    • The rostral migratory stream (RMS) is the main pathway by which newly born subventricular zone cells reach the olfactory bulb (OB) in rodents. However, the RMS in the adult human brain has been elusive. We demonstrate the presence of a human RMS, which is unexpectedly organized around a lateral ventricular extension reaching the OB, and illustrate the neuroblasts in it. The RMS ensheathing the lateral olfactory ventricular extension, as seen by magnetic resonance imaging, cell-specific markers, and electron microscopy, contains progenitor cells with migratory characteristics and cells that incorporate 5-bromo-2'-deoxyuridine and become mature neurons in the OB.

    • Radiofrequency (RF) generated thermal brain lesions are widely used in functional neurosurgery. The size, shape and development of the lesions depends on system parameter settings and the electrode configuration. Difficulties in studying the effect of these factors in vivo stimulated us to develop an in vitro system for standardized comparison between different electrodes and physical parameters. A computer-assisted video system was set-up allowing continuous video recording of RF-generated coagulations in either a standard albumin solution or in the fresh white of a hen's egg as transparent test substrates. Ten lesions were made with each test electrode (two bipolar and three monopolar) in each of the two substrates at 70 degrees, 80 degrees and 90 degrees C (t = 60 sec). Due to the better homogeneity the lesions in the albumin solution were much more regular and reproducible. This made it possible to calculate the size (width 2.2 +/- 0.1 to 5.3 +/- 0.1 mm and length 3.0 +/- 0.1 to 8.7 +/- 0.3 mm) as well as the volume (8.5 +/- 1.4 mm3 to 133.5 +/- 26.8 mm3). It is concluded that this in vitro system offers a reproducible way to study and document the effect of different electrode configurations and RF-generator settings on the formation of a heat lesion. Even if the results are not directly applicable to the living human brain they give an estimate of the form and size of a coagulation lesion and can be of value for standardized comparisons between different electrodes.

  • Dynamics of the immune response against extracellular products of group A streptococci during infection

    • The immune response against the infecting group A streptococcus (GAS) extracellular products (EP) was determined in acute- and convalescent-phase sera from 75 patients with different clinical manifestations of GAS infection. All EP elicited a high proliferative response in human peripheral blood mononuclear cells. In patients with bacteremia, low neutralization in acute-phase sera was associated with development of streptococcal toxic shock syndrome. Lack of neutralization in acute-phase sera was more common in patients infected with the T1emm1 serotype. The majority of patients did not develop the ability to neutralize the mitogenic activity of their infecting isolate despite a significant increase in enzyme-linked immunosorbent assay titer in early convalescent-phase sera. In patients with the ability to neutralize GAS EP, the immune response remained high over at least 3 years. In contrast, the neutralization capacity conferred by intravenous immunoglobulin and/or plasma treatment disappeared within 3 months.

  • Chronic bronchitis in West Sweden - a matter of smoking and social class

    • Background: Although chronic bronchitis is associated with impaired quality of life, hospitalisations and increased mortality, it has been less in focus after the introduction of the term chronic obstructive pulmonary disease (COPD). There are no recent published data on the prevalence of chronic bronchitis from the Scandinavian countries. Aim: The main aim of the present study was to estimate the prevalence of chronic bronchitis in West Sweden by using data from a large-scale epidemiological study of the general population. A further aim was to identify current risk factors for chronic bronchitis in a population with a major decrease in the proportion of smokers. Methods: From the 18,087 questionnaire responders out of 30,000 invited to participate at the West Sweden Asthma Study, 2,000 subjects were randomly selected and invited to detailed clinical examinations performed during 2009-2013. A total of 1,172 subjects aged 17-79 participated in the examinations which included, among others, spirometry and structured interviews. Chronic bronchitis was defined according to reported symptoms. Results: The overall prevalence of chronic bronchitis was 7.2% (men 7.6%; women 6.8% ns), and it was 8.7% in subjects older than age 60. Chronic bronchitis was strongly associated with smoking, defined both as current smoking status and pack-years. Other risk factors were increasing age, low socio-economic class and urban living. Of those with chronic bronchitis, 22% fulfilled the GOLD criteria of COPD. Conclusion: The prevalence of chronic bronchitis was somewhat lower than found by studies in Sweden in the 1980s and the prevalence was now similar in men and women. Although smoking was still the dominating risk factor for chronic bronchitis, the relative importance of smoking had decreased parallel with a decreasing smoking prevalence, while the relative importance of other factors than smoking had increased compared to previous studies.

    • Background: The smoking prevalence in Sweden has steadily decreased during three decades. The prevalence of COPD in Sweden in the 1990s and around the millennium shift was similar to neighboring European countries, i.e. estimated at 11-17%, and of moderate to severe COPD to 7-11%, in ages over 40y.Aim: Has the prevalence of chronic airway obstruction (CAO) and of COPD in Sweden decreased after decades of decreasing smoking prevalence?Methods: Within two large scale population studies in progress, the West Sweden Asthma Study (WSAS) and the Obstructive Lung Disease in Northern Sweden Studies (OLIN), the prevalence of COPD in ages 41-72y was calculated among randomly selected subjects from the general population in 2009-2012. The following post-bronchodilator definitions were used; CAO: FEV1/FVCResults: The prevalence of CAO based on the FEV1/FVCConclusion: As prevalence of COPD defined as chronic airway obstruction before and around the millennium shift was estimated at 11-17% in ages >40 years, the prevalence of COPD has decreased in Sweden, and the decrease in smoking over decades is probably the main causal factor.

    • The previously established graptolite and chitinozoan Hirnanian-Telychian biostratigraphy in the unique Osmundsberget North outcrop in the Siljan region, south-central Sweden, is integrated with new conodont biostratigraphy and C-13(org), C-13(carb) and S-34(pyr) chemostratigraphy. At this locality, the middle Hirnantian (latest Ordovician) topmost part of the Boda Limestone is overlain by the latest Hirnantian Glisstjarn Formation, and the late Aeronianearly Telychian (Llandovery) Kallholn Formation rests unconformably on the Glisstjarn Formation. Previous conodont work showed that the Glisstjarn Formation belongs to the lower Ozarkodina hassi Zone. New samples from calcareous interbeds in the dominantly shaly Kallholn Formation, some of which contain hundreds of condont elements, yielded Distomodus staurognathoides, Aspelundia fluegeli and other taxa indicating the D.staurognathoides Zone. In the East Baltic succession, the coeval interval, which is in the uppermost Raikkulalowermost Adavere stages just below the geographically widespread Osmundsberg K-bentonite, has yielded a conodont fauna similar to that of the lower Kallholn Formation. A regional review of the D. staurognathoides Zone shows that there are possible equivalents to our study interval also in Norway and the Welsh Borderland, but equivalent strata are missing in large parts of North America, or have not produced diagnostic conodonts. The C-13(org) values from the study section are relatively uniform (mostly ranging between 29 parts per thousand and 30 parts per thousand), and the late Aeronian and Valgu positive excursions have not been recognized.

    • The delta C-13(org) chemostratigraphy of the Hirnantian and lower Rhuddanian in the biostratigraphically well-controlled Rostanga-1 drillcore from west-central Scania is used for an improved integration of the Hirnantian Isotope Carbon Excursion (HICE) with the standard graptolite zonation. In this drillcore succession, the end of the HICE corresponds to the top of the range of Metabolograptus persculptus. Baseline delta C-13(org) values occur in the uppermost Hirnantian Avitograptus avitus Faunal Interval as well as in the Rhuddanian Akidograptus ascensus Zone, and the isotope curve is also tied to the Swedish uppermost Katian and Hirnantian trilobite zonation. Chemostratigraphic data from sections in Vastergotland confirm that the beginning of the HICE is at, or very close to, the base of the Skultorp Member of the Loka Formation. The biostratigraphically less precisely controlled end of the HICE is at least locally in the Upper Member of the same formation. The graptolite biostratigraphy in the Mt. Kinnekulle succession indicates that the lowermost Kallholn Formation, which has long been known as the Leonaspis (formerly Acidaspis) Shale, is of Hirnantian rather than earliest Silurian age which is consistent with the age of the lowermost Kallholn Formation in the Rostanga-1 drillcore. Comparisons with Hirnantian sections in the United Kingdom, North America and China make it possible to improve the calibration of the HICE with conodont and graptolite biostratigraphy and confirm the usefulness of delta C-13(org) chemostratigraphy for detailed correlations. The upper Katian carbon chemostratigraphy in key sections in North America and eastern Baltoscandia indicates that the Elkhorn and Paroveja excursions are the same. Available data are used for a new Hirnantian eustasy-climate-faunal evolution model.
