  • Person-centred care on the move : An interview study with programme directors in Swedish higher education

    • BACKGROUND: There is an increasing trend towards person-centred care (PCC) worldwide, suggesting that PCC should be mastered by future health care professionals. This study aims to explore programme directors' views on facilitators and barriers to implementing PCC in four of the largest national study programmes in Sweden training future health care professionals.METHODS: A qualitative design was applied and interviews were conducted with 19 programme directors of Swedish national study programmes in medicine, nursing, occupational therapy and physiotherapy. The interviews were analysed using qualitative content analysis. Themes were sorted according to the Consolidated Framework for Implementation Research (CFIR) in an abductive approach. COREQ guidelines were applied.RESULTS: The overarching theme, as interpreted from the programme directors' experiences, was 'Person-centred care is on the move at different paces.' The theme relates to the domains identified by the CFIR as outer setting, innovation, inner setting and process. PCC was understood as something familiar but yet new, and the higher education institutions were in a state of understanding and adapting PCC to their own contexts. The movement in the outer setting consists of numerous stakeholders advocating for increased patient influence, which has stirred a movement in the inner setting where the higher educational institutions are trying to accommodate these new demands. Different meanings and values are ascribed to PCC, and the concept is thus also 'on the move', being adapted to traditions at each educational setting.CONCLUSION: Implementation of PCC in Swedish higher education is ongoing but fragmented and driven by individuals with a specific interest. There is uncertainty and ambiguity around the meaning and value of PCC and how to implement it. More knowledge is needed about the core of PCC as a subject for teaching and learning and also didactic strategies suitable to support students in becoming person-centred practitioners.

    • Purpose: To evaluate the effect of a communication partner training programme directed to enrolled nurses working with people with communication disorders in nursing homes, using an individualised approach. Method: Five dyads consisting of a person with stroke-induced aphasia (n = 4) or Parkinson’s disease (n = 1) living in different nursing homes and his/her enrolled nurse participated in the study, which had a replicated single subject design with multiple baselines across individuals. The main element of the intervention was supervised analysis of video-recorded natural interaction in everyday nursing situations and the formulation of individual goals to change particular communicative strategies. Results: Outcome was measured via blinded assessments of filmed natural interaction obtained at baseline, intervention and follow-up and showed an increased use of the target communicative strategies. Subjective measures of goal attainment by the enrolled nurses were consistent with these results. Measures of perceived functional communication on behalf of the persons with communication disorders were mostly positive; four of the five participants with communication disorders and two of five enrolled nurses reported improved functional communication after intervention. Conclusions: The use of an individualised communication partner training programme led to significant changes in natural interaction, which contributes importantly to a growing body of knowledge regarding communication partner training.

    • Introduction One aspect of the concept of functional communication is the focus on ability to get information across and conversation partners may be trained to support communication in relation to communication disorders (Simmons-Mackie et al., 2010). However, if the main aim for an interaction is to get a message across it may have consequences for the amount of repair needed to accomplish the goal. As frequent and prolonged repair sequences may be seen as revealing incompetence in participants in interaction, such goals may be in conflict with goals related to the notion of face (Goffman, 1955; Wilkinson et al, 2003). The aim of this study is to describe the effects of a communication partner training program on everyday interaction between a person with aphasia and a professional carer at a nursing home. Method The study presented here is a case study with a time series design supplemented with conversation analysis. At each of seven baseline sessions and eight training sessions a sample of natural conversational interaction between the person with aphasia and his carer was video-recorded. Outcome of intervention was assessed with a rating scale by an assessor, blinded to where in the process of intervention the recordings were obtained. The scale is measuring the caregivers’ ability to apply strategies to support communication with this particular person with aphasia. The result from this quantitative measure is supplemented with qualitative analysis of the effects from the training on repair sequences in the interaction. Results and discussion Analyses of data are presently being performed and results will be discussed and related to a potential conflict between aspects of the concept of functional communication versus the concept of face in interaction in relation to communication disorders.   • Centredness in health care: A systematic overview of reviews

    • Introduction The introduction of effective, evidence-based approaches to centredness in health care is hindered by the fact that research results are not easily accessible. This is partly due to the large volume of publications available and because the field is closely linked to and in some ways encompasses adjoining fields of research, for example, shared decision making and narrative medicine. In an attempt to survey the field of centredness in health care, a systematic overview of reviews was conducted with the purpose of illuminating how centredness in health care is presented in current reviews. Methods Searches for relevant reviews were conducted in the databases PubMed, Scopus, Cinahl, PsychINFO, Web of Science and EMBASE using terms connected to centredness in health care. Filters specific to review studies of all types and for inclusion of only English language results as well as a time frame of January 2017-December 2018, were applied. Results The search strategy identified 3697 unique reviews, of which 31 were included in the study. The synthesis of the results from the 31 reviews identified three interrelated main themes: Attributes of centredness (what centredness is), Translation from theory into practice (how centredness is done) and Evaluation of effects (possible ways of measuring effects of centredness). Three main attributes of centeredness found were: being unique, being heard and shared responsibility. Aspects involved in translating theory into practice were sufficient prerequisites, strategies for action and tools used in safeguarding practice. Further, a variety and breadth of measures of effects were found in the included reviews. Conclusions Our synthesis demonstrates that current synthesized research literature on centredness in health care is broad, as it focuses both on explorations of the conceptual basis and the practice, as well as measures of effects. This study provides an understanding of the commonalities identified in the reviews on centredness in healthcare overall, ranging from theory to practice and from practice to evaluation. Patient or Public Contribution Patient representatives were involved during the initiation of the project and in decisions about its focus, although no patient or public representatives made direct contributions to the review process.

    • Background and objectives: Managing communicative disability is a pervasive issue in long-term care facilities. The aim of this study was to explore how enrolled nurses experience their everyday interactions with residents in nursing homes, particularly focusing on interactions with residents with communicative disability. Design: A qualitative exploratory design including content analysis was used. Participants and setting: Eight individuals working at six nursing homes in western Sweden were interviewed. Method: The interviews were semi-structured with questions about the participants’ experiences in communicating with residents, feelings associated with interactions involving residents with communicative disability, meaning ascribed to interactions, and factors influencing interactions. The interviews were analysed using content analysis. Results: A dynamic interplay between interpersonal relations, daily interactions and the managing of communicative disability was revealed. The enrolled nurses had good knowledge of supportive strategies and an awareness of the importance of the development of personal relationships with residents in order to facilitate interaction. However, factors in the environment presented barriers to communication. Conclusions/Implications: The organisation and physical environment of nursing homes prevent the enrolled nurses from taking full advantage of the communicative resources they have in interaction with residents with communicative disability, hence affecting staff-resident relationships and the delivery of person-centred care.

  • Enterprise Social Media in Project-based Knowledge Work : A contextualized view through the lens of activity theory

    • Project-based knowledge work is characterised by decentralised and dynamic team-based structures, different from the tradi­tional, bureaucratic organisation. Arguably, this flexible and collaborative work setting could benefit from enterprise social media, which helps facilitate how people connect, collaborate and share information across organisational boundaries.This thesis applies the theoretical lens of activity theory to explore the adoption, use and integration of enterprise social media in project-based knowledge work. Moreover, it develops a rich sociocultural understanding of how these platforms are situated in work activities. In particular, the focus lies on information sharing and the potential to connect different types of work, roles and responsibilities.Through a qualitative research design, this thesis contributes towards under­standing the subtle interplay between enterprise social media, employees’ infor­mation sharing and their situated work activities. Key findings reveal that enter­prise social media created long-term strategic benefits, such as a shared knowledge base, situation awareness, horizon scanning, as well as making project-based knowledge work more intertwined. Furthermore, support was also found for a grassroots approach for adoption, as employees were personally invested and more likely to find meaningful use in their activities, through exploration, innovation and trial and error.This thesis also identifies contradictions that relate to the role of enterprise social media for information sharing. It became evident that different platforms could co-exist in functional arrangements, with a specific place and purpose in the wider ecology. However, contradictions emerge in terms of overlaps between technolo-gies, asymmetry in usage and the appearance of different information sharing practices and dependencies.This work presents new theoretical insights into how enterprise social media are used for information sharing in work activities, bringing attention to what people are aiming to achieve, and highlighting how sociocultural elements can have an influence.
