    • Ingestion of aliphatic hydrocarbons--even in small amounts--can cause severe respiratory distress in children. Such accidents are becoming increasingly common in Sweden. A case of severe hydrocarbon ingestion is reported, where a 16-month-old boy, initially treated with mechanical ventilation for respiratory distress, subsequently developed a reversible radiological complication, pneumatocele. Patients at risk of severe hydrocarbon intoxication can be identified by careful evaluation of their history and continuous clinical observation. Controlled ventilation should be instituted early in respiratory distress, and any radiological changes followed up in a few weeks.

  • Mammography screening reduces rates of advanced and fatal breast cancers : Results in 549,091 women

    • Background: It is of paramount importance to evaluate the impact of participation in organized mammography service screening independently from changes in breast cancer treatment. This can be done by measuring the incidence of fatal breast cancer, which is based on the date of diagnosis and not on the date of death.Methods: Among 549,091 women, covering approximately 30% of the Swedish screening‐eligible population, the authors calculated the incidence rates of 2473 breast cancers that were fatal within 10 years after diagnosis and the incidence rates of 9737 advanced breast cancers. Data regarding each breast cancer diagnosis and the cause and date of death of each breast cancer case were gathered from national Swedish registries. Tumor characteristics were collected from regional cancer centers. Aggregated data concerning invitation and participation were provided by Sectra Medical Systems AB. Incidence rates were analyzed using Poisson regression.Results: Women who participated in mammography screening had a statistically significant 41% reduction in their risk of dying of breast cancer within 10 years (relative risk, 0.59; 95% CI, 0.51‐0.68 [P  < .001]) and a 25% reduction in the rate of advanced breast cancers (relative risk, 0.75; 95% CI, 0.66‐0.84 [P  < .001]).Conclusions: Substantial reductions in the incidence rate of breast cancers that were fatal within 10 years after diagnosis and in the advanced breast cancer rate were found in this contemporaneous comparison of women participating versus those not participating in screening. These benefits appeared to be independent of recent changes in treatment regimens.

  • From Eye to Mind : Early Visuomotor Performance and Developmental Trajectories in Children Born Preterm

    • Children born very preterm, at less than 32 weeks of gestation, have an increased risk of developing problems with attention, cognition, perception and motor function. Despite this, the developmental trajectories leading from preterm birth to later impairments are not fully understood.This thesis focused on investigating the development of the visuomotor system in infants born very preterm, in relation to neurodevelopment at 2.5-3 years (including cognitive, language and motor functions) and 6.5 years (including cognitive functions and attention). Two early visuomotor functions were investigated: at 4 months of corrected age we looked at their visual tracking of moving objects and at 8 months of corrected age we examined how they reached for moving objects. Visual tracking performance is the ability to follow, sustain attention on and predict the future positions of a moving object. When infants reach for moving objects, these requirements are enhanced by movements of the arm and hand.This was a prospective, population-based cohort study of children born very preterm in 2004-2007, who were longitudinally assessed from the neonatal period until 6.5 years of age. The results showed that the two most important early visual tracking parameters were gaze gain, which is effectively combining eye and head movements to follow the trajectory of the object, and timing of gaze to the motion of the object. Gaze gain correlated with neurodevelopment at 2.5-3 years and with cognition and attention at 6.5 years. Early reaching was also related to outcomes at 2.5-3 years, but the pattern differed for children born before and after 28 weeks of gestation. Parameters of prediction and movement planning were the most important factors for the children of the lowest gestational age, but strategies and success had greater associations with later function in those who were less premature.The results confirmed that children born very preterm in the first decade of the 2000’s still faced increased risks of developmental delays. Early visual tracking performance, and reaching for moving objects, were shown to be important visuomotor functions in the developmental process of children born very preterm. The studies in this thesis illustrate how early basic functions were related to different neurodevelopmental areas at a later stage and offer important new insights into developmental trajectories of children at risk of developmental impairments. These findings suggest that attention and predictive elements of adjusting one´s movements to motion, may be key mechanisms and possible targets in future intervention studies.

    • The aim of this dissertation is to show how a handicraft-aesthetics are shaped and reshaped, with a particular focus on the interplay between domestic and international handicraft. This is studied in a number of forums, and the negotiations often revolve around what it is that characterises good craft. Specific emphasis is put on investigating the conditions under which and where the interface between these categories might occur. With the aim of providing a perspective of the Swedish situation, a comparative study of how the American patriotic women’s movement, Daughters of the American Revolution, took part in the formation of domestic traditions, ideals and aesthetics in the USA during the early part of the 20th century has also been carried out. From an historical perspective it is illustrated how the images we have both of ourselves and others are mutually dependent and ever-changing. The dissertation is based on fieldwork at costume parades, craft markets, costume shows and exhibitions, as well as in projects involving immigrant women where Swedish handicraft is used as a means of teaching the Swedish language. It is also based on archive research (in various Swedish archives, the Ellis Island archive, New York and the National Archive of the Daughters of the American Revolution, Washington DC). Since the 1970s, international handicraft, in connection with immigration policy and the Handicraft Movement network, has been referred to as an “enrichment” and “vitalisation” of Swedish cultural life and Swedish handicraft. In spite of this the actual presence of international craftwork and craft workers with foreign backgrounds is limited within the Movement. International handicraft and Sámi crafts are both included at the same time as they are distinguished as special interests in relation to normative domestic craftwork. Other handicraft traditions have thus constituted the domestic handicraft’s constituent facades.

    • BACKGROUND: Typically developing infants track moving objects with eye and head movements in a smooth and predictive way at 4 mo of age, but this ability is delayed in very preterm infants. We hypothesized that visual tracking ability in very preterm infants predicts later neurodevelopment. METHOD: In 67 very preterm infants (gestational age<32wk), eye and head movements were assessed at 4 mo corrected age while the infant tracked a moving object. Gaze gain, smooth pursuit, head movements, and timing of gaze relative the object were analyzed off line. Results of the five subscales included in the Bayley Scales of Infant Development (BSID-III) at 3 y of age were evaluated in relation to the visual tracking data and to perinatal risk factors. RESULTS: Significant correlations were obtained between gaze gain and cognition, receptive and expressive language, and fine motor function, respectively, also after controlling for gestational age, severe brain damage, retinopathy of prematurity, and bronchopulmonary dysplasia. CONCLUSION: This is the first study demonstrating that the basic ability to visually track a moving object at 4 mo robustly predicts neurodevelopment at 3 y of age in children born very preterm.

Butikerna på landsbygden slår igen en efter en beroende både på avfolkning och på förändrade inköpsmönster hos befolkningen. Studien visar på omfattningen av "butiksdöden" på landsbygden. Intervjuer med de boende på orten Hedesunda tyder på att de som drabbas av butiksdöden inte ser särskilt allvarligt på detta. De har redan när de valt att bo på landsbygden valt bort en del av servicen menar de. Författaren drar slutsatsen att detta betyder att konsumentens Kommersiella geografiska vardag har fått en ny innebörd och utvidgats.

    • The process of photoactivation has been studied in dark grown cells of Chlamydomonas reinhardtii. A mutant, FUD 39, lacking the Cl-concentrating 23-kDa psbP protein of photosystem II was found to have a decreased capability to perform photoactivation. The yield of the process never reached wild type level, and contrary to the wild type, it was highly dependent on the intensity of the activating light, with a very narrow optimum around 1 µE m2 s1. The different behavior in the mutant can be explained by a requirement for a longer dark period, between the two photoacts, during the photoactivation. This is proposed to reflect the decreased Cl affinity in the mutant. Photoactivation in the mutant was also found to be very sensitive to competing photoinhibitory processes. The inhibition was located to the donor side of photosystem II and affected the photoactivation capability before electron transfer from Tyrz was inhibited. We propose an extended model for photoactivation in which an intermediate that is sensitive to photoinhibition is formed if Cl is not functionally bound to the manganese cluster.
