  • Dapagliflozin for Heart Failure According to Body Mass Index : The DELIVER Trial.

    • AIMS: Obesity is common and associated with unique phenotypic features in heart failure with preserved ejection fraction (HFpEF). Therefore, understanding the efficacy and safety of new therapies in HFpEF patients with obesity is important. The effects of dapagliflozin were examined according to body mass index (BMI) among patients in the Dapagliflozin Evaluation to Improve the LIVEs of Patients With PReserved Ejection Fraction Heart Failure trial. METHODS AND RESULTS: Body mass index was analysed by World Health Organization (WHO) categories and as a continuous variable using restricted cubic splines. Body mass index ranged from 15.2 to 50 kg/m2 with a mean value of 29.8 (standard deviation +/- 6.1) kg/m2. The proportions, by WHO category, were: normal weight 1343 (21.5%); overweight 2073 (33.1%); Class I obesity 1574 (25.2%); Class II obesity 798 (12.8%); and Class III obesity 415 (6.6%). Compared with placebo, dapagliflozin reduced the risk of the primary outcome to a similar extent across these categories: hazard ratio (95% confidence interval): 0.89 (0.69-1.15), 0.87 (0.70-1.08), 0.74 (0.58-0.93), 0.78 (0.57-1.08), and 0.72 (0.47-1.08), respectively (P-interaction = 0.82). The placebo-corrected change in Kansas City Cardiomyopathy Questionnaire total symptom score with dapagliflozin at 8 months was: 0.9 (-1.1, 2.8), 2.5 (0.8, 4.1), 1.9 (-0.1, 3.8), 2.7 (-0.5, 5.8), and 8.6 (4.0, 13.2) points, respectively (P-interaction = 0.03). The placebo-corrected change in weight at 12 months was: -0.88 (-1.28, -0.47), -0.65 (-1.04, -0.26), -1.42 (-1.89, -0.94), -1.17 (-1.94, -0.40), and -2.50 (-4.4, -0.64) kg (P-interaction = 0.002). CONCLUSIONS: Obesity is common in patients with HFpEF and is associated with higher rates of heart failure hospitalization and worse health status. Treatment with dapagliflozin improves cardiovascular outcomes across the spectrum of BMI, leads to greater symptom improvement in patients with obesity, compared with those without, and has the additional benefit of causing modest weight loss.

  • TRAFIR: Characterization of TRAvelling FIRes in large compartments

    • Inspection of recent fire events in large compartments reveals them to have a great deal of non-uniformity, they generally burn locally and move across floor plates over a period of time. This phenomenon which generates transient heating of the structure is idealized as “travelling fire”.A first series of tests was launched to define a fire load representative of an office building according to Eurocodes. Additional tests where the fire dynamics were controlled were launched to develop an understanding of the fire exposure to steel structures.Then, a second series of large scale tests were performed in real building dimensions. These tests had no artificial control over the dynamics, which allowed a realistic characterization of the fire. The fire load was identical for all tests, only the openings were modified.CFD numerical models were developed to reproduce the experimental campaign and to launch parametrical analyses. This allowed to provide information concerning the conditions which may lead (or not) to a travelling fire scenario.An analytical model for the characterization of a travelling fire was developed and implemented in a simple calculation tool. It allows to evaluate the fire location, the gas temperatures in the flames, the heat fluxes in the different parts of the compartment and the temperature in a steel member. In addition, the methodology is introduced in the FEM software SAFIR and OpenSees.Ultimately, a design guide was prepared including worked examples which are detailed step-by-step and for which the influence of the inputs on the results is analysed.

  • An engineering CFD model for fire spread on wood cribs for travelling fires

    • The temperature heterogeneity due to fire in large open-plan office compartments is closely associated with fire spread behaviour and has been historically limited to experimental investigations using timber cribs. This study explores the ability of Computational Fluid Dynamics (CFD) models, specifically the Fire Dynamics Simulator (FDS), to reproduce the results of full-scale tests involving fire spread over timber cribs for continuous fuel-beds. Mesh schemes are studied, with a fine mesh over the crib and 2 × 2 cells in the wood stick cross-section by default, this being relaxed in the surrounding regions to enhance computational efficiency. The simple pyrolysis model considers the charring phase and moisture. In application to the TRAFIR-Liège LB7 test, this calibrated “stick-by-stick” representation shows a good agreement for interrelated parameters of heat release rate, fire spread, gas phase temperature, and burn-away, a set of agreements which has not been demonstrated in previous studies. Fire spread shows relatively high sensitivities to: heat of combustion, ignition temperature, thermal inertia, radiation fraction, heat release rate per unit area, and the fuel load density. An approximately linear regression was found between the different fire modes and the thermal exposures, with “travelling” (and decaying) fires characterised by heat fluxes associated with the fire plume, while the growing fires were associated with proportionally higher heat fluxes on the horizontal surfaces of the sticks, in conditions where these receive more pre-heating. The trends in the overall HRR are more dependent on the fire spread rates than variations in the stick burning rates. © 2022 The Author(s)

  • Baseline Characteristics of Patients With HF With Mildly Reduced and Preserved Ejection Fraction : DELIVER Trial.

    • OBJECTIVES: This report describes the baseline clinical profiles and management of DELIVER (Dapagliflozin Evaluation to Improve the Lives of Patients With Preserved Ejection Fraction Heart Failure) trial participants and how these compare with those in other contemporary heart failure with preserved ejection fraction trials. BACKGROUND: The DELIVER trial was designed to evaluate the effects of the sodium-glucose cotransporter-2 inhibitor dapagliflozin on cardiovascular death, heart failure (HF) hospitalization, or urgent HF visits in patients with HF with mildly reduced and preserved left ventricular ejection fraction (LVEF). METHODS: Adults with symptomatic HF and LVEF $>$40%, with or without type 2 diabetes mellitus, elevated N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels, and evidence of structural heart disease were randomized to dapagliflozin 10 mg once daily or matching placebo. RESULTS: A total of 6,263 patients were randomized (mean age: 72 +/- 10 years; 44% women; 45% type 2 diabetes mellitus; 45% with body mass index $>$/=30 kg/m(2); and 57% with history of atrial fibrillation or flutter). Most participants had New York Heart Association functional class II symptoms (75%). Baseline mean LVEF was 54.2 +/- 8.8% and median NT-proBNP of 1,399 pg/mL (IQR: 962 to 2,210 pg/mL) for patients in atrial fibrillation/flutter compared with 716 pg/mL (IQR: 469 to 1,281 pg/mL) in those who were not. Patients in both hospitalized and ambulatory settings were enrolled, including 10% enrolled in-hospital or within 30 days of a hospitalization for HF. Eighteen percent of participants had HF with improved LVEF. CONCLUSIONS: DELIVER is the largest and broadest clinical trial of this population to date and enrolled high-risk, well-treated patients with HF with mildly reduced and preserved LVEF.   • Dapagliflozin in Heart Failure with Mildly Reduced or Preserved Ejection Fraction.

    • BACKGROUND: Sodium-glucose cotransporter 2 (SGLT2) inhibitors reduce the risk of hospitalization for heart failure and cardiovascular death among patients with chronic heart failure and a left ventricular ejection fraction of 40% or less. Whether SGLT2 inhibitors are effective in patients with a higher left ventricular ejection fraction remains less certain. METHODS: We randomly assigned 6263 patients with heart failure and a left ventricular ejection fraction of more than 40% to receive dapagliflozin (at a dose of 10 mg once daily) or matching placebo, in addition to usual therapy. The primary outcome was a composite of worsening heart failure (which was defined as either an unplanned hospitalization for heart failure or an urgent visit for heart failure) or cardiovascular death, as assessed in a time-to-event analysis. RESULTS: Over a median of 2.3 years, the primary outcome occurred in 512 of 3131 patients (16.4%) in the dapagliflozin group and in 610 of 3132 patients (19.5%) in the placebo group (hazard ratio, 0.82; 95% confidence interval [CI], 0.73 to 0.92; P$<$0.001). Worsening heart failure occurred in 368 patients (11.8%) in the dapagliflozin group and in 455 patients (14.5%) in the placebo group (hazard ratio, 0.79; 95% CI, 0.69 to 0.91); cardiovascular death occurred in 231 patients (7.4%) and 261 patients (8.3%), respectively (hazard ratio, 0.88; 95% CI, 0.74 to 1.05). Total events and symptom burden were lower in the dapagliflozin group than in the placebo group. Results were similar among patients with a left ventricular ejection fraction of 60% or more and those with a left ventricular ejection fraction of less than 60%, and results were similar in prespecified subgroups, including patients with or without diabetes. The incidence of adverse events was similar in the two groups. CONCLUSIONS: Dapagliflozin reduced the combined risk of worsening heart failure or cardiovascular death among patients with heart failure and a mildly reduced or preserved ejection fraction.
