  • Quality Improvement in Healthcare : Experiences from a Swedish County Council Initiative

    • Quality improvement (QI) has become an important issue in healthcare settings. A central question for many healthcare systems is how to manage improvement initiatives adequately. All county councils and regions managing healthcare in Sweden have started to work with QI at an organizational system level, to varied extents. The Kalmar county council improvement initiative constitutes the empirical basis of this thesis. The aim of the thesis is to provide knowledge about different aspects of a county-wide improvement initiative, and a broader understanding of factors and strategies that affect participation, management and outcomes. The overall study design is based on a case study.The first two studies illuminate the practice-based (micro level), bottom-up perspective. Inductively five different areas (categories) were identified. Factors influencing participation in improvement initiatives provided the basis for the next study. The result showed that different staff categories were attracted by different initiatives. The next two studies illuminate the top-down (macro/meso) management perspective. Managers’ views of how patients can participate were investigated and a content analysis of the written answers was made. Four main areas (categories) were identified. A survey study investigated all of the county council managers’ experiences of the whole improvement initiative. Overall the managers thought that the improvement work was worth the effort. To evaluate the Breakthrough Collaborative program, a survey was developed and tested. This survey was used to investigate process and outcome of the BC program. The majority of the respondents were satisfied with their work, but wanted more time for teams to meet and work. To find out if an improvement program can affect outcome and contribute to sustainable changes, interviews were made with project applicants (n=202). Almost half (48%) of the projects were funded, and of those 51% were sustained. Of the rejected (not funded) projects, 28% were accomplished and sustained anyway. The results in this thesis cannot show that the “golden mean” exists, or that a single best way to manage changes and improvements in a healthcare organization has been found, but the way QI initiatives are organized does affect participation and outcomes. The intention, from the management topdown system level, encouraging staff and units and letting practice-based ideas develop at all system levels, can stimulate and facilitate improvement work.

    • The purpose of this paper is to explore the adequacy of business excellence models (BEMs) to address corporate sustainability, which is conceptualised in terms of economic, social, and environmental bottom lines. How organisations may manage corporate sustainability in the absence of a comprehensive sustainability management system standard is also explored. A survey of literature has been carried out. The European Foundation for Quality Management (EFQM) excellence model and the Baldrige Criteria for Performance Excellence (BCPE) are analysed with regard to their considerations of sustainability. The findings reveal that while triple bottom-line considerations are addressed to some extent in the studied BEMs, the models per se do not comprehensively address sustainability issues and economic prosperity remains a dominant consideration. Suggestions for the improvement of the EFQM excellence model and BCPE are discussed. To provide a comprehensive approach for addressing corporate sustainability, an integrated quality-sustainability framework is proposed. The essence of the framework is that sustainability indicators, such as those provided in the Global Reporting Initiative framework, could be integrated with core business processes using the structures and infrastructure provided by BEMs. The proposed improvements should be considered in future revisions of the EFQM excellence model and BCPE.

  • Individual Assessment of Students Working in Project Teams

    • Many engineering courses include assignments where students work together in projects. The approach promotes students, among other things, to be educated and skilled in project management and managing system thinking within complex engineering environments. However, a problem with project-based learning is to accomplish a fair and valid assessment of individuals in a team setting. For example, in many project-based courses, students are only graded pass or fail, or graded collectively as a group. This paper presents results from a new course design based on CDIO-principles, with the aim to increase our understanding of individual assessment and grading in a project-based course. A preliminary conclusion is that it is possible to introduce individual assessment in a project team, assessing the learning outcomes and obtain a high level of student satisfaction. The course development process described in this study has implied a lot of struggling for the teachers involved. Hence, some general aspects of introducing CDIO-principles in a project-based course are also discussed.

  • Alive and kicking–but will Quality Management be around tomorrow? : A Swedish academia perspective

    • Purpose: There is a lack of a recognized conception of quality management (QM) comprises of, as well as a clear roadmap of where QM is heading. The purpose of this article is to investigate how QM is perceived today by scholars at three Swedish universities, but also how and into what QM is expected to develop into in twenty years.Methodology: Data have been collected through three structured workshops using affinity diagrams with scholars teaching and performing research in the QM field affiliated with three different Swedish universities.Findings: The results indicate that current QM is perceived similarly among the universities today, although the taxonomy differs slightly. QM is described as a fairly wide discipline consisting of a set of core of principles that in turn guide which methods and tools that currently by many are perceived as the core of the discipline. The outlook for the future differs more where three possible development directions for QM are seen: [1] searching for a “discipline X” where QM can contribute while keeping its toolbox, [2] focus on a core based on the traditional quality technology toolbox with methods and tools, and [3] a risk that QM, as it is today, may seize to exist and be diffused into other disciplines. Originality/value: This article contributes with a viewpoint on QM today and its future development from the academicians’ perspective.
