  • Evaluating integrated lighting projects - A Procedure to Post-Occupancy Evaluation of Daylight and Electrical Lighting Integrated Projects

    • This report presents tools for post-occupancy evaluation (POE) to evaluate indoor lighting of commissioned projects (‘case studies’) under a common framework. In this report, POE includes technical environmental assessment (TEA) and observer-based environmental assessment (OBEA). The framework proposed in this report evaluates four key aspects of the case study:1. Energy use (electrical lighting systems),2. Visual effects (Indoor lighting environment /photometry)3. Non-visual effects (circadian potential), and4. The user (subjective/surveys and observations)The report targets industry professionals, building designers, lighting designers, building managers, researchers and/or owners wishing to evaluate projects where lighting is supplied by a combination of electrical lighting, daylighting systems (e.g., fenestrations) and assisted technologies (e.g., smart sensors). The framework in this report makes available methods and procedures related to the evaluation of integrated lighting performance in residential and non-residential buildings and its impact on users, and it summarises and categorize methods and procedures in an accessible and industry-oriented language.The content of this document is based on methods and procedures used by participating experts in IEA SHC Task 61 for monitoring twenty-five worldwide integrated daylighting and electric lighting case studies. Since integrated lighting projects are different in type and scopes, the methods and procedures included in the framework do not follow a rigid protocol. Practitioners should use the framework to define the scope of POE monitoring in terms of the aims of the project, context, and resources available. The document is thus a toolbox for planning and executing the monitoring of their integrated lighting projects.

  • Integrating daylighting and lighting in practice : Lessons learned from international case studies

    • This report presents lessons learned from twenty-five worldwide real-life case studies implementing the integration of daylighting and electric lighting. The case studies were monitored with respect to energy use for lighting, visual performance, non-visual performance, and users’ satisfaction. The monitoring is largely based on field measurements, but it is also complemented with simulations and calculations where needed.The report is divided in two parts. The first part provides an overview of the case studies and the overall lessons learned. The second part provides factsheets for each of the case studies; the factsheets include details on the monitoring, results, and specific lessons learned.Based on the lessons learned from the case studies, this report concludes that:• The energy demand for lighting is drastically reduced thanks to the combined effect of more efficient light sources, advances in controls, and raised awareness in the integration of daylighting and electric lighting.• Integrative lighting is currently driving the innovation in lighting technology and wider implementation is expected as knowledge in the field of non-visual requirements for lighting expands.• However, the current integration of the integrative lighting concept with daylighting in practice is limited, which may result in significant energy rebound (increases).• Daylighting integration is of utmost importance for achieving quality beyond energy savings.• Integrated daylighting and electric lighting design is facing new challenges: questions connected with comfort and health are yet to be answered

  • Acute Myocarditis Associated With Desmosomal Gene Variants

    • BACKGROUND The risk of adverse cardiovascular events in patients with acute myocarditis (AM) and desmosomal gene variants (DGV) remains unknown.OBJECTIVES The purpose of this study was to ascertain the risk of death, ventricular arrhythmias, recurrent myocarditis, and heart failure (main endpoint) in patients with AM and pathogenic or likely pathogenetic DGV.METHODS In a retrospective international study from 23 hospitals, 97 patients were included: 36 with AM and DGV (DGV[+]), 25 with AM and negative gene testing (DGV[-]), and 36 with AM without genetics testing. All patients had troponin elevation plus findings consistent with AM on histology or at cardiac magnetic resonance (CMR). In 86 patients, CMR changes in function and structure were re-assessed at follow-up.RESULTS In the DGV(+) AM group (88.9% DSP variants), median age was 24 years, 91.7% presented with chest pain, and median left ventricular ejection fraction (LVEF) was 56% on CMR (P = NS vs the other 2 groups). Kaplan-Meier curves demonstrated a higher risk of the main endpoint in DGV(+) AM compared with DGV(-) and without genetics testing patients (62.3% vs 17.5% vs 5.3% at 5 years, respectively; P < 0.0001), driven by myocarditis recurrence and ventricular arrhythmias. At follow-up CMR, a higher number of late gadolinium enhanced segments was found in DGV(+) AM. CONCLUSIONS Patients with AM and evidence of DGV have a higher incidence of adverse cardiovascular events compared with patients with AM without DGV. Further prospective studies are needed to ascertain if genetic testing might improve risk stratification of patients with AM who are considered at low risk.   • Safety and Outcome of Revascularization Treatment in Patients With Acute Ischemic Stroke and COVID-19: The Global COVID-19 Stroke Registry.

    • COVID-19-related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19.This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT).Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16-2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20-2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23-1.99), 24-hour mortality (OR 2.47; 95% CI 1.58-3.86), and 3-month mortality (OR 1.88; 95% CI 1.52-2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26-1.60).Patients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non-COVID-19 patients receiving treatment. Current available data do not allow direct conclusions to be drawn on the effectiveness of revascularization treatments in patients with COVID-19 or to establish different treatment recommendations in this subgroup of patients with ischemic stroke. Our findings can be taken into consideration for treatment decisions, patient monitoring, and establishing prognosis.The study was registered under ClinicalTrials.gov identifier NCT04895462.

  • Blood cell DNA methylation biomarkers in preclinical malignant pleural mesothelioma : the EPIC prospective cohort

    • Malignant pleural mesothelioma (MPM) is a rare and aggressive cancer mainly caused by asbestos exposure. Specific and sensitive noninvasive biomarkers may facilitate and enhance screening programs for the early detection of cancer. We investigated DNA methylation (DNAm) profiles in MPM prediagnostic blood samples in a case-control study nested in the European Prospective Investigation into Cancer and nutrition (EPIC) cohort, aiming to characterise DNAm biomarkers associated with MPM. From the EPIC cohort, we included samples from 135 participants who developed MPM during 20 years of follow-up and from 135 matched, cancer-free, controls. For the discovery phase we selected EPIC participants who developed MPM within 5 years from enrolment (n = 36) with matched controls. We identified nine differentially methylated CpGs, selected by 10-fold cross-validation and correlation analyses: cg25755428 (MRI1), cg20389709 (KLF11), cg23870316, cg13862711 (LHX6), cg06417478 (HOOK2), cg00667948, cg01879420 (AMD1), cg25317025 (RPL17) and cg06205333 (RAP1A). Receiver operating characteristic (ROC) analysis showed that the model including baseline characteristics (age, sex and PC1wbc) along with the nine MPM-related CpGs has a better predictive value for MPM occurrence than the baseline model alone, maintaining some performance also at more than 5 years before diagnosis (area under the curve [AUC] < 5 years = 0.89; AUC 5-10 years = 0.80; AUC >10 years = 0.75; baseline AUC range = 0.63-0.67). DNAm changes as noninvasive biomarkers in prediagnostic blood samples of MPM cases were investigated for the first time. Their application can improve the identification of asbestos-exposed individuals at higher MPM risk to possibly adopt more intensive monitoring for early disease identification.

  • Investigating the multisensory representation of the hand and the space around it using fMRI

    • The human brain incessantly receives a staggering amount of information from all our senses. The dynamic integration of relevant multisensory signals is an evolutionarily optimal strategy to assist adaptive behavior. Arguably, one physical object stands out as the unique recipient of internal and external multisensory signals alike: the body. Recent progress within neurology, psychology, and neuroscience has emphasized the role of multisensory mechanisms in the construction of a coherent neural representation of the body and its surrounding space. In turn, this representation supports essential defensive and goal-directed behaviors, facilitates the processing of incoming sensory signals, and may lead to the emergence of the feeling of ownership of the body, a foundation of self-awareness. However, the experimental accounts of the neural mechanisms underlying the construction of this central multisensory representation of the body and its link to self-perception remain unclear. The aim of the work presented in this thesis was to employ functional magnetic resonance imaging to pinpoint the neural multisensory representation of the hand and the space around it. Furthermore, this thesis aims at relating these multisensory mechanisms to the self-perception of the hand through the interpretation of converging behavioral, neural, and physiological measures. In Studies I, II, and V, we used a combination of ecologically valid paradigms and setups based on virtual reality technology to characterize a set of interconnected premotor, posterior parietal, and subcortical regions that integrate congruent visual, somatosensory, and proprioceptive signals from the upper limb. Furthermore, we provided evidence for the idea that the representation of the hand forms multisensory predictions about self-specific incoming sensory events. In Studies III and IV, we implemented an fMRI-adaptation paradigm to gain evidence of the existence of neuronal populations in the hand-related multisensory areas with visual receptive fields restricted to the space close to the hand and anchored to the upper limb. Complementary neural, behavioral, and physiological measures consistently related these multisensory mechanisms to the feeling of ownership of the hand, which is contingent upon the spatio-temporal congruence of multisensory signals within the peri-hand space. In conclusion, the experiments presented in this thesis describe a set of multisensory brain regions that draw from multiple sensory modalities to build a self-specifying representation of the hand and the space around it. These results are interpreted with respect to earlier findings in non-human primates and humans, and the key functions of this representation, including its role in supporting essential behaviors and the multifaceted self-perception of the hand.

    • Previous research has shown that the integration of multisensory signals from the body in fronto-parietal association areas underlies the perception of a body part as belonging to ones physical self. What are the neural mechanisms that enable the perception of ones entire body as a unified entity? In one behavioral and one fMRI multivoxel pattern analysis experiment, we used a full-body illusion to investigate how congruent visuo-tactile signals from a single body part facilitate the emergence of the sense of ownership of the entire body. To elicit this illusion, participants viewed the body of a mannequin from the first-person perspective via head-mounted displays while synchronous touches were applied to the hand, abdomen, or leg of the bodies of the participant and the mannequin; asynchronous visuo-tactile stimuli served as controls. The psychometric data indicated that the participants perceived ownership of the entire artificial body regardless of the body segment that received the synchronous visuo-tactile stimuli. Based on multivoxel pattern analysis, we found that the neural responses in the left ventral premotor cortex displayed illusion-specific activity patterns that generalized across all tested pairs of body parts. Crucially, a tripartite generalization analysis revealed the whole-body specificity of these premotor activity patterns. Finally, we also identified multivoxel patterns in the premotor, intraparietal, and lateral occipital cortices and in the putamen that reflected multisensory responses specific to individual body parts. Based on these results, we propose that the dynamic formation of a whole-body percept may be mediated by neuronal populations in the ventral premotor cortex that contain visuo-tactile receptive fields encompassing multiple body segments.

  • Evaluation of integrated daylighting and electric lighting design projects: Lessons learned from international case studies

    • This article presents and discusses the lessons learned from the monitoring of 25 integrated daylighting and electric lighting international case study projects. The case studies consist of real occupied buildings that have been monitored as part of the International Energy Agency (IEA) SHC Task 61/EBC Annex 77 Programme. The general goal of the case studies was to balance lighting energy use with occupants’ visual and non-visual requirements. This was achieved using innovative solutions for daylighting and electric lighting with advanced controls, but also implementing simple and out-of-the-box strategies. The findings suggest that energy demands for lighting can significantly be reduced by combining sensible daylight provision, efficient lighting sources, and advances in controls. Yet, the effective achievement of project goals requires adequate monitoring, fine-tuning, and verification. The findings also suggest that the adoption of “integrative” lighting – that is, lighting systems that address both visual and non-visual responses – is getting increasingly popular. Catering to non-visual requirements will likely drive further innovation in lighting technology. Currently, there is limited investment available for developing daylighting systems for integrative lighting, and the current related electric strategies often come at the risk of energy rebound effects. Overall, providing daylighting and understanding user requirements are fundamental steps towards achieving quality projects, with potential benefits beyond saving energy.
