  • Digital Transformation for Crisis Volunteerism : A study in the aftermath of the Swedish Forest Fires Crisis in 2018

    • In the summer of 2018, Sweden was taken by surprise and engulfed by forest fires on an unprecedented scale. Various forms of crisis volunteerism (CV) proved essential in the large scale response that ensued. Previous information systems (IS) research gives insights, both theoretically and practically, on the potential of IS and digitalisation to enhance and support crisis response. However, the forest fires demonstrated that CV practices and practitioners in Sweden have practically no such support. CV is, in other words, an essential part of the crisis response system, but unsupported by dedicated digitalisation. The aim of this thesis is therefore to understand what is required to enable a digital transformation (DT) that will significantly improve CV practice and the overall crisis response system.Sweden is not unique in this respect. From an international perspective, IS research devotes much attention to the area of crisis response. However, this is most commonly done using the perspective and needs of crisis management stakeholders as a departing point. Also, despite research on and the availability of IS solutions, the degree of practical implementation would appear to be low. This thesis is therefore also based on the perceived knowledge gap that the lack of IS and digitalisation supporting CV is, in part, a result of the lack of research focus on volunteer stakeholders and the CV practice itself. Another gap noted is the lack of knowledge regarding the real-world practice of CV which contributes to undermining the development and use of IS solutions for CV.This thesis, therefore, provides a much needed holistic understanding of real-world CV. This understanding moves the perspective of volunteer stakeholders to the forefront without losing the perspective of crisis management. This has been done via an empirical study in the aftermath of the Swedish forest fires crisis as well as via studies of international IS literature on crisis response and CV. This has resulted in a unique holistic and comprehensive model that relates to the complexity, dynamics and emergence involved in CV practice. Via this model it is possible to relate specific parts or aspects of CV practice, to the whole practice area and continue modelling attributes in greater detail, as required, depending on DT or IS design needs.Underlying the focus in this thesis lies a mild critique of previous IS research with more reductionist approaches, whereby the relevance of broader contextual understandings has been downplayed. The thesis aligns with perspectives on digitalisation and IS design, proposing the relevance of developing broader holistic understandings of research topics. That is, understandings that have greater potential to reveal how phenomena come into being and are adapted in environmental contexts.DT is understood in terms of a process whereby broader understandings of phenomena are used to identify needs (triggers), strategies and capabilities that will inform IS design initiatives. An analytical framework depicting this process and its main components and relationships has been designed in this thesis to contribute to an understanding of what is required to digitally transform CV practice.In summary, the thesis provides a new holistic approach and understanding for the CV practice area and how it may be digitally transformed. The thesis also contributes to a new perspective on DT, applied to a complex and non-organisation based setting. This knowledge is of relevance to both practitioners and IS researchers in crisis response and CV. The CV practice meta-model and the analytical framework for digital transformation can be used to enable and inform future digital transformation strategies and policy in Sweden and internationally. They can also contribute to guiding the initiation of practical IS design initiatives, with greater potential to enhance and improve both CV and overall crisis response.The research was performed within the information systems (IS) discipline.

    • Artikeln, författad av forskare, förstelärare och skolledande personal, bygger på ett samverkansprojekt med målet att utveckla en gymnasieskolas kollektiva lärande. Artikeln syftar till att bidra till förståelse av vad ett sådant utvecklingsarbete kan innebära. Metoder som användes var enkätundersökning och intervjuer. Dataanalys genomfördes med stöd av tematisk analys i olika steg (jfr Braun & Clarke, 2006) och teoretiska begrepp som kollektivt och kollegialt lärande (Larsson, 2018) samt olika former av begreppet tillit (Jederlund, 2019). Resultaten visar att förstelärare som leder utvecklingsarbete påverkas av tidigare erfarenheter från utvecklingsarbete och att det är viktigt att lärare som medverkar känner tillit till enskilda kollegor, till den större grupp de ingår i samt till processen bakom utvecklingsarbetet. Samverkansprojektet bidrog till skapande av strukturer för fortsatt utveckling samt till insikt om att utvecklingsarbete främst handlar om att utveckla arbetssätt som är lämpliga för den egna verksamheten.

  • Upstream land use with microbial downstream consequences : iron and humic substances link to Legionella spp

    • Intensified land use can disturb water quality, potentially increasing the abundance of bacterial pathogens, threatening public access to clean water. This threat involves both direct contamination of faecal bacteria as well as indirect factors, such as disturbed water chemistry and microbiota, which can lead to contamination. While direct contamination has been well described, the impact of indirect factors is less explored, despite the potential of severe downstream consequences on water supply. To assess direct and indirect downstream effects of buildings, farms, pastures and fields on potential water sources, we studied five Swedish lakes and their inflows. We analysed a total of 160 samples in a gradient of anthropogenic activity spanning four time points, including faecal and water-quality indicators. Through species distribution modelling, Random Forest and network analysis using 16S rRNA amplicon sequencing data, our findings highlight that land use indirectly impacts lakes via inflows. Land use impacted approximately one third of inflow microbiota taxa, in turn impacting ∌20–50 % of lake taxa. Indirect effects via inflows were also suggested by causal links between e.g. water colour and lake bacterial taxa, where this influenced the abundance of several freshwater bacteria, such as Polynucleobacter and Limnohabitans. However, it was not possible to identify direct effects on the lakes based on analysis of physiochemical- or microbial parameters. To avoid potential downstream consequences on water supply, it is thus important to consider possible indirect effects from upstream land use and inflows, even when no direct effects can be observed on lakes. Legionella (a genus containing bacterial pathogens) illustrated potential consequences, since the genus was particularly abundant in inflows and was shown to increase by the presence of pastures, fields, and farms. The approach presented here could be used to assess the suitability of lakes as alternative raw water sources or help to mitigate contaminations in important water catchments. Continued broad investigations of stressors on the microbial network can identify indirect effects, avoid enrichment of pathogens, and help secure water accessibility.

    • För att kunna hantera framtida klimatrelaterade händelser behöver samhällets beredskap anpassas och sannolikt utökas. Kunskapsläget över vilken förmåga som behövs för att hantera risker i ett förändrat klimat behöver sammanställas och framtida behov behöver jämföras mot nuvarande beredskapsnivåer för att identifiera lämpliga åtgärder. En analys behöver göras kring eventuella brister, utifrån vilken åtgärder sedan kan prioriteras och vidtas avseende olika delar i förmågan att hantera händelser, bland annat avseende tillhandahållande av statliga förstärkningsresurser. Denna studie analyserar vilken systematik som kan vara aktuell för att möjliggöra sådana analyser och utredningar.Studiens övergripande syfte är att ge förslag kring hur bedömningar kan göras om framtidens behov av statliga förstärkningsresurser i syfte att möta framtidens klimatrelaterade risker. Detta inkluderar en sammanställning av kunskapsläget kring risker och scenarion avseende framtida naturhändelser utifrån ett klimatförändringsperspektiv, med information om var mer kunskap kan hittas. Det inkluderar också att titta på metoder för att dels analysera behov av förmåga på olika nivåer att hantera de klimatrelaterade riskerna och dels hur dimensionering av förmåga kan göras för att möta behovet.Studien består av två delar. I del 1 ges en sammanställning av kunskapsläget om riskbilder och scenarion i Sverige vad gäller framtida naturhändelser utifrån ett klimatförändringsperspektiv och tidsperioden fram till ca år 2100. I del 2 presenteras initialt ett konceptuellt ramverk för kartläggning av beredskap uppdelat i fyra huvuddomäner: Behov, Resurs, Styrning och Beroenden. Baserat på ramverket presenteras därefter en metodik för att analysera samhällets förmåga att hantera framtida händelser i ett förändrat klimat. För att kunna identifiera en lämplig framtida beredskapsnivå, behöver samhällets förmåga att hantera händelser gentemot en framtida riskbild analyseras. Del 2 fokuserar på hur relevanta myndigheter ska kunna gå tillväga för att göra en sådan analys som slutligen kan utgöra beslutsunderlag avseende framtida beredskap. Avslutningsvis ges ett exempel på hur metoden kan användas där händelsen skogsbrand, av omfattningen att nationella förstärkningsresurser i form av MSBs upphandlade helikoptrar och flygplan för skogsbrandbekämpning kan bli aktuella att nyttja, utgör själva händelsen.

  • Peptide receptor radionuclide therapy in gastroenteropancreatic NEN G3 : a multicenter cohort study

    • Peptide receptor radionuclide therapy (PRRT) is an established treatment of metastatic neuroendocrine tumors grade 1-2 (G1-G2). However, its possible benefit in high-grade gastroenteropancreatic (GEP) neuroendocrine neoplasms (NEN G3) is largely unknown. We therefore aimed to assess the benefits and side effects of PRRT in patients with GEP NEN G3. We performed a retrospective cohort study at 12 centers to assess the efficacy and toxicity of PRRT in patients with GEP NEN G3. Outcomes were response rate, disease control rate, progression-free survival (PFS), overall survival (OS) and toxicity. We included 149 patients (primary tumor: pancreatic n = 89, gastrointestinal n = 34, unknown n = 26). PRRT was first-line (n = 30), second-line (n = 62) or later-line treatment (n = 57). Of 114 patients evaluated, 1% had complete response, 41% partial response, 38% stable disease and 20% progressive disease. Of 104 patients with documented progressive disease before PRRT, disease control rate was 69%. The total cohort had median PFS of 14 months and OS of 29 months. Ki-67 21-54% (n = 125) vs Ki-67 >= 55% (n = 23): PFS 16 vs 6 months (P < 0.001) and OS 31 vs 9 months (P < 0.001). Well (n = 60) vs poorly differentiated NEN (n = 62): PFS 19 vs 8 months (P < 0.001) and OS 44 vs 19 months (P < 0.001). Grade 3-4 hematological or renal toxicity occurred in 17% of patients. This large multicenter cohort of patients with GEP NEN G3 treated with PRRT demonstrates promising response rates, disease control rates, PFS and OS as well as toxicity in patients with mainly progressive disease. Based on these results, PRRT may be considered for patients with GEP NEN G3.

  • A temporal record of microplastic pollution in Mediterranean seagrass soils

    • Plastic pollution is emerging as a potential threat to the marine environment. In the current study, we selected seagrass meadows, known to efficiently trap organic and inorganic particles, to investigate the concentrations and dynamics of microplastics in their soil. We assessed microplastic contamination and accumulation in 210Pb dated soil cores collected in Posidonia oceanica meadows at three locations along the Spanish Mediterranean coast, with two sites located in the Almería region (Agua Amarga and Roquetas) and one at Cabrera Island (Santa Maria). Almería is known for its intense agricultural industry with 30 000 ha of plastic-covered greenhouses, while the Cabrera Island is situated far from urban areas. Microplastics were extracted using enzymatic digestion and density separation. The particles were characterized by visual identification and with Fourier-transformed infrared (FTIR) spectroscopy, and related to soil age-depth chronologies. Our findings showed that the microplastic contamination and accumulation was negligible until the mid-1970s, after which plastic particles increased dramatically, with the highest concentrations of microplastic particles (MPP) found in the recent (since 2012) surface soil of Agua Amarga (3819 MPP kg-1), followed by the top-most layers of the soil of the meadows in Roquetas (2173 kg-1) and Santa Maria (68-362 kg-1). The highest accumulation rate was seen in the Roquetas site (8832 MPP m-2 yr-1). The increase in microplastics in the seagrass soil was associated to land-use change following the intensification of the agricultural industry in the area, with a clear relationship between the development of the greenhouse industry in Almería and the concentration of microplastics in the historical soil record. This study shows a direct linkage between intense anthropogenic activity, an extensive use of plastics and high plastic contamination in coastal marine ecosystems such as seagrass meadows. We highlight the need of proper waste management to protect the coastal environment from continuous pollution.

  • Cutting Edge Bionics in Highly Impaired Individuals: A Case of Challenges and Opportunities

    • Highly impaired individuals stand to benefit greatly from cutting-edge bionic technology, however concurrent functional deficits may complicate the adaptation of such technology. Here, we present a case in which a visually impaired individual with bilateral burn injury amputation was provided with a novel transradial neuromusculoskeletal prosthesis comprising skeletal attachment via osseointegration and implanted electrodes in nerves and muscles for control and sensory feedback. Difficulties maintaining implant hygiene and donning and doffing the prosthesis arose due to his contralateral amputation, ipsilateral eye loss, and contralateral impaired vision necessitating continuous adaptations to the electromechanical interface. Despite these setbacks, the participant still demonstrated improvements in functional outcomes and the ability to control the prosthesis in various limb positions using the implanted electrodes. Our results demonstrate the importance of a multidisciplinary, iterative, and patient-centered approach to making cutting-edge technology accessible to patients with high levels of impairment.

    • Nordic nursing studies have a strong tradition within the qualitative approach. This approach has been applied to explore the lived experience of critical illness and intensive care therapy from the perspectives of the patient and the patient’s family. In 2003, the Nordic Association for Intensive Care Nursing Research (NOFI) was established by nursing scholars from Denmark, Norway, and Sweden. The purpose of NOFI was to encourage research in intensive care nursing in the Nordic countries, to establish a network of intensive care nursing scholars, to exchange research experience and research outcomes among the Nordic research communities, and to increase the visibility of research in intensive care nursing in the Nordic countries. Although the focus was on nursing research, an interdisciplinary approach has also been encouraged with joint research and conference activities. NOFI have arranged biannual conferences for members and other individuals interested in the activities of the network. The venue has rotated among the tree Nordic countries, and the themes of the meetings have been: Intensive Care Unit (ICU) environment and acoustics, relatives of ICU-patients, ways of knowing, sedation, ethics, patient diaries, multimodal interventions, research designs, ICU-delirium, psychometrics, burns, and mechanical ventilation. Other activities have been presentations of newly completed PhD-studies in the Nordic countries. Since 2006, nursing scholars from the three Scandinavian countries have collaborated on a study of diaries written for ICU patients (patient diaries). The three countries share common values, culture and language, which has facilitated the exchange of ideas among the Nordic nurses. Intensive care nursing is a young domain of research. We still need to encourage more scholars within this area to increase our knowledge base, improve the ICU-experience and formalize rehabilitation of post-ICU patients. Intensive care is a collaborative practice and future research should reflect the interdisciplinary aspect of the field.
