    • In the current paper it is shown that tensile and compression strength of sheets from hardwood high yield pulps (HYP), which has been manufactured at a low energy input in refining, can be multiplied by improving the fibre-fibre bond strength and the number of contact areas between fibre surfaces in the sheet structure. At fibre surface treatment with cationic starch the properties are improved at almost preserved bulk (i.e., preserved density). Remarkable improvements are achieved at densification of the sheet structures by pressing in a hot press nip at temperatures well above the softening temperature of wet lignin. The former technique is above all to prefer in manufacturing of middle plies in paperboard composites, whereas the latter technique is of special interest in manufacturing of packaging papers with high demands upon strength. For instance, it is shown in the paper how tensile index on sheets of CTMP from birch (CSF 480 ml) can be enhanced from 20 to 50 kNm/kg and compression index (SCT) from 10 to 35 kNm/kg by the tested techniques.

  • Determination of the glycaemic index of foods: interlaboratory study.

    • Objective: Practical use of the glycaemic index (GI), as recommended by the FAO/WHO, requires an evaluation of the recommended method. Our purpose was to determine the magnitude and sources of variation of the GI values obtained by experienced investigators in different international centres. Design: GI values of four centrally provided foods (instant potato, rice, spaghetti and barley) and locally obtained white bread were determined in 8-12 subjects in each of seven centres using the method recommended by FAO/WHO. Data analysis was performed centrally. Setting: University departments of nutrition. Healthy subjects (28 male, 40 female) were studied. Results: The GI values of the five foods did not vary significantly in different centres nor was there a significant centre´food interaction. Within-subject variation from two centres using venous blood was twice that from five centres using capillary blood. The s.d. of centre mean GI values was reduced from 10.6 (range 6.8-12.8) to 9.0 (range 4.8-12.6) by excluding venous blood data. GI values were not significantly related to differences in method of glucose measurement or subject characteristics (age, sex, BMI, ethnicity or absolute glycaemic response). GI values for locally obtained bread were no more variable than those for centrally provided foods. Conclusions: The GI values of foods are more precisely determined using capillary than venous blood sampling, with mean between-laboratory s.d. of approximately 9.0. Finding ways to reduce within-subject variation of glycaemic responses may be the most effective strategy to improve the precision of measurement of GI values.

  • Mechanical circulatory support with the Impella 5.0 device for postcardiotomy cardiogenic shock: a three-center experience

    • AIM:Postcardiotomy cardiogenic shock (PCCS) is associated with high mortality rates, despite full conventional treatment. Although the results of treatment with surgically implantable ventricular assist devices have been encouraging, the invasiveness of this treatment limits its applicability. Several less invasive devices have been developed, including the Impella system. The objective of this study was to describe our three-center experience with the Impella 5.0 device in the setting of PCCS.METHODS:From January 2004 through December 2010, a total of 46 patients were diagnosed with treatment-refractory PCCS and treated with the Impella 5.0 percutaneous left ventricular assist device at three european heart centers. Baseline and follow-up characteristics were collected retrospectively and entered into a dedicated database.RESULTS:Within the study cohort of 46 patients, mean logistic and additive EuroSCORES were 24 ± 19 and 10 ± 4. The majority of patients underwent coronary artery bypass grafting (48%) or combined surgery (33%). Half of all patients had been treated with an intra-aortic balloon pump before 5.0-implantation, 1 patient had been treated with an Impella 2.5 device. All patients were on mechanical ventilation and intravenous inotropes. The Kaplan-Meier estimate of overall 30-day survival was 39.5%.CONCLUSION:Thirty-day survival rates for patients with PCCS, refractory to aggressive conventional treatment and treated with the Impella 5.0 device, are comparable to those reported in studies evaluating surgically implantable VADs, whereas the Impella system is much less invasive. Therefore, mechanical circulatory support with the Impella 5.0 device is a suitable treatment modality for patients with severe PCCS.

  • Decreasing mortality and changes in treatment patterns in patients with acromegaly from a nationwide study

    • Context: New therapeutic strategies have developed for the management of acromegaly over recent decades. Whether this has improved mortality has not been fully elucidated. Objective: The primary aim was to investigate mortality in a nationwide unselected cohort of patients with acromegaly. Secondary analyses included time trends in mortality and treatment patterns. Design: A total of 1089 patients with acromegaly were identified in Swedish National Health Registries between 1987 and 2013. To analyse time trends, the cohort was divided into three periods (1987–1995, 1996–2004 and 2005–2013) based on the year of diagnosis. Main outcome measures: Using the Swedish population as reference, standardized mortality ratios (SMRs) were calculated with 95% confidence intervals (CIs). Results: Overall SMR was 2.79 (95% CI: 2.43–3.15) with 232 observed and 83 expected deaths. Mortality was mainly related to circulatory diseases (SMR: 2.95, 95% CI: 2.35–3.55), including ischemic heart disease (2.00, 1.35–2.66) and cerebrovascular disease (3.99, 2.42–5.55) and malignancy (1.76, 1.27–2.26). Mortality decreased over time, with an SMR of 3.45 (2.87–4.02) and 1.86 (1.04–2.67) during the first and last time period, respectively (P=.015). During the same time periods, the frequency of pituitary surgery increased from 58% to 72% (P<0.001) and the prevalence of hypopituitarism decreased from 41% to 23% (P<0.001). Conclusions: Excess mortality was found in this nationwide cohort of patients with acromegaly, mainly related to circulatory and malignant diseases. Although still high, mortality significantly declined over time. This could be explained by the more frequent use of pituitary surgery, decreased prevalence of hypopituitarism and the availability of new medical treatment options. © 2018 European Society of Endocrinology Printed in Great Britain.

    • To predict the brightness reduction in a chemithermomechanical process as well as the bleachability of pulps produced from different species of wood raw materials there are, at present, no shortcuts available. Pulps have to be manufactured in pilot or mill scale and bleached; evaluating and predicting the bleachability has consequently been very resource consuming and thus expensive - until now! Due to this, we have used a laboratory method based on 60 × 60 mm wood shavings making it possible to predict the bleachability of primarily chemithermomechanical pulps but also other mechanical pulps. Our experiments showed that retention time and impregnation temperature appears to have no impact on the final brightness of birch shavings. When being subjected to a simulated chemithermomechanical pulp process, eucalyptus exhibited an increase in chromophore content before the bleaching stage. In addition, eucalyptus suffered from severe alkaline darkening. After the bleaching stage, the alkaline darkening diminished. A higher retention temperature resulted in lower brightness of the eucalyptus shavings compared to when using a lower retention temperature. Birch reached the highest final brightness levels and had a higher overall yield too.

  • Visualisation of sulphur on single fibre level for pulping industry

    • In the pulp and paper industry, about 5 Mt/y chemithermomechanical pulp (CTMP) are produced globally from softwood chips for production of carton board grades. For tailor making CTMP for this purpose, wood chips are impregnated with aqueous sodium sulphite for sulphonation of the wood lignin. When lignin is sulphonated, the defibration of wood into pulp becomes more selective, resulting in enhanced pulp properties. On a microscopic fibre scale, however, one could strongly assume that the sulphonation of the wood structure is very uneven due to its macroscale size of wood chips. If this is the case and the sulphonation could be done significantly more evenly, the CTMP process could be more efficient and produce pulp even better suited for carton boards. Therefore, the present study aimed to develop a technique based on X-ray fluorescence microscopy imaging (µXRF) for quantifying the sulphur distribution on CTMP wood fibres. Firstly, the feasibility of µXRF imaging for sulphur homogeneity measurements in wood fibres needs investigation. Therefore, clarification of which spatial and spectral resolution that allows visualization of sulphur impregnation into single wood fibres is needed. Measurements of single fibre imaging were carried out at the Argonne National Laboratory’s Advanced Photon Source (APS) synchrotron facility. With a synchrotron beam using one micrometre scanning step, images of elemental mapping are acquired from CTMP samples diluted with non-sulphonated pulp under specified conditions. Since the measurements show significant differences between sulphonated and non-sulphonated fibres, and a significant peak concentration in the shell of the sulphonated fibres, the proposed technique is found to be feasible. The required spatial resolution of the µXRF imaging for an on-site CTMP sulphur homogeneity measurement setup is about 15 µm, and the homogeneity measured along the fibre shells is suggested to be used as the CTMP sulphonation measurement parameter.

    • Minimizing the fiber property distribution would have the potential to improve the pulp properties and the process efficiency of chemimechanical pulp. To achieve this, it is essential to improve the level of knowledge of how evenly distributed the sulfonate concentration is between the individual chemimechanical pulp fibers. Due to the variation in quality between pulpwood and sawmill chips, as well as the on-chip screening method, it is difficult to develop an impregnation system that ensures the even distribution of sodium sulfite (Na2SO3) impregnation liquid. It is, therefore, crucial to measure the distribution of sulfonate groups within wood chips and fibers on a microscale. Typically, the degree of unevenness, i.e., the amount of fiber sulfonation and softening prior to defibration, is unknown on a microlevel due to excessively robust or complex processing methods. The degree of sulfonation at the fiber level can be determined by measuring the distribution of elemental sulfur and counterions of sulfonate groups, such as sodium or calcium. A miniaturized energy-dispersive X-ray fluorescence (ED-XRF) method has been developed to address this issue, enabling the analysis of sulfur distributions. It is effective enough to be applied to industrial laboratories for further development, i.e., improved image resolution and measurement time.
