    • Denna rapport är en del av den avslutande rapporteringen av projektet Trestad2. Trestad2 är en av Socialdepartementet finansierad satsning för att i landets tre storstadskommuner arbeta fram insatser och åtgärder för att minska användandet av cannabis och ungdomar och unga vuxna. De tre städerna har på olika sätt själva eller i samarbete tillskapat en mängd olika projekt för att upptäcka och förebygga användande av cannabis samt behandla de som drabbats. En oberoende forskargrupp har utsetts av de tre städerna att utvärdera arbetet som utförts under projektets gång från 2012 till 2015 och därefter rapportera resultaten till Socialdepartementet. Denna rapport är alltså en del av denna rapportering. Forskargruppen har varit med från projektets start och har löpande tagit del av hur arbetet i projektet utvecklats. Utvärderingen har fokuserat på att undersöka hur väl projektet lyckats uppnå sina målsättningar. I huvudsak har arbetsmetoden varit att granska projektdokument från de olika delprojekten i Trestad2, men forskargruppen har också genomfört intervjuer med nyckelpersoner i de tre städerna. Denna rapport består av tre olika delar. I det första avsnittet finns en redogörelse för den projektansökan som ligger till grund för tillkomsten av Trestad2 och det görs en samlad bedömning huruvida projektet som helhet haft en inriktning i linje med de mål som formulerades från början. I detta avsnitt finns också en redogörelse för hur städerna organiserat sig för att klara av att nå projektets målsättningar och likheter och skillnader lyfts fram samt de olika lokala förutsättningar städerna har haft för att jobba med projektet. I nästa avsnitt följer en genomgång av de skilda delprojekten i Trestad2. Där beskrivs delprojektens innehåll och genomförande och det görs en bedömning av projektens utfall i olika relevanta avseenden. I det sista avsnittet diskuteras vad forskargruppen anser att projektet lett fram till. Där lyfts fram vilka erfarenheter som gjorts, vad som lyckats särskilt väl och de svårigheter som funnits. I rapportens slut finns en förteckning av publicerade rapporter inom Trestad2 för vidare läsning.

Halvtidsdokumentationen avser perioden från januari 2012 till september 2013 och har tillkommit på initiativ av den övergripande forskargruppen för att dokumentera storstadssatsningens förlopp samt för att skapa ett underlag som kan användas såväl under den återstående projekttiden, som vid slutrapportering av hela storstadssatsningen. Rapporten presenterades på ett seminarium med de tre medverkande städerna i Malmö den 3 februari 2014.

  • A web-based group course intervention for 15-25-year-olds whose parents have substance use problems or mental illness : Study protocol for a randomized controlled trial

    • Background: Depending on the definitions used, between 5 and 20 % of all Swedish children grow up with at least one parent suffering from alcohol problems, while 6 % have at least one parent who has received inpatient psychiatric care, conditions that may affect the children negatively. Nine out of ten Swedish municipalities therefore provide support resources, but less than 2 % of these children are reached by such support. Delivering intervention programs via the Internet is a promising strategy. However, web-based programs targeting this at-risk group of children are scarce. We have previously developed a 1.5-h-long web-based self-help program, Alcohol & Coping, which appears to be effective with regards to adolescents' own alcohol consumption. However, there is a need for a more intense program, and therefore we adapted Kopstoring, a comprehensive Dutch web-based psycho-educative prevention program, to fit the Swedish context. The purpose of the program, which in Swedish has been called Grubbel, is to strengthen protective factors, such as coping skills and psychological well-being, prevent the development of psychological disorders, and reduce alcohol consumption. Methods/design: The aim of the current study is to evaluate the effectiveness of Grubbel, which targets 15-25-year-olds whose parents have substance use problems and/or mental illness. Specific research questions relate to the participants' own coping strategies, mental health status and substance use. The study was initiated in the spring of 2016 and uses a two-armed RCT design. Participants will be recruited via social media and also through existing agencies that provide support to this target group. The assessment will consist of a baseline measurement (t0) and three follow-ups after six (t1), 12 (t2), and 24 months (t3). Measures include YSR, CES-DC, Ladder of Life, Brief COPE, AUDIT-C, and WHOQOL-BREF. Discussion: Studies have revealed that the majority of children whose parents have substance use or mental health problems are not reached by the existing support. Thus, there is an urgent need to develop, implement, and evaluate novel intervention programs and disseminate successful programs to a broader audience. This study, investigating the effects of a web-based intervention, therefore makes an important contribution to this field of research. Trial registration: ISRCTN10099247. Retrospectively registered on August 31, 2016.

  • Drug use at licensed premises : prevalence and prevention

    • Background: There are a number of public health concerns related to the use of illicit drugs such as violence, injuries, unprotected sex, and drug dependence. Particularly, there is a strong need to reach adolescents and young adults with drug prevention programs. It is therefore necessary to identify new settings in the community to target young people. The nightlife setting is one arena where many young people socialize, where illicit drugs are available, and where young people are introduced to and start experimenting with club drugs, frequently under the influence of alcohol. However, little is known about the prevalence of club drug use in Sweden. Furthermore, club drug prevention at licensed premises is a new field for intervention research. Objective: The overall aim of this research is to increase knowledge on the prevalence and prevention of illicit club drug use at licensed premises. The thesis investigates methods to measure the prevalence of club drug use and the potential for community- based club drug prevention at licensed premises. Method: This thesis is built around four articles (articles I to IV). Two studies (I and IV) were conducted to measure the prevalence of drug use. Different methodologies (self-report and biological markers) and study populations (staff at licensed premises, and electronic music dance event attendees) were used. To explore the possibilities of conducting community-based club drug prevention work such a program was developed and implemented based on a systems approach to prevention. The multi- component program included community mobilization, drug-training, policy work, increased enforcement, environmental changes, and media advocacy and public relations work. In articles II and III, the effects of this program on doormen intervention rates towards drug-intoxicated guests were evaluated using a pseudopatron methodology with a pretest – posttest (longitudinal repeated measures) design. Results: Drug use among staff and guests at licensed premises was found to be high. Evaluations of the long-term effects of the community-based intervention showed a significant increase in doormen intervention rates towards drug-intoxicated guests. Also, staff reported significantly lower levels of self-reported drug use as well as significantly lower levels of observed drug use among guests at the second measurement. Further, our results indicated that it was feasible to conduct alcohol breath and oral fluid drug testing in a Swedish club setting. Discussion and conclusion: Increased doormen interventions, reduced rates of drug use among staff, and reduced observed drug use among guests suggest positive outcomes of the community-based intervention, as no other explanations for these improvements have been found. The findings demonstrate that the systems model for prevention of substance use was an appropriate model to guide club drug prevention efforts. We successfully used the model to select our intervention methods and implementation strategies.

  • Risky drinking cultures among affluent youth in Sweden

    • There is a growing scientific interest in drinking behavior among young people in affluent areas, who report higher levels of alcohol consumption compared to youth in less privileged areas. This phenomenon has been observed in several Western countries. The research has been dominated by variable-oriented analyses and has presented interesting explanations, but there has been little research into these young people's own experiences of and attitudes toward alcohol consumption. To develop interventions targeting this group, we need to understand their lifeworld. This study aims to develop an in-depth understanding of the high alcohol consumption among young people in affluent areas and how they themselves experience it. In the spring of 2019, we conducted 20 in-depth interviews with adolescents in upper secondary school (aged 15–19) in one of the most affluent area in Sweden. The empirical material was analyzed thematically. Theoretically, the phenomenon is understood by relating to social identity processes and considering the group's material, social and cultural means through Bourdieu's metaphors of capital. We found that affluent youth link their social identities to alcohol consumption. Alcohol is a social beverage that opens social networks and contributes to a sense of community. The consumption of alcohol gives experience capital leading to status in this context, with clear norms and expectations governing alcohol consumption. Parties are arranged in protected spaces where young people are free to drink out of the adults' sight. Affluent youths also have considerable purchasing power which contributes to drinking, and they are socialized into a pre-existing adult alcohol culture characterized by a liberal view on alcohol. Finally, when alcohol consumption escalates, the youths perceive that it is difficult to get adequate help from the adult world. The findings are important for future preventive interventions for subgroups of adolescents at high risk for heavy drinking.

  • Effects of an automated digital brief prevention intervention targeting adolescents and young adults with risky alcohol and other substance use : study protocol for a randomised controlled trial

    • Adolescence and young adulthood is a period in life when individuals may be especially vulnerable to harmful substance use. Several critical developmental processes are occurring in the brain, and substance use poses both short-term and long-term risks with regard to mental health and social development. From a public health perspective, it is important to prevent or delay substance use to reduce individual risk and societal costs. Given the scarcity of effective interventions targeting substance use among adolescents and young adults, cost-effective and easily disseminated interventions are warranted. The current study will test the effectiveness of a fully automated digital brief intervention aimed at reducing alcohol and other substance use in adolescents and young adults aged 15 to 25 years. Methods and analysis A two-arm, double-blind, randomised controlled trial design is applied to assess the effectiveness of the intervention. Baseline assessment, as well as 3-month and 6-month follow-up, will be carried out. The aim is to include 800 participants with risky substance use based on the screening tool CRAFFT (Car, Relax, Alone, Forget, Friends, Trouble). Recruitment, informed consent, randomisation, intervention and follow-up will be implemented online. The primary outcome is reduction in alcohol use, measured by Alcohol Use Disorders Identification Test total score. Secondary outcomes concern binge drinking, frequency of alcohol consumption, amount of alcohol consumed a typical day when alcohol is consumed, average daily drinks per typical week, other substance use, mental health, sexual risk behaviours and perceived peer pressure. Moreover, the study involves analyses of potential moderators including perfectionism, openness to parents, help-seeking and background variables. Ethics and dissemination The study was approved by the Swedish Ethical Review Authority (no. 2019-03249). The trial is expected to expand the knowledge on digital preventive interventions for substance using adolescents and young adults. Results will be disseminated in research journals, at conferences and via the media.

  • How to prevent alcohol and illicit drug use among students in affluent areas : a qualitative study on motivation and attitudes towards prevention

    • Background: The use of alcohol and illicit drugs during adolescence can lead to serious short- and long-term health related consequences. Despite a global trend of decreased substance use, in particular alcohol, among adolescents, evidence suggests excessive use of substances by young people in socioeconomically affluent areas. To prevent substance use-related harm, we need in-depth knowledge about the reasons for substance use in this group and how they perceive various prevention interventions. The aim of the current study was to explore motives for using or abstaining from using substances among students in affluent areas as well as their attitudes to, and suggestions for, substance use prevention. Methods: Twenty high school students (age 15-19 years) in a Swedish affluent municipality were recruited through purposive sampling to take part in semi-structured interviews. Qualitative content analysis of transcribed interviews was performed. Results: The most prominent motive for substance use appears to be a desire to feel a part of the social milieu and to have high social status within the peer group. Motives for abstaining included academic ambitions, activities requiring sobriety and parental influence. Students reported universal information-based prevention to be irrelevant and hesitation to use selective prevention interventions due to fear of being reported to authorities. Suggested universal prevention concerned reliable information from credible sources, stricter substance control measures for those providing substances, parental involvement, and social leisure activities without substance use. Suggested selective prevention included guaranteed confidentiality and non-judging encounters when seeking help. Conclusions: Future research on substance use prevention targeting students in affluent areas should take into account the social milieu and with advantage pay attention to students' suggestions on credible prevention information, stricter control measures for substance providers, parental involvement, substance-free leisure, and confidential ways to seek help with a non-judging approach from adults.
