  • The effect of dosing strategies on the therapeutic efficacy of artesunate-amodiaquine for uncomplicated malaria : a meta-analysis of individual patient data

    • Background: Artesunate-amodiaquine (AS-AQ) is one of the most widely used artemisinin-based combination therapies (ACTs) to treat uncomplicated Plasmodium falciparum malaria in Africa. We investigated the impact of different dosing strategies on the efficacy of this combination for the treatment of falciparum malaria. Methods: Individual patient data from AS-AQ clinical trials were pooled using the WorldWide Antimalarial Resistance Network (WWARN) standardised methodology. Risk factors for treatment failure were identified using a Cox regression model with shared frailty across study sites. Results: Forty-three studies representing 9,106 treatments from 1999-2012 were included in the analysis; 4,138 (45.4%) treatments were with a fixed dose combination with an AQ target dose of 30 mg/kg (FDC), 1,293 (14.2%) with a non-fixed dose combination with an AQ target dose of 25 mg/kg (loose NFDC-25), 2,418 (26.6%) with a non-fixed dose combination with an AQ target dose of 30 mg/kg (loose NFDC-30), and the remaining 1,257 (13.8%) with a co-blistered non-fixed dose combination with an AQ target dose of 30 mg/kg (co-blistered NFDC). The median dose of AQ administered was 32.1 mg/kg [IQR: 25.9-38.2], the highest dose being administered to patients treated with co-blistered NFDC (median = 35.3 mg/kg [IQR: 30.6-43.7]) and the lowest to those treated with loose NFDC-25 (median = 25.0 mg/kg [IQR: 22.7-25.0]). Patients treated with FDC received a median dose of 32.4 mg/kg [IQR: 27-39.0]. After adjusting for reinfections, the corrected antimalarial efficacy on day 28 after treatment was similar for co-blistered NFDC (97.9% [95% confidence interval (CI): 97.0-98.8%]) and FDC (98.1% [95% CI: 97.6%-98.5%]; P = 0.799), but significantly lower for the loose NFDC-25 (93.4% [95% CI: 91.9%-94.9%]), and loose NFDC-30 (95.0% [95% CI: 94.1%-95.9%]) (P < 0.001 for all comparisons). After controlling for age, AQ dose, baseline parasitemia and region; treatment with loose NFDC-25 was associated with a 3.5-fold greater risk of recrudescence by day 28 (adjusted hazard ratio, AHR = 3.51 [95% CI: 2.02-6.12], P < 0.001) compared to FDC, and treatment with loose NFDC-30 was associated with a higher risk of recrudescence at only three sites. Conclusions: There was substantial variation in the total dose of amodiaquine administered in different AS-AQ combination regimens. Fixed dose AS-AQ combinations ensure optimal dosing and provide higher antimalarial treatment efficacy than the loose individual tablets in all age categories.

    • This study is based on the analysis of field-measured nitrous oxide (N2 O) emissions from a Sahelian semi-arid grassland site in Senegal (Dahra), tropical humid agricultural plots in Kenya (Mbita region) and simulations using a 1D model designed for semi arid ecosystems in Dahra. This study aims at improving present knowledge and inventories of N2 O emissions from the African continent. N2 O emissions were larger at the agricultural sites in the Mbita region (range: 0.0 ± 0.0 to 42.1 ± 10.7 ngN m−2 s−1) than at the Dahra site (range: 0.3 ± 0 to 7.4 ± 6.5 ngN m−2 s−1). Soil water and nitrate (NO3−) contents appeared to be the most important drivers of N2 O emissions in Dahra at the seasonal scale in both regions. The seasonal pattern of modelled N2 O emissions is well represented, though the model performed better during the rainy season than between the rainy and dry seasons. This study highlighted that the water-filled pore space threshold recognised as a trigger for N2 O emissions should be reconsidered for semi-arid ecosystems. Based on both measurements and simulated results, an annual N2 O budget was estimated for African savanna/grassland and agricultural land ranging between 0.17–0.26 and 1.15–1.20 TgN per year, respectively.

  • Diel, seasonal, and inter-annual variation in carbon dioxide effluxes from lakes and reservoirs

    • Accounting for temporal changes in carbon dioxide (CO2) effluxes from freshwaters remains a challenge for global and regional carbon budgets. Here, we synthesize 171 site-months of flux measurements of CO2 based on the eddy covariance method from 13 lakes and reservoirs in the Northern Hemisphere, and quantify dynamics at multiple temporal scales. We found pronounced sub-annual variability in CO2 flux at all sites. By accounting for diel variation, only 11% of site-months were net daily sinks of CO2. Annual CO2 emissions had an average of 25% (range 3%-58%) interannual variation. Similar to studies on streams, nighttime emissions regularly exceeded daytime emissions. Biophysical regulations of CO2 flux variability were delineated through mutual information analysis. Sample analysis of CO2 fluxes indicate the importance of continuous measurements. Better characterization of short- and long-term variability is necessary to understand and improve detection of temporal changes of CO2 fluxes in response to natural and anthropogenic drivers. Our results indicate that existing global lake carbon budgets relying primarily on daytime measurements yield underestimates of net emissions.

    • Plant traits-the morphological, anatomical, physiological, biochemical and phenological characteristics of plants-determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait-based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits-almost complete coverage for 'plant growth form'. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait-environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements. This can only be achieved in collaboration with other initiatives.

  • Greenhouse gas emission from tropical reservoirs : Spatial and temporal dynamics

    • The emission of methane (CH4) and carbon dioxide (CO2) from reservoirs has been estimated to make up for about 1.3% of the global anthropogenic greenhouse gas emission. The impoundment of a river leads to the accumulation of sediment that is brought in from inflowing rivers, and the sediment organic matter is degraded to CH4 and CO2. CH4 is of particular concern as its global warming potential is 34 times stronger than that of CO2. In the tropics, high temperatures and high availability of fresh organic matter from high net primary production fuel CH4 and CO2 production. As the construction of hydropower plants is currently undergoing a boom, especially in the tropics, reservoir emission is probably bound to increase.The emission of CH4 and CO2 from reservoir surfaces is, however, highly variable, which makes current estimates uncertain. This thesis is built on the hypothesis that the spatial and temporal variability of greenhouse gas emission in tropical reservoirs, particularly of CH4 ebullition (the emission via gas bubbles), is so large that the sampling strategy affects whole-system estimates of greenhouse gas emission.This thesis shows that greenhouse gas emission from the four studied tropical reservoirs in Brazil varied greatly at different timescales – over 24 hours, between days and between seasons. Seasonal variability was identified as the most important temporal scale to be covered for CH4 ebullition inventories. In addition, the spatial variability of gas emission was large for all pathways. The variability of CH4 ebullition across space, for example, was estimated to be almost as large as its variability between seasons, and patterns of spatial variability in diffusive CH4 and CO2 emission differed between seasons. For both ebullition and diffusion, river inflow areas were prone to elevated greenhouse gas emission.This thesis shows that for retrieving solid emission estimates, there is no alternative to time-consuming measurements in the field. Measurements should be repeated at least once during each hydrological season (i.e. falling and rising water level). The seasonal surveys should cover space at a high resolution, including areas with and without river inflows, and different water column depths. CH4 ebullition made up for 60–99% of the total CO2-equivalent emission from the whole water surface of the studied reservoirs, with the highest contribution in the most productive reservoir. The most variable greenhouse gas emission pathway is therefore the most important one to be measured at appropriate resolution, particularly in productive reservoirs.

    • BackgroundPlasmodium falciparum malaria is associated with anaemia-related morbidity, attributable to host, parasite and drug factors. We quantified the haematological response following treatment of uncomplicated P. falciparum malaria to identify the factors associated with malarial anaemia.MethodsIndividual patient data from eligible antimalarial efficacy studies of uncomplicated P. falciparum malaria, available through the WorldWide Antimalarial Resistance Network data repository prior to August 2015, were pooled using standardised methodology. The haematological response over time was quantified using a multivariable linear mixed effects model with nonlinear terms for time, and the model was then used to estimate the mean haemoglobin at day of nadir and day 7. Multivariable logistic regression quantified risk factors for moderately severe anaemia (haemoglobin < 7 g/dL) at day 0, day 3 and day 7 as well as a fractional fall >= 25% at day 3 and day 7.ResultsA total of 70,226 patients, recruited into 200 studies between 1991 and 2013, were included in the analysis: 50,859 (72.4%) enrolled in Africa, 18,451 (26.3%) in Asia and 916 (1.3%) in South America. The median haemoglobin concentration at presentation was 9.9 g/dL (range 5.0-19.7 g/dL) in Africa, 11.6 g/dL (range 5.0-20.0 g/dL) in Asia and 12.3 g/dL (range 6.9-17.9 g/dL) in South America. Moderately severe anaemia (Hb < 7g/dl) was present in 8.4% (4284/50,859) of patients from Africa, 3.3% (606/18,451) from Asia and 0.1% (1/916) from South America. The nadir haemoglobin occurred on day 2 post treatment with a mean fall from baseline of 0.57 g/dL in Africa and 1.13 g/dL in Asia. Independent risk factors for moderately severe anaemia on day 7, in both Africa and Asia, included moderately severe anaemia at baseline (adjusted odds ratio (AOR) = 16.10 and AOR = 23.00, respectively), young age (age < 1 compared to >= 12 years AOR = 12.81 and AOR = 6.79, respectively), high parasitaemia (AOR = 1.78 and AOR = 1.58, respectively) and delayed parasite clearance (AOR = 2.44 and AOR = 2.59, respectively). In Asia, patients treated with an artemisinin-based regimen were at significantly greater risk of moderately severe anaemia on day 7 compared to those treated with a non-artemisinin-based regimen (AOR = 2.06 [95%CI 1.39-3.05], p < 0.001).ConclusionsIn patients with uncomplicated P. falciparum malaria, the nadir haemoglobin occurs 2 days after starting treatment. Although artemisinin-based treatments increase the rate of parasite clearance, in Asia they are associated with a greater risk of anaemia during recovery.

    • Freshwater reservoirs are a known source of greenhouse gas (GHG) to the atmosphere, but their quantitative significance is still only loosely constrained. Although part of this uncertainty can be attributed to the difficulties in measuring highly variable fluxes, it is also the result of a lack of a clear accounting methodology, particularly about what constitutes new emissions and potential new sinks. In this paper, we review the main processes involved in the generation of GHG in reservoir systems and propose a simple approach to quantify the reservoir GHG footprint in terms of the net changes in GHG fluxes to the atmosphere induced by damming, that is, ´€˜what the atmosphere sees’. The approach takes into account the pre-impoundment GHG balance of the landscape, the temporal evolution of reservoir GHG emission profile as well as the natural emissions that are displaced to or away from the reservoir site resulting from hydrological and other changes. It also clarifies the portion of the reservoir carbon burial that can potentially be considered an offset to GHG emissions.

    • Emission of greenhouse gases (GHGs) from inland waters is recognized as highly important and an understudied part of the terrestrial carbon (C) biogeochemical cycle. These emissions are still poorly quantified in subarctic regions that contain vast amounts of surface C in permafrost peatlands. This is especially true in NE European peatlands, located within sporadic to discontinuous permafrost zones which are highly vulnerable to thaw. Initial measurements of C emissions from lentic waters of the Bolshezemelskaya Tundra (BZT; 200,000 km2) demonstrated sizable CO2 and CH4 concentrations and fluxes to the atmosphere in 98 depressions, thaw ponds, and thermokarst lakes ranging from 0.5 × 106 to 5 × 106 m2 in size. CO2 fluxes decreased by an order of magnitude as waterbody size increased by > 3 orders of magnitude while CH4 fluxes showed large variability unrelated to lake size. By using a combination of Landsat‐8 and GeoEye‐1 images, we determined lakes cover 4% of BZT and thus calculated overall C emissions from lentic waters to be 3.8 ± 0.65 Tg C yr−1 (99% C‐CO2, 1% C‐CH4), which is two times higher than the lateral riverine export. Large lakes dominated GHG emissions whereas small thaw ponds had a minor contribution to overall water surface area and GHG emissions. These data suggest that, if permafrost thaw in NE Europe results in disappearance of large thermokarst lakes and formation of new small thaw ponds and depressions, GHG emissions from lentic waters in this region may decrease.
