    • PURPOSE Allogeneic hematopoietic cell transplantation (HCT) can cure bone marrow failure in patients with Fanconi anemia (FA). Data on outcomes in patients with pretransplantation cytogenetic abnormalities, myelodysplastic syndrome (MDS), or acute leukemia have not been separately analyzed. PATIENTS AND METHODS We analyzed data on 113 patients with FA with cytogenetic abnormalities (n = 54), MDS (n = 45), or acute leukemia (n = 14) who were reported to the Center for International Blood and Marrow Transplant Research from 1985 to 2007. Results Neutrophil recovery occurred in 78% and 85% of patients at days 28 and 100, respectively. Day 100 cumulative incidences of acute graft-versus-host disease grades B to D and C to D were 26% (95% CI, 19% to 35%) and 12% (95% CI, 7% to 19%), respectively. Survival probabilities at 1, 3, and 5 years were 64% (95% CI, 55% to 73%), 58% (95% CI, 48% to 67%), and 55% (95% CI, 45% to 64%), respectively. In univariate analysis, younger age was associated with superior 5-year survival (≤ v > 14 years: 69% [95% CI, 57% to 80%] v 39% [95% CI, 26% to 53%], respectively; P = .001). In transplantations from HLA-matched related donors (n = 82), younger patients (≤ v > 14 years: 78% [95% CI, 64% to 90%] v 34% [95% CI, 20% to 50%], respectively; P < .001) and patients with cytogenetic abnormalities only versus MDS/acute leukemia (67% [95% CI, 52% to 81%] v 43% [95% CI, 27% to 59%], respectively; P = .03) had superior 5-year survival. CONCLUSION Our analysis indicates that long-term survival for patients with FA with cytogenetic abnormalities, MDS, or acute leukemia is achievable. Younger patients and recipients of HLA-matched related donor transplantations who have cytogenetic abnormalities only have the best survival.

    • Background: Oesophagectomy is associated with reduced appetite, weight loss and postprandial hypoglycaemia, the pathophysiological basis of which remains largely unexplored. This study aimed to investigate changes in enteroendocrine function after oesophagectomy. Methods: In this prospective study, 12 consecutive patients undergoing oesophagectomy were studied before and 10 days, 6, 12 and 52weeks after surgery. Serial plasma total fasting ghrelin, and glucagon-like peptide 1 (GLP-1), insulin and glucose release following a standard 400-kcal mixed-meal stimulus were determined. CT body composition and anthropometry were assessed, and symptom scores calculated using European Organisation for Research and Treatment of Cancer (EORTC) questionnaires. Results: At 1 year, two of the 12 patients exhibited postprandial hypoglycaemia, with reductions in bodyweight (mean(s. e. m.) 17.1(3.2) per cent, P < 0.001), fat mass (21.5(2.5) kg versus 25.5(2.4) kg before surgery; P = 0.014), lean body mass (51.5(2.2) versus 54.0(1.8) kg respectively; P = 0.003) and insulin resistance (HOMA-IR: 0.84(0.17) versus 1.16(0.20); P = 0.022). Mean(s. e. m.) fasting ghrelin levels decreased from postoperative day 10, but had recovered by 1 year (preoperative: 621.5(71.7) pg/ml; 10 days: 415.1(59.80) pg/ml; 6weeks: 309.0(42.0) pg/ml; 12weeks: 415.8(52.1) pg/ml; 52weeks: 547.4(83.2) pg/ml; P< 0.001) and did not predict weight loss (P = 0.198). Postprandial insulin increased progressively at 10 days, 6, 12 and 52weeks (mean(s. e. m.) insulin AUC0-30 min: fold change 1.7(0.4), 2.0(0.4), 3.5(0.7) and 4.0(0.8) respectively; P = 0.001). Postprandial GLP-1 concentration increased from day 10 after surgery (P < 0.001), with a 3.3(1.8)-fold increase at 1 year (P < 0.001). Peak GLP-1 level was inversely associated with the postprandial glucose nadir (P = 0.041) and symptomatic neuroglycopenia (Sigstad score, P = 0.017, R2 = 0.45). GLP-1 AUC predicted loss of weight (P = 0.008, R2 = 0.52) and fat mass (P = 0.010, R2 = 0.64) at 1 year. Conclusion: Altered enteroendocrine physiology is associated with early satiety, weight loss and postprandial hypoglycaemia after oesophagectomy.

    • Objective: To prospectively characterize changes in body weight, satiety, and postprandial gut hormone profiles following esophagectomy. Background: With improved oncologic outcomes in esophageal cancer, there is an increasing focus on functional status and health-related quality of life in survivorship. Early satiety and weight loss are common after esophagectomy, but the pathophysiology of these phenomena remains poorly understood. Methods: In this prospective study, consecutive patients undergoing esophagectomy with gastric conduit reconstruction were studied preoperatively and at 10 days, 6 weeks, and 3 months postoperatively. Glucagon-like peptide 1 (GLP-1) immunoreactivity of plasma collected immediately before and at 15, 30, 60, 90, 120, 150, and 180 minutes after a standardized 400-kcal mixed meal was determined. Gastrointestinal symptom scores were computed using European Organization for Research and Treatment of Cancer questionnaires. Results: Body weight loss at 6 weeks and 3 months postoperatively among 13 patients undergoing esophagectomy was 11.1 +/- 2.3% (P < 0.001) and 16.3 +/- 2.2% (P < 0.0001), respectively. Early satiety (P = 0.043), gastrointestinal pain and discomfort (P = 0.01), altered taste (P = 0.006), and diarrhea (P = 0.038) scores increased at 3 months postoperatively. Area under the curve for the satiety gut hormone GLP-1 was significantly increased from 10 days postoperatively (2.4 +/- 0.2-fold increase, P < 0.01), and GLP-1 peak increased 3.8 +/- 0.6-, 4.7 +/- 0.8-, and 4.4 +/- 0.5-fold at 10 days, 6 weeks, and 3 months postoperatively (all P < 0.0001). Three months postoperatively, GLP-1 area under the curve was associated with early satiety (P = 0.0002, R-2 = 0.74), eating symptoms (P = 0.007, R-2 = 0.54), and trouble enjoying meals (P = 0.0004, R-2 = 0.73). Conclusions: After esophagectomy, patients demonstrate an exaggerated postprandial satiety gut hormone response, which may mediate postoperative changes in satiety, body weight, and gastrointestinal quality of life.

  • SOFIA-EXES Mid-IR Observations of [Fe II] Emission from the Extended Atmosphere of Betelgeuse

    • We present a NASA-DLR SOFIA-Echelon Cross Echelle Spectrograph (EXES) and NASA Infrared Telescope Facility-Texas Echelon Cross Echelle Spectrograph (TEXES) mid-IR R ≃ 50,000 spectral study of forbidden Fe II transitions in the early-type M supergiants, Betelgeuse (α Ori: M2 Iab) and Antares (α Sco: M1 Iab + B3 V). With EXES, we spectrally resolve the ground term [Fe II] 25.99 μm ( a 6DJ= 7/2-9/2: Eup = 540 K) emission from Betelgeuse. We find a small centroid blueshift of 1.9 ± 0.4 km s-1 that is a significant fraction (20%) of the current epoch wind speed, with a FWHM of 14.3 ± 0.1 km s-1. The TEXES observations of [Fe II] 17.94 μm (a 4FJ= - 7/2 9/2: Eup = 3400 K) show a broader FWHM of 19.1 ± 0.2 km s-1, consistent with previous observations, and a small redshift of 1.6 ± 0.6 km s-1 with respect to the adopted stellar center-ofmass velocity of VCoM = 20.9 ± 0.3 km s-1. To produce [Fe II] 25.99 μm blueshifts of 20% wind speed requires that the emission arises closer to the star than existing thermal models for α Ori's circumstellar envelope predict. This implies a more rapid wind cooling to below 500 K within 10R∗ (q∗ = 44 mas, dist = 200 pc) of the star, where the wind has also reached a significant fraction of the maximum wind speed. The line width is consistent with the turbulence in the outflow being close to the hydrogen sound speed. EXES observations of [Fe II] 22.90 μm ( a 4DJ= 5/2-7/2: Eup = 11,700 K) reveal no emission from either star. These findings confirm the dominance of cool plasma in the mixed region where hot chromospheric plasma emits copiously in the UV, and they also constrain the wind heating produced by the poorly understood mechanisms that drive stellar outflows from these low variability and weak-dust signature stars.

    • We first present spatially resolved ALMA and VLA continuum observations of the early-M red supergiant Antares to search for the presence of a chromosphere at radio wavelengths. We resolve the free-free emission of the Antares atmosphere at 11 unique wavelengths between 0.7 mm (ALMA band 8) and 10 cm (VLA S band). The projected angular diameter is found to continually increase with increasing wavelength, from a low of 50.7 mas at 0.7 mm up to a diameter of 431 mas at 10 cm, which corresponds to 1.35 and 11.6 times the photospheric angular diameter, respectively. All four ALMA measurements show that the shape of the atmosphere is elongated, with a flattening of 15% at a similar position angle. The disk-averaged gas temperature of the atmosphere initially rises from a value of 2700 K at 1.35 R∗ (i.e., 0.35 R∗ above the photosphere) to a peak value of 3800 K at ∼2.5 R∗, after which it then more gradually decreases to 1650 K at 11.6 R∗. The rise in gas temperature between 1.35 R∗ and ∼2.5 R∗ is evidence for a chromospheric temperature rise above the photosphere of a red supergiant. We detect a clear change in the spectral index across the sampled wavelength range, with the flux density Sν ν1.42 between 0.7 mm and 1.4 cm, which we associate with chromosphere-dominated emission, while the flux density Sν ν0.8 between 4.3 cm and 10 cm, which we associate with wind-dominated emission. We show that the Antares MOLsphere is transparent at our observed wavelengths, and the lukewarm chromosphere that we detect is therefore real and not just an average of the cool MOLsphere and hot ultraviolet emitting gas. We then perform nonlocal thermal equilibrium modeling of the far-ultraviolet radiation field of another early-M red supergiant, Betelgeuse, and find that an additional hot (i.e., > 7000 K) chromospheric photoionization component with a much smaller filling factor must also exist throughout the chromospheres of these stars.

  • Temporal evolution of the size and temperature of Betelgeuse's extended atmosphere

    • Spatially resolved multi-wavelength centimeter continuum observations of cool evolved stars can not only constrain the morphology of the radio emitting regions, but can also directly probe the mean gas temperature at various depths of the star's extended atmosphere. Here, we use the Very Large Array (VLA) in the A configuration with the Pie Town (PT) Very Long Baseline Array (VLBA) antenna to spatially resolve the extended atmosphere of Betelgeuse over multiple epochs at 0.7, 1.3, 2.0, 3.5, and 6.1 cm. The extended atmosphere deviates from circular symmetry at all wavelengths while at some epochs we find possible evidence for small pockets of gas significantly cooler than the mean global temperature. We find no evidence for the recently reported e-MERLIN radio hotspots in any of our multi-epoch VLA/PT data, despite having sufficient spatial resolution and sensitivity at short wavelengths, and conclude that these radio hotspots are most likely interferometric artefacts. The mean gas temperature of the extended atmosphere has a typical value of 3000 K at 2 R∗ and decreases to 1800 K at 6 R∗, in broad agreement with the findings of the single epoch study from Lim et al. (1998, Nature, 392, 575). The overall temperature profile of the extended atmosphere between 2 R∗ ≲ r ≲ 6 R∗ can be described by a power law of the form Tgas(r) ∝ r-0.6, with temporal variability of a few 100 K evident at some epochs. Finally, we present over 12 yr of V band photometry, part of which overlaps our multi-epoch radio data. We find a correlation between the fractional flux density variability at V band with most radio wavelengths. This correlation is likely due to shock waves induced by stellar pulsations, which heat the inner atmosphere and ionize the more extended atmosphere through radiative means. Stellar pulsations may play an important role in exciting Betelgeuse's extended atmosphere.
