Implicating genes, pleiotropy, and sexual dimorphism at blood lipid loci through multi-ancestry meta-analysis.

- Genetic variants within nearly 1000 loci are known to contribute to modulation of blood lipid levels. However, the biological pathways underlying these associations are frequently unknown, limiting understanding of these findings and hindering downstream translational efforts such as drug target discovery.To expand our understanding of the underlying biological pathways and mechanisms controlling blood lipid levels, we leverage a large multi-ancestry meta-analysis (N=1,654,960) of blood lipids to prioritize putative causal genes for 2286 lipid associations using six gene prediction approaches. Using phenome-wide association (PheWAS) scans, we identify relationships of genetically predicted lipid levels to other diseases and conditions. We confirm known pleiotropic associations with cardiovascular phenotypes and determine novel associations, notably with cholelithiasis risk. We perform sex-stratified GWAS meta-analysis of lipid levels and show that 3-5% of autosomal lipid-associated loci demonstrate sex-biased effects. Finally, we report 21 novel lipid loci identified on the X chromosome. Many of the sex-biased autosomal and X chromosome lipid loci show pleiotropic associations with sex hormones, emphasizing the role of hormone regulation in lipid metabolism.Taken together, our findings provide insights into the biological mechanisms through which associated variants lead to altered lipid levels and potentially cardiovascular disease risk.

Serving the needs of us? An audience analysis of community media

- This paper will present the results of an audience study carried out in Korogocho slum, Nairobi, Kenya, during spring 2009. The study is based on a survey and a series of group interviews with youth living in Korogocho slum. It focuses on the attitudes towards community media and media access and use. Do the young inhabitants of the slum agree that community media belongs to the community and caters to their needs? Has the community radio station any affect on their identity and perception of themselves? Have they changed their media habits once the option of community media arose, or are they still mainly using commercial mainstream media?

Voicing the Slum : Youth, Community Media and Social Change in Nairobi

- Since late 2006, several small media projects have emerged in the slums of Nairobi with the aim to counterbalance the ignorance from mainstream media, provide the slums residents with news, information and an opportunity to voice their needs and discuss relevant issues. These media are best labelled community media, since their main concern is to serve the interests of the community, in this context the slums. The aim of this project is to assess the potential impact community media have on the community in which they operate. Moreover, it considers the role community media play in promoting community development and democracy, especially in relation to young people living in the slums of Nairobi. Through ethnographic fieldwork in Nairobi (January 2007 to April 2010) including interviews with producers and audience, the study not only maps the establishment of the community media landscape in the slums of Nairobi but the advent of community broadcasting. The study reveals that community media and community radio in particular play an import role in the local youth’s identity construction. By promoting a “slum identity” and ascribing to it positive connotations they help the youth strengthening a sense of pride in who they are and where they come from. Moreover, community media and especially community broadcasting provide the audience with information and a platform for debate where the community can interact directly or indirectly with civil society group, local power holders and experts whether in health, law and finance. This can improve the living situations of the audience but also their engagement as citizens. On a macro level, community media’s biggest contribution to social change is their proactive work to combat tribalism by encouraging their audiences to perceive themselves as Kenyans rather than clinging on to identities based on tribal belonging, which is further reflected in their use of Swahili. The political economy of community media is the biggest challenge that prevents the media projects from fully fulfilling their objectives and being a progressive force for social change. The weak financial situation not only affects their output negatively, it makes them dependent on external funding and (mis)use youth as unpaid labour.

Implementing A person-centred CommunicaTION (ACTION) educational intervention for in-home nursing assistants – a study protocol

- Background: In this study, the focus is on how to support the competence development needed for nursing assistants in home care. Home care services for older persons can be challenging concerning the nature of the interpersonal interaction and communication needed to care for and respond to the diverse needs of older people who seek to live well in our communities. This implies a need to offer more person-centred care (PCC) to older persons. However, there is a lack of knowledge on how to develop such competence. We, therefore, developed A Person-centred CommunicaTION (ACTION) programme, which is a web-based educational intervention aimed at supporting competence development for nursing assistants. The research objective is to evaluate the ACTION programme with respect to participants’ responses to and the effect of the intervention. Methods: A multicentre case–control study with pre- and post-assessments was designed. The ACTION programme will be implemented at home care units, in two different geographic areas in Sweden. A total of 300 nursing assistants will be recruited: 150 for the intervention group and 150 for the control group. We will evaluate the impact measures and the process. Pre- and post-assessments will be performed with data collected via a) audio recordings of communication, b) a questionnaire on self-efficacy communication skills, PCC, empathy and job satisfaction, c) user data, evaluation forms, field notes and observations, and d) interviews. The data will be analysed with descriptive and analytic statistics and/or qualitative methods for meanings. Discussion: This study has the potential to contribute to the evidence supporting competence development required to offer person-centred and quality home care to older persons and to meet upcoming needs for flexible and easily accessible competence development.
