    • AIMS: To investigate the medium-term effects of estrogen plus progesterone therapy (EPT) on vascular reactivity, endothelial function and hemodynamic responses in 20 hypertensive postmenopausal women. METHODS: This randomized, double-blind, cross-over, placebo-controlled study investigates the effect of 6 months of EPT (conjugated equine estrogen plus medroxyprogesterone). Blood pressure (office and ambulatory), heart rate and heart rate variability (HRV) were measured at baseline and following EPT/placebo treatment. In eight women, we used a wire-myograph to assess endothelial function and contractile response of subcutaneous arteries to transmural nerve stimulation (TNS) and exogenous noradrenaline. RESULTS: EPT decreased vascular reactivity to cumulative TNS compared with baseline (p<0.01) and placebo (p<0.05). Moreover, EPT diminished sensitivity to exogenous noradrenaline (p<0.05). Although EPT reinforced response to acetylcholine, we observed no difference in maximal relaxation induced by substance P or acetylcholine. EPT did not affect ambulatory blood pressure, heart rate or HRV. CONCLUSIONS: Oral combined medium-term EPT reduces adrenergic reactivity in subcutaneous arteries from treated hypertensive postmenopausal women. EPT might act postjunctionally at the adrenergic vascular receptor level. In the present study, EPT neither reduces sympathetic activity nor increases vagal tone, and thus does not support an effect on the central hemodynamic system.

  • Variables associated with HbA1c and weight reductions when adding liraglutide to multiple daily insulin injections in persons with type 2 diabetes (MDI Liraglutide trial 3)

    • Objective To evaluate variables associated with hemoglobin A1c (HbA1c) and weight reduction when adding liraglutide to persons with type 2 diabetes treated with multiple daily insulin injections (MDI). Research design and methods This was a reanalysis of a previous trial where 124 patients were enrolled in a double-blind, placebo-controlled, multicenter randomized trial carried out over 24 weeks. Predictors for effect on change in HbA1c and weight were analyzed within the treatment group and with concurrent interaction analyses. Correlation analyses for change in HbA1c and weight from baseline to week 24 were made. Results The mean age at baseline was 63.7 years, 64.8% were men, the mean number of insulin injections was 4.4 per day, the mean daily insulin dose was 105 units and the mean HbA1c was 74.5 mmol/mol (9.0%). The mean HbA1c and weight reductions were 12.3 mmol/mol (1.13%; P<0.001) and 3.8 kg (P<0.001) greater in liraglutide than placebo-Treated persons. There was no significant predictor for greater effect on HbA1c that existed in all analyses (univariate, multivariate and interaction analyses against controls). For a greater weight reduction when adding liraglutide, a lower HbA1c level at baseline was a predictor (liraglutide group P=0.002, P=0.020 for liraglutide group vs placebo). During follow-up in the liraglutide group, no significant correlation was found between change in weight and change in HbA1c (r=0.09, P=0.46), whereas a correlation existed between weight and insulin dose reduction (r=0.44, P<0.001). Conclusion Weight reduction becomes greater when adding liraglutide in patients with type 2 diabetes treated with MDI who had a lower HbA1c level compared with those with a higher HbA1c level. There was no correlation between reductions in HbA1c and weight when liraglutide was added, that is, different patient groups responded with HbA1c and weight reductions.     • Deficiency of thiamine (vitamin B1) has been demonstrated in several species in the northern hemisphere and is suggested as a cause for declining populations. European perch from the Baltic Sea show negative temporal trends for several health biomarkers and poor recruitment of unknown cause. In this study, thiamine status of perch liver from the Baltic Sea was studied with emphasis on seasonal variation. During spring the thiamine concentration increased, reached a higher level during the summer and then decreased again during autumn. Despite this variation the thiamine concentration was always sufficient in the perch liver. These results indicate that direct thiamine deficiency is an unlikely explanation for the health effects observed in adult female perch from the Baltic Sea.

    • The aim of this thesis is to contribute to the knowledge of how early childhood literacy can be understood in pre-school and school, focusing on ages 1-8 years. We assume that children are competent and creative and generate meaning in the way they deal with text. Our research question in all the studies is: How should we as teachers in pre-school encourage, react to and contribute to developing early childhood literacy? The theoretical framework of our studies is an example of research inspired by action research. Our didactic starting-point is that children’s learning should be understood from the perspective of the learner. The data is analysed and discussed from the perspective of phenomenography and variation theory. We also apply a sociocultural perspective in order to see learning to express oneself in writing as a communicative process and in the light of social and cultural conditions for children’s knowledge and learning. In two of the empirical studies (I & II), the data were derived from interviews and writing tasks, in one of the studies (III) the data consisted of observations, while in the last study (IV), the data consisted of observations and interviews. The results of the four studies comprising our thesis show that children establish an understanding and an approach to literacy learning at an early age, and that this tends to be stable; in pre-school and school there are traditions and things that are taken for granted with regard to children’s literacy learning that should be rethought in order to realise the goals of the curricula; if pre-school and school fail to collaborate and if the teacher does not take the perspective of the learner/child in learning to express themselves in writing, the individual child will be affected negatively; it is a challenge for teachers in pre-school to create an environment that stimulates early childhood literacy, an environment with rich opportunities for functional literacy; it should be possible to utilise and develop the multimodal opportunities, such as pictures, colours, shapes, design etc. that have been a tradition in pre-school.
