  • The human and mouse repertoire of the adhesion family of G-protein-coupled receptors

    • The adhesion G-protein-coupled receptors (GPCRs) (also termed LN-7TM or EGF-7TM receptors) are membrane-bound proteins with long N-termini containing multiple domains. Here, 2 new human adhesion-GPCRs, termed GPR133 and GPR144, have been found by searches done in the human genome databases. Both GPR133 and GPR144 have a GPS domain in their N-termini, while GPR144 also has a pentraxin domain. The phylogenetic analyses of the 2 new human receptors show that they group together without close relationship to the other adhesion-GPCRs. In addition to the human genes, mouse orthologues to those 2 and 15 other mouse orthologues to human were identified (GPR110, GPR111, GPR112, GPR113, GPR114, GPR115, GPR116, GPR123, GPR124, GPR125, GPR126, GPR128, LEC1, LEC2, and LEC3). Currently the total number of human adhesion-GPCRs is 33. The mouse and human sequences show a clear one-to-one relationship, with the exception of EMR2 and EMR3, which do not seem to have orthologues in mouse. EST expression charts for the entire repertoire of adhesion-GPCRs in human and mouse were established. Over 1600 ESTs were found for these receptors, showing widespread distribution in both central and peripheral tissues. The expression patterns are highly variable between different receptors, indicating that they participate in a number of physiological processes.

  • Senior alert: A quality registry to support a standardized, structured, and systematic preventive care process for older adults

    • The average life expectancy and the proportion of the elderly in the Western countries are increasing. The care processes used for the elderly are known to differ between the care providers in Sweden. Accordingly, the need to develop a system to support the processes in order to attain a standardized, structured, and systematic approach to improve preventive care processes for the elderly has been called for. The County Council of Jönköping developed a national Web-based quality registry, Senior Alert, with a focus on the following areas: falls, pressure ulcers, malnutrition, and oral health. The patients are evaluated using validated risk assessment instruments, and the care is planned, executed, evaluated. The registry supports the users to work with preventive care systematically and in a standardized way and provides feedback to the care providers on their preventive care processes. The registry helps the caregivers fulfill the preventive care according to the best available clinical knowledge and practice. The registry also provides the government and health care politicians with data for setting aims for elderly care. The registry is used in 90% of the municipalities and county councils throughout the country. The total number of risk assessments completed from 2009 to 2014 exceeded 1 000 000.

    • The genes of the major histocompatibility complex (MHC) code for proteins involved in antigen recognition and triggering of the adaptive immune response, and are therefore likely to be under selection from parasites. These selection regimes may vary in space and time. Here we report a strong geographical structure in MHC class II B genes of a migrating bird, the great snipe (Gallinago media). Genetic differentiation in the MHC between two ecologically distinct distributional regions (Scandinavian mountain populations vs. East European lowland populations) was still present after statistically controlling for the effect of selectively neutral variation (microsatellites) using partial Mantel tests. This suggests a role for selection in generating this spatial structure and that it represents local adaptation to different environments. Differentiation between populations within the two regions was negligible. Overall, we found a high number of MHC alleles (50, from 175 individuals). This, together with a tendency for a higher rate of nonsynonymous than synonymous substitutions in the peptide binding sites, and high Tajima's D in certain regions of the gene, suggests a history of balancing selection. MHC variation is often thought to be maintained by some form of balancing selection, but the nature of this selection remains unclear. Our results support the hypothesis that spatial variation in selection regimes contributes to the high polymorphism.

  • Dynamic Coupling of Computational Fluid Dynamics and Thermodynamics Software : Applied on a Top Blown Converter

    • A novel modeling approach is presented where a computational fluid dynamics software is coupled to thermodynamic databases to obtain dynamic simulations of metallurgical process phenomena. The modeling approach has been used on a fundamental model of a top-blown converter. Reactions between gas-steel, gas-slag, steel-slag and gas-steel-slag have been considered. The results show that the mass transport in the surface area is totally controlled by convection. Also, that a large amount of CO produced during the decarburization might slow down the rate of decarburization in droplets ejected from the bath. For the present simulation conditions reflecting laboratory experiments, it was also seen that the amount of slag (FeO and/or SiO2) created is close to zero, i.e. only gas (CO+CO2) is created as the oxygen jet hits the steel bath. It was also illustrated how an extrapolation of the decarburization rate, sampled from a few seconds of simulation, could be done to get a rough estimate of the carbon content at a later stage in the process as long as the carbon content is relatively high. The overall conclusion is that it is possible to make a dynamic coupling of the Thermo-Calc databases and a CFD software to make dynamic simulations of metallurgical processes such as a top-blown converter.

    • A dynamic modeling approach is presented where a computational fluid dynamics software is coupled to a thermodynamics software to obtain simulations of reactions between steel, slag and gas in a top-blown converter. For each simulation the transport of momentum, energy and mass of species as well as the thermodynamic equilibrium in each cell containing at least two phases was treated. The overall conclusion is that the present calculation procedure is successful for dynamic simulations of interaction between an oxygen gas jet with a melt and a slag. The predicted rate of decarburization was found to agree well with experimental data from laboratory trials. In addition, four cases where simulated for which the temperature, the dissolved carbon content and the dissolved oxygen content were varied. The most important findings from these comparisons were that: i) a higher initial oxygen concentration in the melt yields a larger decarburization rate, ii) carbon content also plays a big role for the desiliconization where a low carbon content is required for desiliconization to take place, iii) decarburization and desiliconization is largely influenced by the temperature at which reactions take place, where low temperature favors desiliconization and iv) the region affected by a lower carbon/silicon concentration (hot-spot region) directly below the jet was approximately 10 mm for the current setup.

    • A coupling between computational fluid dynamics (CFD) and thermodynamics has recently been done. In the current model improvement, a more realistic model was developed, where the numbers of gas species and slag phases were increased. For each simulation the transport of momentum, energy and mass of species as well as the thermodynamic equilibrium in each cell containing at least two phases was treated. Read how this calculation procedure can handle dynamic simulations of interaction between an oxygen gas jet, a melt and a slag. How is the agreement between the predicted rate of decarburization and experimental data? Which findings were achieved from the simulation of four cases varying the temperature, the dissolved carbon content and the dissolved oxygen content?

    • Creating better value in health care service today is very challenging. The social pressure to do so is real for every health care system and its leadership. Real benefit has been achieved in manufacturing sector work by the use of "value-chain" thinking, which assumes that the work is a series of linked processes necessary to make a product. For those activities in health care systems that are similar, this model may be very helpful. Attempts to "install" the value chain widely in health care systems have, however, been frustrating. As a result, well-meaning leaders seeking better value have resorted to programs of cost reduction, rather than service redesign. Professionals have not been very happy or willing participants. The work of health care service invites an expanded model of value creation, one that better matches the work. This paper proposes a networked architecture that can mobilize and integrate the resources of health care professionals, interested patients, family, and other community members in the delivery and improvement of health care systems. It also suggests how this value-creation architecture might contribute to research and the development of new knowledge. Two cases illustrate the proposed architecture and its implications for system design and practice, technology development, and roles and responsibilities of all actors involved in health care systems. We believe that this model better fits the need of making and improving health care services. This expanded understanding of how value is created invites attention by senior leaders, by those attempting to facilitate the improvement of current systems, by patients and clinicians involved in the daily work of health care service coproduction, by those charged with the preparation and formation of future professionals, by those who measure and conduct research in health care services, and by those leading policy, payment, and reimbursement systems.

    • We report eight novel members of the superfamily of human G protein-coupled receptors (GPCRs) found by searches in the human genome databases, termed GPR97, GPR110, GPR111, GPR112, GPR113, GPR114, GPR115 and GPR116. Phylogenetic analysis shows that these are additional members of a family of GPCRs with long N-termini, previously termed EGF-7TM, LNB-7TM, B2 or LN-7TM. Five of the receptors form their own phylogenetic cluster, while three others form a cluster with the previously reported HE6 and GPR56 (TM7XN1). All the receptors have a GPS domain in their N-terminus and long Ser/Thr-rich regions forming mucin-like stalks. GPR113 has a hormone binding domain and one EGF domain. GPR112 has over 20 Ser/Thr repeats and a pentraxin domain. GPR116 has two immunoglobulin-like repeats and a SEA box. We found several human EST sequences for most of the receptors showing differential expression patterns, which may indicate that some of these receptors participate in reproductive functions while others are more likely to have a role in the immune system.
