  • Exhaustion and Impaired Work Performance in the Workplace : Associations With Presenteeism and Absenteeism

    • Objective: The aim of this study was to investigate the association between presenteeism and absenteeism during the previous year and the current levels of exhaustion and impaired work performance in a Swedish university setting.Methods: In a study of 3525 employees, an ordinal logistic regression and general linear model was used to examine the association between presenteeism and absenteeism during the previous year and the current exhaustion and impaired work performance, respectively.Results: Presenteeism, but not absenteeism, during the previous year independently increased the risk of having moderate or severe exhaustion. Presenteeism, absenteeism, and exhaustion remained positively associated with impaired work performance when health status and other confounders had been adjusted for.Conclusions: Presenteeism, but not absenteeism, was associated with exhaustion. Both presenteeism and absenteeism were the salient correlates of impaired work performance.

  • Individual preferences for physical exercise as secondary prevention for non-specific low back pain : a discrete choice experiment

    • Background: Exercise is effective in improving non-specific low back pain (LBP). Certain components of physical exercise, such as the type, intensity and frequency of exercise, are likely to influence participation among working adults with non-specific LBP, but the value and relative importance of these components remain unknown. The study's aim was to examine such specific components and their influence on individual preferences for exercise for secondary prevention of non-specific LBP among working adults. Methods: In a discrete choice experiment, working individuals with non-specific LBP answered a webbased questionnaire. Each respondent was given ten pairs of hypothetical exercise programs and asked to choose one option from each pair. The choices comprised six attributes of exercise (i.e., type of training, design, intensity, frequency, proximity and incentives), each with either three or four levels. A conditional logit regression that reflected the random utility model was used to analyze the responses. Results: The final study population consisted of 112 participants. The participants' preferred exercise option was aerobic (i.e., cardiovascular) rather than strength training, group exercise with trainer supervision, rather than individual or unsupervised exercise. They also preferred high intensity exercise performed at least once or twice per week. The most popular types of incentive were exercise during working hours and a wellness allowance rather than coupons for sports goods. The results show that the relative value of some attribute levels differed between young adults (age <= 44 years) and older adults (age <= 45 years) in terms of the level of trainer supervision required, exercise intensity, travel time to exercise location and financial incentives. For active study participants, exercise frequency (i.e., twice per week, 1.15; CI: 0.25; 2.06) influenced choice of exercise. For individuals with more than one child, travel time (i.e., 20 minutes, - 0.55; CI: 0.65; 3.26) was also an influential attribute for choice of exercise, showing that people with children at home preferred to exercise close to home. Conclusions: This study adds to our knowledge about what types of exercise working adults with back pain are most likely to participate in. The exercise should be a cardiovascular type of training carried out in a group with trainer supervision. It should also be of high intensity and preferably performed twice per week during working hours. Coupons for sports goods do not appear to motivate physical activity among workers with LBP. The findings of the study could have a substantial impact on the planning and development of exercise provision and promotion strategies to improve non-specific LBP. Providers and employers may be able to improve participation in exercise programs for adults with non-specific LBP by focusing on the exercise components which are the most attractive. This in turn would improve satisfaction and adherence to exercise interventions aimed at preventing recurrent non-specific LBP.

    • Objective: To examine the associations between sickness presenteeism (SP) reported as number of days with SP reported as number of times and to evaluate their responsiveness.Methods: The study population (n = 454) consisted of employed individuals, at risk of long-term sickness absence. Correlation analyses were performed to examine associations between the two SP measures and external constructs such as work performance, general health, and registered sick leave. Both SP constructs were measured several times to examine responsiveness.Results: The SP measures are moderately correlated. They moderately correlated with work performance and health status measures. SP reported as number of times seems to be more sensitive than number of days in detecting changes after rehabilitation.Conclusions: Numerical or categorical constructs are valid sources of data on SP. However, categorized SP seems to be more responsive.

    • A number of smaller municipalities have decreasing population rates. Generally, the young move out, increasing the proportion of older people. To increase our understanding of the living conditions of an ageing population in small municipalities, a postal survey was conducted in three small, semi-rural municipalities in southern Sweden. In the survey the respondents answered questions about their living situation and their housing plans. The aim of this study was to investigate the housing situation and housing plans of the very old in semi-rural areas and research questions analysed for this study concerned the current housing situation and plans for future housing. A total of 1386 surveys were sent out in March 2014, to all inhabitants aged 80 years or more, residing in the ordinary housing market in the three municipalities, the response rate was 60%. The results show that most of the respondents were firmly rooted in the area as most of them had lived in the municipality for more than 20 years and 60% had lived in their current dwelling for more than 20 years. Ageing in place was the dominating plan, although one quarter of the respondents answered that they did not know what would happen in the future. Those who planned to move wanted to move to housing that required less maintenance and to a more central location. Residential mobility is at play also in old age as 27% of the respondents had moved at some point during the last 10 years, i.e., after the age of 70.

    • Many municipalities in rural areas have decreasing population rates. Generally the young move out, increasing the share of older people and influencing the way life is led in rural areas. In this paper focus is on the housing situation and housing plans of the oldest individuals in three small, semi-rural municipalities in southern Sweden. In order to increase our understanding of the living conditions of an ageing rural population, a postal survey was conducted. In one part of the survey the respondents answered questions about their current housing situation and their housing plans for the future. The research questions analysed for this study regarded stated reasons for housing decisions and plans. 1386 surveys were sent out, i.e., to all inhabitants aged 80 years and older residing in the ordinary housing market, in the three municipalities, 817 surveys were returned providing a response rate at 60%. The aim was to determine the main reasons for remaining put in a current housing situation or reasons for why relocation could be a future option. Results show that place attachment is strong, 90% of the respondents have lived in the municipality for more than 20 years and 60% have lived in the current dwelling for more than 20 years. Ageing in place is the dominating plan although 23% answered that they do not know what will happen in future. Those that plan to move, want to move to housing that needs less maintenance than the current dwelling and to a more central location.

    • I vår tids diskussion om välfärd är omsorgsbegreppet centralt. Men vad är omsorg och på vilket sätt kommer den till uttryck i sammanhang som gäller äldre och åldrande? Det är några av de frågeställningar som diskuteras i den här antologin.I äldre­omsorgsforskning har man hittills som regel tagit offentlig äldreomsorg som en självklar utgångspunkt.Här vidgas istället perspektivet och flera andra av omsorgsbegreppets olika dimensioner, gestaltningar och sammanhang inkluderas och diskuteras - sådana som har betydelse i vår tid men som ofta har förbisetts i forskningen.Det innebär också att antologin fokuserar på åldrande och omsorg i vid bemärkelse och inte bara på kommunal äldreomsorg. Med utgångspunkt i begrepp som tid, plats och aktörer och samspelet mellan dessa lyfter vi fram flera olika sammanhang för omsorg: Det är fråga om civilsamhället som plats och utförare av omsorg, liksom omsorg som berör migration och etniska relationer.Vi diskuterar också den fysiska miljön, boendet och platsens betydelse för omsorgens utformning och innebörder. Slutligen belyser vi livets sista tid och omsorgens olika existentiella dimensioner under detta skede.Åldrande och omsorgens gestaltningar - mot nya perspektiv riktar sig främst till högskolestuderande inom samhälls- och beteende­vetenskap, socialt arbete, social omsorg och vård, liksom till yrkesverksamma inom fältet.     • Many municipalities in Sweden have decreasing population rates combined with an increasing proportion of older people. Such a demographic shift will influence the way life is led as the foundation for service provision and social activities becomes undermined. This leads us to question the extent to which shrinking municipalities can be considered socially sustainable. The aim of the paper was to investigate older people's participation in the local community and to study the perceived changes in the local community as reported by older people and how these are experienced. A postal survey was sent out to all inhabitants aged 80 years and older living in their own household in three small, semi-rural municipalities in southern Sweden, in total 1386. The response rate was 60%. Thus, focus was on the experiences of the oldest individuals. The research questions analysed for this study concerned the ageing population's social networks, community involvement, car dependence and service provision. The results are used to discuss the social sustainability of the societies in which these people have lived for a long time.

    • Brown adipose tissue (BAT) thermogenesis relies on blood flow to be supplied with nutrients and oxygen, and for the distribution of the generated heat to the rest of the body. It is therefore fundamental to understand the mechanisms by which blood flow is regulated and its relation to thermogenesis. Here we present high-resolution laser-Doppler imaging (HR-LDR) as a novel method for noninvasive, in vivo measurement of BAT blood flow in mice. Using HR-LDR, we found that norepinephrine stimulation increases BAT blood flow in a dose-dependent manner, and that this response is profoundly modulated by environmental temperature acclimation. Surprisingly, we found that mice lacking uncoupling protein 1 (UCP1) have fully preserved BAT blood flow response to norepinephrine, despite failing to perform thermogenesis. BAT blood flow was not directly correlated to systemic glycaemia, but glucose injections could transiently increase tissue perfusion. Inguinal white adipose tissue, also known as a brite/beige adipose tissue, was also sensitive to cold acclimation and similarly increased blood flow in response to norepinephrine. In conclusion, using a novel non-invasive method to detect BAT perfusion, we demonstrate that adrenergically-stimulated BAT blood flow is qualitatively and quantitatively fully independent of thermogenesis, and is therefore not a reliable parameter for the estimation of BAT activation and heat generation.
