    • Open experimentation with operational Mobile Broadband (MBB) networks in the wild is currently a fundamental requirement of the research community in its endeavor to address the need of innovative solutions for mobile communications. Even more, there is a strong need for objective data about stability and performance of MBB (e.g., 3G/4G) networks, and for tools that rigorously and scientifically assess their status. In this paper, we introduce the MONROE measurement platform: an open access and flexible hardware-based platform for measurements and custom experimentation on operational MBB networks. The MONROE platform enables accurate, realistic and meaningful assessment of the performance and reliability of 11 MBB networks in Europe. We report on our experience designing, implementing and testing the solution we propose for the platform. We detail the challenges we overcame while building and testing the MONROE testbed and argue our design and implementation choices accordingly. We describe and exemplify the capabilities of the platform and the wide variety of experiments that external users already perform using the system

    • Mobile broadband (MBB) networks underpin numerous vital operations of the society and are arguably becoming the most important piece of the communications infrastructure. In this demo paper, our goal is to showcase the potential of a novel multi-homed MBB platform for measuring, monitoring and assessing the performance of MBB services in an objective manner. Our platform, MONROE, is composed of hundreds of nodes scattered over four European countries and a backend system that collects the measurement results. Through a user-friendly web client, the experimenters can schedule and deploy their experiments. The platform further embeds traffic analysis tools for real-time traffic flow analysis and a powerful visualization tool.

    • Ossification of the Internet transport-layer architecture is a significant barrier to innovation of the Internet. Such innovation is desirable for many reasons. Current applications often need to implement their own mechanisms to receive the transport service they need, but many do not have the breadth of adapting to all possible network characteristics. An updated transport architecture can do much to make the Internet more flexible and extensible. New ground-breaking services often require different or updated transport protocols, could benefit from better signalling between application and network, or desire a more flexible choice of which network path is used for which traffic. This document therefore proposes a new transport architecture. Such architecture lowers the barrier to service innovation by proposing a “transport system”, the NEAT System, that can leverage the rich set of available transport protocols. It paves the way for an architectural change of the Internet where new transport-layer services can seamlessly be integrated and quickly made available, minimising deployment difficulties, and allowing Internet innovators to take advantage of them wherever possible. The document provides a survey of the state-of-the-art to identify the architectural obstacles to, and opportunities for, evolution of the transport layer. It also details a set of general requirements for a new transport architecture. This new architecture is motivated by a set of use-cases, followed by a description of the NEAT architecture for a transport system, designed to permit applications to select appropriate transports based on their needs and the available transport services.

  • Deliverable D3.1 - Initial Report on the Extended Transport System

    • The NEAT System offers an enhanced API for applications that disentangles them from the actual transport protocol being used. The system also enables applications to communicate their service requirements to the transport system in a generic, transport-protocol independent way. Moreover, the architecture of the NEAT System promotes the evolution of new transport services. Work Package 3 (WP3) enhances and extends the core parts of the NEAT Transport. Efforts have been devoted to developing transport-protocol mechanisms that enable a wider spectrum of NEAT Transport Services, and that assist the NEAT System in facilitating several of the commercial use cases. Work has also started on the development of optimal transport-selection mechanisms; mechanisms that enable for the NEAT System to make optimal transport selections on the basis of application requirements and network measurements. Lastly, another research activity has been initiated on how to use SDN to signal application requirements to routers, switches, and similar network elements. This document provides an initial report on all these WP3 activities—both on completed and on near-termplanned work.

  • Deliverable D3.3 - Extended Transport System and Transparent Support of Non-NEAT Applications

    • This deliverable summarises and concludes our work in Work Package 3 (WP3) to extend the transport services provided by the NEAT System developed in Work Package 2, and to enable non-NEAT applications to harness the transport services offered by NEAT. We have demonstrated how a policy- and information-based selection of transport protocol by NEAT could provide a more efficient transport service for web applications. The information on which NEAT makes its transport selection decisions resides in the Characteristics Information Base (CIB). The CIB is populated by various CIB sources, and in WP3 we have designed, implemented, and evaluated various CIB sources, including meta data from mobile broadband networks, passive measurements, IPv6 Provisioning Domain protocols and the Happy Eyeballs mechanism, which caches the outcome of its connection attempts. A key property of NEAT is that it not only “vertically” decouples applications from transport protocols, but also “horizontally”. Particularly, it enables applications to harness information about resource availability and policies from Software Defined Networking (SDN) controllers in managed networks, without these applications actually being SDN-aware. To extend the use of NEAT to non-NEAT applications, we have implemented a BSDcompatible sockets API on top of NEAT and a NEAT proxy that intercepts and replaces standard TCP connections with NEAT flows, i.e., with the transport solutions deemed most appropriate by NEAT.We have also proposed a way for non-NEAT applications to make use of NEAT through the deployment of NEAT-enabled virtual appliances in SDN-controlled networks: connections from these applications are routed via an SDN-controlled proxy that terminates the original connection and replaces it with a NEAT-selected connection.

    • There are few long-term follow-up studies on psychological treatment of anxiety disorders carried out in clinical mental health settings, so called effectiveness studies. The present paper presents a four year follow-up of patients with obsessive-compulsive disorder treated by the Bergen 4-day treatment (B4DT), a concentrated form of exposure and response prevention (ERP). A total of 77 obsessive–compulsive disorder (OCD) patients received treatment during four consecutive days and were assessed with the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) pre, post, and at follow-ups after 3 and 6 months, and 4 years post-treatment. The Y-BOCS mean score changed from 25.9 at pre- to 10.0 post-treatment and 9.9 at long-term follow-up. The proportion fulfilling the strict international consensus criteria for remission was 73% at post-treatment and 69% at follow-up. When taking declining rate, attrition rate, remission, relapse, and further improvement during the follow-up period into account, 72% were recovered on a long-term basis. A comparison with previously published effectiveness studies of ERP indicated that the 4-day treatment yielded significantly higher proportions of remission at post-treatment and recovery at follow-up, as well as within-group effect size on the Y-BOCS. The implications of these results are discussed.

  • Problematic opioid use among osteoarthritis patients with chronic post-operative pain after joint replacement: analyses from the BISCUITS study

    • Objectives: Opioids are commonly used to manage pain, despite an increased risk of adverse events and complications when used against recommendations. This register study uses data of osteoarthritis (OA) patients with joint replacement surgery to identify and characterize problematic opioid use (POU) prescription patterns.Methods: The study population included adult patients diagnosed with OA in specialty care undergoing joint replacement surgery in Denmark, Finland, Norway, and Sweden during 1 January 2011 to 31 December 2014. Those with cancer or OA within three years before the first eligible OA diagnosis were excluded. Patients were allocated into six POU cohorts based on dose escalation, frequency, and dosing of prescription opioids post-surgery (definitions were based on guidelines, previous literature, and clinical experience), and matched on age and sex to patients with opioid use, but not in any of the six cohorts. Data on demographics, non-OA pain diagnoses, cardiovascular diseases, psychiatric disorders, and clinical characteristics were used to study patient characteristics and predictors of POU.Results: 13.7% of patients with OA and a hip/knee joint replacement were classified as problematic users and they had more comorbidities and higher pre-surgery doses of opioids than matches. Patients dispensing high doses of opioids pre-surgery dispensed increased doses post-surgery, a pattern not seen among patients prescribed lower doses pre-surgery. Being dispensed 1-4,500 oral morphine equivalents in the year pre-surgery or having a non-OA pain diagnosis was associated with post-surgery POU (OR: 1.44-1.50, and 1.11-1.20, respectively).Conclusions: Based on the discovered POU predictors, the study suggests that prescribers should carefully assess pain management strategies for patients with a history of comorbidities and pre-operative, long-term opioid use. Healthcare units should adopt risk assessment tools and ensure that these patients are followed up closely. The data also demonstrate potential areas for further exploration in improving patient outcomes and trajectories.

  • Concentrated ERP Delivered in a Group Setting : A Replication Study

    • Background: In a previous effectiveness study (Havnen et al., 2014), 35 obsessive compulsive disorder (OCD) patients underwent Concentrated Exposure Treatment (cET), which is a newly developed group treatment format delivered over four consecutive days. Aims: The primary aims of the present study were to evaluate the treatment results for a new sample of OCD patients receiving the cET treatment approach and to replicate the effectiveness study described in Havnen et al. (2014). Method: Forty-two OCD patients underwent cET treatment. Treatment was delivered by different therapists than in Havnen et al. (2014), except for two groups led by the developers of the treatment. Assessments of OCD symptom severity, treatment satisfaction, and occupational impairment were included. Results: The results showed a significant reduction in Yale-Brown Obsessive Compulsive Scale scores from pre-treatment to post-treatment, which was maintained at 6-month follow-up. At post-treatment, 74% of the sample was remitted; at 6-month follow-up, 60% were recovered. The sample showed a very high degree of overall treatment satisfaction. The results from the present study were statistically compared with those obtained in the previous study. The analyses showed that the study samples had comparable demographic data and equal application of treatment. The outcome of the present and original study did not differ significantly on primary and secondary outcome measures. Conclusions: This study shows that cET was successfully replicated in a new patient sample treated by different therapists than the original study. The results indicate that cET is well accepted by the patients, and the potential for dissemination is discussed.

  • Concentrated ERP delivered in a group setting : An effectiveness study

    • The aims of the present study were to evaluate the acceptability and long term gains of a highly concentrated exposure and response prevention (ERP) for obsessive compulsive disorder (OCD). Treatment was individually tailored and delivered in a group format over four consecutive days in an outpatient OCD treatment unit, part of the ordinary specialist health care. A total of 35 patients accepted the offer of treatment; 23 of the patients were classified with severe to extreme OCD, and 74% of the sample had previously received treatment for their OCD (20% of these with ERP). The results showed that more than 90% of the patients expressed a high degree of satisfaction with the concentrated ERP. At six months follow-up 27 (77%) were classified as recovered and a significant improvement was seen in depressive symptoms as well. The majority of the sample also showed improvement with regard to employment status 12 months after treatment. The results indicate that this highly concentrated treatment may be a feasible format of delivering ERP.
