Human cystatin C: Role of the N-terminal segment in the inhibition of human cysteine proteinases and in its inactivation by leucocyte elastase

- Leucocyte elastase in catalytic amounts was observed to rapidly cleave the Val-10-Gly-11 bond of the human cysteine-proteinase inhibitor cystatin C at neutral pH. The resulting modified inhibitor had size and amino acid composition consistent with a cystatin C molecule devoid of the N-terminal Ser-1-Val-10 decapeptide. Leucocyte-elastase-modified cystatin C had more than 240-fold lower affinity than native cystatin C for papain. Removal of the N-terminal decapeptide of human cystatin C also decreased inhibition of human cathepsins B and L by three orders of magnitude, but decreased inhibition of cathepsin H by only 5-fold. A tripeptidyldiazomethane analogue of of the N-terminal portion of cystatin C was a good inhibitor of cathepsins B and L but a poor inhibitor of cathepsin H. It therefore appears that amino acid side chains of the N-terminal segment of cystatin C bind in the substrate-binding pockets of cathepsins B and L but not in those of cathepsin H. It is argued that the N-terminal cystatin C interaction with cathepsin B is physiologically important and hence that leucocyte elastase could have a function as a regulator of extracellular cysteine-proteinase inhibitory activity at sites of inflammation.

Amplitude modulation of light from various sources

- This work was done to obtain basic knowledge about various light sources and specially about light modulation, or flicker. The study showed that the modulation of light varied a lot between different ordinary light sources. In general the 100 Hz component in the flickering light dominated. For incandescent lights the modulation increased with decreasing power and was in the range 10-22%. Light from the tungsten-halogen lamps had 2-6% modulation. The most common light sources (single-colour fluorescent light) had a modulation of about 20%. Fluorescent tubes with better colour rendering (full-colour fluorescent and full-colour special fluorescent tubes) had a higher modulation, 30-40%, while light from high-frequency (HF) fluorescent tubes had a modulation of 0.9%. Light from conventional compact fluorescent tubes had a modulation degree of about 44%, and for HF compact fluorescent tubes it was 2-7%. Modulation of light from HF compact fluorescent tubes had a tendency to decrease with increasing tube size. The modulation of light from discharge lamps was in the range of 52% up to 100% and the curve form of the light from low pressure sodium vapour lamp (100% modulation) contained several high frequency components.

Multisystemisk terapi för ungdomar med allvarliga beteendeproblem : Resultat efter två år

- Multisystemisk terapi (MST) är en strukturerad öppenvårdsbehandling som vänder sig till familjer med ungdomar som har beteendeproblem. MST har utvecklats i USA som ett alternativ till placering utom hemmet. MST lägger stor vikt vid att arbeta med alla de system som en ung människa finns i (t.ex. familjen, skolan kamratkretsen). Arbetet styrs av konkreta behandlingsmål som i sin tur bestämmer valet av interventioner. Behandlingen utvärderas regelbundet genom intervjuer med föräldrarna. MST-arbetet utförs inom små team bestående av tre till fyra terapeuter och en teamledare. Varje terapeut arbetar med familjen under fyra till sex månader. Terapeuterna/teamet finns tillgängliga dygnet runt.I den här studien jämförs MST med socialtjänstens traditionella insatser (t.ex. placering i dygnsvård, samtalsstöd, familjeterapi). Undersökningsgruppen utgörs av 156 ungdomar mellan 12 och 17 år med allvarliga beteendeproblem motsvarande kriterier i den psykiatriska diagnosen uppförandestörning (eng. conduct disorder). Ungdomarna har fördelats till MST eller traditionell behandling genom randomisering. Utvärderingen berör 27 kommunala förvaltningar och sammanlagt sex MST-team från Göteborg, Halmstad, Malmö och Stockholm. Totalt har elva olika mätinstrument använts för att beskriva ungdomarnas psykiska problem, normbrytande beteende (inkl. kriminalitet), drogbruk, sociala förmågor, förhållande till kamrater, familjerelationer, känsla av sammanhang samt föräldrarnas psykiska hälsa. Dessutom har information om insatser och dess kostnader inhämtats från socialtjänstens akter samt från Statens institutionsstyrelse (SiS). I den här rapporten beskrivs ungdomarnas situation efter två år samt en ekonomisk analys av samhällets kostnader

Treatment as usual in effectiveness studies : what is it and does it matter?

- A hallmark of an evidence-based practice (EBP) is the systematic appraisal of research related to the effectiveness of interventions. This study addressed the issue of interpreting results from effectiveness studies that use treatment-as-usual (TAU) as a comparator. Using randomised controlled studies that evaluate the effectiveness of multisystemic therapy as an illustrative example, we show that TAU includes a wide variety of treatment alternatives. Estimated treatment effects on recidivism suggest that TAU seems to contain a greater variation in underlying risk than experimental conditions, supporting the hypothesis that the content of TAU could affect outcomes. Implications for the realisation of an EBP are discussed.

Ultrasound anatomy in the neonatal clubfoot

- Our objective was to assess clubfoot anatomy by US in newborn babies before and in the early phase of treatment. Reproducible US projections and measurements were carried out on 30 untreated clubfeet in 22 children, consecutively included in the study. The position of the navicular in relation to the head of the talus was visualised in all feet. The mean distance between the medial malleolus and the navicular was significantly shorter in the clubfeet than in normal feet. There was a tendency to medial displacement of the cuboid. Soft tissue thickness at the medial side of the foot was increased in all deformed feet. The measurements showed an acceptable intra- and inter-observer reliability for the different variables assessed (r=0.71-0.96, Pearson's correlation coefficient). With US it is possible to obtain well-defined planes of investigation that give important information about the clubfoot deformity concerning the talo-crural, the talonavicular and the calcaneo-cuboid joints. The method is simple enough to be used in everyday clinical practice and we recommend it as a guide during the non-operative treatment and for preoperative planning.
