  • Do young adults with cancer receive information about treatment- related impact on sex life? : Results from a population-based study

    • Background Sexual dysfunction is common following a cancer diagnosis in young adulthood (18-39 years) and problems related to sex life are ranked among the core concerns in this age group. Yet, few studies have investigated to what extent adults younger than 40, receive information from healthcare providers about the potential impact of cancer and its treatment on their sex life.Methods A population-based cross-sectional survey study was conducted with 1010 young adults 1.5 years after being diagnosed with cancer (response rate 67%). Patients with breast, cervical, ovarian and testicular cancer, lymphoma, and brain tumors were identified in national quality registries. Sociodemographic and clinical factors associated with receiving information were examined using multivariable binary logistic regression.Results Men to a higher extent than women reported having received information about potential cancer-related impact on their sex life (68% vs. 54%, p < 0.001). Receipt of information varied across diagnoses; in separate regression models, using lymphoma as reference, both women and men with brain tumors were less likely to receive information (women: OR 0.10, CI = 0.03-0.30; men: OR 0.37, CI = 0.16-0.85). More intensive treatment was associated with higher odds of receiving information in both women (OR 1.89; CI = 1.28-2.79) and men (OR 2.08; CI = 1.09-3.94). None of the sociodemographic factors were associated with receipt of information.Conclusions To improve sexual health communication to young adults with cancer, we recommend diagnosis-specific routines that clarify when in the disease trajectory to discuss these issues with patients and what to address in these conversations.

  • Ambient air pollution and inflammation-related proteins during early childhood

    • Background and aim: Experimental studies show that short-term exposure to air pollution may alter cytokine concentrations. There is, however, a lack of epidemiological studies evaluating the association between long-term air pollution exposure and inflammation-related proteins in young children. Our objective was to examine whether air pollution exposure is associated with inflammation-related proteins during the first 2 years of life. Methods: In a pooled analysis of two birth cohorts from Stockholm County (n = 158), plasma levels of 92 systemic inflammation-related proteins were measured by Olink Proseek Multiplex Inflammation panel at 6 months, 1 year and 2 years of age. Time-weighted average exposure to particles with an aerodynamic diameter of <10 mu m (PM10), <2.5 mu m (PM2.5), and nitrogen dioxide (NO2) at residential addresses from birth and onwards was estimated via validated dispersion models. Stratified by sex, longitudinal cross-referenced mixed effect models were applied to estimate the overall effect of preceding air pollution exposure on combined protein levels, "inflammatory proteome", over the first 2 years of life, followed by cross-sectional protein-specific bootstrapped quantile regression analysis. Results: We identified significant longitudinal associations of inflammatory proteome during the first 2 years of life with preceding PM2.5 exposure, while consistent associations with PM10 and NO2 across ages were only observed among girls. Subsequent protein-specific analyses revealed significant associations of PM10 exposure with an increase in IFN-gamma and IL-12B in boys, and a decrease in IL-8 in girls at different percentiles of proteins levels, at age 6 months. Several inflammation-related proteins were also significantly associated with preceding PM10, PM2.5 and NO2 exposures, at ages 1 and 2 years, in a sex-specific manner. Conclusions: Ambient air pollution exposure influences inflammation-related protein levels already during early childhood. Our results also suggest age-and sex-specific differences in the impact of air pollution on children's inflammatory profiles.

  • Knowledge of and attitude to oral health and oral diseases among young adolescents in Sweden

    • The aim of this study was to investigate the knowledge of and attitudes to oral health among 12- and 15-year-old students in Sweden. The results are designed to act as a baseline survey to evaluate future interventions. From all 85 schools in the County of Uppsala, ten schools were randomly selected. In all, 993 students were offered the chance to participate in the study. 793 (80%) individuals answered a questionnaire consisted of fifteen structured questions about their knowledge and attitudes to oral health. The result showed that the level of knowledge by adolescents is relatively high. A large majority of the subjects felt that their teeth were important. Most of the adolescents had learned about oral health from the dental team. With the exception of the question about the meaning of the word “periodontitis”, all the questions produced a distribution of correct answers in favour of the girls. Older students showed more knowledge compared with younger students. In none of the questions did the younger students display more knowledge than the older. The adolescents with an immigrant background showed less knowledge compared with natives of Sweden in several areas. Despite having less knowledge of oral health, more students with an immigrant background than natives of Sweden stated that their teeth were important. Several of the survey questions call for answers in the form of factual knowledge. Knowledge is therefore regarded as a quantitative measure of a reproduction, where the adolescents are expected to reproduce something that they have already learned. Increasing criticism has been levelled against this view of knowledge. Facts are not enough to induce young people to change their health behaviour. Facts are an important part of the message but must be complemented by reflection and consideration of how the receiver understands it.

    • AbstractAim The aim of this study was to describe young adults’ own perspectives on the experience of having a parent who developed cancer when the young adult was an adolescent. Method Narrative interviews were conducted with six young adults aged between 20 and 26. The interviews were analysed using qualitative content analysis. Results The main message that the young adults communicated in the interviews was interpreted as the overarching theme ‘Loneliness despite the presence of others’. Two domains with three categories each emerged: distance, comprising a feeling of loneliness, lacking the tools to understand, and grief and anger; and closeness, comprising belief in the future, comfort and relief, and a need for support. The young adults felt a loneliness that they had never experienced before, and they lacked the tools to understand the situation. They felt grief and anger over what the cancer had caused. However, they had still managed to regain faith in the future. They found comfort and relief in the thought that this would not necessarily happen to them again, and they gained support from talking to family and friends. Conclusion If all family members are given the same information, it becomes easier to talk about what is happening. This can reduce adolescent children’s experience of loneliness. Contact with health care professionals should be maintained throughout the period of illness. Many short informal contacts create relationships and trust that can be helpful if the worst happens and the parent dies.

  • Expression of LRIG proteins as possible prognostic factors in primary vaginal carcinoma

    • Background: Primary vaginal carcinoma (PVC) is a rare malignancy. Established prognostic factors include tumour stage and age at diagnosis. The leucine-rich repeats and immunoglobuline-like domains (LRIG)-1 protein functions as a tumour suppressor, but less is known about the functions of LRIG2 and LRIG3. The present study aimed to evaluate the expression of LRIG proteins and analyse their possible associations with clinical characteristics and survival in a cohort of PVC patients.Methods: We used immunohistochemistry to investigate LRIG1, LRIG2, and LRIG3 expression in tumour samples from a consecutive cohort of 70 PVC patients. The association between LRIG protein expression and clinical characteristics and cancer-specific survival was investigated using univariate and multivariate analyses.Results: The majority of PVC patients (72%) had > 50% LRIG1-and LRIG2-positive cells, and no or low LRIG3-positive cells. HPV status was significantly correlated with LRIG1 expression (p = 0.0047). Having high LRIG1 expression was significantly correlated with superior cancer-specific survival in univariate and multivariate analyses. LRIG2 and LRIG3 expression did not significantly correlate with clinical characteristics or survival.Conclusion: LRIG1 expression might be of interest as a prognostic marker in PVC patients, whereas the role of LRIG2 and LRIG3 expression remains to be clarified.
