  • Activation of complement factor B contributes to murine and human myocardial ischemia/reperfusion injury

    • The pathophysiology of myocardial injury that results from cardiac ischemia and reperfusion (I/R) is incompletely understood. Experimental evidence from murine models indicates that innate immune mechanisms including complement activation via the classical and lectin pathways are crucial. Whether factor B (f B), a component of the alternative complement pathway required for amplification of complement cascade activation, participates in the pathophysiology of myocardial I/R injury has not been addressed. We induced regional myocardial I/R injury by transient coronary ligation in WT C57BL/6 mice, a manipulation that resulted in marked myocardial necrosis associated with activation of fB protein and myocardial deposition of C3 activation products. In contrast, in f13(-/-) mice, the same procedure resulted in significantly reduced myocardial necrosis (% ventricular tissue necrotic; fB(-/-) mice, 20 4%; WT mice, 45 3%; P< 0.05) and diminished deposition of C3 activation products in the myocardial tissue (fB(-/-) mice, 0 0%; WT mice, 31 6%; P<0.05). Reconstitution of fB(-/-) mice with WT serum followed by cardiac I/R restored the myocardial necrosis and activated C3 deposition in the myocardium. In translational human studies we measured levels of activated fB (Bb) in intracoronary blood samples obtained during cardio-pulmonary bypass surgery before and after aortic cross clamping (AXCL), during which global heart ischemia was induced. Intracoronary Bb increased immediately after AXCL, and the levels were directly correlated with peripheral blood levels of cardiac troponin I, an established biomarker of myocardial necrosis (Spearman coefficient = 0.465, P < 0.01).   • Survey of transfusion practices in preterm infants in Europe

    • Preterm infants commonly receive red blood cell (RBC), platelet and fresh frozen plasma (FFP) transfusions. The aim of this Neonatal Transfusion Network survey was to describe current transfusion practices in Europe and to compare our findings to three recent randomised controlled trials to understand how clinical practice relates to the trial data.MethodsFrom October to December 2020, we performed an online survey among 597 neonatal intensive care units (NICUs) caring for infants with a gestational age (GA) of <32 weeks in 18 European countries.ResultsResponses from 343 NICUs (response rate: 57%) are presented and showed substantial variation in clinical practice. For RBC transfusions, 70% of NICUs transfused at thresholds above the restrictive thresholds tested in the recent trials and 22% below the restrictive thresholds. For platelet transfusions, 57% of NICUs transfused at platelet count thresholds above 25×109/L in non-bleeding infants of GA of <28 weeks, while the 25×109/L threshold was associated with a lower risk of harm in a recent trial. FFP transfusions were administered for coagulopathy without active bleeding in 39% and for hypotension in 25% of NICUs. Transfusion volume, duration and rate varied by factors up to several folds between NICUs.ConclusionsTransfusion thresholds and aspects of administration vary widely across European NICUs. In general, transfusion thresholds used tend to be more liberal compared with data from recent trials supporting the use of more restrictive thresholds. Further research is needed to identify the barriers and enablers to incorporation of recent trial findings into neonatal transfusion practice.

  • Fungal communities in groundwater springs along the volcanic zone of Iceland

    • Aquatic fungi are a largely unexplored group of organisms with a still unknown diversity of ecological niches. Groundwater biomes comprise vast but poorly explored habitats. In this study, we sampled the Icelandic groundwater, a unique system that has been separated and isolated into distinct basins for millions of years due to volcanic activity. The aim was to explore the fungal diversity of the basins and its connection with the surface waters. We screened the DNA-based fungal diversity of 11 Icelandic groundwater springs and their emerging surface waters by ITS metabarcoding, revealing that the species turnover was indeed distinct for each groundwater basin. Furthermore, the groundwater taxa, which include many psychrophilic yeast-like and ascomycete fungi, seem to serve as a constant inoculum for the surface water. Nevertheless, the groundwater springs had a lower sequence proportion of early diverging, primary aquatic fungal lineages (10%) and completely unknown lineages (13%) than the surface waters (41% and 22%, respectively), likely explained by the lower diversity of suitable hosts for these presumed parasitic lineages. Our results highlight the importance of overlooked organism groups in the resolution of overarching research questions in ecology, nutrient circulation, and global change biology at large.
