    • Neonatal mortality, often due to birth asphyxia, remains stubbornly high in sub-Saharan Africa. Guidelines for neonatal resuscitation, where achieving adequate positive pressure ventilation (PPV) is key, have been implemented in low-resource settings. However, the actual clinical practices of neonatal resuscitation have rarely been examined in these settings. The primary aim of this prospective observational study was to detail the cumulative proportion of time with ventilation during the first minute on the resuscitation table of neonates needing PPV at the Mulago National Referral Hospital in Kampala, Uganda.MethodsFrom November 2015 to January 2016, resuscitations of non-breathing neonates by birth attendants were video-recorded using motion sensor cameras. The resuscitation practices were analysed using the application NeoTapAS and compared between those taking place in the labour ward and those in theatre through Fisher’s exact test and Wilcoxon rank-sum test.ResultsFrom 141 recorded resuscitations, 99 were included for analysis. The time to initiation of PPV was 66 (42–102) s overall, and there was minimal PPV during the first minute in both groups with 0 (0–10) s and 0 (0–12) s of PPV, respectively. After initiating PPV the overall duration of interruptions during the first minute was 28 (18–37) s. Majority of interruptions were caused by stimulation (28%), unknown reasons (25%) and suction (22%).ConclusionsOur findings show a low adherence to standard resuscitation practices in 2015–2016. This emphasises the need for continuous educational efforts and investments in staff and adequate resources to increase the quality of clinical neonatal resuscitation practices in low-resource settings.

    • Child health is taking the back seat in development strategies. In summarising a newly released collaborative report, this paper calls for a novel conceptual model where child health takes centre stage in relation to the 2030 Agenda and the Sustainable Development Goals. It lays out five principles by which renewed effort and focus would yield the most benefit for children and adolescents. These include: re-defining global child health in the post-2015 era by placing children and adolescents at the centre of the Sustainable Development Goals; striving for equity; realising the rights of the child to thrive throughout the life-course; facilitating evidence informed policy-making and implementation; and capitalising on interlinkages within the SDGs to galvanise multisectoral action. These five principles offer models that together have the potential of improving design, return and quality of global child health programs while re-energising the 2030 Agenda and the Sustainable Development Goals.

  • Interactions between Health and the Sustainable Development Goals: The Case of the Democratic Republic of Congo

    • A systematic and contextualized assessment of the interactions between the Sustainable Development Goals and health in the Democratic Republic of Congo is currently lacking. This study aimed to characterize and classify the linkages between the Sustainable Development Goals in the DRC with a focus on health and well-being. In this semi-qualitative participatory study, 35 experts assessed 240 interactions between 16 of the 17 SDGs during a two-day workshop in Kinshasa, Democratic Republic of the Congo, using a scale from +3 (strongly promoting) to −3 (strongly restricting). SDG 16 (Peace, justice, and strong institutions) had the strongest promoting influence on other goals and was identified as a key priority for the DRC to attain the SDGs. Progress on SDG 3 (good health and well-being) was perceived as promoting progress on most SDGs, and through second-order interactions, a positive feedback loop was identified. Furthermore, progress on the other SDGs was deemed to promote progress on SDG 3, with SDG 16 having the greatest positive potential when second-order interactions were taken into account. Our results show the importance of recognizing synergies and trade-offs concerning the interactions between health and other SDGs and that it is imperative to set up structures bringing together different sectors to accelerate work towards achieving the 2030 Agenda.

  • Greenness in semi-arid areas across the globe 1981-2007 - an Earth Observing Satellite based analysis of trends and drivers

    • Semi-arid areas, defined as those areas of the world where water is an important limitation for plant growth, have become the subject of increased interest due to the impacts of current global changes and sustainability of human lifestyles. While many ground-based reports of declining vegetation productivity have been published over the last decades, a number of recent publications have shown a nuanced and, for some regions, positive picture. With this background, the paper provides an analysis of trends in vegetation greenness of semi-arid areas using AVHRR GIMMS from 1981 to 2007. The vegetation index dataset is used as a proxy for vegetation productivity and trends are analyzed for characterization of changes in semi-arid vegetation greenness. Calculated vegetation trends are analyzed with gridded data on potential climatic constraints to plant growth to explore possible causes of the observed changes. An analysis of changes in the seasonal variation of vegetation greenness and climatic drivers is conducted for selected regions to further understand the causes of observed inter-annual vegetation changes in semi-arid areas across the globe. It is concluded that semi-arid areas, across the globe, on average experience an increase in greenness (0.015 NDVI units over the period of analysis). Further it is observed that increases in greenness are found both in semi-arid areas where precipitation is the dominating limiting factor for plant production (0.019 NDVI units) and in semi-arid areas where air temperature is the primarily growth constraint (0.013 NDVI units). Finally, in the analysis of changes in the intra-annual variation of greenness it is found that seemingly similar increases in greenness over the study period may have widely different explanations. This implies that current generalizations, claiming that land degradation is ongoing in semi-arid areas worldwide, are not supported by the satellite based analysis of vegetation greenness.
