    • The effect of postoperative radiotherapy after sector resection for stage I-II lymph node-negative breast cancer was evaluated in a patient population with access to public mammographical screening. 1187 women were randomised to no further treatment or postoperative radiotherapy following a standardised sector resection and axillary dissection. Radiation was administered to a dose of 48-54 Gy. Median age was 60 years, and median size of the detected tumours was 12 mm. Of the women 65% had their tumours detected by mammographical screening. The relative risk (RR) of ipsilateral breast recurrence was significantly higher in the non-irradiated patients compared with the irradiated patients, RR=3.33 (95% Confidence Interval (CI) 2.13-5.19, P<0.001). The corresponding cumulative incidence at 5 years was 14% versus 4%, respectively. Overall survival (OS) was similar, RR=1.16 (95% CI 0.81-1.65, P=0.41), with 5 year probabilities of 93 and 94%, respectively. Recurrence-free survival (RFS) at 5 years was significantly lower in the non-irradiated women, 77% versus 88% (P<0.001). Although women above 49 years of age, whose tumours were detected with mammographical screening, had the lowest rate of ipsilateral breast recurrence in this study, the cumulative incidence of such event amounted to 10% at 5 years if radiotherapy was not given. Such a recurrence rate has been considered as unacceptably high, but is, however, in the same range as that reported after lumpectomy and postoperative radiotherapy in published series.

    • This article first reviews the principal monitoring requirements of the Water Framework Directive (WFD) of the European Union (EU) and assesses how contaminant monitoring may fit into a risk-assessment approach. In this context, we show the limited ability of conventional trace-contaminant-monitoring methods to fulfil all of the WFD requirements. We then clearly define and exemplify the roles and the functions of a new set of monitoring tools, using three case studies based on datasets that we obtained during a field trial in the River Meuse as part of the Screening methods for Water data InFormaTion (SWIFT-WFD) project in support of implementing the WFD.

    • Clinical assessment tools for adult attention deficit hyperactivity disorder (ADHD) such as rating scales, interviews and behavior observations are often based on subjective judgments which enhance the risk of overlooking or misinterpreting symptoms. In search for objective measures of adult ADHD, the present study investigated levels of sensitivity and specificity for the Quantified Behavior Test Plus, QbTest-Plus, in adult patients (N = 19) awaiting clinical assessment for ADHD. QbTest-Plus report objective measures of ADHD core symptoms using an infrared motion tracking system and a continuous performance test. The measures were collected and evaluated previous to clinical assessment and compared regarding the diagnosis of ADHD. Sensitivity for detecting ADHD with QbTest-Plus was 83 % and specificity was 57 %. The results, possibly affected by confounding factors, suggest further examination of calibrated and objective measure for the QbTest-Plus with regard to ADHD in adults.

    • We studied the effect of postoperative radiotherapy (RT) after breast sector resection for ductal carcinoma in situ (DCIS). The study protocol stipulated radical surgery but microscopically clear margins were not mandatory. We randomised 1,046 operated women to postoperative RT or control between 1987 and 1999. The primary endpoint was ipsilateral local recurrence. Secondary endpoints were contralateral breast cancer, distant metastasis and death. After a median follow-up of 5.2 years (range 0.1-13.8) there were 44 recurrences in the RT group corresponding to a cumulative incidence of 0.07 (95% confidence interval (CI) 0.05-0.10). In the control group there were 117 recurrences giving a cumulative incidence of 0.22 (95% CI 0.18-0.26) giving an overall hazard ratio of 0.33 (95% CI 0.24-0.47, p < 0.0001). Twenty two percent of the patients had microscopically unknown or involved margins. We found no evidence for different effects of RT on the relative risk of invasive or in situ recurrence. Secondary endpoints did not differ. Women undergoing sector resection for DCIS under conditions of population based screening mammography benefit from postoperative RT to the breast. Seven patients needed RT-treatment to prevent one recurrence.

    • Environmental, developmental and personality factors are explored as predictors of motivation to treatment for alcoholism, using concepts such as social situation on admission, treatment career, age education and adventurous patterns of behaviour. The original population consists of 603 severely dependent alcoholics (mean age 40 years, 24% women, 42% compulsorily committed). In structural equation model estimation, 85% of the original cases were used, and included compulsorily as well as voluntarily admitted patients. A reduced model was tried on voluntary patients alone. In the main model, patterns of behaviour and age were not related to the motivation to treatment, while treatment career had some negative impact. Social problems were related to less motivation to treatment. In turn, a worse social situation was related to more treatment experience. The reduced model on voluntary patients alone confirmed the main findings, although more previous treatment tended to be related to more motivation. In conclusion, motivation was mostly related to a better social situation, having �more to lose�. A collapse in the social situation was more an obstacle than a promoter of motivation. There was no support for the statements that �maturing out� would result in more motivation and inconclusive findings concerning the impact on motivation from having many previous treatment experiences. The pattern of behaviour was not directly related to the level of motivation. Based on these findings, the relevance of social work for motivation to treatment is discussed.

    • MAP-NET (Measure and Analysis of Personal NETwork) is a computerised instrument for assessment of the personal networks of persons with substance use disorders and related problems, with information on individual, relational and structural levels, designed to give immediate feedback to be used in clinical interviews, and to provide relevant measures for research. The study investigates its validity and reliability, by using a combination of validation strategies including factor analysis and internal consistency of scales, convergent and predictive validity, sensitivity to social desirability and agreement with collateral interviews. The scales demonstrate satisfactory construct validity and acceptable to excellent internal consistency and convergent and predictive validity with relevant other scales. There is low sensitivity to social desirability and other bias. There is substantial agreement with collateral interviews on the most sensitive variables. Thus, MAP-NET has the qualities needed for clinical use and research.

    • Selective as well as waste forming active centers were defined for MoVNbTeO and MoVTaTeO catalysts in the ammoxidation of propane to acrylonitrile and all catalytic functionalities were assigned to specific elements at the respective active centers. Symbiosis between M1 and M2 phases of these catalysts was observed, with phase cooperation being more extensive in the Nb than Ta containing compositions. The difference in catalytic effectiveness arises most likely because contact and surface area exposure of the two respective, cooperating phase pairs are not equal. The M1 phase of the catalysts is reducible by propane and ammonia in the absence of dioxygen and is regenerable to its original, fully oxidized state by dioxygen (air). No structural collapse is observed even after 120 C3H8 + NH3 reduction pulses. The so induced reduction of the catalyst extends up to 70 layers deep. The product distribution over the first few pulses is very similar to that under catalytic conditions, supporting the concept that lattice oxygen is involved in the catalytic ammoxidation process. Therefore, the ammoxidation of paraffins is a redox process, as is of course the well-known olefin ammoxidation process.
