    • Observations of bulk plasma rotation in radio frequency (RF) heated JET discharges are reported. This study is concentrated on RF heated L-mode plasmas. In particular, the toroidal rotation profiles in plasmas heated by ion cyclotron resonance frequency (ICRF) waves and lower hybrid (LH) waves have been analysed. It is the first time that rotation profiles in JET plasmas with LH waves have been measured in dedicated discharges. It is found that the toroidal plasma rotation in the outer region of the plasmas is in the co-current direction irrespective of the heating scenario. An interesting feature is that the toroidal rotation profile appears to be hollow in many discharges at low plasma current, but a low current in itself does not seem to be a sufficient condition for finding such profiles. Fast ion transport and finite orbit width effects are mechanisms that could explain hollow rotation profiles. This possibility has been investigated by numerical simulations of the torque on the bulk plasma due to fast ICRF accelerated ions. The obtained torque is used in a transport equation for the toroidal momentum density to estimate the effect on the thermal bulk plasma rotation profile.

  • Two worlds, one goal : A Clinician’s Perspective on Laboratory Analyses in Anticoagulant Treatment

    • Almost precisely a century ago, in the 1920s and 1930s, cattle bled to death in North America after being fed moldy hay containing sweet clover, the yellow Melilotus officinalis, and the white Melilotus albus. The toxic substance in the hay inhibiting blood coagulation was identified and named dicumarol. Further development resulted in warfarin, an oral anticoagulant that has been used for over 70 years and still is, even though newer direct-acting oral anticoagulants (DOACs) are mainly replacing it. For some patients, warfarin is still the drug of choice. A safe warfarin treatment needs repeated blood sample analysis (PT-INR), and with the new DOACs come new laboratory challenges. The aim of this thesis was to investigate ways laboratory methods can contribute to improving oral anticoagulant treatment. Paper I explores genetic variants of the enzyme targeted by warfarin, VKORC1. The result shows that the haplotype VKORC1*2 is the most important of the VKORC1 haplotypes for warfarin dosage, with a lower dose requirement. The VKORC1*2 haplotype was also related to more unstable PT-INR levels. Paper II describes a cross-section study comparing warfarin treatment control, as PT-INRs within the intended therapeutic range, in primary health care centers (PHCCs) and specialized anticoagulation clinics (ACCs). Both settings showed good therapeutic control, with at least as good therapeutic control in the PHCCs as in the ACCs. Today, almost all warfarin treatment in our region is centralized to ACCs. Paper III focuses on the modification of a point-of-care PT method. A ratio of PT from two different dilutions of each patient sample was calculated and used as an indirect measure of DOAC activity. There were close correlations between the PT ratio and drug concentrations measured at the hospital laboratory. The detection level varies between DOACs and may limit its use in some situations. Paper IV evaluated the MRX PT DOAC, an assay based on the PT ratio principle. It was found to be able to detect potentially interfering DOAC levels in plasma samples. Confirmatory testing is recommended, as is sensitivity improvement for the detection of specific interferences.

    • ICRH is often used in experiments to simulate destabilization of Alfvén eigenmodes by thermonuclear α-particles. Whereas the slowing down distribution of α-particles is nearly isotropic, the ICRH creates an anisotropic distribution function with non-standard orbits. The ICRH does not only build up gradients in phase space, which destabilizes the AEs, but it also provides a strong phase decorrelation mechanism between ions and AEs. Renewal of the distribution function by thermonuclear reactions and losses of α-particles to the wall lead to a continuous drive of the AEs. Simulations of the non-linear dynamics of AEs and the impact they have on the heating profile due to particle redistribution are presented.

    • Alfvén eigenmodes (AEs) excited by α particles in a burning plasma can degrade the heating efficiency by spatial redistribution of the resonant α particles. Changes of the orbit invariants in phase space by collisions and other waves, such as magnetosonic waves during ion cyclotron resonance heating (ICRH), lead to changes in the phase between the αs and AEs, causing a decorrelation of the interactions. ICRH lead to an increased decorrelation of the AE interactions and hence a stronger radial redistribution of the thermonuclear α particles by the AEs. Renewal of the distribution function by thermonuclear reactions and losses of α particles to the wall lead to a continuous drive of the AEs and a radial redistribution of the α particles. The redistribution results in a degradation of the heating efficiency.

Changes of the orbit invariants in phase space by collisions and interactions with other waves, such as magnetosonic waves during ion cyclotron resonance heating (ICRH), lead to changes in the phase between the α-particles and AEs, causing a decorrelation of the interactions and stronger redistribution of the α-particles. Cyclotron interactions increase the decorrelation of the AE interactions with the high-energy ions and hence a stronger radial redistribution of the high-energy α-particles by the AEs. Renewal of the distribution function by thermonuclear reactions and losses of α-particles to the wall lead to a continuous drive of the AEs and a radial redistribution of the α-particles. The condition for excitation of AEs is shown to depend on the heating scenario where heating at the low field side creates a significant population of high-energy non-standard orbits which drive the modes. The redistribution results in a reduction in the averaged α-particle energy and a degradation of the heating efficiency. The effect on the distribution function in the presence of several unstable modes is not additive and the particle redistribution is found to saturate with an increasing number of modes.

    • In this single-center cohort study, we applied a panel of laboratory markers to characterize hemostatic function in 217 consecutive patients that underwent testing for COVID-19 as they were admitted to Linkoping ¨ University Hospital between April and June 2020. In the 96 patients that tested positive for SARS-CoV-2 (COVID-19+), the cumulative incidences of death and venous thromboembolism were 24.0% and 19.8% as compared to 12.4% (p = 0.031) and 11.6% (p = 0.13) in the 121 patients that tested negative (COVID-19− ). In COVID-19+ patients, we found pronounced increases in plasma levels of von Willebrand factor (vWF) and fibrinogen. Excess mortality was observed in COVID-19+ patients with the following aberrations in hemostatic markers: high D-dimer, low antithrombin or low plasmin-antiplasmin complex (PAP) formation, with Odds Ratios (OR) for death of 4.7 (95% confidence interval (CI95) 1.7–12.9; p = 0.003) for D-dimer >0.5 mg/L, 5.9 (CI95 1.8–19.7; p = 0.004) for antithrombin (AT) ˂0.85 kIU/l and 4.9 (CI95 1.3–18.3; p = 0.019) for PAP < 1000 μg/L. Compounding increases in mortality was observed in COVID-19+ patients with combined defects in markers of fibrinolysis and coagulation, with ORs for death of 15.7 (CI95 4.3–57; p < 0.001) for patients with PAP <1000 μg/L and D-dimer >0.5 mg/L and 15.5 (CI95 2.8–87, p = 0.002) for patients with PAP <1000 μg/L and AT ˂0.85 kIU/L. We observed an elevated fraction of incompletely degraded D-dimer fragments in COVID-19+ patients with low PAP, indicating impaired fibrinolytic breakdown of cross-linked fibrin.

    • Aim:  To investigate the association between perinatal risk factors and neonatal complications and early oculo-motor development in very preterm infants. Methods:  Perinatal risk factors were identified, and the potential association with early oculo-motor development was evaluated by measuring smooth pursuit eye movements (SP) at 2 and 4 months' corrected age (CA) in a population of very preterm infants born in Uppsala County 2004-2007 (n = 113). Results:  Among the 15 tested factors, eight showed significant association in univariate analysis with lower levels of SP at 4 months' CA, namely administration of prenatal corticosteroids, gestational age, birthweight, bronchopulmonary dysplasia, retinopathy of prematurity, periventricular leukomalacia, intraventricular haemorrhage >grade 2, and persistent ductus arteriosus. At 2 months' CA, only retinopathy of prematurity >stage 2 was associated with lower levels of SP. When all factors significant in the univariate tests were included in multiple regressions aimed to assess each factor's independent relation to SP, periventricular leukomalacia was the only significant independent factor. When adding 2-5 of the significant factors using multiple regression analysis, the levels of SP became lower. Conclusion:  Perinatal risk factors were associated with lower levels of SP. This could be interpreted as delayed or disturbed development of normal oculomotor ability.

    • Aim:  To investigate early oculo-motor development in a population-based cohort of very preterm infants.Methods:  Early oculo-motor development was prospectively studied by measuring smooth pursuit eye movements at 2 and 4 months corrected age in a population of very preterm infants born in Uppsala County 2004–2007. Eighty-one preterm infants were studied, and 32 healthy term infants constituted the control group.Results:  The study group consisted of infants with a mean gestational age of28 + 5 weeks. At 2 and 4 months corrected age, infants born very preterm showed lower gain (p < 0.001) and proportion of smooth pursuit eyemovements (p < 0.001) compared to the control group. The boys showed higher gain of smooth pursuit eye movements at both 2 and 4 months corrected age, compared to girls.Conclusions:  Oculo-motor development measured by smooth pursuit eye movements is delayed in very preterm infants at 2 and 4 months corrected age. This might be a risk factor or early indicator of later perceptual and behavioural impairment.
