  • An Integrated Development Approach for Monitoring and Simulation to Predict Functional Product Availability

    • For nearly two decades, business models such as Functional Products have been in focus within research and of interest in the manufacturing industry. Functional product offers consist of hardware, software, service -support systems and management of operation which, when developed in an integrated manner, together provide the customer with an agreed-upon function with a specified level of availability. Compared to product-oriented sales, this type of business model can provide added value to customers, usually through an increase in the service content. Due to the total care commitment, offering Functional Products requires management of reliability and maintainability in order to meet the availability requirement of the function provided. The development of the Functional Product must include holistic analysis and prediction of the functional product availability performance to reduce technical and economic risks and ensure that the function is delivered according to contract. The research performed in this thesis presents an integrated development approach for monitoring and simulation to predict functional product availability. It is shown how the constituents of a functional product can be modelled in an integrated manner in order to simulate and predict functional product availability. A part of this modelling strategy is demonstrated through a simulation case example to show that is possible through this approach to evaluate the availability of different functional product designs. To support the development of the monitoring capability needed for availability simulations it is shown how it is possible to develop fault detection and diagnosis methods for fault detection systems based on data stream management systems. It is also shown how data stream forecasting can be used to predict failures due to faults occurring at short notice. Different fault detection methods have been developed, tested and evaluated on real industrial applications to verify applicability as queries on data streams, managed by data stream management systems. The results from these tests have been evaluated for their predictive performance and detection accuracy. Finally, methodological and technological approaches to monitoring and analysis in functional product development and similar business models to functional products are reviewed. The results showed that few research contributions address the information perspective in functional product development and similar business models holistically. The integrated development approach presented is a pragmatic approach to functional product development which is based on the merged research results of the papers included and knowledge domain presented.

  • Outcome of Radical Surgical Treatment of Abdominal Aortic Graft and Endograft Infections Comparing Extra-anatomic Bypass with In Situ Reconstruction : A Nationwide Multicentre Study

    • Objective: Abdominal aortic graft and endograft infection (AGI) is primarily treated by resection of the infected graft and restoration of distal perfusion through extra-anatomic bypass (EAB) or in situ reconstruction/repair (ISR). The aim of this study was to compare these surgical strategies in a nationwide multicentre retrospective cohort study.Methods: The Swedish Vascular Registry (Swedvasc) was used to identify surgically treated abdominal AGIs in Sweden between January 1995 and May 2017. The primary aim was to compare short and long term survival, as well as complications for EAB and ISR.Results: Some 126 radically surgically treated AGI patients were identified – 102 graft infections and 24 endograft infections – treated by EAB: 71 and ISR: 55 (23 neo-aorto-iliac systems, NAISs). No differences in early 30 day (EAB 81.7% vs. ISR 76.4%, p =.46), or long term five year survival (48.2% vs. 49.9%, p =.87) were identified. There was no survival difference comparing NAIS to other ISR strategies. The frequency of recurrent graft infection during follow up was similar: EAB 20.3% vs. ISR 17.0% (p =.56). Survival and re-infection rates of the new conduit did not differ between NAIS and other ISR strategies. Age ≥ 75 years (odds ratio [OR] 4.0, confidence interval [CI] 1.1 – 14.8), coronary artery disease (OR 4.2, CI 1.2 – 15.1) and post-operative circulatory complications (OR 5.2, CI 1.2 – 22.5) were associated with early death. Prolonged antimicrobial therapy (> 3 months) was associated with reduced long term mortality (HR 0.3, CI 0.1 – 0.9).Conclusion: In this nationwide multicentre study comparing outcomes of radically treated AGI, no differences in survival or re-infection rate could be identified comparing EAB and ISR.

  • Semi-Conservative Treatment Versus Radical Surgery in Abdominal Aortic Graft and Endograft Infections

    • Objective: Abdominal aortic graft and endograft infections (AGIs) are rare complications following aortic surgery. Radical surgery (RS) with resection of the infected graft and reconstruction with extra-anatomical bypass or in situ reconstruction is the preferred therapy. For patients unfit for RS, a semi-conservative (SC), graft preserving strategy is possible. This paper aimed to compare survival and infection outcomes between RS and SC treatment for AGI in a nationwide cohort.Methods: Patients with abdominal AGI related surgery in Sweden between January 1995 and May 2017 were identified. The Management of Aortic Graft Infection Collaboration (MAGIC) criteria were used for the definition of AGI. Multivariable regression was performed to identify factors associated with mortality.Results: One hundred and sixty-nine patients with surgically treated abdominal AGI were identified, comprising 43 SC (14 endografts; 53% with a graft enteric fistula [GEF] in total) and 126 RS (26 endografts; 50% with a GEF in total). The SC cohort was older and had a higher frequency of cardiac comorbidities. There was a non-significant trend towards lower Kaplan -Meier estimated five year survival for SC vs. RS (30.2% vs. 48.4%; p = .066). A non-significant trend was identified towards worse Kaplan -Meier estimated five year survival for SC patients with a GEF vs. without a GEF (21.7% vs. 40.1%; p = .097). There were significantly more recurrent graft infections comparing SC with RS (45.4% vs. 19.3%; p < .001). In a Cox regression model adjusting for confounders, there was no difference in five year survival comparing SC vs. RS (HR 1.0, 95% CI 0.6 -1.5).Conclusion: In this national AGI cohort, there was no mortality difference comparing SC and RS for AGI when adjusting for comorbidities. Presence of GEF probably negatively impacts survival outcomes of SC patients. Rates of recurrent infection remain high for SC treated patients.

    • Within the growing order of Chlamydiales, there are a number of pathogens. One is Chlamydia psittaci, a zoonotic pathogen, with birds as natural hosts that may be transmitted to humans and cause severe respiratory disease, psittacosis. The prevalence of this pathogen in Antarctic birds is almost unknown as well as the ramifications of its potential spread in na < ve bird populations. To investigate the prevalence of chlamydia organisms, cloacal and fecal samples were collected from 264 penguins and 263 seabirds on the Antarctic Peninsula and in Southern Chile. No C. psittaci could be detected by 23S rRNA real-time PCR. However, DNA sequencing of the 16S rRNA 298-bp signature sequence revealed a Chlamydiaceae-like bacterium previously found in seabirds from the subarctic zone, demonstrating that this not yet fully characterized bacterium is widespread. In conclusion, the prevalence of C. psittaci among wild birds on the Antarctic Peninsula seems to be low, but other types of chlamydial organisms are common. Further studies are required to taxonomically define and finally understand the role of these non-classified Chlamydiae.

    • This Letter presents a search for the Standard Model Higgs boson in the decay channel H -> ZZ((*)) -> l(+)l(-)l(-)+l(')-, where l,l' = e or mu, using proton-proton collisions at root s = 7 TeV recorded with the ATLAS detector and corresponding to an integrated luminosity of 4.8 fb(-1). The four-lepton invariant mass distribution is compared with Standard Model background expectations to derive upper limits on the cross section of a Standard Model Higgs boson with a mass between 110 GeV and 600 GeV. The mass ranges 134-156 GeV, 182-233 GeV, 256-265 GeV and 268-415 GeV are excluded at the 95% confidence level. The largest upward deviations from the background-only hypothesis are observed for Higgs boson masses of 125 GeV, 244 GeV and 500 GeV with local significances of 2.1, 2.2 and 2.1 standard deviations, respectively. Once the look-elsewhere effect is considered, none of these excesses are significant.
