    • Introduction: Impaired isometric muscle strength was previously reported in adults with Fontan circulation. However, it is unclear if this impairment is present in children and adolescents with Fontan circulation. We investigated isometric muscle strength of the lower limb in patients (6–18 years) with Fontan circulation in comparison with healthy controls.Method: In this cross-sectional study, 43 patients (6–18 years) with Fontan circulation and 43 age- and sex-matched controls were included. Isometric knee extension and plantar flexion muscle strength were assessed using dynamometry (Newton, N). Lean mass of the legs was assessed with dual-energy X-ray absorptiometry. Analyses were performed on group level (n = 43), and for subgroups that included children aged 6–12 years (n = 18) and adolescents aged 13–18 years (n = 25).Results: On group level, the patients with Fontan circulation had impaired isometric knee extension strength in comparison with the controls (p = 0.03). In subgroup analyses, impaired isometric knee extension strength was present in the adolescents (p = 0.009) but not in the children groups. For plantar flexion, there was no difference between patients and controls. There was no difference in lean mass between patients and controls (9.6 ± 4.3 kg vs. 10.8 ± 5.6 kg, p = 0.31). However, the lean mass was highly correlated to isometric knee extension strength (patients r = 0.89, controls r = 0.96, p < 0.001) and isometric plantar flexion strength (patients r = 0.7, controls r = 0.81, p < 0.001).Conclusion: The finding of impaired isometric knee extension muscle strength in adolescents (13–18 years) with Fontan circulation and no corresponding impairment in the children group (6–12 years) could imply that isometric muscle strength gets more impaired with age.

  • Sources of radiographic distortion in conventional and computed tomography of the temporal bone.

    • OBJECTIVES: To identify those bony regions of the glenoid fossa where, due to the inclination, there is an increased risk of radiographic distortion in conventional and computed tomography (CT).METHODS: The inclination of the roof and posterior wall of the glenoid fossa was determined relative to established imaging planes. Measurements were performed on 50 corrected coronal MR and 50 axial CT images and 200 sagittal cryosections of 50 temporomandibular joints (TMJs). The location of regions with unfavourable bone wall inclination was identified using the condyle as a reference.RESULTS: The inclination of parts of the fossa roof exceeded the limit for reliable depiction in corrected sagittal and coronal planes in 40% and 8% of the joints respectively. The inclination of parts of the posterior wall of the fossa exceeded the limit for reliable depiction in corrected sagittal and in true sagittal planes in 100% and 84% of the joints respectively. In 84% of the joints the inclination exceeded the limit for reliable depiction in the axial plane. For both bone walls the regions with unfavourable inclination were in the medial part of the joint.CONCLUSIONS: The angulation of parts of the roof and posterior wall of the glenoid fossa in relation to established imaging planes makes them highly susceptible to distortion. The oblique coronal projection is well suited for depiction of the roof of the fossa and preferable to a sagittal projection. An oblique axial projection is required for the posterior wall.

  • The temporomandibular joint : Tomopraphic and CT assesment of its bone demarcations with reference to adjacent organs

    • The wall of the temporal bone separating the temporomandibular joint (TMJ) from surroundings organs, can be very thin and also have development defects. Distortion effects in the radiographic reproduction of these bone walls can result in misinterpretations when exanimating suspected pathologic changes in the region. These areas need to be radiographic identified prior to taking any invasive measures. Incorrect assessment of bone thickness may lead to serious sequelae due to the risk of penetration injury during invasive investigation or treatment of the TMJ or ear. The purpose of this project was to gain more detailed knowledge about the anatomy and topography of the TMJ with special reference to its bone demarcations regarding adjacent organs and to evaluate the tomographic and computed tomographic (CT) depiction of these bone walls. To obtain a basic analysis of the tissue anatomy and tomographic and CT reproduction of the TMJ region, autopsy specimens were studied. After CT and conventional tomography, the specimens were sectioned in a microtom. Three- dimensional orientation systems allowed identification of section depth in the radiograms and in the histologic sections, allowing the radiograms in turn to be correlated with the true anatomy. The angle of inclination relative to the perpendicular to established imaging planes the bone walls studied was examined in three projections in order to identify regions where the bone demarcation showed an unfavorable inclination regarding the possibility of valid radiographic representation. The thickness of the bone wall between the TMJ and the middle cranial fossa, measured in the thinnest part, varied between 0.08 and 3.62 mm, averaging 1.14 mm. The bone wall between the TMJ and the middle ear showed less variation in thickness ranging from 0.00 to 1.80 mm. The thickness of the bone wall separating the TMJ from the external auditory canal varied between  1.50 mm (lateral part) and 1.21 mm (central part), with a range of between 0.21 and 4.10 mm. Development defects of this bone wall were found in 5.2 % of the examined joints. The validity in tomographic depiction of these walls was highly dependent on an optimal orientation of the bone wall in relation to the image plane. The variations in the anatomy and sagittal dimension of the external auditory canal led to variations in tomographic blurring, and suggested the need for examinations after patient repositioning in cases of suspected bone resorbing lesions so that image aberration due to unfavorable inclination of the bone wall relative to the image plane may be excluded. CT of these bone walls was valid (± 10 %) for walls thicker than approximately 1 mm, forming an angle of less than 35® with the perpendicular to the scan plane when the bone wall thickness was determined as the full-width-at-half-maximum (FWHM). For bone walls thinner than 1 mm, and for those thicker than 1 mm and at an angle exceeding 35®, partial volume averaging effects resulted in a progressively increasing magnification of bone dimensions. Observer estimations of bone thickness from images obtained using conventional bone window settings (c=400, W=2000) showed good agreement for bone walls thicker than 1 mm and with an angle of inclination relative to the perpendicular to the image plane of less than approximately 25®. For bone walls thinner than 1 mm and for thicker than 1 mm with an inclination exceeding approximately 25®, the estimations resulted in a progressively increasing overestimation amounting 200% for gracile bone walls with an inclination of 45® to 50®. Determination of width or absence of the central white zone in images obtained with the described parameters could help to reduce the risk of overestimation of bone thickness. A considerable part of the bone walls separating the TMJ from the middle cranial fossa and the external auditory canal/middle ear, respectively, have dimensions and inclinations to established imaging planes used at TMJ examinations that make the depiction of these walls highly susceptible to image distortion.

  • Validity of computed tomography in imaging thin walls of the temporal bone

    • OBJECTIVES: To evaluate the validity of computed tomography (CT) for reproduction of the bone margins of the temporomandibular joint (TMJ).METHODS: Seven TMJ specimens were examined with a CT and then cryosectioned. The bone separating the TMJ from the middle cranial fossa, middle ear and external auditory canal was measured as the full width at half maximum (FWHM). Measurements were compared with the true thickness of the bone wall.RESULTS: There was good agreement when the bone walls were thicker than 1 mm: accuracy was influenced only by the angle of the bone wall to the scanning plane. Conversely, bone walls thinner than 1 mm were reproduced with a magnification that increased with decreasing bone thickness. The difference increased further as the inclination of the bone wall became greater.CONCLUSION: Measurements performed at FWHM are reliable within +/- 10% for bone walls more than approximately 1 mm thick which form an angle of less than 35 degrees to the perpendicular of the scanning plane. For bone walls thinner than 1 mm and for those thicker than 1 mm with an inclination exceeding approximately 35 degrees, partial volume effects result in a progressively increasing magnification of bone thickness.

  • Deterioration of intraoral hole size identification after treatment of oral and pharyngeal cancer.

    • Thirty-one patients with a diagnosed malignant tumour of the oral cavity or pharynx were tested in hole size identification on four test occasions: before all treatment, after radiotherapy and 6 months and 1 year after surgical treatment. They were compared within groups as well as with a group of healthy reference individuals of the same age who underwent the same test procedure at a 2 months' interval. The oral group did not decline in hole size identification after radiotherapy, but did after surgery. The deterioration was persistent 1 year after surgery. The pharyngeal group did not change performance in hole size identification after radiotherapy, nor after surgery. It is obvious that surgery of the oral structures causes the deterioration. No correlation with damage to the lingual nerve could be registered. The oral cavity reacts as one unit, despite sensory input from two sides. The non-operated side does not compensate for the operated side. It is plausible that decreased oral sensory acuity, in recognizing hole size of the bolus, contributes to postoperative swallowing problems.

    • Thirty patients with diagnosed malignant tumors of the oral cavity or pharynx were tested in regards to intraoral shape recognition at 4 test occasions: before all treatment, after radiotherapy, 6 months after surgery, and 1 year after surgery. They were compared within groups as well as with a group of healthy reference individuals of the same age who underwent the same test procedure at a 2-month interval. The tumor itself did not influence the capability of shape recognition. The reference individuals demonstrated significantly better results on the second test occasion, which is known as a learning effect. Learning improvement was not seen in the patients whose second test occasions were after radiotherapy, implying an impediment amounting to the magnitude of the learning effect. At 6 months after surgery the patients' capabilities of shape recognition had deteriorated significantly with no difference between the oral cancer group and the pharyngeal cancer group. No spontaneous rehabilitation had taken place 1 year after surgery. The presence or absence of surgical lingual nerve damage did not influence the results. The nonoperated side does not compensate for the operated one. It is plausible that decreased oral sensory acuity in recognizing the shape of the bolus contributes to postoperative swallowing problems.

  • Impairment of intra-oral sensation, discrimination ability, and swallowing function following radiotherapy and surgery for oral and pharyngeal cancer

    • Oral and pharyngeal cancer is commonly treated with a combination of radiotherapy and surgery. It is a clinical knowledge that patients often experience severe swallowing disorders following treatment. Since surgical sequelae are instantaneous and obvious, little attention has been paid to other concurrent effects of the treatment. To shed light on this subject, the aim of this thesis was twofold (i) to make a retrospective inventory of the sequelae following treatment and (ii) to perform a prospective, inceptive examination at diagnosis, and to follow-up after radiotherapy, six months and 12 months after surgery. The files of ninety-nine patients revealed that following treatment one-third had to use gastric fistulas and more than nine of ten patients had restricted swallowing capacity. Every second patient could only swallow puréed or liquid food. Adequate intra-oral sensation and discrimination ability is essential for bolus preparation and bolus control, for appropriate elicitation of the swallowing reflex and, hence, for the oral phase of swallowing. At the inceptive examination, the prospective part of the study demonstrated intra-oral discrimination ability in patients was equal to that in healthy controls but was impaired six months after treatment and there was no significant improvement after 12 months. It had been expected that the patient’s healthy, non-tumor side would compensate but it did not. An explanation was found when it was revealed that radiotherapy induced a delayed decline in intra-oral sensation. Sensory decline was not demonstrated within a month after radiotherapy but was manifest six months later. Since the radiotherapy field includes the neck, because of the risk for metastasis, it is highly plausible that pharyngeal sensation declines in a manner corresponding to that found intra-orally when the healthy side is irradiated. In accord with this presumption, pharyngeal swallowing function deteriorated in patents with oral tumors. Cineradiographic evaluation of oral and pharyngeal swallowing function disclosed a significant association between the degree of swallowing dysfunction and the degree of sensory decline and with the degree of impairment of shape recognition. Conclusions: Delayed intra-oral sensory decline, found to be induced by radiotherapy, can be expected to appear in the entire radiation field, including the oral cavity and the pharynx, with adverse effect on swallowing. Testing intra-oral sensation close to the last radiotherapy session is not advisable, because sensory decline does not develop immediately after radiotherapy but manifests after six months. Spontaneous sensory rehabilitation cannot be expected after six months. The significant association between degree of swallowing dysfunction and degree of intra-oral sensory decline and impaired discrimination ability must be considered in the quest for functional rehabilitation of patients treated for oral or pharyngeal cancer.

    • BACKGROUND: Patients with unilateral oral or pharyngeal cancer often receive bilateral radiotherapy because of the potential for metastases. Because postoperative sequelae are evident on the tumor side, to date little attention has been paid to sensory alterations after radiotherapy on the healthy, nontumor side. The objective of this study was to investigate possible sensory alterations. METHODS: Intraoral sensation was tested bilaterally at standardized sites in 27 patients and 20 controls. Preoperative radiotherapy was bilateral in 19 patients and unilateral in eight patients. Patients were tested before treatment, after radiotherapy, and after surgery at 6 months and 1 year. Comparisons were performed interindividually and intraindividually and between groups. RESULTS: A delayed deterioration of sensation was revealed on the nontumor side 6 months after radiotherapy. There was no recovery 1 year after treatment. CONCLUSIONS: Intraoral sensation cannot be evaluated directly after radiotherapy. It is plausible that sensory deterioration after radiotherapy has an impact on functional rehabilitation after tumor treatment.

  • Bilateral TMJ Disk Displacement Induces Mandibular Retrognathia

    • Unilateral non-reducing TMJ disk displacement has been shown to retard mandibular growth on the ipsilateral side, with facial asymmetry a sequela. We hypothesized that bilateral affliction would impair mandibular growth bilaterally, generating mandibular retrognathia. Non-reducing TMJ disk displacement was surgically created in 10 growing New Zealand White rabbits. Ten additional rabbits served as a sham-operated control group. Facial growth was followed in serial cephalograms, with tantalum implants, during a period corresponding to childhood and adolescence in man. The results verified that bilateral non-reducing TMJ disk displacement retarded mandibular growth bilaterally, the extent corresponding to mandibular retrognathia in man. Maxillary growth was also retarded, but to a lesser degree. Growth impairment fluctuated over time, the most striking retardation occurring during periods of general growth acceleration. This should be taken into consideration when orthodontic treatment, aimed at stimulating mandibular growth, is initiated in adolescent individuals with non-reducing TMJ disk displacement.
