  • Leptin independently predicts development of sepsis and its outcome

    • Background: Sepsis is a life-threatening condition and obesity is related to the clinical outcome. The underlying reasons are incompletely understood, but the adipocyte derived hormones leptin and adiponectin may be involved.Methods: Patients aged 18 years or more with documented first time sepsis events were included in a nested case-referent study if they had participated in previous health surveys. Two matched referents free of known sepsis were identified. Circulating levels of leptin and adiponectin were determined in stored plasma, and their impact on a future sepsis event and its outcome was evaluated.Results: We identified 152 patients (62% women) with a sepsis event and a previous participation in a health survey. Eighty-three % had also blood samples from the acute event. Hyperleptinemia at health survey associated with a future sepsis event (OR 1.77, 95% CI 1.04-3.00) and with hospital death. After adjustment for BMI leptin remained associated with sepsis in men, but not in women. High levels in the acute phase associated with increased risk for in hospital death in women (OR 4.18, 95% CI 1.17-15.00), while being protective in men (OR 0.05, 95% CI 0.01-0.48). Furthermore, leptin increased more from baseline to the acute phase in men than in women. Adiponectin did not predict sepsis and did not relate to outcome.Conclusions: Hyperleptinemia independently predicted the development of sepsis and an unfavourable outcome in men, and inertia in the acute response related to worse outcome.

    • We report on the final electroweak measurements performed with data taken at the Z resonance by the experiments operating at the electron-positron colliders SLC and LEP. The data consist of 17 million Z decays accumulated by the ALEPH, DELPHI, L3 and OPAL experiments at LEP, and 600 thousand Z decays by the SLID experiment using a polarised beam at SLC. The measurements include cross-sections, forward-backward asymmetries and polarised asymmetries. The mass and width of the Z boson, m(Z) and Gamma(Z), and its couplings to fermions, for example the p parameter and the effective electroweak mixing angle for leptons, are precisely measured: m(Z) = 91.1875 +/- 0.0021 GeV, Gamma(Z) = 2.4952 +/- 0.0023 GeV, rho(l) = 1.0050 +/- 0.0010, sin(2)theta(eff)(lept) = 0.23153 +/- 0.00016. The number of light neutrino species is determined to be 2.9840 +/- 0.0082, in agreement with the three observed generations of fundamental fermions. The results are compared to the predictions of the Standard Model (SM). At the Z-pole, electroweak radiative corrections beyond the running of the QED and QCD coupling constants are observed with a significance of five standard deviations, and in agreement with the Standard Model. Of the many Z-pole measurements, the forward-backward asymmetry in b-quark production shows the largest difference with respect to its SM expectation, at the level of 2.8 standard deviations. Through radiative corrections evaluated in the framework of the Standard Model, the Z-pole data are also used to predict the mass of the top quark, m(t) = 173(+10)(+13) GeV, and the mass of the W boson, m(W) = 80.363 +/- 0.032 GeV. These indirect constraints are compared to the direct measurements, providing a stringent test of the SM. Using in addition the direct measurements of m(t) and m(W), the mass of the as yet unobserved SM Higgs boson is predicted with a relative uncertainty of about 50% and found to be less than 285 GeV at 95% confidence level.     • Denna inventering av violgubbe (Gomphus clavatus) och rödlistade fjälltaggsvampar (Sarcodon spp.) har utförts på uppdrag av Länsstyrelsen i Västmanlands län. Syftet har varit att öka kunskapen om arternas förekomst i länet samt behovet av skydd och andra åtgärder på de olika lokalerna. Inventeringen har genomförts inom ramen för åtgärdsprogrammen (ÅGP) för bevarande av violgubbe och rödlistade fjälltaggsvampar. Inventeringen har genomförts under september 2008 av Leif Andersson, Kurt- Anders Johansson, Rolf-Göran Carlsson, Stig Jacobsson och Tomas Fasth. Alla kända lokaler för violgubbe och rödlistade fjälltaggsvampar besöktes. Även lokaler för arter med likartade miljökrav har besökts och områden kända för kalkrik berggrund. Under inventeringen har mycel för de eftersökta arterna noterats och koordinater för dessa tagits. Även andra svampar har noterats om än i olika omfattning. Totalt har ett hundratal lokaler besökts varav svampar har noterats på 80 lokaler. Säsongen var mycket god för fjälltaggsvampar, möjligen med undantag för brödtaggsvamp (Sarcodon versipellis). För violgubbe var säsongen något sämre. Totalt noterades violgubbe och/eller rödlistade fjälltaggsvampar på 17 lokaler. Av de 19 kända lokalerna för violgubbe noterades den på sex och en ny lokal upptäcktes. Tidigare var 20 lokaler kända för de olika rödlistade fjälltaggsvamparna varav återfynd gjordes på sex av dessa. Dessutom gjordes 14 nya fynd av rödlistade fjälltaggvampar. Nya för Västmanlands län är blåfotad taggsvamp (Sarcodon glaucopus), sammetstaggsvamp (Sarcodon martioflavus) och den ännu obeskrivna Sarcodon ”modestum”. Den goda svamptillgången 2008 gjorde att särskilt många rödlistade spindelskivlingar noterades. Flera arter är nya för Västmanlands län. Även andra nyfynd av rödlistade svampar för Västmanlands län gjordes t. ex. narrvaxskivlingen (Hygrophorus subviscifer) som noterades på två platser. Totalt gjordes 146 fynd av 42 rödlistade svampar inklusive violgubbe och rödlistade fjälltaggsvampar. Ett antal lokaler och områden har kristalliserats ut som särskilt intressanta för de eftersökta svamparna. I Sala kommun finns många värdefulla områden, särskilt nära tätorten, ofta i närheten av där gruvdrift bedrivits. Särskilt Snarpsätraskogen norr om staden samt områden söder och sydväst om staden hyste många hotade arter. Områdena söder om staden ingår i Salakalkens Natura 2000-område, men skogsnaturtyperna är i dagsläget inte anmälda som naturtyp. I Fagersta och Norberg finns också värdefulla stråk för violgubbe och rödlistade fjälltaggsvampar. Särskilt ska här framhållas Jättåsarna i Fagersta kommun. Flera områdena är inte utpekade som naturvärden eller nyckelbiotoper och saknar också formellt områdesskydd. Ett antal lokaler för de eftersökta svamparna finns i naturreservat och biotopskydd.

  • High-risk HPV and survival in patients with oral and oropharyngeal squamous cell carcinoma : 5-year follow up of a population-based study

    • CONCLUSION: No statistically significant 5-year survival difference was seen in patients with oral and oropharyngeal squamous cell carcinoma (OOPSCC) between high-risk HPV-positive and -negative groups in this population-based study. OBJECTIVES: To see if the formerly observed higher risk for recurrence or second primary tumour (SPT) in high-risk HPV-positive patients with OOPSCC corresponds to worse survival. METHODS: A total of 128 consecutive, previously untreated patients with OOPSCC, who were part of a population-based case-control study in southern Sweden during 2000-2004, were included. A mouthwash sample was collected and exfoliated cells were collected with cotton-tipped swabs from the tonsillar fossa and the tumour. Specimens were analysed for HPV DNA using nested polymerase chain reaction (PCR). Disease-specific survival (DSS) and DSS difference between HPV-negative and HPV-positive patients were calculated. The relationship between age, stage, high-risk HPV status and DSS was assessed. Oral and oropharyngeal tumours were assessed separately. RESULTS: Mean DSS in months was 80.7/68.6 (high-risk HPV-negative/high-risk HPV-positive) for oral cavity tumours (p = 0.18) and 67.6/78.3 (high-risk HPV-negative/high-risk HPV-positive) for oropharyngeal tumours (p = 0.47). For oral cavity tumours, age, T status, N status and stage all showed significant differences in DSS. For oropharyngeal tumours, no significant difference regarding DSS was found.

  • Comparisons between comorbid conditions and health care consumption in rheumatoid arthritis patients with or without biological disease-modifying antirheumatic drugs : a register-based study

    • BACKGROUND: Symptoms and prognosis of patients with rheumatoid arthritis (RA) have improved with more intensive therapy, including the biological disease-modifying anti-rheumatic drugs (bDMARDs). Real life data concerning how comorbidities are distributed among patients treated or not treated with bDMARDs are scarce. Our objective was to investigate differences in comorbidity and health care consumption in RA patients, with and without bDMARDs.METHODS: This cross-sectional study was performed in the Southwestern part of Sweden. Patients, aged ≥ 18 years and diagnosed with RA in secondary health care during 2009-2010, were identified in the regional health care database. Aggregated data of comorbidity and health care consumption were retrieved between 2006 and 2010. RA patients treated with bDMARDs on 31st December 2010 were identified in the Swedish Rheumatology Quality Register (SRQ), which includes the biologics register Anti-Rheumatic Therapy in Sweden (ARTIS). Descriptive, comparative, univariate and multiple logistic regression analyses were used to identify factors associated with bDMARDs.RESULTS: Seven thousand seven hundred and twelve (7712) RA patients were identified (age 64.8 ± 14.9 years, women 74.3%), of whom 1137 (14.7%) were treated with bDMARDs. Overall, the most common comorbidities were infections (69.2%), hypertension (41.1%), chronic respiratory disease (15.3%), ischemic heart disease (14.0%) and malignancy (13.7%). Patients without bDMARDs were older and had more comorbidity. In the multiple logistic regression analysis, older age, cerebrovascular and chronic respiratory disease, heart failure, depression and malignancy were all associated with no present bDMARDs. Infections were associated with bDMARDs. Patients treated with bDMARDs consumed more secondary outpatient care but less visits in primary health care compared to patients without bDMARDs.CONCLUSIONS: Patients treated with bDMARDs versus no bDMARDs were younger and had significantly lower period prevalence for most common comorbidities, with the exception of infections. Differences in comorbidities between RA patients with or without bDMARDs should be taken into consideration when evaluating effectiveness and safety of bDMARDs in ordinary care.

    • This article discusses the so-called verb-oriented method and its role in the research project Gender and Work in early modern Sweden (GaW), which is based at Uppsala University. It provides a presentation of the GaW-database, which has been designed to allow analysis according to the verb-method. Finally, the article points out that this method can be combined with a number of different theoretical approaches as long as the focus is on practices. It is therefore compatible with the approaches of e.g., Judith Butler, Michel de Certeau, and Amartya Sen. Work is defined as "time-use with the purpose of making a living" and the article discusses why data on time-use, or actual work activities, are better suited for research into early modern Swedish working life than other types of data. It shows that activities are usually described in the sources by verb-phrases, and explains how and from what sources verb-phrases are collected and analyzed within the project. In order to allow for generalizations the verb-method presupposes large amounts of data. This is the rationale for the GaW-database, which at present includes around 5000 verb-phrases and 75000 data posts.
