- Approaches exploiting trait distribution extremes may be used to identify loci associated with common traits, but it is unknown whether these loci are generalizable to the broader population. In a genome-wide search for loci associated with the upper versus the lower 5th percentiles of body mass index, height and waist-to-hip ratio, as well as clinical classes of obesity, including up to 263,407 individuals of European ancestry, we identified 4 new loci (IGFBP4, H6PD, RSRC1 and PPP2R2A) influencing height detected in the distribution tails and 7 new loci (HNF4G, RPTOR, GNAT2, MRPS33P4, ADCY9, HS6ST3 and ZZZ3) for clinical classes of obesity. Further, we find a large overlap in genetic structure and the distribution of variants between traits based on extremes and the general population and little etiological heterogeneity between obesity subgroups.

- Aim: To describe telenurses’ experiences of difficult calls.Design: A qualitative approach with a descriptive design was used to gain a deeper understanding of the telenurses’ experiences.Methods: The data were collected in spring 2017 through semi-structured interviews with 19 telenurses at call centres and primary healthcare centres and were analysed with qualitative content analysis.Results: Becoming emotionally concerned is central to the telenurse’s experiences of difficult calls. Difficult calls are accompanied by feelings such as inadequacy, uncertainty and anxiety, which can be described as emotional tension. Emotional tension refers to situations when the caller’s expressed emotions were conveyed to the telenurses and altered their state of mind. The telenurses stated that difficult calls that cause them to become anxious remain in their thoughts and go through their minds repeatedly, making a deep impression.

- En representativ sample drogberoende patienter på sjukhus 1988 - 89 (n=125) följdes upp vid 5 år och vid 15 år. Kliniska ata och utfallsdata registrerades vid indevårdtillfället och vid uppföljningarna med en semistrukturerad intervju omfattande demografiska karakteristika, kliniska diagnoser kompletterade med kliniska test och skattningsskalor. Registerdata används för validering och jämförelse. Säkre utfallsdata vid 5 år finns registrerade för 112 personer av hela samplet (90 %) och för 110 (88 %) vid 15 år.

Psykisk och somatisk belastning hos tvångsvårdade kvinnor – En kohortstudie av LVM- och LVU-vårdade kvinnor vid Lunden

- Data avseende bakgrund, sjukdomsbild; både fysisk och psykisk med data hämtades ur ADAD och DOK och genom intervjuer, social- och sjukvårdsjournaler i ett konsekutivt material om 216 klienter som vårdats på Lunden mellan januari 1997 till juni 2000. Totalt 115 flickor/kvinnor (80 LVM- och 52 LVU-flickor) testades med två test BCT och CMPS, psykiska symtom (SCL-90) och personlighetsstörning enligt DSM IV (SCID II). PCL-R-skattning har också gjorts. Uppväxtförhållandena präglades av sociala och psykiska problem med missbruk och psykiska problem i familjen, ensamt eller i kombination. Bland LVM- hade 60 procent och bland LVU-kvinnorna hade 70 procent försökt att begå självmord. Narkotikamissbruk med ett utpräglat blandmissbruk dominerade bilden. Större delen av de som missbrukade hade ett beroendetillstånd (DSM-IV). Opiater var vanligast bland de äldre och centralstimulantia bland de yngre. Självrapporterad kriminalitet var mycket hög; drygt 70 % av LVM- och 85 % av LVU-kvinnorna hade någon gång under sin livstid ha begått kriminella handlingar. Prostitution var vanligare bland LVM-kvinnorna (40 %) än i LVU-gruppen (20%). Drygt 70 % i bägge grupperna hade någon form av personlighetsstörning. Säkerställd depressionsdiagnos förekom i bägge grupperna bland 13 %. En ångestdiagnos kunde ställas för 23 procent av LVM-kvinnorna. Toxisk psykos fanns hos 17 % i LVM- mot 15 procent i LVU-gruppen. Psykisk sjukdom som schizofreni eller bipolär affektiv sjukdom förekom enbart hos två flickor på LVU. Andelen kvinnor med psykopati motsvarade den i ett oselekterat material av tunga narkotikamissbrukare med 5,6 % psykopati bland de yngre flickorna och 3 % i LVM-gruppen. Profilerna på personlighetstesten Basic Character Trait test (BCT) och Cesarec Marke Personality Scheme (CMPS) visar huvudsakligen normala värden med staninepoäng mellan 4 och 6 med undantag av en klar förhöjning på behov av att bli om-händertagen (succorance) och ja-sägartendens (aquiescense). SCL-90 slutligen indikerar nor-mala symtomnivåer (T < 60) på de flesta kliniska skalor.

Meta-analysis identifies 13 new loci associated with waist-hip ratio and reveals sexual dimorphism in the genetic basis of fat distribution

- Waist-hip ratio (WHR) is a measure of body fat distribution and a predictor of metabolic consequences independent of overall adiposity. WHR is heritable, but few genetic variants influencing this trait have been identified. We conducted a meta-analysis of 32 genome-wide association studies for WHR adjusted for body mass index (comprising up to 77,167 participants), following up 16 loci in an additional 29 studies (comprising up to 113,636 subjects). We identified 13 new loci in or near RSPO3, VEGFA, TBX15-WARS2, NFE2L3, GRB14, DNM3-PIGC, ITPR2-SSPN, LY86, HOXC13, ADAMTS9, ZNRF3-KREMEN1, NISCH-STAB1 and CPEB4 (P = 1.9 × 10⁻⁹ to P = 1.8 × 10⁻⁴⁰) and the known signal at LYPLAL1. Seven of these loci exhibited marked sexual dimorphism, all with a stronger effect on WHR in women than men (P for sex difference = 1.9 × 10⁻³ to P = 1.2 × 10⁻¹³). These findings provide evidence for multiple loci that modulate body fat distribution independent of overall adiposity and reveal strong gene-by-sex interactions.

- Research statement/question: The main concern in this panel (colloquium) presentation was to show how a discourse and communication based approach in the context of the health and elderly care, provides a basis for reflecting on vulnerability. How can the patient`s and/or elderly client´s vulnerability be reflected and reduced within the context of health and elderly care? How can professional vulnerability be understood in the light of patient involvement? In general, the panel (colloquium) analyzed the challenges of different kind of vulnerability in client encounters e.g. to be or not to treated as accountable.Theoretical framework: From a four-dimensional, interdisciplinary perspective. Discourse Analysis, Micro-sociology, Phenomenology and Nursing, the panel (colloquium) featured five different presentations.Methods: Different kind (video, audio and interview) of transcribed and translated data from talk encounters between care professionals and clients, have been analyzed. Used analytical methods were qualitative method of Conversation Analysis; Meta-synthesis; activity analysis and phenomenographic analysis.Findings: When looking for possible convergence between principles characterizing the approaches used by the authors and their findings, in this panel (colloquium) there are key-ideas which also have to do with what we would call communicative vulnerability. E.g., considering the vulnerability of older persons with dementia, the authors argue that a flexibility of practices may be a step towards implementing a client-centred approach in care plan meetings; it is also suggested that the caring aspect of professional skills in client encounter is to support hope; and that awareness of and understanding of the own professional vulnerability may gain understanding of implicit motives for decision-making.Conclusions:Humanization of health and eldercare professionals´ work by identifying the aspect of patients´ vulnerability is dealt with in this panel (colloquium) through carefully applied research methods from areas as discourse analysis, phenomenology, phenomenographic, and micro-sociology. This panel (colloquium) attests to relevant productivity of the researcher network: Talk, Text and Tools (3T), Scandinavian Interdisciplinary Health Communication Research.
