    • Aims: This paper describes the Heterodyne Instrument for the Far-Infrared (HIFI) that was launched onboard ESA's Herschel Space Observatory in May 2009. Methods: The instrument is a set of 7 heterodyne receivers that are electronically tuneable, covering 480-1250 GHz with SIS mixers and the 1410-1910 GHz range with hot electron bolometer (HEB) mixers. The local oscillator (LO) subsystem comprises a Ka-band synthesizer followed by 14 chains of frequency multipliers and 2 chains for each frequency band. A pair of auto-correlators and a pair of acousto-optical spectrometers process the two IF signals from the dual-polarization, single-pixel front-ends to provide instantaneous frequency coverage of 2 × 4 GHz, with a set of resolutions (125 kHz to 1 MHz) that are better than 0.1 km s-1. Results: After a successful qualification and a pre-launch TB/TV test program, the flight instrument is now in-orbit and completed successfully the commissioning and performance verification phase. The in-orbit performance of the receivers matches the pre-launch sensitivities. We also report on the in-orbit performance of the receivers and some first results of HIFI's operations. Herschel is an ESA space observatory with science instruments provided by European-led Principal Investigator consortia and with important participation from NASA.

  • Water vapor toward starless cores : The Herschel view

    • Aims: Previous studies by the satellites SWAS and Odin provided stringent upper limits on the gas phase water abundance of dark clouds (x(H2O) < 7 × 10-9). We investigate the chemistry of water vapor in starless cores beyond the previous upper limits using the highly improved angular resolution and sensitivity of Herschel and measure the abundance of water vapor during evolutionary stages just preceding star formation. Methods: High spectral resolution observations of the fundamental ortho water (o-H2O) transition (557 GHz) were carried out with the Heterodyne Instrument for the Far Infrared onboard Herschel toward two starless cores: Barnard 68 (hereafter B68), a Bok globule, and LDN 1544 (L1544), a prestellar core embedded in the Taurus molecular cloud complex. Detailed radiative transfer and chemical codes were used to analyze the data. Results: The RMS in the brightness temperature measured for the B68 and L1544 spectra is 2.0 and 2.2 mK, respectively, in a velocity bin of 0.59 km s-1. The continuum level is 3.5 ± 0.2 mK in B68 and 11.4 ± 0.4 mK in L1544. No significant feature is detected in B68 and the 3σ upper limit is consistent with a column density of o-H2O N(o-H2O) < 2.5 × 1013 cm-2, or a fractional abundance x(o-H2O) < 1.3 × 10-9, more than an order of magnitude lower than the SWAS upper limit on this source. The L1544 spectrum shows an absorption feature at a 5σ level from which we obtain the first value of the o-H2O column density ever measured in dark clouds: N(o-H2O) = (8 ± 4) × 1012 cm-2. The corresponding fractional abundance is x(o-H2O) ≃ 5 × 10-9 at radii >7000 AU and ≃2 × 10-10 toward the center. The radiative transfer analysis shows that this is consistent with a x(o-H2O) profile peaking at ≃10-8, 0.1 pc away from the core center, where both freeze-out and photodissociation are negligible. Conclusions: Herschel has provided the first measurement of water vapor in dark regions. Column densities of o-H2O are low, but prestellar cores such as L1544 (with their high central densities, strong continuum, and large envelopes) appear to be very promising tools to finally shed light on the solid/vapor balance of water in molecular clouds and oxygen chemistry in the earliest stages of star formation.   • Water abundances in high-mass protostellar envelopes : Herschel observations with HIFI

    • Aims: We derive the dense core structure and the water abundance in four massive star-forming regions in the hope of understanding the earliest stages of massive star formation. Methods: We present Herschel/HIFI observations of the para-H2O 111-000 and 202-111 and the para-H_218O 111-000 transitions. The envelope contribution to the line profiles is separated from contributions by outflows and foreground clouds. The envelope contribution is modeled with Monte-Carlo radiative transfer codes for dust and molecular lines (MC3D and RATRAN), and the water abundance and the turbulent velocity width as free parameters. Results: While the outflows are mostly seen in emission in high-J lines, envelopes are seen in absorption in ground-state lines, which are almost saturated. The derived water abundances range from 5×10-10 to 4×10-8 in the outer envelopes. We detect cold clouds surrounding the protostar envelope, thanks to the very high quality of the Herschel/HIFI data and the unique ability of water to probe them. Several foreground clouds are also detected along the line of sight. Conclusions: The low H2O abundances in massive dense cores are in accordance with the expectation that high densities and low temperatures lead to freeze-out of water on dust grains. The spread in abundance values is not clearly linked to physical properties of the sources.   • Herschel-PACS spectroscopy of the intermediate mass protostar NGC 7129 FIRS 2

    • Aims. We present preliminary results of the first Herschel spectroscopic observations of NGC 7129 FIRS2, an intermediate mass star-forming region. We attempt to interpret the observations in the framework of an in-falling spherical envelope. Methods. The PACS instrument was used in line spectroscopy mode ( R = 1000-5000) with 15 spectral bands between 63 and 185 mu m. This provided good detections of 26 spectral lines seen in emission, including lines of H2O, CO, OH, O I, and C II. Results. Most of the detected lines, particularly those of H2O and CO, are substantially stronger than predicted by the spherical envelope models, typically by several orders of magnitude. In this paper we focus on what can be learned from the detected CO emission lines. Conclusions. It is unlikely that the much stronger than expected line emission arises in the (spherical) envelope of the YSO. The region hot enough to produce such high excitation lines within such an envelope is too small to produce the amount of emission observed. Virtually all of this high excitation emission must arise in structures such as as along the walls of the outflow cavity with the emission produced by a combination of UV photon heating and/or non-dissociative shocks.

  • Diagnostic and Prognostic Utility of the Extracellular Vesicles Subpopulations Present in Pleural Effusion

    • Extracellular vesicles (EVs), comprising exosomes, microvesicles, and apoptotic bodies, are released by all cells into the extracellular matrix and body fluids, where they play important roles in intercellular communication and matrix remodeling in various pathological conditions. Malignant pleural mesothelioma (MPM) is a primary tumor of mesothelial origin, predominantly related to asbestos exposure. The detection of MPM at an early stage and distinguishing it from benign conditions and metastatic adenocarcinomas (AD) is sometimes challenging. Pleural effusion is often the first available biological material and an ideal source for characterizing diagnostic and prognostic factors. Specific proteins have previously been identified as diagnostic markers in effusion, but it is not currently known whether these are associated with vesicles or released in soluble form. Here, we study and characterize tumor heterogeneity and extracellular vesicle diversity in pleural effusion as diagnostic or prognostic markers for MPM. We analyzed extracellular vesicles and soluble proteins from 27 pleural effusions, which were collected and processed at the department of pathology and cytology at Karolinska University Hospital, representing three different patient groups, MPM (n = 9), benign (n = 6), and AD (n = 12). The vesicles were fractionated into apoptotic bodies, microvesicles, and exosomes by differential centrifugation and characterized by nanoparticle tracking analysis and Western blotting. Multiplex bead-based flow cytometry analysis showed that exosomal markers were expressed differently on EVs present in different fractions. Further characterization of exosomes by a multiplex immunoassay (Luminex) showed that all soluble proteins studied were also present in exosomes, though the ratio of protein concentration present in supernatant versus exosomes varied. The proportion of Angiopoietin-1 present in exosomes was generally higher in benign compared to malignant samples. The corresponding ratios of Mesothelin, Galectin-1, Osteopontin, and VEGF were higher in MPM effusions compared to those in the benign group. These findings demonstrate that relevant diagnostic markers can be recovered from exosomes.

  • Intersectoral and integrated approaches in achieving the right to health for refugees on resettlement: A scoping review

    • Background Better understanding, documentation and evaluation of different refugee health interventions and their means of health system integration and intersectoral collaboration are needed. Objectives Explore the barriers and facilitators to the integration of health services for refugees; the processes involved and the different stakeholders engaged in levaraging intersectoral approaches to protect refugees' right to health on resettlement. Design Scoping review. Methods A search of articles from 2000 onward was done in MEDLINE, Web of Science, Global Health and PsycINFO, Embase. Two frameworks were applied in our analysis, the â framework for analysing integration of targeted health interventions in systems' and â Health in All Policies' framework for country action. A comprehensive description of the methods is included in our published protocol. Results 6117 papers were identified, only 18 studies met the inclusion criteria. Facilitators in implementation included: Training for providers, colocation of services, transportation services to enhance access, clear role definitions and appropriate budget allocation and financing. Barriers included: Lack of a participatory approach, insufficient resources for providers, absence of financing, unclear roles and insufficient coordination of interprofessional teams; low availability and use of data, and turf wars across governance stakeholders. Successful strategies to address refugee health included: Networks of service delivery combining existing public and private services; system navigators; host community engagement to reduce stigma; translation services; legislative support and alternative models of care for women and children. Conclusion Limited evidence was found overall. Further research on intersectoral approaches is needed. Key policy insights gained from barriers and facilitators reported in available studies include: Improving coordination between existing programmes; supporting colocation of services; establishing formal system navigator roles that connect relevant programmes; establishing formal translation services to improve access and establishing training and resources for providers. © Author(s) (or their employer(s)) 2019.   • Sulphide mineral flotation : a new insight into oxidation mechanisms

    • Formation of hydrogen peroxide (H2O2), an oxidizing agent stronger than oxygen, by sulphide minerals during grinding was investigated. It was found that pyrite (FeS2), chalcopyrite (CuFeS2), sphalerite ((Zn,Fe)S), and galena (PbS), which are the most abundant sulphide minerals on Earth, generated H2O2 in pulp liquid during wet grinding in the presence or devoid of dissolved oxygen in water and also when the freshly ground solids are placed in water immediately after dry grinding. Pyrite generated more H2O2 than other sulphide minerals and the order of H2O2 production by the minerals found to be pyrite > chalcopyrite > sphalerite > galena. The pH of water influenced the extent of hydrogen peroxide formation where higher amounts of H2O2 are produced at highly acidic pH. The amount of H2O2 formed also increased with increasing sulphide mineral loading and grinding time due to increased surface area and its interaction with water. The sulphide surfaces are highly catalytically active due to surface defect sites and unsaturation because of broken bonds and capable of breaking down the water molecule leading to hydroxyl free radicals. Type of grinding medium on formation of hydrogen peroxide by pyrite revealed that the mild steel produced more H2O2 than stainless steel grinding medium, where Fe2+ and/or Fe3+ ions played a key role in producing higher amounts of H2O2.Furthermore, the effect of mixed sulphide minerals, i.e., pyrite–chalcopyrite, pyrite–galena, chalcopyrite–galena and sphalerite–pyrite, sphalerite–chalcopyrite and sphalerite–galena on the formation of H2O2 showed increasing H2O2 formation with increasing pyrite fraction in chalcopyrite–pyrite composition. In pyrite–sphalerite, chalcopyrite–sphalerite or galena–sphalerite mixed compositions, the increase in pyrite or chalcopyrite proportion, the concentration of H2O2 increased but with increase in galena proportion, the concentration of H2O2 decreased. Increasing pyrite proportion in pyrite–galena mixture, the concentration of H2O2 increased and also in the mixture of chalcopyrite–galena, the concentration of H2O2 increased with increasing chalcopyrite fraction. The results of H2O2 formation in pulp liquid of sulphide minerals and mixed minerals at different experimental conditions have been explained by Eh–pH diagrams of these minerals and the existence of free metal ions that are equally responsible for H2O2 formation besides surfaces catalytic activity. The results also corroborate the amount of H2O2 production with the rest potential of the sulphide minerals; higher is the rest potential more is the formation of H2O2. Most likely H2O2 is answerable for the oxidation of sulphide minerals and dissolution of non-ferrous metal sulphides in the presence of ferrous sulphide besides the galvanic interactions. This study highlights the necessity of revisiting into the electrochemical and/or galvanic interactions between the grinding medium and sulphide minerals, and interaction mechanisms between pyrite and other sulphide minerals in terms of their flotation behaviour in the context of inevitable H2O2 existence in the pulp liquid.

  • Intersectoral approaches and integrated services in achieving the right to health for refugees upon resettlement: a scoping review protocol

    • Introduction Global insecurity and climate change are exacerbating the need for improved management of refugee resettlement services. International standards hold states responsible for the protection of the right of non-citizens to an adequate standard of physical and mental health while recognising the importance of social determinants of health. However, programmes to protect refugees' right to health often lack coordination and monitoring. This paper describes the protocol for a scoping review to explore barriers and facilitators to the integration of health services for refugees; the content, process and actors involved in protecting refugee health; and the extent to which intersectoral approaches are leveraged to protect refugees' right to health on resettlement, especially for vulnerable groups such as women and children. Methods and analysis Peer-reviewed (through four databases including MEDLINE, Web of Science, Global Health and PsycINFO) and grey literature were searched to identify programmes and interventions designed to promote refugee health in receiving countries. Two reviewers will screen articles and abstract data. Two frameworks for integration and intersectoral action will be applied to understand how and why certain approaches work while others do not and to identify the actors involved in achieving success at different levels of integration as defined by these frameworks. Ethics and dissemination Findings from the scoping review will be shared in relevant conferences and meetings. A brief will be created with lessons learnt from successful programmes to inform decision making in design of refugee programmes and services. Ethical approval is not required as human subjects are not involved.

  • Sustainable food systems - a health perspective

    • Malnutrition in all forms, ranging from undernourishment to obesity and associated diet-related diseases, is one of the leading causes of death worldwide, while food systems often have major environmental impacts. Rapid global population growth and increases in demands for food and changes in dietary habits create challenges to provide universal access to healthy food without creating negative environmental, economic, and social impacts. This article discusses opportunities for and challenges to sustainable food systems from a human health perspective by making the case for avoiding the transition to unhealthy less sustainable diets (using India as an exemplar), reducing food waste by changing consumer behaviour (with examples from Japan), and using innovations and new technologies to reduce the environmental impact of healthy food production. The article touches upon two of the challenges to achieving healthy sustainable diets for a global population, i.e., reduction on the yield and nutritional quality of crops (in particular vegetables and fruits) due to climate change; and trade-offs between food production and industrial crops. There is an urgent need to develop and implement policies and practices that provide universal access to healthy food choices for a growing world population, whilst reducing the environmental footprint of the global food system.

    • Aim: To assess the effectiveness of a cognitive-behavioral treatment (CBT) program on weight reduction among Iranian adolescents who are overweight. Methods: Using a randomized controlled trial design, 55 adolescents who were overweight (mean [SD] age = 14.64 [1.69] years; zBMI = 2.18 [0.65]) were recruited in the CBT program and 55 in the treatment as usual (TAU; mean age = 14.88 [1.50]; zBMI = 2.09 [0.57]) group. All the participants completed several questionnaires (Child Dietary Self-Efficacy Scale; Weight Efficacy Lifestyle questionnaire; Physical Exercise Self-Efficacy Scale; Pediatric Quality of Life Inventory; and self-reported physical activity and diet) and had their anthropometrics measured (height, weight, waist and hip circumferences, and body fat). Results: The CBT group consumed significantly more fruits and juice, vegetables, and dairy in the 6-month follow-up as compared with the TAU group (p-values <0.001). The CBT group consumed significantly less sweet snacks, salty snacks, sweet drinks, sausages/processed meat, and oils in the six-month follow-up compared with the TAU group (p-values<0.001). Additionally, the waist circumference, BMI, waist-hip ratio, and fat mass were significantly decreased in the CBT group in the six-month follow-up compared with the TAU group (p-values<0.005). The CBT group significantly improved their psychosocial health, physical activity, and health-related quality of life (p-values<0.001). Conclusion: The CBT program showed its effectiveness in reducing weight among Iranian adolescents who were overweight. Healthcare providers may want to adopt this program to treat excess weight problems among adolescents.
