  • Global Biobank Meta-analysis Initiative : Powering genetic discovery across human disease

    • Biobanks facilitate genome-wide association studies (GWASs), which have mapped genomic loci across a range of human diseases and traits. However, most biobanks are primarily composed of individuals of European ancestry. We introduce the Global Biobank Meta-analysis Initiative (GBMI)-a collaborative network of 23 biobanks from 4 continents representing more than 2.2 million consented individuals with genetic data linked to electronic health records. GBMI meta-analyzes summary statistics from GWASs generated using harmonized genotypes and phenotypes from member biobanks for 14 exemplar diseases and endpoints. This strategy validates that GWASs conducted in diverse biobanks can be integrated despite heterogeneity in case definitions, recruitment strategies, and baseline characteristics. This collaborative effort improves GWAS power for diseases, benefits understudied diseases, and improves risk prediction while also enabling the nomination of disease genes and drug candidates by incorporating gene and protein expression data and providing insight into the underlying biology of human diseases and traits.

  • High therapeutic efficacy of artemether-lumefantrine and dihydroartemisinin-piperaquine for the treatment of uncomplicated falciparum malaria in Somalia

    • Background: Artemether-lumefantrine (AL) and dihydroartemisinin-piperaquine (DHA/PPQ) are the recommended first- and second-line treatments, respectively, for uncomplicated falciparum malaria in Somalia. The studies reported here were conducted to assess the efficacy of these artemisinin-based combinations and the mutations in Plasmodium falciparum K13-propeller (Pfk13) domain and amplification in Pfplasmepsin 2 (Pfpm2) gene in Somalia. Methods: One-arm prospective studies were conducted to assess the clinical and parasitological responses to DHA/PPQ and AL at two sites in 2016 and 2017, respectively, using the standard WHO protocol. The patterns of molecular markers associated with artemisinin and PPQ resistance were investigated for the first time in Somalia. Results: A total of 339 patients were enrolled with 139 for AL and 200 for DHA/PPQ. With AL, no parasite recurrence was observed among patients treated at either site, corresponding to 100% clinical and parasitological responses. For DHA-PPQ, an adequate clinical and parasitological response rate > 97% was observed. All study patients on both treatments at both sites were parasite-free on day 3. Of the 138 samples with interpretable results for the polymorphism in Pfk13, only one (0.7%), from Bosaso, contained a non-synonymous mutation (R622I), which is not one of the known markers of artemisinin resistance. No Pfpm2 amplification was observed among the 135 samples with interpretable results. Conclusions: AL and DHA/PPQ were highly effective in the treatment of uncomplicated falciparum malaria, and there was no evidence of resistance to artemisinin or PPQ. These two combinations are thus relevant in the chemotherapeutic strategy for malaria control in Somalia.

    • Hydrogen production from methanol rather than the traditional source, methane, is considered to be advantageous in ease of transportation and storage. However, the current copper-based catalysts utilized in methanol steam reforming are associated with challenges of sintering at high temperature and production of CO which could poison fuel cells. In addressing these challenges, ZnO nanorods were grown hydrothermally on the surface of cordierite and impregnated with Cu to produce catalysts for methanol steam reforming. The catalysts were characterized using SEM, XRD, FTIR, XPS, BET and Raman Spectroscopy. A fixed-bed reactor was used for testing the catalysts while the reaction products were characterized using a GC fitted with FID and TCD. The effects of temperature, methanol concentration and particle size of catalysts on methanol steam reforming were investigated. The experiments were carried out between 180 and 350 degrees C. CO selectivity of 0% was observed for temperatures between 180 and 230 degrees C for 0.8 MeOH:1H(2)O with an average H-2 selectivity of 98% for that temperature range. XPS showed that the catalyst was relatively unchanged after reaction while Raman spectroscopy revealed coke formation on the catalyst surface for reactions carried out above 300 degrees C.   • Effects of synthesis methods on performance of CuZn/MCM-41 catalysts in methanol steam reforming

    • CuZn-based catalysts are active in production of hydrogen by methanol steam reforming. However, there is a need to have further insight on their physico-chemical properties to improve selectivity to hydrogen. Therefore, a series of CuZn/MCM-41 catalysts was synthesized by four different routes; one pot hydrothermal synthesis (OPMCM), co-impregnation (COMCM), serial impregnation (SRMCM) and copper impregnated on Zn-MCM-41 (ZNMCM). Samples of catalysts were characterized by X-ray diffraction (XRD), transmission electron microscopy (TEM), scanning electron microscopy (SEM) coupled with energy dispersive X-ray spectroscopy (EDS), inductively coupled plasma (ICP) emission spectrometry, Fourier transform infrared (FTIR) spectroscopy, thermogravimetric analysis (TGA) and X-ray photoelectron spectroscopy (XPS). XRD revealed disruption in the ordered pore network typical in MCM-41 for all catalysts synthesized and also showed that the one pot synthesis catalyst had wide spread dispersion of Cu and Zn. SEM micrographs captured irregularly shaped particles of different sizes. While XPS showed that different Cu and Zn species were formed within the catalyst matrix. XPS also confirmed that there was wide spread dispersion and interaction of Cu and Zn with MCM-41 matrix in the OPMCM catalyst. COMCM and OPMCM demonstrated the highest activity with 88 and 65% methanol conversion with corresponding H2 selectivity of 91 and 86% respectively. They are better than SRMCM and ZNMCM which had average H2 selectivity of 19% and 31% respectively. CO selectivity was less than 1.8% for the COMCM and OPMCM catalysts. While SRMCM and ZNMCM had CO selectivity's as high as 8.9% and 7.2% respectively. The data generated shows that catalytic activity is largely affected by the nature of Cu species within the catalyst matrix.

  • Introducing evidence based public health practice through a journal club for public health officers in Somaliland

    • Evidence based public health is one of the basic training tools of public health students and young officers in decision making. The training tools for early career specialists and trainees in public health is journal clubs (JC). It keeps the knowledge of professionals up to date and assist them in receiving information to design, plan, implement health care services, policies and strategies.The intention of the JC team was to raise awareness of methods for public health literature search, appraising it and applying this knowledge in their daily practices. A senior public health practitioner in Somaliland (the corresponding author) invited medical students and residents to have JCs as a part of their training. They did not accept the offer so the organizer invited practicing junior public health professionals instead. The JC team members were a general practitioner and 3 public health workers from Ministry of Health, public hospital physician, public health school and field public health officer. A weekly or twice weekly journal club took place to train them in critical appraisal. This continued for 15 months in a hybrid mentorship for the health care professionals.The team mentor selected a paper for discussion. Mentees choose a study design appraisal tool from the Critical Appraisal Skills Program (CASP) that matched the study to present. In the process of appraisal, a team member led the discussion using the checklist.The mentees presented their critical appraisal either orally or via a presentation. The checklist and paper were compared for assessing the study design and structure of the paper of the week.This approach of empowering junior public health officers in Somalia is a way forward for encouraging the professionals to use evidence based practice in their daily practices. This will improve their selection of research tools and translating the scientific work into their practice and services.

  • Novel ancestry-specific primary open-angle glaucoma loci and shared biology with vascular mechanisms and cell proliferation

    • Primary open -angle glaucoma (POAG), a leading cause of irreversible blindness globally, shows disparity in prevalence and manifestations across ancestries. We perform meta -analysis across 15 biobanks (of the Global Biobank Meta -analysis Initiative) (n = 1,487,441: cases = 26,848) and merge with previous multiancestry studies, with the combined dataset representing the largest and most diverse POAG study to date (n = 1,478,037: cases = 46,325) and identify 17 novel significant loci, 5 of which were ancestry specific. Gene -enrichment and transcriptome-wide association analyses implicate vascular and cancer genes, a fifth of which are primary ciliary related. We perform an extensive statistical analysis of SIX6 and CDKN2B-AS1 loci in human GTEx data and across large electronic health records showing interaction between SIX6 gene and causal variants in the chr9p21.3 locus, with expression effect on CDKN2A/B. Our results suggest that some POAG risk variants may be ancestry specific, sex specific, or both, and support the contribution of genes involved in programmed cell death in POAG pathogenesis.
