  • Circulating concentrations of biomarkers and metabolites related to vitamin status, one-carbon and the kynurenine pathways in US, Nordic, Asian, and Australian populations

    • Background: Circulating concentrations of biomarkers that are related to vitamin status vary by factors such as diet, fortification, and supplement use. Published biomarker concentrations have also been influenced by the variation across laboratories, which complicates a comparison of results from different studies. Objective: We robustly and comprehensively assessed differences in biomarkers that are related to vitamin status across geographic regions. Design: The trial was a cross-sectional study in which we investigated 38 biomarkers that are related to vitamin status and one-carbon and tryptophan metabolism in serum and plasma from 5314 healthy control subjects representing 20 cohorts recruited from the United States, Nordic countries, Asia, and Australia, participating in the Lung Cancer Cohort Consortium. All samples were analyzed in a centralized laboratory. Results: Circulating concentrations of riboflavin, pyridoxal 5'-phosphate, folate, vitamin B-12, all-trans retinol, 25-hydroxyvitamin D, and a-tocopherol as well as combined vitamin scores that were based on these nutrients showed that the general B-vitamin concentration was highest in the United States and that the B vitamins and lipid soluble vitamins were low in Asians. Conversely, circulating concentrations of metabolites that are inversely related to B vitamins involved in the one-carbon and kynurenine pathways were high in Asians. The high B-vitamin concentration in the United States appears to be driven mainly by multivitamin-supplement users. Conclusions: The observed differences likely reflect the variation in intake of vitamins and, in particular, the widespread multivitamin-supplement use in the United States. The results provide valuable information about the differences in biomarker concentrations in populations across continents.

  • Impaired functional vitamin B6 status is associated with increased risk of lung cancer

    • Circulating vitamin B6 levels have been found to be inversely associated with lung cancer. Most studies have focused on the B6 form pyridoxal 5'-phosphate (PLP), a direct biomarker influenced by inflammation and other factors. Using a functional B6 marker allows further investigation of the potential role of vitamin B6 status in the pathogenesis of lung cancer. We prospectively evaluated the association of the functional marker of vitamin B6 status, the 3-hydroxykynurenine:xanthurenic acid (HK:XA) ratio, with risk of lung cancer in a nested case-control study consisting of 5,364 matched case-control pairs from the Lung Cancer Cohort Consortium (LC3). We used conditional logistic regression to evaluate the association between HK:XA and lung cancer, and random effect models to combine results from different cohorts and regions. High levels of HK:XA, indicating impaired functional B6 status, were associated with an increased risk of lung cancer, the odds ratio comparing the fourth and the first quartiles (OR4th vs. 1st) was 1.25 (95% confidence interval, 1.10-1.41). Stratified analyses indicated that this association was primarily driven by cases diagnosed with squamous cell carcinoma. Notably, the risk associated with HK:XA was approximately 50% higher in groups with a high relative frequency of squamous cell carcinoma, i.e., men, former and current smokers. This risk of squamous cell carcinoma was present in both men and women regardless of smoking status.

  • Circulating Folate, Vitamin B6, and Methionine in Relation to Lung Cancer Risk in the Lung Cancer Cohort Consortium (LC3)

    • Background: Circulating concentrations of B vitamins and factors related to one-carbon metabolism have been found to be strongly inversely associated with lung cancer risk in the European Prospective Investigation into Cancer and Nutrition (EPIC) study. The extent to which these associations are present in other study populations is unknown.Methods: Within 20 prospective cohorts from the National Cancer Institute Cohort Consortium, a nested case-control study was designed including 5364 incident lung cancer case patients and 5364 control subjects who were individually matched to case patients by age, sex, cohort, and smoking status. Centralized biochemical analyses were performed to measure circulating concentrations of vitamin B6, folate, and methionine, as well as cotinine as an indicator of recent tobacco exposure. The association between these biomarkers and lung cancer risk was evaluated using conditional logistic regression models.Results: Participants with higher circulating concentrations of vitamin B6 and folate had a modestly decreased risk of lung cancer risk overall, the odds ratios when comparing the top and bottom fourths (OR 4vs1 ) being 0.88 (95% confidence interval [CI] = 0.78 to 1.00) and 0.86 (95% CI = 0.74 to 0.99), respectively. We found stronger associations among men (vitamin B6: OR 4vs1 = 0.74, 95% CI = 0.62 to 0.89; folate: OR 4vs1 = 0.75, 95% CI = 0.61 to 0.93) and ever smokers (vitamin B6: OR 4vs1 = 0.78, 95% CI = 0.67 to 0.91; folate: OR 4vs1 = 0.87, 95% CI = 0.73 to 1.03). We further noted that the association of folate was restricted to Europe/Australia and Asia, whereas no clear association was observed for the United States. Circulating concentrations of methionine were not associated with lung cancer risk overall or in important subgroups.Conclusions: Although confounding by tobacco exposure or reverse causation cannot be ruled out, these study results are compatible with a small decrease in lung cancer risk in ever smokers who avoid low concentrations of circulating folate and vitamin B6.

  • Modelling the early to mid-Holocene Arctic climate

    • In the recent past it has become evident that the Earth's climate is changing, and that human activity play a significant role in these changes. One of the regions where the ongoing climate change has been most evident is in the Arctic: the surface temperature has increased twice as much in this region as compared to the global average, in addition, a significant decline in the Arctic sea-ice extent has been observed in the past decades. Climate model studies of past climates are important tools to understand the ongoing climate change and how the Earth's climate may respond to changes in the forcing.This thesis includes studies of the Arctic climate in simulations of the early and mid-Holocene, 9 000 and 6 000 years before present. Changes in the Earth's orbital parameters resulted in increased summer insolation as compared to present day, especially at high northern latitudes. Geological data imply that the surface temperatures in the early to mid Holocene were similar to those projected for the near future. In addition, the geological data implies that the Arctic sea ice cover was significantly reduced in this period. This makes the early to mid-Holocene an interesting period to study with respect to the changes observed in the region at present.Several model studies of the mid-Holocene have been performed through the Paleoclimate Modeling Intercomparison Project (PMIP1 to PMIP3). The simulations have been performed with climate models of varying complexity, from atmosphere-only models in the first phase to fully coupled models with the same resolution as used for future climate simulations in the third phase. The first part of this thesis investigates the simulated sea ice in the pre-industrial and mid-Holocene simulations included in the PMIP2 and PMIP3 ensemble. As the complexity of the models increases, the models simulate smaller extents and thinner sea ice in the Arctic; the sea-ice extent suggested by the proxy data for the mid-Holocene is however not reproduced by the majority of the models.One possible explanation for the discrepancy between the simulated and reconstructed Arctic sea ice extent is missing or inadequate representations of important processes. The representation of atmospheric aerosol direct and indirect effects in past climates is a candidate process. Previous studies of deeper time periods have concluded that the representation of the direct and indirect effects of the atmospheric aerosols can influence the simulated climates, and reduce the equator to pole temperature gradient in past warm climates, in better agreement with reconstructions. The second part of the thesis investigates the influence of aerosol on the early Holocene climate. The indirect effect of reduced aerosol concentrations as compared to the present day is found to cause an amplification of the warming, especially in the Arctic region. A better agreement with reconstructed Arctic sea ice extent is thus achieved.

  • Urban political decentralisation : Six Scandinavian cities

    • Intra-municipal decentralisation in Scandinavian cities is unique with regard to the volume of services devolved to urban districts. The book investigates the democratic and efficiency effects of decentralisation reforms in six major Scandinavian cities. Special attention is given to consequences of different institutional arrangements as well as to providing an understanding of reform processes. Data for the descriptions and analyses are provided by a number of sources: Surveys with district politicians and samples of citizens, published evaluation reports and interview data from three case studies especially done for this study. Institutional arrangements differ between Copenhagen with relatively autonomous districts and Swedish and Norwegian cities whose districts are more closely integrated with the centre. These differences do matter, both with regard to reform effects and the trajectories of reform processes. Institutionalisation, adaptation and power seeking behaviour of political parties are important factors for understanding the reform processes.

  • Genomic and Phenotypic Characterisation of Campylobacter jejuni Isolates From a Waterborne Outbreak

    • Campylobacter infections are the leading cause of bacterial gastroenteritis. In Europe, over 246,000 cases are confirmed annually. Infections are often transmitted via contaminated food, such as poultry products, but water may be the source of infection as well. The aim of this study was to characterise a selection of Campylobacter jejuni human isolates, together with a water isolate, from a waterborne outbreak in Norway in 2019, including human isolates from early, mid-, and late epidemic. The isolates were characterised with whole-genome sequencing, analysing the expression of putative virulence genes and demonstrating the pathogenic potential in an in vitro adhesion model using HT-29 cells. All isolates belonged to the multilocus sequence type 1701 and ST45 clonal complex. In the genomic analysis, the water isolate clustered somewhat separately from the human isolates. There was some variation between the human isolates, but the water isolate seemed to display the greatest pathogenic potential, demonstrated by the highest levels of virulence gene expression, adhesion to epithelial cells and IL-8 induction. These results suggest that the water isolate of the study has potential to cause human infections, and that some bacterial changes due to host or environmental adaptation, may occur during a waterborne Campylobacter epidemic. This is, to the best of our knowledge, the first study on C. jejuni isolates from a waterborne outbreak, including both human isolates and a water isolate, characterised with genomic and phenotypic approaches.

    • Loss-of-function mutations protective against human disease provide in vivo validation of therapeutic targets, but none have yet been described for type 2 diabetes (T2D). Through sequencing or genotyping of ∼150,000 individuals across 5 ancestry groups, we identified 12 rare protein-truncating variants in SLC30A8, which encodes an islet zinc transporter (ZnT8) and harbors a common variant (p.Trp325Arg) associated with T2D risk and glucose and proinsulin levels. Collectively, carriers of protein-truncating variants had 65% reduced T2D risk (P = 1.7 × 10(-6)), and non-diabetic Icelandic carriers of a frameshift variant (p.Lys34Serfs*50) demonstrated reduced glucose levels (-0.17 s.d., P = 4.6 × 10(-4)). The two most common protein-truncating variants (p.Arg138* and p.Lys34Serfs*50) individually associate with T2D protection and encode unstable ZnT8 proteins. Previous functional study of SLC30A8 suggested that reduced zinc transport increases T2D risk, and phenotypic heterogeneity was observed in mouse Slc30a8 knockouts. In contrast, loss-of-function mutations in humans provide strong evidence that SLC30A8 haploinsufficiency protects against T2D, suggesting ZnT8 inhibition as a therapeutic strategy in T2D prevention.

  • Healthcare professionals' experiences of targeted health dialogues in primary health care

    • BACKGROUND: In Sweden, population-based targeted health dialogues are an important part of health promotion and disease prevention in primary health care. Targeted health dialogues are performed with a pedagogical approach to allow individuals to reflect over their resources, situation and motivation to change lifestyle habits together with a healthcare professional.AIM: The aim of this study was to explore healthcare professionals' experiences of targeted health dialogues in primary health care.METHODS: Three focus group interviews were conducted with 20 healthcare professionals. The interviews were analysed using qualitative content analysis.RESULTS: The main category A possibility to promote healthy behaviours and prevent disease describes how the targeted health dialogues were experienced as a valuable opportunity to promote health among inhabitants. The significance of the primary healthcare centre's health promotion and prevention strategies was emphasised to enable the targeted health dialogues as a part of the assignment to promote health. These strategies were expressed as shared focus and organisational space and support making it possible for example to reach all socioeconomic groups. The work with targeted health dialogue was described as a complex task requiring extensive competence. Furthermore, the pedagogical tool including the visual health profile was experienced to have an important impact on the dialogue offering direction for actions to promote health and prevent disease.CONCLUSIONS: Targeted health dialogues can be a valuable opportunity for healthcare professionals in primary health care to promote a healthy lifestyle among inhabitants. Certain preconditions at both the meso- and the micro level is however required for this to come about.

  • Research methods in nursing students’ Bachelor's theses in Sweden : A descriptive study

    • Background: During the nursing programme in Sweden, students complete an independent project that allows them to receive both a professional qualification as a nurse and a Bachelor's degree. This project gives students the opportunity to develop and apply skills such as critical thinking, problem-solving and decision-making, thus preparing them for their future work. However, only a few, small-scale studies have analysed the independent project to gain more insight into how nursing students carry out this task.Objectives: The aim of the present study was to describe the methods, including ethical considerations and assessment of data quality, applied in nursing students’ independent Bachelor's degree projects in a Swedish context.Design: A descriptive study with a quantitative approach.Methods: A total of 490 independent projects were analysed using descriptive statistics.Results: Literature reviews were the predominant project form. References were often used to support the analysis method. They were not, however, always relevant to the method. This was also true of ethical considerations. When a qualitative approach was used, and data collected through interviews, the participants were typically professionals. In qualitative projects involving analysis of biographies/autobiographies or blogs participants were either persons with a disease or next of kin of a person with a disease.Conclusions: Although most of the projects were literature reviews, it seemed unclear to the nursing students how the data should be analysed as well as what ethical issues should be raised in relation to the method. Consequently, further research and guidance are needed. In Sweden, independent projects are not considered research and are therefore not required to undergo ethics vetting. However, it is important that they be designed so as to avoid possible research ethics problems. Asking persons about their health, which occurred in some of the empirical projects, may therefore be considered questionable.

  • Life in a World Heritage City : A case study of discussions and contested values in Angra do Heroísmo, the Azores

    • The objectives of this study are to critically map and analyse past and current  discussions, negotiations and social processes that take place and relate to conditions created by living in- or monitoring the World Heritage City Angra do Heroísmo in the Azores, Portugal. Based on ethnographic fieldwork and qualitative interviews with the stakeholders in the city, the purpose has been to study how, and with what result and consequences, contested values, interests, rhetoric and powers are mobilized and made into dynamic forces for these stakeholders. Hence, this thesis gives a chronological presentation of selected issues and discussions, which have been taking place in the city from 1980 to 2012. The first subjects attended to are the 1980 earthquake and the nomination process which led to the inscription of Angra to the World Heritage List in 1983.However, some of the core objectives relate to the implications of living in a World Heritage City, given the preservation provisions which follow such a classified area. The aim has further been to assess the point of departure for the official monitors and their views on monitoring a vibrant historical area. The discussions analysed relate to the predicaments occurring when modern development is set up against preservation. However, the analyses show how policies and preservation ideals change, as well as how powers and authorities are challenged and affected by forces within and outside the “authorized heritage discourse”. Finally, this study has assessed the notions of pride, local attachment and identity among the inhabitants in Angra. The World Heritage Status can trigger a double sense of feelings; on the one side it holds emotions related to pride and recognition, on the other it represents obstacles for living- and developing the city in accordance with the present needs.World Heritage and tourism are closely linked, and this investigation explores how the stakeholders in Angra relate to the possibilities and threats which tourism holds. Angra represents a site where there is a limited influx of tourist, and we can see how most stakeholders speak for a quality-based tourism. In this respect, one could say there is a prevailing consensus among the stakeholders. Finally, this study has assessed the notions of pride, local attachment and identity among the inhabitants in Angra. These sensations are related to the historical cityscape, an environment which also frames personal experiences and processes. The World Heritage Status can trigger a double sense of feelings; on the one side it holds emotions related to pride and recognition, on the other it represents obstacles for living- and developing the city in accordance with the present needs. Angra do Heroísmo is studied for its particularities, yet comparisons made with other studies reveal that the matters and aspects addressed in this thesis can be universal rather than particular.
