  • Oxytocin improves cytological and histological profiles of vaginal atrophy in postmenopausal women

    • OBJECTIVE: To investigate if topical oxytocin can reverse vaginal atrophy, as assessed by cytological and histological examination of the vaginal mucosal epithelium, in postmenopausal women after 12 weeks of treatment as compared to placebo.STUDY DESIGN: Sixty-eight postmenopausal women diagnosed with vaginal atrophy were randomized for this multicenter, double-blinded, placebo-controlled trial. Thirty-three women received 600 IU vagitocin, an oxytocin containing gel, and 35 women received a placebo gel intravaginally. The dose was 600 IU daily for the first two weeks and thereafter 600 IU twice a week for 10 weeks. All participant women underwent four visits and a subgroup of 20 women had a further fifth visit. Vaginal smears for cytological evaluation were collected at all visits. Vaginal biopsies were taken in 20 women before and after 12 weeks of treatment for histological analysis. In these women a vaginal smear was also collected after 14 weeks.RESULTS: The increase in the percentage of superficial cells between 0 and 2 weeks was significantly greater after treatment with vagitocin in comparison with placebo (p = 0.04). The difference in the maturation value between 0 and 12 weeks was significantly higher in the vagitocin than in the placebo group (p = 0.01). The reduction in the scores of atrophy was according to the histological investigation significantly greater in the vagitocin group than in the placebo group at 12 weeks (p < 0.04).CONCLUSION: Daily intravaginal treatment with vagitocin 600 IU improves expressions of vaginal atrophy as recorded by cytological investigation of vaginal smears and histological analysis of vaginal biopsies. Treatment twice weekly seems to be less effective regarding the increase in superficial cells.

    • To explore the efficacy of local oxytocin for the treatment of post-menopausal vaginal atrophy. Design Double-blinded randomised controlled trial. Setting Healthy post-menopausal women in Stockholm, Sweden. Participants Sixty four post-menopausal women between February and June 2012 at the Karolinska University Hospital Huddinge/Sweden. Main outcome measures The efficacy of oxytocin for treatment of vaginal atrophy after seven weeks and cytological evaluation. Results The percentage of superficial cells in the vaginal smears and the maturation values were significantly increased after seven weeks of treatment with vagitocin 400 IU ( p = 0.0288 and p = 0.0002, respectively). The vaginal pH decreased significantly after seven weeks of treatment with vagitocin 100 IU ( p = 0.02). The scores of vaginal atrophy, according to the histological evaluation, were significantly reduced after administration of vagitocin 100 IU ( p = 0.03). The thickness of the endometrium did not differ between the treatment and placebo groups after seven weeks of treatment. The symptom experienced as the most bothersome was significantly reduced after seven weeks of treatment in the women receiving vagitocin 400 IU compared to women in the placebo group ( p = 0.0089). Conclusions Treatment with intravaginally applied oxytocin could be an alternative to local estrogen treatment in women with post-menopausal vaginal atrophy.

    • There is a large inter-individual variation in response to clopidogrel treatment, and previous studies have indicated higher risk of thrombotic events in those with high residual platelet reactivity (HPR). Less is known about individual variation over time. The aim of this prospective cohort study was to investigate intra-individual variation in platelet reactivity. Platelet aggregation in whole blood was assessed in 77 patients, at 3 days, 8 days and 6 months after admission for acute myocardial infarction and loading dose of clopidogrel. All patients were treated with aspirin and clopidogrel through 6-month follow-up. We found a significant increase in median ADP-stimulated aggregation from third to eighth day (195 vs. 250 AU*min, p-value = 0.001) but not from day 8 to 6 months (250 vs. 223 AU*min, p-value = 0.666). There was no significant change in the overall rate of HPR (15.6% vs 20.8%, p-value 0.503) or low platelet reactivity (LPR) (37.7% vs 33.8%, p-value = 0.609) from day 8 to 6-month follow-up. In contrast, more than one in four changed HPR status, 15.6% from non-HPR to HPR and 10.4% HPR to non-HPR. A shift in LPR status appeared even more frequent, occurring in about one of three patients. In spite of similar median aggregation and rate of HPR during 6-month follow-up, about one in four of the patients changed HPR status and one in three changed LPR status. This may be important information for a concept of risk stratification based on a single aggregation value early after an acute coronary syndromes.

    • INTRODUCTION: There is a large inter-individual variation in response to clopidogrel treatment and previous studies have indicated higher risk of thrombotic events in patients with high residual platelet reactivity (HRPR), but the optimal time-point for testing is not established. The aim of this study was to investigate the optimal time-point for aggregometry testing and the risk of major adverse cardiac events associated with HRPR.METHOD AND RESULTS: We included 125 patients with ACS (73 with STEMI, and 71 received abciximab). The prevalence of HRPR varied substantially over time. The rate of HRPR in patients treated and not treated with abciximab were 43% vs 67% (p=0.01) before, 2% vs 23% (p=0.001) 6-8h after, 8% vs 9% (p=0.749) 3days after, and 23% vs 12% (p=0.138) 7-9 days after loading dose of clopidogrel. We found HRPR in 18% of the patients but only four ischemic events during 6months follow-up, with no significant difference between HRPR patients compared to the rest of the population. There were 3 TIMI major bleedings, all of which occurred in the low residual platelet reactivity (LRPR) group.CONCLUSION: There is a large variation in platelet reactivity over time, also depending on adjunctive therapy, which has a large impact on optimal time-point for assessment. We found HRPR in almost 1 in 5 patients, but very few MACE, and not significantly higher in HRPR patients. In a contemporary ACS population, with low risk for stent thrombosis, the predictive value of HRPR for ischemic events will probably be low.

    • The present study deals with the expression of frontal orientation in dynamic contexts as lexicalised in the Swedish particle fram and its French equivalents (s')avancer/en avant. The study is based on a parallel corpus containing approximately one millon words of Swedish and one million words of French fiction and it consists of a comparison made on the basis of monolingual and crosslinguistic data. The study links with current trends in both cognitive linguistics and corpus linguistics. The aim of the study is twofold : first it sets out to measure to what extent these forms express the same type of orientations and to what extent they do use these in domains other than the spatial one; second it sets out to measure to what extent these forms are used as translational equivalents as well as to list the other types of translational equivalents that occur in the corpus. This is done in order to evaluate the salience of the concept of frontal orientation in the both languages compared and to get a view of other types of conceptualizations that underlie the linguistic expression of the other language when the Swedish and the French texts do not make the same use of the expression of frontal orientation.

    • The focus of this study is use of discourse markers (DM). Data consists of taped interactions of informants. From this data, four particular DM are analyzed with regard to distribution in turn taking. The concepts of cohesion and coherence in text or discourse, together with informants´ communicative and discourse strategies, in a group of bilingual speakers (BS) and a control group of monolingual speakers (MS), are also analyzed. The findings confirm that BS, compared to the MS, make more frequent use of y with a pragmatic meaning when introducing various types of questions that constitute requests for elucidation directed to an interlocutor and thereby continue the interaction. In general, however, the MS use the DM y more frequently than the BS, demonstrating a more elaborate sense of stylistics in the coordination of their turn as narrations and descriptions. With regard to pero, the BS prefer to use this DM with a pragmatic meaning, in the continuation of an intervention or at the end of a turn, in order to question that which has been stated. The MS use pero primarily with a semantic meaning, to express counter-arguments inside a turn. It is exclusively the BS who use the DM así que with pragmatic meaning of consequence, in interactional exchanges, when they wish to introduce questions and assertions, where así que begins their reaction to the utterance of the interlocutor. Such questions may be regarded as a discourse strategy that permits possibilities for avoiding taking responsibility for how an utterance is interpreted. In addition, both the BS and the MS use así que at the end of turns, providing the discourse with meta-pragmatic meaning whereby it allows possibilities for interpreting this as finalizing. Through their use of es que, the BS gradually explain and justify statements made in utterances during their turns while the MS, through use in their discourse of this DM, allows them to introduce explications, justifications or excuses in the event of possible disagreement. In addition, the MS use justification introduced through this DM as a strategy to take a turn or to attempt to take over the turn, where es que conveys discourse-pragmatic meaning and introduces a tone of verbal courtesy.

    • Le pronom on renferme un paradoxe sémantique. D’une part, on établit la présence d’un ou de plusieurs êtres humains comme le sujet concerné par une situation décrite. D’autre part, l’identification de ce ou ces être(s) humain(s) repose sur toute la complexité des indications contextuelles, des connaissances communes et des idées préconçues des locuteurs. Cette tension entre intension et extension, qui n’arrête pas d’intriguer les chercheurs en linguistique française, est abordée ici de plusieurs manières différentes. Ainsi le fonctionnement de on est étudié dans trois genres différents (fiction romanesque, article scientifique, conversation spontanée) et de trois perspectives théoriques (textuelle, polyphonique et contrastive).
