    • Due to concerns related to treatment with erythropoietin (EPO) and possible negative effects on tumour control, a workshop was organised by the Medical Products Agency of Sweden with the aim to revise national treatment guidelines if needed. In patients with solid tumours, conflicting results have been reported with respect to tumour control and survival. Until further notice it is therefore recommended that EPO should be used restrictively in the treatment of patients with cancer and that the anticipated improvement in quality of life should be evaluated against potential risks.

    • Human embryonic stem cell (hESC) derived cardiomyocytes are in the present study being used for testing drug-induced cardiotoxicity in a biosensor set-up. The design of an in vitro testing alternative provides a novel opportunity to surpass previous methods based on rodent cells or cell lines due to its significantly higher toxicological relevance. In this report we demonstrate how hESC-derived cardiomyocytes release detectable levels of two clinically decisive cardiac biomarkers, cardiac troponin T and fatty acid binding protein 3, when the cardiac cells are exposed to the well-known cardioactive drug compound. doxorubicin. The release is monitored by the immuno-biosensor technique surface plasmon resonance, particularly appropriate due to its capacity for parallel and high-throughput analysis in complex media.

  • Fatigue Assessment and LEFM Analysis of Cruciform Joints Fabricated with Different Welding Processes

    • In this study fatigue testing and defect assessment were carried out on specimens welded with robotic and manualwelding using fl ux cored (FCAW) and metal cored (MCAW) fi ller materials in order to study the effect of the weldingmethod on the fatigue strength and weld quality. Thirteen different batches were investigated of which two wasshot peened before fatigue testing. The local weld geometry was measured for all the specimens before testing.The specimens welded with fl ux cored weld wire showed the best fatigue strength, small defects and low residualstresses. Large scatter in the fatigue data is observed, especially when manual welding is employed. The few largestdefects were removed by the shot peening process, although small defects survived. This led to a smaller scatterin fatigue live for the shot peened specimens. Linear elastic fracture mechanics, LEFM, was employed for analysisof the fatigue test results. The fatigue life predictions using a 2D LEFM FE-model for simulating a continuous coldlap defect along the weld toe showed a qualitative agreement with the fatigue test results. The 2D analysis showedthat a continuous cold lap defect should be no more than 0.5 mm deep in order to comply with the requirement offatigue lives for normal weld quality according to the IIW design rules. For larger defects (> 0.8 mm) an increased toeradius will have a small effect on the fatigue strength. A 3D LEFM analysis of crack growth from a spatter-inducedcold lap defect was also carried out. This showed similar trends in crack growth compared to the 2D analysis ofa continuous cold lap, although the spatter-induced cold lap defect (semi-elliptical) had a longer fatigue life (x2.7),and hence is less dangerous from a fatigue point of view.

  • Chronic rhinosinusitis impairs sleep quality : results of the GA(2)LEN study

    • STUDY OBJECTIVES: To analyse the prevalence of sleep problems in subjects with CRS and to determine whether the disease severity of CRS affects sleep quality.METHODS: Questionnaires were sent to a random sample of 45 000 adults in four Swedish cities. Questions on CRS, asthma, allergic rhinitis, co-morbidities, tobacco use, educational level and physical activity were included. CRS was defined according to the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) epidemiological criteria. The disease severity of CRS was defined by the number of reported CRS symptoms. Sleep quality was assessed using the Basic Nordic Sleep Questionnaire.RESULTS: Of the 26 647 subjects, 2249 (8.4%) had CRS. Reported sleep problems were 50-90% more common among subjects with CRS compared with those without or the total population. The prevalence of reported sleep problems increased in conjunction with the severity of CRS. After adjusting for gender, BMI, age, tobacco use, asthma, somatic diseases, physical activity level and educational level, participants with four symptoms of CRS (compared with subjects without CRS symptoms) displayed a higher risk of snoring (adj. OR (95% CI): 3.13 (2.22-4.41)), difficulties inducing sleep (3.98 (2.94-5.40)), difficulties maintaining sleep (3.44 (2.55-4.64)), early morning awakening (4.71 (3.47-6.38)) and excessive daytime sleepiness (4.56 (3.36-6.18)). The addition of persistent allergic rhinitis to CRS further increased the risk of sleep problems.CONCLUSIONS: Sleep problems are highly prevalent among subjects with CRS. The disease severity of CRS negatively affects sleep quality.

    • The aim of this study was to assess the feasibility and results of repeat surgery without preoperative angiography in limbs with patent infrainguinal bypass grafts. Between January 1995 and December 1999, 73 surgical interventions were performed for correction of inflow, graft, or runoff-related lesions in limbs with patent infrainguinal bypass grafts. Fifty-six of the 73 cases were operated on based on the findings obtained from duplex scanning alone. There were 53 vein and 3 prosthetic grafts in the series. The indications for intervention without angiography were stenotic or occlusive lesions in 35, graft aneurysm in 7, and arteriovenous fistulae in 14. There were no deviations from the preoperatively planned surgical strategy in patients undergoing surgery without preoperative angiography. Cumulative life table primary, (stenosis free) and primary-assisted patency rates, at 12 months following graft revisions (excluding arteriovenous fistulae ligatures) without preoperative angiography, were 64% and 85%, respectively. The corresponding figures for revisions performed with preoperative angiography were 58% and 84%, respectively. There were no significant differences between patients undergoing surgery with or without preoperative diagnostic angiography with regard to patency rates. Surgical interventions for correction of infrainguinal graft-related stenotic or aneurysmal lesions can be safely performed based on findings obtained from duplex scanning.

  • Logistic regression for clustered data from environmental monitoring programs

    • Large-scale surveys, such as national forest inventories and vegetation monitoring programs, usually have complex sampling designs that include geographical stratification and units organized in clusters. When models are developed using data from such programs, a key question is whether or not to utilize design information when analyzing the relationship between a response variable and a set of covariates. Standard statistical regression methods often fail to account for complex sampling designs, which may lead to severely biased estimators of model coefficients. Furthermore, ignoring that data are spatially correlated within clusters may underestimate the standard errors of regression coefficient estimates, with a risk for drawing wrong conclusions. We first review general approaches that account for complex sampling designs, e.g. methods using probability weighting, and stress the need to explore the effects of the sampling design when applying logistic regression models. We then use Monte Carlo simulation to compare the performance of the standard logistic regression model with two approaches to model correlated binary responses, i.e. cluster-specific and population-averaged logistic regression models. As an example, we analyze the occurrence of epiphytic hair lichens in the genus Bryoria; an indicator of forest ecosystem integrity. Based on data from the National Forest Inventory (NFI) for the period 1993-2014 we generated a data set on hair lichen occurrence on > 100,000 Picea abies trees distributed throughout Sweden. The NFI data included ten covariates representing forest structure and climate variables potentially affecting lichen occurrence. Our analyses show the importance of taking complex sampling designs and correlated binary responses into account in logistic regression modeling to avoid the risk of obtaining notably biased parameter estimators and standard errors, and erroneous interpretations about factors affecting e.g. hair lichen occurrence. We recommend comparisons of unweighted and weighted logistic regression analyses as an essential step in development of models based on data from large-scale surveys.

    • Presents some theoretical principles and empirical evidence relating to the new Swedish production systems for final assembly of motor vehicles. Contends that in these production systems it is possible simultaneously to enhance efficiency and quality of working life. Briefly sketches three production forms as they apply to the final assembly of automobiles and discusses the societal environments in which these different forms of production have evolved. It focuses on Volvo Uddevalla plant as one of the main examples so far of a reflective production system for final assembly of automobiles. Amplifies the analysis of different production flow patterns for final assembly and in particular addresses the issue of semi‐parallel mechanistic production flow and parallel organic production flow as alternatives to serial flow on a conventional line assembly. A comment: This publication is authored together with e.g. senior research competencies (1) within vocational learning and training as well as (2) within time-geography with whom Engström have had long time cooperation procedures. But also personal at our department/research group (Mats Johansson) well as our knowledgeable contact person (Bertil Johansson) within Volvo contributed (he has really helped us out for decades and was responsible for the continuous contacts with Volvo in most any matter).
