    • AIMS: Standardized data definitions are essential for monitoring and assessment of care and outcomes in observational studies and randomized controlled trials (RCTs). The European Unified Registries for Heart Care Evaluation and Randomized Trials (EuroHeart) project of the European Society of Cardiology aimed to develop contemporary data standards for atrial fibrillation/flutter (AF/AFL) and catheter ablation.METHODS AND RESULTS: We used the EuroHeart methodology for development of data standards and formed a Working Group comprising 23 experts in AF/AFL and catheter ablation registries, as well as representatives from the European Heart Rhythm Association and EuroHeart. We conducted a systematic literature review of AF/AFL and catheter ablation registries and data standard documents to generate candidate variables. We used a modified Delphi method to reach consensus on a final variable set. For each variable, the Working Group developed permissible values and definitions, and agreed as to whether the variable was mandatory (Level 1) or additional (Level 2). In total, 70 Level 1 and 92 Level 2 variables were selected and reviewed by a wider Reference Group of 42 experts from 24 countries. The Level 1 variables were implemented into the EuroHeart IT platform as the basis for continuous registration of individual patient data.CONCLUSION: By means of a structured process and working with international stakeholders, harmonized data standards for AF/AFL and catheter ablation for AF/AFL were developed. In context of the EuroHeart project, this will facilitate country-level quality of care improvement, international observational research, registry-based RCTs and post-marketing surveillance of devices and pharmacotherapies.

    • Rapporten Tilliten till det svenska samhällskontraktet, skriven av Rebecka Andersenoch Lars Trägårdh, är ett viktigt bidrag från Fores till det alltför begränsade forskningsområdet om tillit.Rapportens viktigaste slutsats är att Sveriges tillitsproblem i allra högsta grad är lokal, men riskerar att spilla över på det nationella om problemen förblir olösta. För att stärka lokalsamhälletilliten krävs det av politiken att säkra den fysiska närvaron av välfärdsinstitutioner över hela Sverige. Det gäller i synnerhet polisnärvaron som är central för människors känsla av trygghet. Studieresultaten visar nämligen att svenskarnas uppfattade trygghet och tillit är mycket starkt sammankopplat.Tilliten till det svenska samhällskontraktet bidrar även med ett unikt Välfärdstillgänglighetsindex, respektive Välfärdsförtroendeindex, som i sin tur delas upp i fyra underkategorier: sjukvård, utbildning, äldreomsorg och polisväsende/trygghet.   • Active and passive seismic methods for investigating the glacially-triggered Burträsk fault

    • Glacially-triggered faults are of high scientific interest since their formation was likely accompanied by major earthquakes and they are still a centre of seismicity in northern Fennoscandia, today. Imaging their deeper structure mainly relies on reflection seismics since the method generally has the best resolving power at depth of all geophysical methods. This thesis uses data acquired at the glacially-triggered Burträsk fault to advance active and passive seismic imaging methods and improve the understanding of the area. Reprocessing of a vintage, crooked-line reflection dataset using a newly developed module for a local cross-dip correction improved the quality of the reflection image significantly and provided important 3D information for the interpretation of the fault and its surroundings. A fault segment to the southwest of the profile was imaged with a dip of approximately 50° but the segment intersecting the profile was not imaged, likely due to insufficient shot coverage close to the fault. Since the Burträsk area is seismically the most active area in Sweden, passive imaging using a dataset of local microearthquakes was attempted. As a first step, the earthquakes were re-located using a stacking-based location method. In spite of the poor azimuthal coverage of the array, the method yielded surprisingly good location results within 30–40 km from the array. The most important factors for success proved to be combined P- and S-wave location with down-weighted S-wave signals, and the use of a polarity-sensitive characteristic function. The distribution of the hypocentres confirmed that the trace of the Burträsk fault is the currently active fault plane. Disappointingly, passive seismic processing using reflection seismic interferometry (RSI) did not image the known reflections. To improve the planning of future studies, different aspects of imaging dipping faults with RSI were investigated using synthetic data. The results showed that reconstructing steeply dipping reflections requires sources in the hanging-wall and that the interpretation of the RSI sections is complicated due to strong artefacts caused by P-S converted arrivals. Thus, passive imaging needs careful planning and a good knowledge of the source positions for distinguishing between artefacts and subsurface structures. This knowledge can be obtained using the stacking-based location method.

  • Innovative seismic imaging of volcanogenic massive sulfide deposits, Neves-Corvo, Portugal - Part 1 : In-mine array

    • To evaluate and upscale the feasibility of using exploration tunnels in an operating mine for active-source seismic imaging, a seismic experiment was conducted at the Neves-Corvo mine, in southern Portugal. Four seismic profiles were deployed in exploration drifts approximately 650 m beneath the ground surface, above the world-class Lombador volcanogenic massive sulfide deposit. In addition to the tunnel profiles, two perpendicular surface seismic profiles were deployed above the exploration tunnels. The survey was possible due to a newly developed prototype global positioning system (GPS) time transmitter enabling accurate GPS synchronization of cabled and nodal seismic recorders, below and on the surface. Another innovative acquisition aspect was a 1.65 t broadband, linear synchronous motor (LSM) driven - electric seismic vibrator (e-vib) used as the seismic source along two of the exploration tunnels. We have evaluated the challenges and innovations necessary for active-source tunnel seismic acquisition, characterized by high levels of vibrational noise from the mining activities. In addition, we evaluated the LSM vibrator's signal and overall seismic-data quality in this hard rock mining environment. Our processing results from the tunnel data and 3D reflection imaging of the Lombador deposit below the exploration tunnels were checked for consistency through constant-velocity 3D ray-tracing traveltime forward modeling. For imaging purposes, 3D Kirchhoff prestack depth and poststack time-migration algorithms were used, with both successfully imaging the targeted deposit. The results obtained show that active-source-seismic imaging using subsurface mining infrastructure of operational mines is possible. However, it requires innovative exploration strategies, a broadband seismic source, an accurate GPS-time system capable of transmitting GPS-time hundreds of meters below the surface, and careful processing. The results obtained open up possibilities for similar studies in different mining or tunneling projects.

    • During the last few decades there has been an increased demand for infrastructure, along with a greater awareness of environmental issues in the construction industry. These factors have contributed to an increased interest in using seismic methods for near surface characterization, particularly in urban environments. Seismic sensors not affected by anthropogenic electromagnetic noise are therefore important, as well as an acquisition system that is easy to deploy, move and non-invasive. To address some of these challenges, a multicomponent broadband MEMS (micro-electro mechanical system) based landstreamer system was developed. The landstreamer system is fully digital, therefore it is less sensitive to electrical or electromagnetic noise. Crosstalk, leakage and tilting tests show that the system is superior to its predecessors. The broadband nature of the sensors (theoretically 0–800 Hz), 3C (three-component) recording and the close spacing of the sensors enable high-resolution imaging. The current streamer configuration consists of 20 sensors 4 m apart and 80 sensors 2 m apart. The streamer can easily be combined with wireless recorders for simultaneous data acquisition. In this study, we present results from testing of the streamer with various sources, such as a shear wave vibrator and different types of impact sources. MEMS-sensors and their high sensitivity allowed recording clear reflections that were not observed with coil-based sensors. A complementary test was also carried out at a planned access ramp for an urban tunnel where potential poor quality rocks had been identified by drilling. First-break traveltime tomography showed that these poor quality rocks correlate with low velocity zones. The presented landstreamer system has great potential for characterizing the subsurface in noisy environments.

  • Adrenal incidentaloma - follow-up results from a Swedish prospective study

    • OBJECTIVES: To examine the risk of developing adrenal carcinomas and clinically overt hypersecreting tumours during short-term follow-up in patients with adrenal incidentalomas. DESIGN: 229 (98 males and 131 females) patients with adrenal incidentalomas were investigated in a prospective follow-up study (median time 25 months; range 3-108 months). The patients were registered between January 1996 and July 2001 and followed until December 2004. Twenty-seven Swedish hospitals contributed with follow-up results. METHODS: Diagnostic procedures were undertaken according to a protocol including reinvestigation with computed tomography scans after 3-6 months, 15-18 months and 27-30 months, as well as hormonal evaluation at baseline and after 27-30 months of follow-up. Operation was recommended when the incidentaloma size increased or if there was a suspicion of a hypersecreting tumour. RESULTS: The median age at diagnosis of the 229 patients included in the follow-up study was 64 years (range 28-84 years) and the median size of the adrenal incidentalomas when discovered was 2.5 cm (range 1-8 cm). During the follow-up period, an increase in incidentaloma size of > or =0.5 cm was reported in 17 (7.4%) and of > or =1.0 cm was reported in 12 (5.2%) of the 229 patients. A decrease in size was seen in 12 patients (5.2%). A hypersecreting tumour was found in 2% of the hormonally investigated patients: Cushing's syndrome (n = 2) and phaeochromocytoma (n = 1). Eleven patients underwent adrenalectomy, but no cases of primary adrenal malignancy were observed. CONCLUSIONS: Patients with adrenal incidentaloma had a low risk of developing malignancy or hormonal hypersecretion during a short-term follow-up period.

    • BACKGROUND:Hyperparathyroidism affects most patients with multiple endocrine neoplasia type 1 (MEN 1). This study investigates expression of the candidate MEN1 gene phospholipase C beta 3 (PLC beta 3) and expression and function of a putative calcium sensing protein (CAS) in hyperparathyroidism of MEN 1.METHODS:In 31 parathyroid glands from 17 patients with MEN 1, CAS distribution was studied immunohistochemically and parallel sections were explored for PLC beta 3 mRNA expression by in situ hybridization. Enzymatically dispersed parathyroid cells were analyzed for cytoplasmic calcium concentrations [Ca2+]i and parathyroid hormone (PTH) release.RESULTS:All glands exhibited a heterogeneously reduced CAS immunoreactivity, especially meager in nodularly assembled parathyroid cells. Calcium regulated [Ca2+]i and PTH release tended to be more deranged in the glands possessing the lowest immunostaining. Parathyroid PLC beta 3 invariably was homogeneously expressed, and this included even MEN 1 patients with reduced PLC beta 3 expression in endocrine pancreatic tumors.CONCLUSIONS:The findings support variable calcium insensitivity of [Ca2+]i and PTH release in hyperparathyroidism of MEN 1, apparently coupled to heterogeneously reduced CAS expression. For clarification of the role of PLC beta 3 in MEN 1 parathyroid tumorigenesis further study of this protein is required.

    • BACKGROUND: Experimental evidence support soluble receptors for tumor necrosis factor alpha as important mediators of the underlying pathology leading to cardiovascular disease and cancer. However, prospective data concerning the relation between circulating soluble tumor necrosis factor receptor-1 (sTNFR1) and mortality in humans are lacking. We aimed to explore and validate the association between sTNFR1 and mortality, and to explore the influence of other established risk factors for mortality, including other inflammatory markers.METHODS: The association between serum sTNFR1and the risk for mortality was investigated in two community-based cohorts of elderly: the Prospective Investigation of the Vasculature in Uppsala Seniors (PIVUS; women 50%, n = 1005, mean age 70 years, median follow-up 7.9 years) and the Uppsala Longitudinal Study of Adult Men (ULSAM, n = 775, mean age 77 years, median follow-up 8.1 years).RESULTS: In total, 101 participants in PIVUS and 274 in ULSAM died during follow-up. In multivariable Cox regression models adjusted for inflammation, lifestyle and established cardiovascular risk factors, one standard deviation (SD) higher sTNFR1 was associated with a hazard ratio (HR) for mortality of 1.37, 95% confidence interval (CI) 1.17-1.60, in PIVUS and HR 1.22, 95% CI 1.10-1.37 in ULSAM. Moreover, circulatingsTNFR1 was associated with cardiovascular mortality (HR per SD of sTNFR1, 1.24, 95% CI 1.07-1.44) and cancer mortality (HR per SD of sTNFR1, 1.32, 95% CI 1.11-1.57) in the ULSAM cohort. High levels of sTNFR1 identified individuals with increased risk of mortality among those with high as well as low levels of systemic inflammation.CONCLUSIONS: An association between circulating sTNFR1 and an increased risk for mortality was found and validated in two independent community-based cohorts. The future clinical role of sTNFR1 to identify high risk patients for adverse outcomes and mortality has yet to be determined.
