    • An extraordinarily diverse and well-preserved material, including the remains of 47 insect taxa and 12 taxa of other invertebrates, extracted from the 17th century burial of Bishop Peder Winstrup in Lund Minster, is presented and discussed in terms of the treatment of the body, activities connected with the burial and faunal significance. The invertebrate assemblages include species from gardens, insects feeding on living plants as well as dried or decaying plant matter. Many of the species are regarded as closely associated with humans (synanthropic), and a number of these are associated with outbuildings, such as stables and cellars. The absence of species associated with cadavers (necrophilous taxa) in the studied insect material is significant. The most plausible explanation is that the bishop died, and was buried during the winter, when such species are inactive, and thus precluded from colonising the body. A number of species were recorded which are today rare or very rare in southern Sweden. This is a strong indication that they once were more common and widespread, perhaps due to a greater prevalence of their preferred habitats. Sweden’s earliest fossil bedbug is also amongst the finds.

  • Ischemic Preconditioning Prior to Intermittent Pringles Maneuver in Liver Resections

    • Background: Continuous inflow vascular occlusion during liver resections causes less severe ischemia and reperfusion injury (IRI) if it is preceded by ischemic preconditioning (IP) or if intermittent inflow occlusion is used during the resection. No previous clinical trial has studied the effects of adding IP to intermittent inflow occlusion.Methods: Consecutive patients (n=32) with suspicion of malignant liver disease had liver resections (minimum 2 segments) performed with inflow occlusion 15/5. Half of the patients were randomized to receive IP (10/10). The patients were stratified according to volume of resection and none had chronic liver disease. The patients were followed for 5 days with microdialysis (μD).Results: All patients completed the study and there were no deaths. No differences were seen between the groups regarding demographics or perioperative parameters (bleeding, duration of ischemia, resection volume, complications and serum lab tests). There were no differences in ALT, AST, Bilirubin or PT-INR levels, but μD revealed lower levels of lactate, pyruvate and glucose in the IP group having major liver resections (ANOVA). Nitrite and nitrate levels in μD decreased postoperatively but no differences were seen between the groups. In one patient an elevated μDglycerol curve was seen before the diagnosis of a stroke was made.Conclusions: IP before intermittent vascular occlusion does not reduce the serum parameters used to assess IRI. IP seems to improve aerobic glucose metabolism as the levels of glucose, pyruvate and lactate locally in the liver were reduced compared to controls in patients having resected >3 segments. μD may be used to monitor metabolism locally.

    • The Swedish Defence Research Agency (FOI) has developed a concept demonstrator called the Information Fusion Demonstrator 2003 (IFD03) for demonstrating information fusion methodology suitable for a future Network Based Defense (NBD) C4ISR system. The focus of the demonstrator is on real-time tactical intelligence processing at the division level in a ground warfare scenario. The demonstrator integrates novel force aggregation, particle filtering, and sensor allocation methods to create, dynamically update, and maintain components of a tactical situation picture. This is achieved by fusing physically modelled and numerically simulated sensor reports from several different sensor types with realistic a priori information sampled from both a high-resolution terrain model and an enemy organizational and behavioral model. This represents a key step toward the goal of creating in real time a dynamic, high fidelity representation of a moving battalion-sized organization, based on sensor data as well as a priori intelligence and terrain information, employing fusion, tracking, aggregation, and resource allocation methods all built on well-founded theories of uncertainty. The motives behind this project, the fusion methods developed for the system, as well as its scenario model and simulator architecture are described. The main services of the demonstrator are discussed and early experience from using the system is shared.

    • Purpose: To investigate whether outpatient blue-light flexible cystoscopy could solve the diagnostic challenge of positive or suspicious urine cytology findings despite normal white-light flexible cystoscopy results and normal findings on computerized tomography urography, in patients investigated for urothelial cancer.Material and methods: In a multicentre study, a total of 70 examinations were performed with the use of blue-light flexible cystoscopy (photodynamic diagnosis) after intravesical instillation of the fluorescence agent hexaminolevulinate. The examination started with a conventional white-light flexible cystoscopy and then the settings were switched to use blue light. Suspicious lesions were biopsied. Afterwards, the patients were interviewed regarding their experience of the examinations.Results: Bladder cancer was diagnosed in 29 out of 70 (41%) cases, among them 14/29 (48%) had malignant lesions seen only in blue light. The majority had carcinoma in situ (21/29). Normal findings were seen in 41 cases that underwent BLFC. During the further course, malignancy of the bladder was detected in six cases (9%) and malignancy of the upper urinary tract was detected in one case (1%). The majority of patients (93%) preferred the blue-light flexible cystoscopy performed at the outpatient clinic instead of the transurethral resection under general anaesthesia.Conclusion: Blue-light flexible cystoscopy at the outpatient clinic may be a useful tool to solve unclear cases of a malignant or suspicious urinary cytology suggestive of bladder cancer. The procedure was well tolerated by the patients.
