  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

  • Development of a standardised training curriculum for robotic surgery: a consensus statement from an international multidisciplinary group of experts

    • Objectives: To explore the views of experts about the development and validation of a robotic surgery training curriculum, and how this should be implemented. Materials and methods: An international expert panel was invited to a structured session for discussion. The study was of a mixed design, including qualitative and quantitative components based on focus group interviews during the European Association of Urology (EAU) Robotic Urology Section (ERUS) (2012), EAU (2013) and ERUS (2013) meetings. After introduction to the aims, principles and current status of the curriculum development, group responses were elicited. After content analysis of recorded interviews generated themes were discussed at the second meeting, where consensus was achieved on each theme. This discussion also underwent content analysis, and was used to draft a curriculum proposal. At the third meeting, a quantitative questionnaire about this curriculum was disseminated to attendees to assess the level of agreement with the key points. Results: In all, 150 min (19 pages) of the focus group discussion was transcribed (21 316 words). Themes were agreed by two raters (median agreement kappa 0.89) and they included: need for a training curriculum (inter-rater agreement kappa 0.85); identification of learning needs (kappa 0.83); development of the curriculum contents (kappa 0.81); an overview of available curricula (kappa 0.79); settings for robotic surgery training ((kappa 0.89); assessment and training of trainers (kappa 0.92); requirements for certification and patient safety (kappa 0.83); and need for a universally standardised curriculum (kappa 0.78). A training curriculum was proposed based on the above discussions. Conclusion: This group proposes a multi-step curriculum for robotic training. Studies are in process to validate the effectiveness of the curriculum and to assess transfer of skills to the operating room.

  • A Comparison and Introduction of Novel Solar Panel’s Fault Diagnosis Technique Using Deep-Features Shallow-Classifier through Infrared Thermography

    • Solar photovoltaics (PV) are susceptible to environmental and operational stresses due to their operation in an open atmosphere. Early detection and treatment of stress prevents hotspots and the total failure of solar panels. In response, the literature has proposed several approaches, each with its own limitations, such as high processing system requirements, large amounts of memory, long execution times, fewer types of faults diagnosed, failure to extract relevant features, and so on. Therefore, this research proposes a fast framework with the least memory and computing system requirements for the six different faults of a solar panel. Infrared thermographs from solar panels are fed into intense and architecturally complex deep convolutional networks capable of differentiating one million images into 1000 classes. Features without backpropagation are calculated to reduce execution time. Afterward, deep features are fed to shallow classifiers due to their fast training time. The proposed approach trains the shallow classifier in approximately 13 s with 95.5% testing accuracy. The approach is validated by manually extracting thermograph features and through the transfer of learned deep neural network approaches in terms of accuracy and speed. The proposed method is also compared with other existing methods.

  • Performance analysis of microchannel heat sink with ribbed pinfins

    • This paper presents a numerical study investigating the effect of ribbed pinfins on the thermal–hydraulic performance of a microchannel heat sink. A three-dimensional numerical model of a single rectangular microchannel with rectangular pinfins having ribs on them is developed, and simulations are performed for different rib configurations under laminar flow conditions with deionized water as the coolant. The study is conducted in two parts. The first part examines the influence of varying numbers of ribs on the pinfins while maintaining a constant rib width. The results show that increasing the number of ribs initially enhances heat transfer, with the case having two ribs (R2) exhibiting the highest Nusselt number ratio and lowest thermal resistance, up to 16 % lower than the nonribbed case (R0). However, a further increase in the number of ribs leads to a decrease in heat transfer performance due to the reduced cross-sectional area for heat conduction through the ribs. Moreover, the highest performance factor is exhibited by the case consisting of seven ribs (R7). The second part explores the impact of varying rib width, using the optimal R7 configuration based on the performance factor from the first part. The results indicate that increasing the rib width significantly improves heat transfer, with the widest rib case (W7) achieving up to a 25 % increase in Nusselt number compared to a plain microchannel. The study provides insights into the design of efficient microchannel heat sinks with ribbed pinfins for effective cooling of high-heat-flux electronic devices.

  • The need for supportive mental wellbeing interventions in bladder cancer patients : A systematic review of the literature

    • Objectives There is an increased awareness of the effect of a bladder cancer diagnosis and its treatments on the mental wellbeing of patients. However, few studies have evaluated the efficacy, feasibility and acceptability of interventions to improve this mental wellbeing. This systematic review is the first phase of the Medical Research Council Framework for developing complex interventions and provides an overview of the published mental wellbeing interventions that could be used to design an intervention specific for BC patients.Methods This review was conducted in accordance with the PRISMA guidelines in January 2019 and studies were identified by conducting searches for Medline, the Cochrane Central Register of Controlled Trials and Ovid Gateway. All included studies met the following criteria: mental wellbeing interventions of adults with medically confirmed diagnosis of any type of urological cancer, reported outcomes for specific HRQoL domains including psychological factors. The quality of evidence was assessed according to Down and Black 27-item checklist.Results A total of 15,094 records were collected from the literature search and 10 studies matched the inclusion and exclusion criteria. Of these, nine interventions were for patients with prostate cancer and one for patients with kidney cancer. No studies were found for other urological cancers. Depression was the most commonly reported endpoint measured. Of the included studies with positive efficacy, three were group interventions and two were couple interventions. In the group interventions, all showed a reduction in depressive symptoms and in the couple interventions, there was a reduction in depressive symptoms and a favourable relationship cohesion. The couple interventions were the most feasible and acceptable, but further research was required for most of the studies.Conclusion While awareness of the importance of mental wellbeing in bladder cancer patients is growing, this systematic literature review highlights the gap of feasible and acceptable interventions for this patient population.

  • Hydroquinone; a novel bioactive compound from plant-derived smoke can cue seed germination of lettuce

    • Plant-derived smoke has been known to play an important role in distribution and growth of vegetation. Using a proficiently designed furnace, we extracted smoke from the leaves of four plant viz. Helianthus annuus,Aloe vera,Ginkgo biloba, and Cymbopogon jwarancusa. Smoke dilutions obtained from these plants were obtained in different concentrations to identify potential lettuce growth promoting smoke solution. Results revealed that smoke obtained from G. biloba significantly enhanced the lettuce seed germination. This solution was then partitioned into ethyl acetate, dichloromethane, n-hexane, chloroform and ether fractions. Ethyl acetate fraction was found to be potent to enhance seed germination. This fraction was subjected to column chromatography and spectroscopic techniques to obtain compound 1. This compound was identified as hydroquinone using 1D and 2D NMR techniques. At low concentrations (5, 10, and 20 ppm), compound 1 enhanced the lettuce seed germination; however, higher concentrations inhibited its growth as compared to control.

    • Silicon carbide is a captivating semiconductor material for electrical and electro-optical applications requiring high temperatures. Silicon carbide is a crucial non-oxide ceramic with a wide range of uses in manufacturing. It has special properties like high rigidity and durability, heat and chemical constancy, a high melting point, oxidation resistance, powerful erosion resistance, etc. Due to all of these properties, silicon carbide is the perfect material for high-power, high-temperature electrical devices as well as erosion and cutting purposes. Silicon carbide materials are frequently used in different fields of nuclear materials and semiconductor materials because of their outstanding radiation resistance, thermal conductivity, oxidation resistance, and mechanical strength. This study presents various K-Banhatti entropies, redefines Zagreb entropies, and the atom-bond sum connectivity entropy of the double and strong double graphs of silicon carbide [Si2C3−I(r,s)], and presents a numerical and graphical analysis of these results. The QSPR analysis of silicon carbide is performed for four characteristics Poisson’s ratio, shear modulus, Young’s modulus and bulk modulus through linear and quadratic regression analysis and found the best prediction. These models provide scientists with a new way of estimating physicochemical properties.

  • Jejunal diverticulosis presenting as intestinal obstruction-A case report of a rare association

    • Jejunal diverticula are rare medical conditions with an incidence of 0.3%-2.5%, mostly discovered perioperatively. Our patient, 60 years old female, presented in an emergency with complaints of constipation, vomiting, abdominal pain, and distension. On examination, her abdomen was markedly distended with generalized tenderness. An erect abdominal X-ray revealed multiple air-fluid levels, which suggests small bowel obstruction. A diagnosis of jejunal diverticula was made on exploratory laparotomy. No evidence of granuloma or malignancy was seen on histopathological examination. Segmental resection of the affected jejunum was carried out, followed by end-to-end primary anastomosis. The patient was discharged on postoperative Day 6 with complete recovery at 2 weeks follow-up visit.

  • Estimated Zika virus importations to Europe by travellers from Brazil

    • BACKGROUND: Given the interconnectivity of Brazil with the rest of the world, Zika virus (ZIKV) infections have the potential to spread rapidly around the world via viremic travellers. The extent of spread depends on the travel volume and the endemicity in the exporting country. In the absence of reliable surveillance data, we did mathematical modelling to estimate the number of importations of ZIKV from Brazil into Europe.DESIGN: We applied a previously developed mathematical model on importations of dengue to estimate the number of ZIKV importations into Europe, based on the travel volume, the probability of being infected at the time of travel, the population size of Brazil, and the estimated incidence of ZIKV infections.RESULTS: Our model estimated between 508 and 1,778 imported infections into Europe in 2016, of which we would expect between 116 and 355 symptomatic Zika infections; with the highest number of importations being into France, Portugal and Italy.CONCLUSIONS: Our model identified high-risk countries in Europe. Such data can assist policymakers and public health professionals in estimating the extent of importations in order to prepare for the scale up of laboratory diagnostic assays and estimate the occurrence of Guillain-Barré Syndrome, potential sexual transmission, and infants with congenital ZIKV syndrome.
