    • This study examines the manufacturing of tailored men’s garments in the workshops at the Swedish royal court during the reigns of Charles IX and Gustavus Adolphus. The focus of the examination has been on the craft of tailoring and the organization of the court tailors. The study is built on both object-based and archival research. The sources used are the extant royal and livery garments from the Swedish court, produced between 1610 and 1635, and the documents from the royal wardrobe. The latter consist of accounts, warrants and inventories produced during the daily contact between the wardrobe staff and the court tailors. The extant garments, being the material evidence of the prevailing tailoring methods, show choices of materials, construction and tailoring techniques, while the documents show the width of the production to which the garments can be compared.The study of the craft is concerned with what was produced, which materials were used, how these functioned in the garments, and what technical solutions were in use. The detailed studies have also brought about questions concerning the technical differences between tailors and seamstresses, the rationality with which the workshops were run, how different aspects of tailoring were valued at the time, and how the tailoring performed at the Swedish court corresponded with European work from the same period.The study argues, seen through the craft, that the tailoring workshops at the Swedish court were run as a well organized and highly efficient business. In this standardized amounts and materials, as well as a uniform craftsmanship were used. The tailors running the workshops usually stayed in royal service for many years. They were responsible for the work produced in their workshops as well as for their staff. The study also suggests that the techniques in practice in the tailoring workshops at the Swedish court were not local but belonged to a western European tradition of the craft. Within this frame was a group of established technical solutions that appear to have been typical for the time.

    • beta-Microseminoprotein (MSP), a 10kDa protein in human seminal plasma, binds human cysteine-rich secretory protein-3 (CRISP-3) with high affinity. CRISP-3 is a member of the family of CRISPs, which are widespread among animals. In this work we show that human as well as porcine MSP binds catrin, latisemin, pseudecin, and triflin, which are CRISPs present in the venoms of the snakes Crotalus atrox, Laticauda semifasciata, Pseudechis porphyriacus, and Trimeresurus flavoviridis, respectively. The CRISPs were purified from the venoms by affinity chromatography on a human MSP column and their identities were settled by gel electrophoresis and mass spectrometry. Their interactions with human and porcine MSPs were studied with size exclusion chromatography and surface plasmon resonance measurements. The binding affinities at 25 degrees C were between 10(-10)M and 10(-7)M for most of the interactions, with higher affinities for the interactions with porcine MSP compared to human MSP and with Elapidae CRISPs compared to Viperidae CRISPs. The high affinities of the bindings in spite of the differences in amino acid sequence between the MSPs as well as between the CRISPs indicate that the binding is tolerant to amino acid sequence variation and raise the question how universal this cross-species reaction between MSPs and CRISPs is.

  • Cultural Competence for Health Professionals : Instrument Development

    • In recent decades, both global migration in general and specifically migration to Sweden have increased. This development compels the need for delivering healthcare to the increasingly diverse populations in Sweden. To support health professionals, for instance occupational therapists, in developing their professional knowledge in encounters with foreign-born clients a self-rating instrument measuring cultural competence is developed. This may contribute to the development of suitable services for foreignborn clients and improve person-centered interventions for these clients.The general aim of this thesis was to develop an instrument for health professionals by examining psychometric properties and utility of the Swedish version of the Cultural Competence Assessment Instrument (CCAI-S) among occupational therapists. The specific aim of study I was to evaluate the content validity and utility of the Swedish version of the Cultural Competence Assessment Instrument (CCAI-S) among occupational therapists. The study had a descriptive and explorative design. Nineteen occupational therapists participated, divided into four focus groups. Qualitative content analysis was used to examine the content validity and utility of the CCAI-S. The specific aim of study II was to examine the clinical relevance, construct validity and reliability of the Swedish version of the Cultural Competence Assessment Instrument (CCAI-S) among Swedish occupational therapists. The study had a cross-sectional design. A web-based questionnaire was e-mailed to a randomised sample of 428 occupational therapists to investigate the construct validity, reliability and utility of the CCAI-S. Factor analysis was performed as well as descriptive statistics.The findings from study I revealed high content validity for all 24 items. However, six items needed reformulations and exemplifications. Regarding utility, the results showed strong support for CCAI-S. The category ‘Interactions with clients’ showed that the CCAI-S could be utilised individually for the health professional and create a higher awareness of cultural questions in practice. The category ‘Workplace and its organisational support’ displayed potential for use in different workplaces regarding CCAI-S and indicated the importance of organisational support for health professionals in the development of cultural competence. The findings from study II regarding construct validity generated a three-factor model with the labels ‘Openness and awareness’, ‘Workplace support’ and ‘Interaction skills’. All three factors showed high factor loadings and contained 12 of the 24 original items. The Cronbach’s Alpha showed high support for the three-factor model. Concerning utility, the participants reported that all 24 items had high clinical relevance.In conclusion, the findings from the two studies indicated good measurement properties and high clinical relevance for the CCAI-S. This may sup-port the utilisation of CCAI-S in the Swedish context for health professionals, for instance occupational therapists. The results of the instrument development show that the upcoming published version of the CCAI-S can be a valuable self-assessment tool for health professionals who strive to improve in person-centred communication in encounters with foreign-born clients. CCAI-S can also be of support for the organisation to serve as a guide for what to focus on to develop cultural competence within the staff. Altogether this presumably influence the effectiveness of the healthcare and enhance the evidence of interventions for foreign-born clients. To develop an instrument is an iterative process requiring several evaluations and tests in various settings and populations. Therefore further psychometric testing and utility studies on the CCAI-S is crucial.

  • Crystal structure of protein C inhibitor provides insights into hormone binding and heparin activation

    • Protein C inhibitor (PCI) is a member of the serpin family that has many biological functions. In blood it acts as a procoagulant, and, in the seminal vesicles, it is required for spermatogenesis. The activity of PCI is affected by heparin binding in a manner unique among the heparin binding serpins, and, in addition, PCI binds hydrophobic hormones with apparent specificity for retinoids. Here we present the 2.4 Angstrom crystallographic structure of reactive center loop (RCL) cleaved PCI. A striking feature of the structure is a two-turn N-terminal shortening of helix A, which creates a large hydrophobic pocket that docking studies indicate to be the retinoid binding site. On the basis of surface electrostatic properties, a novel mechanism for heparin activation is proposed.

    • Antithrombin (AT) is a serine protease inhibitor that has thrombin, factors IXa and Xa as target proteases. In addition to active native AT, two other forms have been identified in plasma: the reactive center loop inserted cleaved and latent, uncleaved forms. Both have been shown to be present in normal human blood. Latent AT forms a dimer with native AT in vitro, thus inactivating the native form. Here we describe a mouse monoclonal antibody, 8C8, that is specific for latent AT. The affinity of 8C8 was found to be 500-fold higher for latent than for native AT and 5000-fold higher for latent than for cleaved AT. A sandwich assay was developed to measure the concentration of latent AT in plasma, which was found to be similar to 4.8 mg L-1 in healthy individuals. The K-D of the interaction between native and latent AT was found to be 51 mu M, i.e. far above the plasma concentration of both native and latent AT, indicating a negligible complex formation in blood.

  • Development of Methods for Measuring Protein C Inhibitor and Antithrombin: Use of Monoclonal Antibodies against the Reactive Center Loop-Inserted Forms of the Serpins

    • Protein C inhibitor (PCI) and antithrombin (AT) are serine protease inhibitors (serpins) that are involved in the regulation of coagulation. Like other inhibitory serpins, PCI and AT adopt different structural conformations that are related to their functions. The cleaved, inactive form is a result of cleavage by a protease, and the latent form, which is also inactive, can arise due to a mutation in the serpin. Methods that quantify the different forms can be useful as diagnostic tools. The aim of this work was to develop such methods for measuring AT and for measuring cleaved PCI in complex with APC (APCI-PCI). The high-affinity monoclonal antibody M36 (KD ~50 pM) was produced against cleaved PCI. M36 is specific enough to discriminate between cleaved and native PCI and thus it was used as the catcher antibody, along with a monoclonal antibody against protein C as detector, in an immunoassay developed to measure the APC-PCI complex in blood plasma. The mean concentration of APC-PCI was found to be 0.30 µg/L in healthy individuals, and the levels of this complex were elevated in patients who had undergone hip surgery and were lowered in patients treated with warfarin. A crystal of cleaved PCI was obtained, which revealed a short helix A, and this is similar to the two hormone-binding serpins called corticosteroid-binding globulin and thyroxine-binding globulin. A potential binding site for retinoic acid was found in PCI, and moreover, possible novel binding sites for heparin were exposed, which suggested an alternative model for heparin-activated inhibition of APC by PCI. The monoclonal antibody M9 against reactive center loop-inserted antithrombin (AT) was produced and was found to have affinities (KD values) of 3.07 and 2.27 µM for latent and cleaved AT, respectively. An immunoassay for measuring cleaved AT was developed using M9 as catcher antibody and the monoclonal antibody M27 (specific for cleaved and native AT) as detector. The median level of cleaved AT was 1.3 mg/L in healthy individuals. No correlation was found between cleaved AT and the thrombin-antithrombin (TAT) complex in patients with deep venous thrombosis. The monoclonal antibody 8C8 was produced against latent AT, and it showed affinities (KD values) of 0.92 nM, 0.57 µM, and 5.65 µM for latent, native and cleaved AT, respectively. An immunoassay was developed for measuring latent AT, using M9 (specific for cleaved and latent AT) as catcher antibody and 8C8 as detector. The assay showed that the median level of latent AT was 4.7 mg/L in healthy individuals. Commercial preparations of native AT were found to contain ?10% latent AT. In addition, a weak interaction (KD = 51 µM) was observed between latent and native AT, which contradicts the formation of a stable dimer in vivo.

    • The risk of venous thromboembolism is enhanced in pregnant carriers of the Factor V Leiden mutation. The primary aim of the study was to compare prothrombin fragments 1+2, soluble fibrin and D-dimer levels in pregnant Factor V Leiden mutation carriers with those in non-carriers. Secondary aims were to evaluate whether these biomarkers could predict placenta-mediated complications or venous thromboembolism, and to study blood coagulation after caesarean section with thromboprophylaxis and after vaginal delivery without thromboprophylaxis.
