  • Essentials of Nursing Care in Randomized Controlled Trials of Nurse-Led Interventions in Somatic Care : A Systematic Review

    • Background: Nursing practice has to contribute to evidence pointing out why there is a need for more nurse-designed randomized control trials (RCTs) focusing on evidence-based practice (EBP). How far this EBP has progressed in different health aspects is usually established by systematic reviews of RCTs. Nurse-led RCTs exist but no study has addressed the essentials of nursing care. Aim: The aim was therefore to determine the essentials of nurses’ interventions by means of nurse-led RCTs in somatic care focusing on the stated context, goals, content, strategies as well as the nurse’s role related to effectiveness. Methods: A systematic review was realized according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data extraction process (n = 55) from PubMed and CINAHL. Results: Of the RCTs in somatic care, 71% showed a positive effectiveness of nurse-led interventions, of which the nurse had a significant role with regard to being the main responsible in 67% of the studies. Also, 47% of the RCTs presented a theoretical standpoint related to the nurse-led interventions and most prominent were international evidence-based guidelines. Goals were found to have either a patient-centered or a professional-centered ambition. Strategies were based on patient-directed initiatives, nurse-patient-directed initiatives or nurse-directed initiatives, while contents were built upon either a patient-nurse interaction or a nursing management plan. Conclusions: This review underlines the necessity of a holistic view of a person, as nurse-led RCTs comprising a patient-centered ambition, patient-directed initiative and patient-nurse interaction plan showed beneficial nursing care effectiveness, particularly if theory-based. In a nurse-led RCT, a basic theoretical perspective is advantageous as well as to elucidate the role of the nurse in relation to the estimated effects.

  • Air-Entrainment in Wall-Jets Using SLIPI in a Heavy-Duty Diesel Engine

    • Mixing in wall-jets was investigated in an optical heavy-duty diesel engine with several injector configurations and injection pressures. Laser-induced fluorescence (LIF) was employed in non-reacting conditions in order to quantitatively measure local equivalence ratios in colliding wall-jets. A novel laser diagnostic technique, Structured Laser Illumination Planar Imaging (SLIPI), was successfully implemented in an optical engine and permits to differentiate LIF signal from multiply scattered light. It was used to quantitatively measure local equivalence ratio in colliding wall-jets under non-reacting conditions. Mixing phenomena in wall-jets were analyzed by comparing the equivalence ratio in the free part of the jet with that in the recirculation zone where two wall-jets collide. These results were then compared to φ predictions for free-jets. It was found that under the conditions tested, increased injection pressure did not increase mixing in the wall-jets. Comparisons with free-jet predictions further indicated that mixing in wall-jets is less effective than in free-jets for identical conditions and downstream distances. The confined nature of the wall-jet in the optical engine is suspected to be the reason for these observations. A rapid leaning-out of the jet after end of injection was observed for all cases, but this enhanced mixing was not transmitted to the wall-jet.

    • Consumer report for the company in question (the Volvo Truck Company) which is financed by the company and partly also by a research foundation. It is a matter of a work that in this particular case were carried out during nine years in a number of experimental workshops located outside the Chalmers University of Technology. These workshops were financed by the Volvo Automobile and Truck companies. This achievement, as the final contribution to the Swedish automotive industry after having already treated this research field/problem area for more than two decades before this particular period if time (involving several junior and senior research competencies, as well as industrial and governmental foundings). Specifically, this publication explains how the authors dealt with the product architecture and product variation of the automotive products disassembled, which proved to necessary to be carried out in one of the experimental workshops (several complete automobiles and one heavy truck chassis were disassembled combined with having product data accessible in form of paper print-outs as well as by appropriate computer connections to the two Volvo companies etc.). That is, by using a special sort of “design analysis by means of axonometric hand-drawn illustrations” (see a conference contribution from 1992 that also is registered and available as o PDF-document at Chalmers Public Library (CPL) (a self-developed method found out together with a junior research competency from the School of Architecture at Chalmers).

This achievement, as the final contribution to the Swedish automotive industry after having already treated this research field/problem area for more than two decades before this particular period if time (involving several junior and senior research competencies, as well as industrial and governmental foundings). A comment: As was the praxis during this (early) period of time at Chalmers University of Technology, as well as probably at some other universities. This means that frequent and vast academic publication by means of e.g. conference papers and refereed contributions to scientific journals was almost unheard of. At least so for some of the applied science disciplines. However, today are the situation indeed very different (i.e. earlier was the publication in many respects focused on trying to satisfy the executive administrator the research foundation in question etc.). To be more precise, this early praxis was definitively the case for the research and development work carried out at the Department of Transportation (a broadly applied science discipline), which was characterized by more practical strives to e.g. engineer research results. Rather than relying on truer scientific efforts in a narrower sense. Functional satisfaction (i.e. something engineered work as planned instead verification/falsification of a hypothesis) was judged as being a primary aim of this research and development work.  Therefore are (in the context of the publications registered in Chalmers Public Library CPL) the author making distinctions between consumer reports, on one hand. And, one the other hand, reports and preprints (manuscripts), while instead the academically orientated publications are various sort of scientific contributions presented at conferences or published in scientific journals (actually it is somewhat more complex, but further explanations are omitted here). The consumer reports were aimed at a various external organization with who the authors cooperated, and these are in many cases not available in PDF format to be easily read by logging in at CPL.  This particular approach has judged as being fair/relevant fair in order to mirror of work conducted by the author or authors during the passing decades. Besides, the competencies at Chalmers Library, who actually have been very helpful, have not managed to provide appropriate answers to the various questions implied being at hand just above (this despite trying to get hold of such answers, hence this very comment).

    • Background: Teamcare should, like all patient care, also contribute to evidence-based practice (EBP). Randomized controlled trials (RCTs) focusing on teamcare have been performed but no study has addressed its essentials. How far this EBP has progressed in different health aspects is generally established in systematic reviews of RCTs.Aim: The aim is to determine the essentials of teamcare including the nurse profession in RCTs of multi- or interdisciplinary interventions in somatic care focusing on the stated context, goals, strategies, content as well as effectiveness of quality of care.Methods: A systematic review was performed according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data selection process of 27 articles from PubMed and CINAHL.Results: Eighty-five percent of RCTs in somatic care showed a positive effectiveness of teamcare interventions, of which interdisciplinary ones showed a greater effectiveness compared with the multidisciplinary approach (100% vs 76%). Also theory-based RCTs presented higher positive effectiveness (85%) compared with non-theory-based RCTs (79%). The RCTs with positive effectiveness showed greater levels for professional-centered ambition in terms of goals and for team-directed initiatives in terms of strategy, and a significantly higher level for patient-team interaction plans in terms of content was shown.Conclusions: Teamcare RCTs are still grounded in the multidisciplinary approach having a professional-centered ambition while interdisciplinary approaches especially those that are theory-based appear to be essential with regard to positive effectiveness and preferable when person-centered careis applied.

    • This present thesis explores the hermeneutical theology of the North American catholic theologian David Tracy and the challenges plurality poses to modern theology. According to Tracy this challenge is to find a way to be open to plurality and the enrichments it brings, but that openness should not degenerate into a repressive tolerance, where anything can be said because nothing is taken seriously.Tracy thinks that theology should meet this double challenge of plurality by being both critical and self-critical. In different periods of his authorship, he has developed three different configurations of this double critique. The first part of this thesis explores those configurations. The first one deals with the content of theology. Through a double critique Tracy employs an explicitly metaphysical mode of reflection. Later he configures the double critique through a mode of conversation where he focuses the method of theology. In the third configuration, where Tracy is interested in question of form in theology, he explores the fragment as a form for theology. A fragment is both fragmenting and fragmented.So the investigation into the different configurations of the double critique in the theology of Tracy deals with three different themes in theology: content, method and form. In the second part of the thesis these three themes are discussed and how they can be understood in a theology where self-critique is not seen as an imported but limited phase in theology but as an integral and vital part of theology’s way of existence.The conclusion of the thesis argues that Christian theology should have a form that is christomorphic in a particular kenotic way. That means that theology should work through forms that are not colonizing the inter-subjectively shared life-world but that actively makes an effort to provide space for the other to form his or her own identity
