    • Rapporten behandlar marknadens möjligheter och begränsningar inom välfärdsområdet både vad gäller finansiering och produktion. Finansieringsaspekten handlar om möjligheterna till ökat inslag av försäkringar avseende t.ex. sjukfrånvaro, arbetslöshet, sjukvård och äldreomsorg. Produktionsaspekten handlar om möjligheterna att intensifiera konkurrensen inom en rad områden som exempelvis skola, vård, omsorg och arbetsförmedling. Det finns ingen enkel formel för privatisering. På vilket sätt privatiseringen kan ske och hur långt man kan gå beror på omständigheterna i varje särskilt fall.

    • Essay I: Return to Education for the Deaf/Hearing Impaired and the Blind/Visually Impaired, 1991-2000    Mincer-equations are estimated for the deaf/hearing impaired and the blind/visually impaired. The results show that the estimates of the coefficient for the education variable are lower than for a comparison group, and that for several years it is not statistically significant that education for the blind/visually impaired has a positive effect on labour income. Essay II: Labour Income Distribution for the Deaf/Hearing Impaired and the Blind/Visually Impaired in Sweden, 1991-2000    Incomes or the deaf/hearing impaired and the blind/visually impaired are studied. They are compared with the income distribution for a comparison group. The results show that the income distribution is most unequal for the blind/visually impaired and that the average and median incomes are clearly lowest for this group. One explanation is a high share of zero earners. Essay III: Wages and Wage Distributions for the Deaf/Hearing Impaired and the Blind/Visually Impaired with and without Wage Subsidy    Mincer-equations with a wage subsidy dummy are estimated and wage distributions are studied for the deaf/hearing impaired and the blind/visually impaired. The coefficient for the wage subsidy dummy is discussed. From the wage distributions for full-time employed people we see that employed people with a wage subsidy have a more compact wage distribution compared to employed people without a wage subsidy. Essay IV: Employment for the Deaf/Hearing Impaired and the Blind/Visually Impaired during the 1990s    Employment rates for the deaf/hearing impaired and the blind/visually impaired are compared with a group that represents the Swedish population. The results show that the pattern and level of the employment rate are similar for the deaf/hearing impaired and the comparison group. The employment rate is clearly lower for the blind/visually impaired people, however.

    • This open study examined the effect of smoking reduction and smoking cessation on established cardiovascular risk factors. Fifty-eight healthy adult smokers (smoking >or=15 cigarettes/day for at least 3 years) were provided with nicotine nasal spray (to be used ad libitum) and asked to stop smoking. The primary goal during the first 8 weeks, however, was to reduce their daily smoking by at least 50%. Subjects were then followed for another 8 weeks; at this point, 33 participants had successfully stopped smoking. Cardiovascular risk factors including fibrinogen, hemoglobin, hematocrit, triglycerides, and cholesterol were measured at baseline and at 9 and 17 weeks. After 8 weeks of smoking reduction, the mean number of cigarettes smoked per day had decreased from 21.5 +/- 0.6 (baseline) to 10.8 +/- 0.6 (p < 0.001). This was accompanied by significant improvements in fibrinogen (from 2.9 +/- 0.1 g/l at baseline to 2.6 +/- 0.1 g/l, p = 0.011), white blood cells (from 7.0 +/- 0.4 to 6.2 +/- 0.3 x 10(9)/l, p = 0.005) and the high-density/low-density lipoprotein (HDL/LDL) ratio (0.33 +/- 0.03 to 0.37 +/- 0.03, p < 0.005). Following 8 weeks of abstinence from smoking, the mean white blood cell count was further reduced (to 6.1 +/- 0.3 x 10(9)/l, p = 0.026 vs. baseline) and there were also significant improvements in HDL (from 1.16 +/- 0.06 mmol/l at baseline to 1.32 +/- 0.06, p < 0.001) and LDL (from 3.78 +/- 0.16 mmol/l at baseline to 3.52 +/- 0.17, p = 0.015). In conclusion, 8 weeks of smoking reduction resulted in clinically significant improvements in established cardiovascular risk factors. These improvements were even greater after an additional period of abstinence from smoking.

    • The purpose of this paper is to compare pension schemes with respect to their intergenerational redistributive effects caused by economic and demographic changes. It is shown how these effects depend on the specific design of the pension scheme, with special attention devoted to the indexation problem. There is a potential trade-off between financial stability of the pension system and a “desired” distribution between generations. A buffer fund is often seen as the remedy to demographic strain and potential conflict. Therefore, the possibility of accumulating (and de-cumulating) a buffer fund is included. A lifecycle perspective is applied and the risk-sharing is measured by different generations’ rate of return. The analysis is carried out within the framework of an over-lapping generation model in the setting of a stylised economy.

  • Designing an Optimal Sickness Insurance. Some Evidence from the Swedish Experience

    • Risk adverse individuals demand a sickness insurance to cover the risk of income loss due to work incapacity. Since health or sickness is not easy to observe, there is asymmetric information causing the well-known problems with adverse selection and moral hazard. Furthermore, sickness, with resulting inability to work, is to some extent private information hidden to the insurer. One effect is that an efficient market solution will not be established. In Sweden, as in many other countries, there is a public sickness insurance. It is compulsory, thus dealing with the adverse selection problem. The way of handling moral hazard, emanating from private information, is co-insurance and control. The degree and form of co-insurance and control determines the individual's cost of sick leave or other forms of absence from work, thus affecting sick leave. The Swedish sickness insurance offers a unique possibility to analyse incentive effects of different designs. The last decade there have been numerous changes, for example the remuneration level has changed 9 times since early 90ies. The purpose of the paper is to - analyse how sickness leave has responded to the changes in rules - how reactions differ by age, sex, socio-economic group, and other factors We give a short description of the Swedish sickness insurance and the changes that have been legislated the last decade. We also give a description of how absence from work has developed during this period. In analysing sickness leave we use a labour supply model within the context of a labour contract model. The budget restriction changes due to changes in the sickness insurance. To study the responses we use two different data sets, both consisting of panel data analysed by count data and limited/dependent variables approaches.
