  • Global burden of peripheral artery disease and its risk factors, 1990-2019 : a systematic analysis for the Global Burden of Disease Study 2019

    • Background: Peripheral artery disease is a growing public health problem. We aimed to estimate the global disease burden of peripheral artery disease, its risk factors, and temporospatial trends to inform policy and public measures.Methods: Data on peripheral artery disease were modelled using the Global Burden of Disease, Injuries, and Risk Factors Study (GBD) 2019 database. Prevalence, disability-adjusted life years (DALYs), and mortality estimates of peripheral artery disease were extracted from GBD 2019. Total DALYs and age-standardised DALY rate of peripheral artery disease attributed to modifiable risk factors were also assessed.Findings: In 2019, the number of people aged 40 years and older with peripheral artery disease was 113 million (95% uncertainty interval [UI] 99 center dot 2-128 center dot 4), with a global prevalence of 1 center dot 52% (95% UI 1 center dot 33-1 center dot 72), of which 42 center dot 6% was in countries with low to middle Socio-demographic Index (SDI). The global prevalence of peripheral artery disease was higher in older people, (14 center dot 91% [12 center dot 41-17 center dot 87] in those aged 80-84 years), and was generally higher in females than in males. Globally, the total number of DALYs attributable to modifiable risk factors in 2019 accounted for 69 center dot 4% (64 center dot 2-74 center dot 3) of total peripheral artery disease DALYs. The prevalence of peripheral artery disease was highest in countries with high SDI and lowest in countries with low SDI, whereas DALY and mortality rates showed U-shaped curves, with the highest burden in the high and low SDI quintiles.Interpretation: The total number of people with peripheral artery disease has increased globally from 1990 to 2019. Despite the lower prevalence of peripheral artery disease in males and low-income countries, these groups showed similar DALY rates to females and higher-income countries, highlighting disproportionate burden in these groups. Modifiable risk factors were responsible for around 70% of the global peripheral artery disease burden. Public measures could mitigate the burden of peripheral artery disease by modifying risk factors.

    • Follow-up is integral for hematopoietic cell transplantation (HCT) care to ensure surveillance and intervention for complications. We characterized the incidence of and predictors for being lost to follow-up. Two-year survivors of first allogeneic HCT (10,367 adults and 3865 children) or autologous HCT (7291 adults and 467 children) for malignant/nonmalignant disorders between 2002 and 2013 reported to the Center for International Blood and Marrow Transplant Research were selected. The cumulative incidence of being lost to follow-up (defined as having missed 2 consecutive follow-up reporting periods) was calculated. Marginal Cox models (adjusted for center effect) were fit to evaluate predictors. The 10-year cumulative incidence of being lost to follow-up was 13% (95% confidence interval [CI], 12% to 14%) in adult allogeneic HCT survivors, 15% (95% CI, 14% to 16%) in adult autologous HCT survivors, 25% (95% CI, 24% to 27%) in pediatric allogeneic HCT survivors, and 24% (95% CI, 20% to 29%) in pediatric autologous HCT survivors. Factors associated with being lost to follow-up include younger age, nonmalignant disease, public/no insurance (reference: private), residence farther from the tranplantation center, and being unmarried in adult allogeneic HCT survivors; older age and testicular/germ cell tumor (reference: non-Hodgkin lymphoma) in adult autologous HCT survivors; older age, public/no insurance (reference: private), and nonmalignant disease in pediatric allogeneic HCT survivors; and older age in pediatric autologous HCT survivors. Follow-up focusing on minimizing attrition in high-risk groups is needed to ensure surveillance for late effects.

    • Smart polymers (SP) have become one important class of polymers and their applications have been increasing significantly. Last two to three decades have witnessed explosive growth in the subject. SP which are also known as stimuli-responsive soluble insoluble polymers or environmentally sensitive polymers have been used in the area of biotechnology, medicine and engineering. The present review is aimed to highlight the applications of SP when these polymers are presented in three common physical forms (i) linear free chains in solution where polymer undergoes a reversible collapse after an external stimulus is applied, (ii) covalently cross-linked reversible gels where swelling or shrinking of the gets can be triggered by environmental change and (iii) chain adsorbed or surface-grafted form, where the polymer reversibly swells or collapses on surface, once an external parameter is changed. Though there are number of reviews coming up in this area in recent times, the present review mainly addresses the developments of SP in the last decade with specific application areas of bioseparations, protein folding, microfluidics and actuators, sensors, smart surfaces and membranes.

  • Genome-wide association analysis identifies six new loci associated with forced vital capacity

    • Forced vital capacity (FVC), a spirometric measure of pulmonary function, reflects lung volume and is used to diagnose and monitor lung diseases. We performed genome-wide association study meta-analysis of FVC in 52,253 individuals from 26 studies and followed up the top associations in 32,917 additional individuals of European ancestry. We found six new regions associated at genome-wide significance (P < 5 × 10(-8)) with FVC in or near EFEMP1, BMP6, MIR129-2-HSD17B12, PRDM11, WWOX and KCNJ2. Two loci previously associated with spirometric measures (GSTCD and PTCH1) were related to FVC. Newly implicated regions were followed up in samples from African-American, Korean, Chinese and Hispanic individuals. We detected transcripts for all six newly implicated genes in human lung tissue. The new loci may inform mechanisms involved in lung development and the pathogenesis of restrictive lung disease.

    • The revised NEO Personality Inventory (NEOPI-R), popularly known as the five-factor model, defines five personality factors: Neuroticism, Extraversion, Openness to Experience, Agreeableness, and Conscientiousness. The structural correlates of these personality factors are still a matter of debate. In this work, we examine the impact of subtle cognitive deficits on structural substrates of personality in the elderly using DTI derived white matter (WM) integrity measure, Fractional Anisotropy (FA). We employed canonical correlation analysis (CCA) to study the relationship between personality factors of the NEOPI-R and FA measures in two population groups: healthy controls and MCI. Agreeableness was the only personality factor to be associated with FA patterns in both groups. Openness was significantly related to FA data in the MCI group and the inverse was true for Conscientiousness. Furthermore, we generated saliency maps using bootstrapping strategy which revealed a larger number of positive correlations in healthy aging in contrast to the MCI status. The MCI group was found to be associated with a predominance of negative correlations indicating that higher Agreeableness and Openness scores were mostly related to lower FA values in interhemispheric and cortico-spinal tracts and a limited number of higher FA values in cortico-cortical and cortico-subcortical connection. Altogether these findings support the idea that WM microstructure may represent a valid correlate of personality dimensions and also indicate that the presence of early cognitive deficits led to substantial changes in the associations between WM integrity and personality factors.

  • Inertial particles in superfluid turbulence : Coflow and counterflow

    • We use pseudospectral direct numerical simulations to solve the three-dimensional (3D) Hall-Vinen-Bekharevich-Khalatnikov (HVBK) model of superfluid helium. We then explore the statistical properties of inertial particles, in both coflow and counterflow superfluid turbulence (ST) in the 3D HVBK system; particle motion is governed by a generalization of the Maxey-Riley-Gatignol equations. We first characterize the anisotropy of counterflow ST by showing that there exist large vortical columns. The light particles show confined motion as they are attracted toward these columns, and they form large clusters; by contrast, heavy particles are expelled from these vortical regions. We characterize the statistics of such inertial particles in 3D HVBK ST: (1) The mean angle (SIC)(tau) between particle positions, separated by the time lag r, exhibits two different scaling regions in (a) dissipation and (b) inertial ranges, for different values of the parameters in our model; in particular, the value of (SIC)(tau), at large r, depends on the magnitude of U-ns. (2) The irreversibility of 3D HVBK turbulence is quantified by computing the statistics of energy increments for inertial particles. (3) The probability distribution function (PDF) of energy increments is of direct relevance to recent experimental studies of irreversibility in superfluid turbulence; we find, in agreement with these experiments, that, for counterflow ST, the skewness of this PDF is less pronounced than its counterparts for coflow ST or for classical fluid turbulence.

  • Large-Scale Genome-Wide Association Studies and Meta-Analyses of Longitudinal Change in Adult Lung Function

    • Background: Genome-wide association studies (GWAS) have identified numerous loci influencing cross-sectional lung function, but less is known about genes influencing longitudinal change in lung function. Methods: We performed GWAS of the rate of change in forced expiratory volume in the first second (FEV1) in 14 longitudinal, population-based cohort studies comprising 27,249 adults of European ancestry using linear mixed effects model and combined cohort-specific results using fixed effect meta-analysis to identify novel genetic loci associated with longitudinal change in lung function. Gene expression analyses were subsequently performed for identified genetic loci. As a secondary aim, we estimated the mean rate of decline in FEV1 by smoking pattern, irrespective of genotypes, across these 14 studies using meta-analysis. Results: The overall meta-analysis produced suggestive evidence for association at the novel IL16/STARD5/TMC3 locus on chromosome 15 (P = 5.71 x 10(-7)). In addition, meta-analysis using the five cohorts with >= 3 FEV1 measurements per participant identified the novel ME3 locus on chromosome 11 (P = 2.18 x 10(-8)) at genome-wide significance. Neither locus was associated with FEV1 decline in two additional cohort studies. We confirmed gene expression of IL16, STARD5, and ME3 in multiple lung tissues. Publicly available microarray data confirmed differential expression of all three genes in lung samples from COPD patients compared with controls. Irrespective of genotypes, the combined estimate for FEV1 decline was 26.9, 29.2 and 35.7 mL/year in never, former, and persistent smokers, respectively. Conclusions: In this large-scale GWAS, we identified two novel genetic loci in association with the rate of change in FEV1 that harbor candidate genes with biologically plausible functional links to lung function.

  • Engineered microstructure evolution in inconel 718 via laser powder bed fusion through a novel element-based approach

    • Engineered microstructure enables localised control in mechanical properties within a single material. In this research, a novel element-based approach is proposed to engineer the microstructure by varying the laser power and scan speed in laser powder bed fusion (L-PBF) process. A sample containing 25 elements were printed at different laser power and scan speed values and the resultant microstructure exhibited that the textured grains increase with laser energy density (LED). Subsequently, the measured hardness of the element printed at highest LED was approximately 14 % higher than the element printed at lowest LED. Current research is a proof of concept for the element-based approach to engineer the microstructure through L-PBF technique which can be extended to the component level by correlating the microstructure with element position and laser parameters.

  • First-passage-time problem for tracers in turbulent flows applied to virus spreading

    • We study the spreading of viruses, such as SARS-CoV-2, by airborne aerosols, via a first-passage-time problem for Lagrangian tracers that are advected by a turbulent flow: By direct numerical simulations of the three-dimensional (3D) incompressible Navier-Stokes equation, we obtain the time t(R) at which a tracer, initially at the origin of a sphere of radius R, crosses the surface of the sphere for the first time. We obtain the probability distribution function P(R, t(R)) and show that it displays two qualitatively different behaviors: (a) for R << L-I, P(R, t(R)) has a power-law tail similar to t(R)(-alpha), with the exponent alpha = 4 and L-I the integral scale of the turbulent flow; (b) for L-I less than or similar to R, the tail of P(R, t(R)) decays exponentially. We develop models that allow us to obtain these asymptotic behaviors analytically. We show how to use P(R, t(R)) to develop social-distancing guidelines for the mitigation of the spreading of airborne aerosols with viruses such as SARS-CoV-2.
