    • Nb(C,N) precipitates were studied in a niobium-stabilised stainless steel (AISI 347) statically aged at 700 degrees C. Scanning electron microscopy and energy filtered transmission electron microscopy were used to determine the volume fraction and precipitate size of primary and secondary Nb(C,N) after ageing times between 0 and 70,000 h. The experimental data were correlated with simulations of Nb(C,N) formation based on the assumption that the process is controlled by diffusion. These simulations provide a rationale for the existence of two sets of mobium carbonitrides in commercial tubes of AISI 347. Growth of primary Nb(C,N) occurred essentially during manufacturing, with no significant growth at 700 degrees C. Rapid dissolution and re-precipitation of secondary Nb(C,N) occurred during manufacturing. Coarsening at 700 degrees C of secondary particles was modelled using the Lifshitz-Slyozov-Wagner theory, which overestimated the coarsening rate. These problems were overcome with a model developed by the authors. This model takes both growth and coarsening into account.

    • Compared to the successful probing of genetic causes of monogenic disorders, dissecting the genetics of complex polygenic diseases has until recently been a fairly slow and cumbersome process. With the introduction of whole genome wide association studies (WGAS) the situation dramatically changed in 2007. The results from several recent WGAS on type 2 diabetes (T2D) and obesity have identified at least eighteen genes consistently associated with T2D. Many of the genes implicate pancreatic beta-cell function in the pathogenesis of T2D whereas only one clearly associate with insulin resistance. The identified genes most likely merely represent the tip of the iceberg in the explanation behind T2D. Refined tools will have to provide a more complete picture of the genetic complexity of T2D over the next few years. In addition to common variants increasing susceptibility for the disease, rare variants with stronger effects, copy number variations, and epigenetic effects like DNA methylation and histone acetylation will become important. Nevertheless, today we are able for the first time to anticipate that the genetics of a complex disease like T2D really can be dissected.

  • Direct exciton quenching in single molecules of MEH-PPV at 77 K

    • Fluorescence blinking behaviour was observed for single molecules of the conjugated polymer poly[2-methoxy,5-(2'-ethylhexyloxy)-p-phenylene-vinylene] (MEH-PPV) at 77 and 300 K. No strict qualitative dependence on temperature was observed, despite of the expected suppression of exciton migration at the low temperature. We assume MEH-PPV molecules to form dense nanoparticles with a characteristic size of about 10 nm at the largest. In such small nanoparticles exciton can be efficiently quenched by a photogenerated quencher due to Forster transfer just after a few hops over the polymer nanoparticle. Thus, long-distance exciton migration is not necessary to have strong fluorescence intensity fluctuations. (C) 2004 Elsevier B.V. All rights reserved.

  • Evaluation of the ECG based Selvester scoring method to estimate myocardial scar burden and predict clinical outcome in patients with left bundle branch block, with comparison to late gadolinium enhancement CMR imaging

    • Background: Myocardial scar burden quantification is an emerging clinical parameter for risk stratification of sudden cardiac death and prediction of ventricular arrhythmias in patients with left ventricular dysfunction. We investigated the relationships among semiautomated Selvester score burden and late gadolinium enhancement-cardiovascular magnetic resonance (LGE-CMR) assessed scar burden and clinical outcome in patients with underlying heart failure, left bundle branch block (LBBB) and implantable cardioverter-defibrillator (ICD) treatment. Methods: Selvester QRS scoring was performed on all subjects with ischemic and nonischemic dilated cardiomyopathy at Skåne University Hospital Lund (2002-2013) who had undergone LGE-CMR and 12-lead ECG with strict LBBB pre-ICD implantation. Results: Sixty patients were included; 57% nonischemic dilated cardiomyopathy and 43% ischemic cardiomyopathy with mean left ventricular ejection fraction of 27.6% ± 11.7. All patients had scar by Selvester scoring. Sixty-two percent had scar by LGE-CMR (n = 37). The Spearman correlation coefficient for LGE-CMR and Selvester score derived scar was r = .35 (p = .007). In scar negative LGE-CMR, there was evidence of scar by Selvester scoring in all patients (range 3%-33%, median 15%). Fourteen patients (23%) had an event during the follow-up period; 11 (18%) deaths and six adequate therapies (10%). There was a moderate trend indicating that presence of scar increased the risk of clinical endpoints in the LGE-CMR analysis (p = .045). Conclusion: There is a modest correlation between LGE-CMR and Selvester scoring verified myocardial scar. CMR based scar burden is correlated to clinical outcome, but Selvester scoring is not. The Selvester scoring algorithm needs to be further refined in order to be clinically relevant and reliable for detailed scar evaluation in patients with LBBB.

    • The study presented in this research is a systematic human factors approach comparing two striking process accidents in Latin America: the Copiapó mining accident (2010), at the San José copper-gold mine, in Chile, and the FPSO CSM accident (2015), at Camarupim offshore oil field, in Brazil. Despite being different industrial segments - mining and O&G - more similarities than differences were observed in the treatment of process safety anomalies, especially those related to major accidents. The intense interactions between workers, equipment and processes, in both industries, have been making significant developments in the edge of innovation and technology, however increasing the complexity of risks in the workplaces. Furthermore, the differences between the preparation and handling of emergency situations show how complex, and critical, process safety is in these industrial areas. Aiming to adequately evidence how this complexity is intrinsically part of the various system that form the entire process, the FRAM (Functional ResonanceAnalysis Method) was utilized to model and analyses both accidents, under a human factors approach. Interactions and interrelations between LOPC, non technical skills, resilience and technical procedures were noticed as crucial for process safety and productivity of daily operations, as well as the preparedness for emergency situations.

    • Background: Idiopathic ventricular fibrillation (IVF) is a rare cause of sudden cardiac arrest which may pose therapeutic and prognostic challenges. To date, the only effective treatment for survivors of cardiac arrest is the insertion of an implantable cardioverter-defibrillator (ICD). We sought to review the long-term outcome of a Swedish cohort with IVF.Methods and results: Fifty patients with IVF diagnosis between 1988 and 2016 (mean age at index 34.3, 56% male), were followed for a median 13.8 years in this retrospective multicenter observational study. No cardiac mortality was reported. 32% (n = 16) of patients had recurrence of ventricular fibrillation or sustained ventricular tachycardia, requiring ICD therapy, at a median time of 1.9 years (range 0.1–20.3) from the index event. Annual incidence rate of ventricular tachyarrhythmia was 3.1%. Abnormal ECG at baseline did not predict appropriate ICD therapy (p = 0.56). During the follow-up period, 14% (n = 7) patients received a cardiac diagnosis. Follow-up genetic testing was low (26%), however did confirm pathogenic mutations in three cases.Conclusion: Idiopathic VF is a rare diagnosis with a relatively good prognosis provided ICD therapy is initiated. Routine clinical follow-up is recommended due to potential late emerging cardiac pathology. ECG changes are common, but have no prognostic value in determining the risk of ventricular arrhythmias recurrence. Screening for genetic diseases has previously been low, and this calls for improvement, especially since cheaper and more comprehensive genetic panels are now readily available.
