    • Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data.

  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • A new cloud dynamics and radiation database (CDRD) precipitation retrieval algorithm for satellite passive microwave (PMW) radiometer measurements has been developed. It represents a modification to and an improvement upon the conventional cloud radiation database (CRD) algorithms, which have always been prone to ambiguity. This part 2 paper of a series describes the methodology of the algorithm and the modeling verification analysis involved in creating a synthetic CDRD database for the Europe/Mediterranean basin region. This is followed by a proof-of-concept analysis, which demonstrates that the underlying CDRD theory based on use of meteorological parameters for reducing retrieval ambiguity is valid. This paper uses a regional/mesoscale model, applied in cloud resolving model (CRM) mode, to produce a large set of numerical simulations of precipitating storms and extended precipitating systems. The simulations are used for selection of millions of meteorological/microphysical vertical profiles within which surface rainfall is identified. For each of these profiles, top-of-atmosphere brightness temperature (TB) vectors are calculated (the vector dimension associated with the number of relevant cm-mm wavelengths and polarizations), based on an elaborate radiative-transfer equation (RTE) model system (RMS) coupled to the CRM. This entire body of simulation information is organized into the CDRD database, then used as a priori knowledge to guide a physical Bayesian retrieval algorithm in obtaining rainfall and associated precipitation parameters from the PMW satellite observations. We first prove the physical validity of our CRM-RMS simulations, by showing that the simulated TBs are in close agreement with observations. Agreement is demonstrated using dual-channel-frequency TB manifold sections, which quantify the degree of overlap between the simulated and observed TBs extracted from the full manifolds. Nevertheless, the salient result of this paper is a proof that the underlying CDRD theory is valid, found by combining subdivisions of the invoked meteorological parameter ranges of values and showing that such meteorological partitioning associates itself with distinct microphysical profiles. It is then shown that these profiles give rise to similar TB vectors, proving the existence of ambiguity in a CRD-type algorithm. Finally, we show that the CDRD methodology provides significant improvements in reducing retrieval ambiguity and retrieval error, especially for land surface backgrounds where contrasts are typically small between the rainfall TB signatures and surface emission signatures.

  • Care of family caregivers of persons with dementia (CaFCa) through a tailor-made mobile app : study protocol of a complex intervention study

    • Background: Globally, family members account for the main source of caregiving of persons with dementia living at home. Providing care to family members with dementia often has negative health consequences for caregivers such as stress, depression and low quality of life. Yet, formal support for family caregivers (FCs) is limited. Telehealth technology has the potential to provide health care and social support to FCs. This study aims to assess the effectiveness of providing support by healthcare professionals (HPs) through a mobile app in reducing stress, depressive symptoms and loneliness, and improving mental health and quality of life of FCs of persons with dementia.Methods: Using a pragmatic intervention design, this study will use pre- and post-intervention assessment to evaluate the effectiveness of the proposed intervention in a sample of 78 FCs of persons with dementia (PWD). The intervention will be implemented by approximately 5 HPs specialized in dementia care based in the municipalities in Sweden. The main thrust of the intervention is to provide professional support, with help of an interactive mobile app, to family members in their caregiving role for PWDs. Qualitative interviews with HPs and FCs form the groundwork of the development of the mobile app. By using the app on smart phone or tablet, the FC, in groups of 8-10, will communicate with peers and a HP exchanging ideas on how to deal with PWD's behavioral and cognitive changes and get support. They will also be able to discuss stressful events and access mindfulness exercises focused on themselves. Quantitative data will be collected before and at three time points after the 8-week intervention to assess changes in the health outcomes of the FCs. In-depth interviews will be conducted after the intervention to capture the experiences of FCs and HPs regarding the ease of use and acceptability of the app.Discussion: This tailor-made mobile app has the high potential to be a practical platform for supporting FCs to alleviate stress and improve mental health irrespective of distance to the nearest health care or social service center.

    • Many popular direction-of-arrival (DOA) estimators rely on the fact that the array response vector of the array is Vandermonde, for example, that of a uniform linear array (ULA). Array interpolation is a preprocessing technique to transform the array response vector of a planar array of arbitrary geometry to that of a ULA over an angular sector. While good approximation within the target sector is attained with the various existing array interpolation approaches, the response of the interpolated array in the out-of-sector region is at best partially controlled. Accordingly, out-of-sector signals, especially those highly correlated with the in-sector signals, can degrade significantly the performance of DOA estimators that rely on the Vandermonde form to work correctly. Recently, we proposed an improved array interpolation approach that takes into account the array response over the full azimuth. In this paper, we develop the idea further and present a simple data-adaptive array interpolation scheme that can provide significantly better accuracy in the DOA estimates. We present examples to demonstrate the effectiveness of our proposal

  • Single-trial event-related potential extraction through one-unit ICA-with-reference

    • Objective. In recent years, ICA has been one of the more popular methods for extracting event-related potential (ERP) at the single-trial level. It is a blind source separation technique that allows the extraction of an ERP without making strong assumptions on the temporal and spatial characteristics of an ERP. However, the problem with traditional ICA is that the extraction is not direct and is time-consuming due to the need for source selection processing. In this paper, the application of an one-unit ICA-with-Reference (ICA-R), a constrained ICA method, is proposed.Approach. In cases where the time-region of the desired ERP is known a priori, this time information is utilized to generate a reference signal, which is then used for guiding the one-unit ICA-R to extract the source signal of the desired ERP directly.Main results. Our results showed that, as compared to traditional ICA, ICA-R is a more effective method for analysing ERP because it avoids manual source selection and it requires less computation thus resulting in faster ERP extraction.Significance. In addition to that, since the method is automated, it reduces the risks of any subjective bias in the ERP analysis. It is also a potential tool for extracting the ERP in online application.
