  • Patients’ and Healthcare Personnel’s Experiences of Health Coaching with Online Self-Management in the Renewing Health Project

    • Background. Telehealth applications have shown positive effects for people with chronic conditions and their awareness of health. Objective. To describe patients’ and healthcare personnel’s experiences of using health coaching with online self-management in primary health care. Method. A pragmatic randomised controlled trial was conducted. Patients in the intervention group measured and reported medical parameters such as blood pressure, blood glucose, prothrombin complex (PK) values, and 2-channel ECG. Data were collected through a questionnaire, individual interviews with patients, and focus group discussions with healthcare personnel. The questionnaire was analysed using statistics; texts from interviews and focus groups were analysed using content analysis. Findings. Patients were satisfied and believed that the intervention had enhanced their care and increased accessibility without causing concerns about privacy. Although being positive, patients commented the lack of support and feedback from healthcare personnel. Healthcare personnel regarded the intervention valuable for the patients’ abilities to perform self-management healthcare tasks but preferred that patients did so without them supporting the patients. Conclusion. Patients expressed satisfaction and acceptance regarding the use of the application. It seems that healthcare personnel are convinced about the benefits for patients and the potential for the intervention but are not convinced about its benefits for healthcare organisations.

  • Providing Breastfeeding Support : Experiences from Child-HealthNurses

    • Background: Breastfeeding problems are common during the early period but can often be prevented or overcome with adequate support. Child-health nurses meet almost all children during their first weeks of life and play an important role in promoting breastfeeding and in strengthening parents’ confidence and their belief in their own ability. It is, therefore, important to gain more knowledge about child-health nurses’ experiences. Objective: To describe child-health nurses’ experiences of providing breastfeeding support. Methods: This qualitative study is descriptive with an inductive approach. A purposive sample of eight child-health nurses recruited from district health care centers participated. Data were collected through focus group interviews and analyzed with content analysis. Results: Child-health nurses consider it to be important to provide early breastfeeding support and that early hospital discharge following birth can complicate breastfeeding. Furthermore, the introduction of infant formula and tiny tastes given to the baby can be a barrier to breastfeeding. Parents’ confidence had an effect on breastfeeding, and breastfeeding is promoted by confident parents. Trends and cultural differences have an influence on parents’ attitudes toward breastfeeding. Child-health nurses stated the importance of having a consensus breastfeeding policy. Conclusion and Recommendation: A number of factors affect breastfeeding, and breastfeeding support from child-health nurses is important in the early stages after birth. To conclude, the support must be individually tailored with a focus on the parents’ needs. There is a need for greater cooperation between the maternal care and child-health care staff in order to provide adequate and continuous breastfeeding support throughout the care chain.

    • The recent development of different genetically modified mice with potentially interesting bone phenotypes has increased the demand for effective non-invasive methods to evaluate effects on bone of mice during growth and development, and for drug evaluation. In the present study, the skeleton was analyzed by repeated in vivo scans using dual energy X-ray absorptiometry (DXA) and peripheral quantitative computed tomography (pQCT). Ovariectomized (ovx) mice treated with parathyroid hormone (PTH) were used as an animal model to evaluate these two techniques at different times after the onset of treatment. Female mice (6 weeks of age) were allocated randomly to four groups: (1) sham-operated+vehicle; (2) ovx+vehicle; (3) sham-operated+PTH(1-84) 150 microg/kg per day; (4) ovx+PTH. Six weeks after ovariectomy the drug treatment began and was continued for 8 weeks. The total body bone mineral content (BMC) and total body areal bone mineral density (BMD) were measured by DXA. Ovariectomy reduced total body BMC and total body areal BMD by 6.2+/-1.7% and 2.6+/-0.9% respectively. No effect of PTH on total body BMC was seen during the treatment period. The trabecular volumetric BMD was measured by pQCT. Ovariectomy reduced the trabecular volumetric BMD by 52+/-6.7%. The pQCT technique detected a clear effect on trabecular volumetric BMD after 2 weeks of PTH treatment (ovx 94+/-29% and sham-operated 46+/-10% more than vehicle-treated). The cortical bone was measured in a mid-diaphyseal pQCT scan of the tibia. Ovariectomy reduced the cortical BMC by 9+/-2%. PTH treatment for 8 weeks increased cortical BMC in ovx mice. In conclusion, the pQCT technique is more sensitive than the DXA technique in the detection of bone loss after ovariectomy and increased bone mass after PTH treatment in mice. Notably, the pQCT, but not the DXA, technique detected a dramatic effect as early as after 2 weeks of PTH treatment. Dynamic pQCT measurements will be useful for monitoring skeletal changes during growth and development, and for drug evaluation in mice.

  • Stimulus Sequence and the Exponent of the Power Function for Loudness

    • In two experiments, 15 and 13 subjects estimated the loudness of 12 sound-pressure levels (38–104 dB; 6-dB intervals) of a 1000-Hz tone by the method of magnitude estimation with a modulus assigned to the first stimulus presented. The tone duration was 1 sec. and the interstimulus interval was 6 sec. The presentation order was systematically ascending-descending in one experiment and balanced-irregular in the other. The results indicate that (1) loudness is a power function of sound pressure with an exponent of 0.60 for the systematic order and 0.29 for the irregular order. (2) For both the irregular and systematic orders, a large step-size (12 or 18 dB) between the stimulus on Trial n and on Trial n-1 (or n-3) results in a slight assimilation effect. This also occurs for the small step-size (6 dB) in the irregular order. (3) The size of momentary exponents (based on two points, Trials n and n-l or n-3) depends on the sound pressures of successive stimuli, whether the steps are positive or negative, and whether the stimuli have been presented in systematic or irregular order. For positive steps, the momentary exponent is lower for a soft tone (Trial n) than for a loud tone, whereas for negative steps the momentary exponent is lower for a loud tone than for a soft tone. These effects are more pronounced when these stimuli are presented in an irregular order. A relative judgment model is offered for magnitude estimation. It assumes that subjects judge the loudness of a stimulus in terms of three reference markers: the minimum and maximum sound pressures as well as the sound pressure of the previous stimulus.

  • Transformation from entrepreneurship to entrepreneurships : creating alternatives?

    • In this paper our ambition is to provide with theoretical and empirical inspiration for studying contemporary constitutions of entrepreneurship. In specific, we seek to highlight how the transformation from entrepreneurship into forms of entrepreneurships has unfolded on various arenas. This means tracing the interplay between criticism of (traditional) entrepreneurship and the outbreak and dissemination of alternative entrepreneurships. In specific, we focus on the positive connotations that come with the alternative forms, a goodness that lures behind each and every corner, to see what it shapes as well as what shape entrepreneurship takes. Even if entrepreneurship research does pay some interest to the changing conditions for entrepreneurship, it seldom links these to changes in conditions for people, organizations and societies.

    • Objective: Cerebral palsy (CP) includes disturbances in muscular control caused by perinatal brain injury. Masticatory muscle involvement hampers functions such as chewing and talking. Bruxism and temporomandibular disorders are overrepresented. Neuromuscular blocks with botulinum toxin type A (BTX-A) may alleviate problems due to muscular hyperactivity. The aim was to evaluate masticatory muscle BTX-A injections in subjects with CP and bruxism. Methods: A prospective, parallel, randomized, placebo-controlled, and double-blind trial in 12 patients with CP was performed. End points were alterations in objective and subjective oral capacities after two BTX-A or corresponding placebo injections. Matched, healthy references were also evaluated. Results: The reference group demonstrated stronger and more efficient oral functions compared with the CP group. Subjective and objective oral capacities appeared to vary considerably between CP patients and also over time in this patient group and were poorly correlated. No significant effect of BTX-A compared with placebo on outcome variables was observed at group level, but continued treatment with BTX-A was requested by the majority of the patients. Conclusion: The evidence is unable to support the use of BTX-A for the treatment of affected masticatory muscles in CP, but the findings are inconclusive in certain respects. Larger, more homogeneous groups of CP patients need to be evaluated in future trials. ©2019 The Authors. Clinical and Experimental Dental Research published by John Wiley & Sons Ltd.

  • Changes in Physical Activity Are Associated with Corresponding Changes in Psychological Well-Being : A Pandemic Case Study

    • Societal crises and personal challenges are often followed by substantial changes in physicalactivity. Is there a link between such changes and psychological well-being? Seeking to answerthis question, we conducted a correlational study on a representative sample in Sweden during thefirst year of the COVID-19 pandemic (N = 1035). About 49% of the sample had decreased theirphysical activity compared to their self-reported activity level prior to the pandemic, whereas 32%had increased it. The results showed a positive and robust association between changes in dailyactivity level and corresponding changes in psychological well-being. Specifically, individuals whohad reduced their physical activity over the last year reported lower life satisfaction than before, andindividuals who had increased their physical activity reported higher life satisfaction than before.The amount of complete physical inactivity (sitting) showed a similar pattern as the exercise data,meaning that individuals who reported increasing inactivity per day also reported a greater declinein life satisfaction. Additional analyses showed that the association between daily activity level andlife satisfaction was somewhat stronger for men than for women, but there was no difference whencomparing individual versus organized activities. The current study was based on a cross-sectionaldesign, measuring self-reported change over time. Recent work from other research teams have usedlongitudinal data and experience-sampling in different settings, finding similar results. We concludethat there is good reason to recommend physical exercise as a coping strategy in difficult times.
