    • The European Hematology Association (EHA) Roadmap for European Hematology Research highlights major achievements in diagnosis and treatment of blood disorders and identifies the greatest unmet clinical and scientific needs in those areas to enable better funded, more focused European hematology research. Initiated by the EHA, around 300 experts contributed to the consensus document, which will help European policy makers, research funders, research organizations, researchers, and patient groups make better informed decisions on hematology research. It also aims to raise public awareness of the burden of blood disorders on European society, which purely in economic terms is estimated at (sic)23 billion per year, a level of cost that is not matched in current European hematology research funding. In recent decades, hematology research has improved our fundamental understanding of the biology of blood disorders, and has improved diagnostics and treatments, sometimes in revolutionary ways. This progress highlights the potential of focused basic research programs such as this EHA Roadmap. The EHA Roadmap identifies nine 'sections' in hematology: normal hematopoiesis, malignant lymphoid and myeloid diseases, anemias and related diseases, platelet disorders, blood coagulation and hemostatic disorders, transfusion medicine, infections in hematology, and hematopoietic stem cell transplantation. These sections span 60 smaller groups of diseases or disorders. The EHA Roadmap identifies priorities and needs across the field of hematology, including those to develop targeted therapies based on genomic profiling and chemical biology, to eradicate minimal residual malignant disease, and to develop cellular immunotherapies, combination treatments, gene therapies, hematopoietic stem cell treatments, and treatments that are better tolerated by elderly patients.

  • Heart transplantation across the antibodies against HLA and ABO

    • We have intentionally performed heart transplantation in a 5-year-old child, despite the most unfavourable risk factors for patient survival; the presence of high level of antibodies against donor's human leucocyte antigen (HLA) class I/II and blood group antigens. Pretransplant treatment by mycophenolate mofetil, prednisolone, tacrolimus, intravenous immunoglobulin, rituximab, protein-A immunoadsorption (IA) and plasma exchange reduced antibody titres against the donor's lymphocytes from 128 to 16 and against the donor's blood group antigen from 256 to 0. The patient was urgently transplanted with a heart from an ABO incompatible donor (A(1) to O). A standard triple-drug immunosuppressive protocol was used. No hyperacute rejection was seen. Antibodies against the donor's HLA antigens remained at a low level despite three acute rejections. Rising anti-A(1) blood group antibodies preceded the second rejection and were reduced by two blood group-specific IAs and remained at a low level. The patient is doing well despite the persistence of donor-reactive antibodies.

    • Recent advances, first and foremost the development of new immunosuppressive agents, have markedly improved the outcome of intestinal transplantation, which is a treatment option for patients with serious intestinal diseases who have become dependent on total parenteral nutrition. The first small bowel transplantation in Sweden was performed at Huddinge Hospital in 1997, in the adult patient with intestinal pseudo-obstruction. The article reports the course of this patient and an update of international progress in intestinal transplantation.

  • Pulmonary function and atherosclerosis in the general population : causal associations and clinical implications

    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

  • The intelligence discourse : the Swedish military intelligence (MUST) as a producer of knowledge

    • The Swedish Military Intelligence and Security Directorate (MUST) is a producer of knowledge, a knowledge that is fundamental for decisionmaking in foreign and security policy. The intelligence knowledge production is often held as objective, value neutral, and with the intention of ‘speaking truth onto power’. However, this study holds that such a perspective on intelligence knowledge production calls for a revision. Hence, the overall purpose of this study is to examine the characteristics of knowledge in intelligence analysis and also to investigate how that knowledge is affected by the social context of its production, the military intelligence service. The source material is of three kinds; first texts and documents, second interviews with intelligence analysts and managers, and third observations of seminars and meetings during the production process of estimates.The results are that there is a strong presence of an implicit interpretive framework that continuously influences and guides the knowledge production and thereby makes the knowledge dependent on one specific perspective contrary to the intentional objectivity within the intelligence service. Further, the study reveals that the social and discursive practices for intelligence knowledge production include a ‘logic of appropriateness’ suggesting the presence of a structured Denkkollektiv with a structured Denkstil. The actions and choices of the individuals are transformed to create conformity to the norms within the social discursive practices. Thus, the inherited frame of interpretation, as well as the socialised norm of staying within the existing accepted frames ofthinking and acting ends up to the stability and duration of the not always accurate and fruitful Denkstil.At the core of political science resides the question of how policy is shaped. Even though this study has focused merely on one organisation in a specific policy field in one country it brings insights to the knowledge and policy nexus.

    • Twenty-five weaned (35 days old) castrated male piglets from five different litters were used to evaluate the effect of chicory (C) and ribwort (R) forage inclusion in a cereal-based diet on the coefficient of total tract apparent digestibility (CTTAD) of organic matter (OM) and the dietary fibre fraction at 3 and 5 weeks post-weaning.Inclusion of chicory and ribwort had a negative impact on the CTTAD of OM (P<0.05), while diet had no effect (P>0.05) on the CTTAD of NDF and total non-starch polysaccharides (NSP). In contrast, the CTTAD of uronic acid increased (P<0.05) with inclusion of both chicory and ribwort. The CTTAD of OM, total NSP, arabinoxylans, arabinose, xylose and uronic acid was higher at 5 than at 3 weeks after weaning (P<0.01). (C) 2010 Elsevier B.V. All rights reserved.

    • Twenty-five weaned 35-day-old piglets were used in a 35-day growth experiment to evaluate the effect of inclusion of chicory and ribwort forage in a cereal-based diet on growth performance, feed intake, digestibility and shedding of faecal coliform bacteria. A total of seven experimental diets were formulated, a cereal-based basal diet (B), and six diets with inclusion of 40, 80 and 160 g/kg chicory (C40, C80 and C160) or ribwort (R40, R80 and R160). Piglets had ad libitum access to feed and water throughout the experiment. Three and five weeks post-weaning faeces samples for determination of digestibility were collected once a day for five subsequent days. Additional faeces samples for determination of coliform counts were collected at days 1, 16 and 35 post-weaning. Piglets fed diet R160 had the lowest average daily feed intake (DFI) and daily weight gain (DWG), and differed (P < 0.05) from piglets fed diets B, R40 and R80. There were no differences in DFI and DWG between the chicory diets and diet B. Inclusion of chicory or ribwort had a minor negative impact on the coefficient of total tract apparent digestibility (CTTAD) of dry matter, organic matter and crude protein, whereas inclusion of both chicory and ribwort resulted in higher CTTAD of non-starch polysaccharides and neutral detergent fibre (NDF). The CTTAD of arabinose were higher for diets C160 and R160 than for diet B (P, 0.05), and the CTTAD of uronic acid was higher for diets C40, C80, C160, R80 and R160 than for diet B (P < 0.05). Age affected the CTTAD for all parameters (P < 0.05) except for NDF, with higher values at 5 than at 3 weeks post-weaning. The coliform counts decreased with increasing age (P < 0.05), but was not affected by treatment. The results indicate that inclusion of up to 160 g/kg of chicory do not negatively affect performance, whereas high inclusion of ribwort have a negative impact on feed consumption and consequently on growth rate. Both herbs have a higher digestibility of fibre compared to cereal fibre. Chicory and ribwort are both promising as feedstuffs to weaned piglets, but the low palatability of ribwort limits the inclusion level.

  • Game intelligence in team sports

    • We set up a game theoretic framework to analyze a wide range of situations from team sports. A fundamental idea is the concept of potential ; the probability of the offense scoring the next goal minus the probability that the next goal is made by the defense. We develop categorical as well as continuous models, and obtain optimal strategies for both offense and defense. A main result is that the optimal defensive strategy is to minimize the maximum potential of all offensive strategies.
