    • Technological progress poses unique challenges for the public sector. New technology should be adopted, but it must always be done within the framework of good administration. It follows laws governing public administration must be continuously adapted. Sweden recently amended its secrecy legislation to facilitate the use of third-party cloud solutions by public authorities. When the amendment was enacted, most public sector organisations had already been using external cloud solutions for a long time. Today, there is as much pressure on authorities to implement AI technology as there ever was to move administration into the cloud. This paper uses traditional legal methodology to investigate if the Swedish secrecy legislation adequately enables the use of cloud-based GenAI solutions. Findings indicate that the recent amendment is likely insufficient and that there are significant practical hurdles for the application of the law, particularly with services from global cloud providers. The paper contributes to the understanding of Swedish law, and of the difficulties that can occur anywhere when policy makers and legislators do not move in tandem.

    • BackgroundIn Sweden, information on seroprevalence of tick-borne encephalitis virus (TBEV) in the population, including vaccination coverage and infection, is scattered. This is largely due to the absence of a national tick-borne encephalitis (TBE) vaccination registry, scarcity of previous serological studies and use of serological methods not distinguishing between antibodies induced by vaccination and infection. Furthermore, the number of notified TBE cases in Sweden has continued to increase in recent years despite increased vaccination.AimThe aim was to estimate the TBEV seroprevalence in Sweden.MethodsIn 2018 and 2019, 2,700 serum samples from blood donors in nine Swedish regions were analysed using a serological method that can distinguish antibodies induced by vaccination from antibodies elicited by infection. The regions were chosen to reflect differences in notified TBE incidence.ResultsThe overall seroprevalence varied from 9.7% (95% confidence interval (CI): 6.6-13.6%) to 64.0% (95% CI: 58.3-69.4%) between regions. The proportion of vaccinated individuals ranged from 8.7% (95% CI: 5.8-12.6) to 57.0% (95% CI: 51.2-62.6) and of infected from 1.0% (95% CI: 0.2-3.0) to 7.0% (95% CI: 4.5-10.7). Thus, more than 160,000 and 1,600,000 individuals could have been infected by TBEV and vaccinated against TBE, respectively. The mean manifestation index was 3.1%.ConclusionA difference was observed between low- and high-incidence TBE regions, on the overall TBEV seroprevalence and when separated into vaccinated and infected individuals. The estimated incidence and manifestation index argue that a large proportion of TBEV infections are not diagnosed.

    • Categorical work in chronic care is increasingly dependent on digital technologies for remote patient care. However, remote care takes many forms and while various types of digital technologies are currently being used, we lack a nuanced understanding of how to design such technologies for specific novel usages. In this paper, we focus on digital technologies for patient-generated health data and how their use changes categorical work in chronic care. Our aim is to understand how categorical work changes, which novel forms of categorical work emerge and what the implications are for the care relation. This paper is based on an ethnographic study of healthcare professionals' work at a pelvic cancer rehabilitation clinic and their interactions with patients. In this setting, supportive talks between patients and nurses are central. To understand the complexities of categorical work in chronic care when patient-generated health data are introduced, we contrast the traditional supportive talks with supportive talks where the nurses had access to the patients' patient-generated health data. We identify and analyze challenges connected to novel forms of categorical work. Specifically, we focus on categorical work and how it can undergo changes. Our empirical findings show how changes occur in the way patients' lived experience of the chronic disease aligns with the categories from chronic care, as well as in the way the nurse works with clinical categories during the talk. These insights help us further understand the implications of patient generated-data use in supportive talks. We contribute to an improved understanding of the use of patient-generated health data in clinical practice and based on this, we identify design implications for how to make categorical work more collaborative.

  • The Net Landscape Carbon Balance—Integrating terrestrial and aquatic carbon fluxes in a managed boreal forest landscape in Sweden

    • The boreal biome exchanges large amounts of carbon (C) and greenhouse gases (GHGs) with the atmosphere and thus significantly affects the global climate. A managed boreal landscape consists of various sinks and sources of carbon dioxide (CO2), methane (CH4), and dissolved organic and inorganic carbon (DOC and DIC) across forests, mires, lakes, and streams. Due to the spatial heterogeneity, large uncertainties exist regarding the net landscape carbon balance (NLCB). In this study, we compiled terrestrial and aquatic fluxes of CO2, CH4, DOC, DIC, and harvested C obtained from tall-tower eddy covariance measurements, stream monitoring, and remote sensing of biomass stocks for an entire boreal catchment (~68 km2) in Sweden to estimate the NLCB across the land–water–atmosphere continuum. Our results showed that this managed boreal forest landscape was a net C sink (NLCB = 39 g C m−2 year−1) with the landscape–atmosphere CO2 exchange being the dominant component, followed by the C export via harvest and streams. Accounting for the global warming potential of CH4, the landscape was a GHG sink of 237 g CO2-eq m−2 year−1, thus providing a climate-cooling effect. The CH4 flux contribution to the annual GHG budget increased from 0.6% during spring to 3.2% during winter. The aquatic C loss was most significant during spring contributing 8% to the annual NLCB. We further found that abiotic controls (e.g., air temperature and incoming radiation) regulated the temporal variability of the NLCB whereas land cover types (e.g., mire vs. forest) and management practices (e.g., clear-cutting) determined their spatial variability. Our study advocates the need for integrating terrestrial and aquatic fluxes at the landscape scale based on tall-tower eddy covariance measurements combined with biomass stock and stream monitoring to develop a holistic understanding of the NLCB of managed boreal forest landscapes and to better evaluate their potential for mitigating climate change.

    • This paper examines recombinability as a quality of digital infrastructures. The recombination of heterogeneous digital capabilities enables and increases the fit between the infrastructure and the practices it supports. However, there is yet limited understanding of how to design for recombinability in digital infrastructures. This issue is critical in the healthcare setting where information and data needs of patients and health personnel vary in scope and time. This study reports from a comparative case study on the design and use of two patient-centered digital infrastructures. We identify two design strategies for recombinability and analyze their rationales and challenges. © 2020, Springer Nature Switzerland AG.

    • In this work we take the point of departure from two thoroughly conducted casestudies, in which we have analysed certain knowledge work activities with followingimplications for design. Both studies clearly stated the call for a ICT-support whensearching information and sharing knowledge. For example, sharing bookmarksamong knowledge workers seemed to be a necessary and useful activity. Importantand valuable bookmarks often mirror what kind of information and knowledgepeople are looking for, and also what kind of forum they usually go to on the worldwide web. In this work we have developed a prototype, KnowMan, which managesbookmarks on the world wide web. The prototype is designed as a user-friendly"pad-interface" in which bookmarks can be collaboratively collected, shared andfurther explored and annotated by other knowledge workers. We end up with ageneral discussion about the possible effects of using KnowMan and what furtherdesign aspects that should be considered.

    • Digital transformation is associated with a fundamental change in the operating models of organizations and industries alike. At the same time, previous research highlights that existing governance practices may act as a deterrent to digital transformation. In this study, we explore how the IT governance of a large university counteracts necessary digital transformation in higher education over time. We show how the adoption of an industry-standard IT governance framework, through a series of generative mechanisms, leads to a vicious cycle that restricts digital transformation into mere computerization, thereby successfully counteracting digital transformation. In other words, the IT governance framework increasingly protects the organization from the organizational change brought on by new digital opportunities. This is discussed in relation to the literature on IT governance and digital transformation with the intent of contributing with a critical perspective on the widespread adoption and use of standard IT governance framework.

    • Most research on platform innovation studies the phenomena from a distance due to lack of access. This paper reports from within an action research case of platform development in a small-scale context. The case is based on a regional business initiative with the goal to establish an arena for mobile commerce and stimulate local industry growth. It was conducted in collaboration between researchers and third-party developers. The article shows how the initial phases of platformization are characterized by socio-technical arrangements, co-creation of boundary resources and intimate knowledge communication. The paper contributes to platform research by acknowledging a small-scale context for platform research. It further develops distributed tuning of boundary resources into an intimate fine-tuning process that we illustrate is valid for a small-scale context.

    • Healthcare is increasingly permeated with digital platforms supporting cooperative care involving different professional groups and also patients. New mobile technologies allow for patients to continuously monitor and document their symptoms to support better healthcare, as well as self-care. The successful design of such multi-user platforms calls for new design approaches involving heterogeneous conditions and goals. This paper analyzes theuse of boundary objects in design as a mediator for different users' needs and conditions. Our research is conducted at a clinic supporting cancer survivors in their struggles with treatment induced illnesses, a treatment heavily dependent on new medical research as well as on patient involvement. The data is collected ethnographically over two years following a design project that developed a digital platform to support the care provided by the clinic. We describe how useful boundary objects transform over time, from rich narratives, to conceptual formulations and finally into concrete prototypes of the platform. We argue that understanding such a transformation can inform the design of healthcare platforms and guide future design processes, where co-designing with boundary objects can be especially useful as a design approach when doing design complex settings, such as healthcare settings.
