  • Improved adherence to Swedish national guidelines for acute myocardial infarction : the Quality Improvement in Coronary Care (QUICC) study

    • BACKGROUND: The adherence to evidence-based treatment guidelines for acute myocardial infarction (AMI) is still suboptimal. Therefore, we designed a study to evaluate the effects of a collaborative quality improvement (QI) intervention on the adherence to AMI guidelines. The intervention used a national web-based quality registry to generate local and regular real-time performance feedback. METHODS: A 12-month baseline measurement of the adherence rates was retrospectively collected, comprising the period July 1, 2001, through June 30, 2002. During the intervention period of November 1, 2002, through April 30, 2003, multidisciplinary teams from 19 nonrandomized intervention hospitals were subjected to a multifaceted QI-oriented intervention. Another 19 hospitals, unaware of their status as controls, were matched to the intervention hospitals. During the postintervention measurement period of May 1, 2003, through April 30, 2004, a total of 6726 consecutive patients were included at the intervention (n = 3786) and control (n = 2940) hospitals. The outcome measures comprised 5 Swedish national guideline-derived quality indicators, compared between baseline and postintervention levels in the control and QUICC intervention hospitals. RESULTS: In the control and QI intervention hospitals, the mean absolute increase of patients receiving angiotensin-converting enzyme inhibitors was 1.4% vs 12.6% (P = .002), lipid-lowering therapy 2.3% vs 7.2% (P = .065), clopidogrel 26.3% vs 41.2% (P = .010), heparin/low-molecular weight heparin 5.3% vs 16.3% (P = .010), and coronary angiography 6.2% vs 16.8% (P = .027), respectively. The number of QI intervention hospitals reaching a treatment level of at least 70% in 4 or 5 of the 5 indicators was 15 and 5, respectively. In the control group, no hospital reached 70% or more in just 4 of the 5 indicators. CONCLUSIONS: By combining a systematic and multidisciplinary QI collaborative with a web-based national quality registry with functionality allowing real-time performance feedback, major improvements in the adherence to national AMI guidelines can be achieved.

    • Tilltal (Tillgängligt kulturarv för forskning i tal, 'Accessible cultural heritage for speech research') is a multidisciplinary and methodological project undertaken by the Institute of Language and Folklore, KTH Royal Institute of Technology, and The Swedish National Archives in cooperation with the National Language Bank and SWE-CLARIN [1]. It aims to provide researchers better access to archival audio recordings using methods from language technology. The project comprises three case studies and one activity and usage study. In the case studies, actual research agendas from three different fields (ethnology, sociolinguistics and interaction analysis) serve as a basis for identifying procedures that may be simplified with the aid of digital tools. In the activity and usage study, we are applying an activity-theoretical approach with the aim of involving researchers and investigating how they use - and would like to be able to use - the archival resources at ISOF. Involving researchers in participatory design ensures that digital solutions are suggested and evaluated in relation to the requirements expressed by researchers engaged in specific research tasks [2]. In this paper we focus on one of the case studies, which investigates the process by which personal experience narratives are transformed into cultural heritage [3], and account for our results in exploring how different types of text material from the archives can be used to find relevant sections of the audio recordings. Finally, we discuss what lessons can be learned, and what conclusions can be drawn, from our experiences of interdisciplinary collaboration in the project.

  • How to implement Illness Management and Recovery (IMR) in mental health service settings : evaluation of the implementation strategy

    • Background: The purpose of this study was to evaluate the implementation strategy used in the first-phase of implementation of the Illness Management and Recovery (IMR) programme, an intervention for adults with severe mental illnesses, in nine mental health service settings in Norway. Methods: A total of 9 clinical leaders, 31 clinicians, and 44 consumers at 9 service settings participated in the implementation of IMR. Implementation was conducted by an external team of researchers and an experienced trainer. Data were gathered on fidelity to the intervention and implementation strategy, feasibility, and consumer outcomes. Results: Although the majority of clinicians scored within the acceptable range of high intervention fidelity, their participation in the implementation strategy appeared to moderate anticipated future use of IMR. No service settings reached high intervention fidelity scores for organizational quality improvement after 12 months of implementation. IMR implementation seemed feasible, albeit with some challenges. Consumer outcomes indicated significant improvements in illness self-management, severity of problems, functioning, and hope. There were nonsignificant positive changes in symptoms and quality of life. Conclusions: The implementation strategy appeared adequate to build clinician competence over time, enabling clinicians to provide treatment that increased functioning and hope for consumers. Additional efficient strategies should be incorporated to facilitate organizational change and thus secure the sustainability of the implemented practice.

  • Comparison of Exact and Approximate methods for the Vehicle Routing Problem with Time Windows

    • This paper presents a comparison of two approaches for solving the vehicle routing problem with time windows (VRPTW). Scheduling of vehicles for pickup and delivery is a common problem in logistics and may be expressed as VRPTW, for which both exact and approximate techniques are available. It is therefore interesting to compare such techniques to evaluate their performance and figure what is the best option based on the instance features and size. In this work, we compared Mixed Integer Linear Programming (MILP) with Set-Based Particle Swarm optimization (S-PSO). Both algorithms are tested on the full 56 instances of the Solomon dataset. The results show that the two algorithms perform similarly for lower number of customers while there are significant differences for the cases with higher number of customers. For higher number of customers MILP consistently performs as good as or better than S-PSO for the clustered data, both with short and long scheduling horizons, while the S-PSO outperforms MILP in most cases with random and mixed random clustered data with long scheduling horizons. Furthermore when the algorithms perform the same with regards to the main objective (number of vehicles), MILP generally achieves a better result in the second objective (distance traveled).

    • Aging of power-optimized commercial 2.3 Ah cylindrical LiFePO4//graphite cells to be used in hybrid electric vehicle is investigated and compared for three different aging procedures; (i) using a simulated hybrid electric vehicle cycle within a narrow SOC-range, (ii) using a constant-current cycle over a 100% SOC-range, and (iii) stored during three years at 22 degrees C. Postmortem analysis of the cells is performed after full-cell electrochemical characterization and discharge. EIS and capacity measurements are made on different parts of the disassembled cells. Material characterization includes SEM, EDX, HAXPES/XPS and XRD. The most remarkable result is that both cycled cells displayed highly uneven aging primarily of the graphite electrodes, showing large differences between the central parts of the jellyroll compared to the outer parts. The aging variations are identified as differences in capacity and impedance of the graphite electrode, associated with different SEI characteristics. Loss of cyclable lithium is mirrored by a varying degree of lithiation in the positive electrode and electrode slippage. The spatial variation in negative electrode degradation and utilization observed is most likely connected to gradients in temperature and pressure, that can give rise to current density and potential distributions within the jellyroll during cycling.     • An acute meal study was performed to determine postprandial glucose and insulin responses after consumption of two fermented oat bran-based beverages (with and without exopolysaccharides) and yoghurt. This randomized, single-blind, within-subject study included 18 healthy, overweight participants. Four breakfast meals, including a reference meal, were tested; all meals contained 50 g of available carbohydrates, but differed in energy and macronutrient composition. All experimental meals reduced the postprandial glucose response compared with the reference meal. The oat drinks as well as the yoghurt elicited higher early (0-15 min) insulin responses, but the overall insulinaemia were similar to the reference meal. A new food product containing fermented liquid oat bran and milk reduced the postprandial blood glucose response as efficiently as yoghurt after a high-glycaemic index white wheat bread meal, but the presence of microbial exopolysaccharides did not affect the outcome.
