  • Mechanical complications after central venous catheterisation in the ultrasound-guided era : a prospective multicentre cohort study

    • BACKGROUND: Limited data are available on the incidence of mechanical complications after ultrasound-guided central venous catheterisation. We aimed to determine the incidence of mechanical complications in hospitals where real-time ultrasound guidance is clinical practice for central venous access and to identify variables associated with mechanical complications.METHODS: All central venous catheter insertions in patients ≥16 yr at four emergency care hospitals in Sweden from March 2, 2019 to December 31, 2020 were eligible for inclusion. Every insertion was monitored for complete documentation and occurrence of mechanical complications within 24 h after catheterisation. Multivariable logistic regression analyses were used to determine associations between predefined variables and mechanical complications.RESULTS: In total, 12 667 catheter insertions in 8586 patients were included. The incidence (95% confidence interval [CI]) of mechanical complications was 7.7% (7.3-8.2%), of which 0.4% (0.3-0.5%) were major complications. The multivariable analyses showed that patient BMI <20 kg m -2 (odds ratio 2.69 [95% CI: 1.17-5.62]), male operator gender (3.33 [1.60-7.38]), limited operator experience (3.11 [1.64-5.77]), and increasing number of skin punctures (2.18 [1.59-2.88]) were associated with major mechanical complication. Subclavian vein catheterisation was associated with pneumothorax (5.91 [2.13-17.26]). CONCLUSIONS: The incidence of major mechanical complications is low in hospitals where real-time ultrasound guidance is the standard of care for central venous access. Several variables independently associated with mechanical complications can be used for risk stratification before catheterisation procedures, which might further reduce complication rates.CLINICAL TRIAL REGISTRATION: NCT03782324.

  • A web-based tool for in silico biomarker discovery based on tissue-specific protein profiles in normal and cancer tissues

    • Here we report the development of a publicly available Web-based analysis tool for exploring proteins expressed in a tissue- or cancer-specific manner. The search queries are based on the human tissue profiles in normal and cancer cells in the Human Protein Atlas portal and rely on the individual annotation performed by pathologists of images representing immunohistochemically stained tissue sections. Approximately 1.8 million images representing more than 3000 antibodies directed toward human proteins were used in the study. The search tool allows for the systematic exploration of the protein atlas to discover potential protein biomarkers. Such biomarkers include tissue-specific markers, cell type-specific markers, tumor type-specific markers, markers of malignancy, and prognostic or predictive markers of cancers. Here we show examples of database queries to generate sets of candidate biomarker proteins for several of these different categories. Expression profiles of candidate proteins can then subsequently be validated by examination of the underlying high resolution images. The present study shows examples of search strategies revealing several potential protein biomarkers, including proteins specifically expressed in normal cells and in cancer cells from specified tumor types. The lists of candidate proteins can be used as a starting point for further validation in larger patient cohorts using both immunological approaches and technologies utilizing more classical proteomics tools.

Läsaren får här ta del av exempelvis idrottskulturen, fritidskulturen och skolans ämne idrott och hälsa. Genomgående handlar texterna om villkoren för barns och ungdomars deltagane och om de olika lärprocesser som sker i anslutning till idrottsutövning.Lars-Magnus Engström har gjort betydande insatser som forskare och lärare samt som professor vid Lärarhögskolan i Stockholm och vid Gymnastik- och idrottshögskolan. I snart fyrtion år har han arbetat med studier kring påverkans- och lärprocesser i idrott. Hans forskning har främst kretsat kring människors idrottsvanor och vilka som utvecklar en fysiskt aktiv livsstil. Idrotts- och motionsutövningar ger både ett så kallat egenvärde och investeringsvärde. Med dessa begrepp bland många andra har Lars-Magnus Engström bidragit till en fördjupad vetenskaplig förståelse av idrottskulturen.De flesta författarna har eller har haft Lars-Magnus Engström som handledare och tillhör forskningsgruppen för pedagogik, idrott och fritidskultur. Redaktörer för boken är Karin Redelius och Håkan Larsson.

    • This book addresses the environmental issues related to shipping and the natural environment, including descriptions of and proposed solutions to the issues. Currently, challenges exist that must be addressed if shipping is to become sustainable and fulfil the zero vision of no harmful emissions to the environment. In this chapter, we evaluate the steps that have been taken (if any) to limit the various environmental issues and discuss possible steps to be taken to improve environmental performance. Furthermore, future challenges must also be addressed, e.g., the current trend of increasing ship operations in the Arctic. In general, three factors could be addressed in order to reach environmentally sustainable shipping: regulations, technical solutions, and increased environmental awareness. © Springer-Verlag Berlin Heidelberg 2016.     • Today plant-based alternatives to animal-agricultural products are made available or developed alongside ‘cultured’ meat, and products utilising genetic modification. To proponents, this signifies the emergence of ‘cellular agriculture’ as a food-production field or the possibility of a ‘post-animal bioeconomy’: a way to safely and sustainably produce animal products without animals. Drawing on previous work on ontological politics enables acknowledging how these novel objects unsettle animal products’ ontological stability, thereby offering a practical case of how the world is multiply produced. An important emphasis within this tradition is the situated nature of reality-making practices. Consequently our analysis, focusing on different practices, sites and objects compared to influential studies of ontological politics, necessitates bringing in hitherto relatively unexplored political-economic relations and legal processes. As global processes and problem formulations, laboratories, and national or regional regulations come together to remake realities the ontological-political dynamics determining the fate of cellular agriculture or a post-animal bioeconomy becomes shaped by a combination of conflicts and budding collaborations between proponents of new technologies and established livestock interests. Understanding these dynamics requires tracing both how post-animal products reshape the world they are introduced into, and acknowledging the friction evident as reality-carrying objects leave their laboratories.

  • Escape of Lyman continuum radiation from local galaxies : Detection of leakage from the young starburst Tol 1247-232

    • Context. It has been suggested that the escape fraction of hydrogen ionizing photons (f(esc)) from galaxies is evolving with time, but the picture is far from clear. While evidence for significant escape fractions has been found at high redshifts in several studies, the picture looks different in the closer universe. The lack of local detections could in principle be a combined effect of an evolving escape fraction, the low number galaxies observed, the selection criteria of these targets, and technical problems associated with the instrument best adapted for low redshift targets, the Far Ultraviolet Spectroscopic Explorer (FUSE). Aims. In an attempt to tackle the last of these effects, we apply a new background routine developed for FUSE data to study galaxies from the archive in search of possible Lyman continuum (LyC) leakage. In the process, for the first time a stacked spectrum in the LyC has been produced for local galaxies. With this small sample, we also make a more tentative approach to looking for possible correlations between f(esc) and physical parameters, such as internal absorption E(B - V)(i), mass, H I mass, specific star formation rate (SSFR), metallicity, and Ly alpha emission. Methods. Eight star forming galaxies with redshifts z > 0.015 from the FUSE archive were re-examined. Also, a subsample of an additional four galaxies with lower redshifts were included, for which the escape fraction was estimated from residual flux in the low ionization interstellar C II lambda 1036 angstrom line. Results. Out of the eight galaxies, only one was found to have significant LyC leakage, Tol 1247-232 (S/N = 5.2). This is the second detection of a leaking galaxy in the local universe. From the first case, Haro 11, we derive an intrinsic Lyman break amplitude for starbursts at this young age of (f(1500) (angstrom)/f(900) (angstrom))(int) = 1.5(-0.5)(+0.6), which gives an absolute escape fraction for Tol 1247-232 of f(esc) = 2.4(-0.8)(+0.9) %. Tol 1247-232 exhibits an extremely blue far-UV slope reminiscent of high redshift LyC leaking galaxies, and although it does not classify as an AGN in the BPT diagram or by other available diagnostics, a minor AGN cannot be completely excluded. The stacked sample shows an excess in the LyC with f(esc) = 1.4(-0.5)(+0.6) %, but we note that there might be important selection biases involved, since the galaxies were originally handpicked for their star forming qualities. With the small sample, we suggest a possible trend toward higher f(esc) with lower mass and with enhanced SSFR. None of the galaxies with high values of E(B - V)(i) were found to show any sign of leakage.

    • Background: To understand cardiac and skeletal muscle function, it is important to define and explore their molecular constituents and also to identify similarities and differences in the gene expression in these two different striated muscle tissues. Here, we have investigated the genes and proteins with elevated expression in cardiac and skeletal muscle in relation to all other major human tissues and organs using a global transcriptomics analysis complemented with antibody-based profiling to localize the corresponding proteins on a single cell level. Results: Our study identified a comprehensive list of genes expressed in cardiac and skeletal muscle. The genes with elevated expression were further stratified according to their global expression pattern across the human body as well as their precise localization in the muscle tissues. The functions of the proteins encoded by the elevated genes are well in line with the physiological functions of cardiac and skeletal muscle, such as contraction, ion transport, regulation of membrane potential and actomyosin structure organization. A large fraction of the transcripts in both cardiac and skeletal muscle correspond to mitochondrial proteins involved in energy metabolism, which demonstrates the extreme specialization of these muscle tissues to provide energy for contraction. Conclusions: Our results provide a comprehensive list of genes and proteins elevated in striated muscles. A number of proteins not previously characterized in cardiac and skeletal muscle were identified and localized to specific cellular subcompartments. These proteins represent an interesting starting point for further functional analysis of their role in muscle biology and disease.
