    • Compared to individuals who work during the day, shift workers are at higher risk of a range of metabolic disorders and diseases (eg, obesity, cardiovascular disease, peptic ulcers, gastrointestinal problems, failure to control blood sugar levels, and metabolic syndrome). At least some of these complaints may be linked to the quality of the diet and irregular timing of eating, however other factors that affect metabolism are likely to play a part, including psychosocial stress, disrupted circadian rhythms, sleep debt, physical inactivity, and insufficient time for rest and revitalization. In this overview, we examine studies on food and nutrition among shift workers [ie, dietary assessment (designs, methods, variables) and the factors that might influence eating habits and metabolic parameters]. The discussion focuses on the quality of existing dietary assessment data, nutritional status parameters (particularly in obesity), the effect of circadian disruptions, and the possible implications for performance at work. We conclude with some dietary guidelines as a basis for managing the nutrition of shift workers.

The present study sought to validate the Karolinska drowsiness score (KDS) as an indicator of physiological sleepiness against driving impairment and eye blink duration during a 45-minute drive in an advanced moving-base driving simulator. METHODS: Data from 19 persons were used in the analysis. Electrooculography, electroencephalography, and electromyography were administered continuously. Physiological sleepiness was quantified by scoring the percentage (0-100%) of the scoring epoch with alpha and theta activity and slow eye movements (KDS). Lateral position and speed were used as measures of driving behavior. Lane departure was defined as two wheels touching the lane markers. Blink duration was used as a secondary indicator of sleepiness. RESULTS: The results showed that, for young drivers, sleepiness increased with time in the task with higher levels. The variability of the lateral position and the mean and variability of the blink duration significantly changed when sleepiness increased to KDS >/=20%. Furthermore, there was an increase in the risk of lane departure for KDS >/=30%. CONCLUSIONS: The results suggest that KDS scoring is a reasonable procedure for estimating physiological sleepiness under conditions of driving. The results also indicate that a younger age is associated with greater sensitivity to sleepiness at the wheel.

    • The relationship between everyday light exposure and sleep was studied for office workers. The study was conducted during the upswing of the COVID-19 pandemic, enabling a comparison between Office and Home Workdays. Fifteen full-time office employees were monitored for a period of 4–6 weeks. They wore a light-tracking device on their clothes and had a sleep tracker at home. Compared to an Office Workday, light exposure was lower in the afternoon and total sleep time was almost 5 minutes longer on a Home Workday. Sleep efficiency was the same on both workday types. A higher median illuminance level in the afternoon was significantly related to later sleep onset on an Office Workday. Higher median illuminance levels in the morning were related to earlier awakening. Counter to expectations, higher light levels in the evening were also related to earlier awakening. Everyday light exposure matters for sleep quality but may affect circadian functioning differently than the often more extreme light interventions employed in laboratory experiments. Moreover, differences in outcomes between Office and Home Workdays signal the need for further investigation to provide supportive light levels during workhours.

  • The Effects of 884 MHz GSM Wireless Communication Signals on Self-reported Symptoms and Sleep : An Experimental Provocation Study

    • In the current study we assessed possible effects of prolonged (3 hours) exposure to 884 MHz GSM wireless communication signals on self-reported symptoms, cognitive function, and electroencephalographically (EEG) recorded sleep. The study group consisted of 36 women and 35 men. Twenty-two women and sixteen men reported symptoms they specifically related to mobile phone use (SG). The rest of the participants reported no mobile phone-related symptoms (NG). Potential participants volunteering for the study were evaluated by physicians, including some biochemical assessments, to rule out medical conditions that could interfere with study variables of interest. Once selected, participants spent three different sessions in the laboratory. The habituation session was followed by two subsequent sessions. In these subsequent sessions, subjects were either exposed to sham exposure (sham) or 884 MHz GSM wireless communication signals for 3 hours (an average of 1.4 W/kg including periods of DTX and Non-DTX. Exposure directed to the left hemisphere). Data was collected before, during and following the exposure/sham sessions. Data collected included self-reported symptoms, including headache, cognitive function, mood, and electroencephalographic recordings. During actual exposure, as compared to sham exposure, sleep initiated one hour after exposure was affected. There was a prolonged latency to reach the first cycle of deep sleep (stage 3). The amount of stage 4 sleep was also decreased in exposed subjects. NG subjects reported more headaches during exposures vs. sham exposure. Neither group (SG and NG) was able to detect the true exposure status more frequently than by chance alone. The study indicates that during laboratory exposure to 884 MHz wireless signals, components of sleep, believed to be important for recovery from daily wear and tear, are adversely affected. Moreover, participants that otherwise have no self-reported symptoms related to mobile phone use, appear to have more headaches during actual radiofrequency exposure as compared to sham exposure. However, subjects were not able to detect the true exposure status more often than would have been expected by statistical chance alone. Additional self-reported findings, biochemical, performance and electrophysiological data are currently being analyzed. Possible health implications from the findings will also be further explored.

    • Social constraints posed by work schedules influence sleep duration and timing. Everyday light exposure can help (or hinder) sleep outcomes. This study investigated the differences in the relationship between light exposure and sleep outcomes on days with and without social constraints using ambulatory assessment for 4–6 weeks for 15 office employees. The effects of light on sleep were investigated for both clock time and wake time (related to individual sleep times). Participants were exposed to more light during the morning and afternoon on workdays, and sleep times were later on days without social constraints. The relationship between light exposure and sleep was more pronounced, or sometimes even only present, for days without social constraints. In addition, no differences were found between clock time and wake time, which underlines the complexity of the relationship between everyday light exposure and sleep. Despite increased light exposure during workdays, the effects of light on sleep were more pronounced on days without social constraints. It may signal that office workers need a more substantial circadian stimulus (i.e. higher light exposure) for light to influence sleep outcomes on days with social constraints.

    • While we do not yet understand all the functions of sleep, its critical role for normal physiology and behaviour is evident. Its amount and temporal pattern depend on species and condition. Humans sleep about a third of the day with the longest, consolidated episode during the night. The change in lifestyle from hunter-gatherers via agricultural communities to densely populated industrialized centres has certainly affected sleep, and a major concern in the medical community is the impact of insufficient sleep on health 1 and 2. One of the causal mechanisms leading to insufficient sleep is altered exposure to the natural light–dark cycle. This includes the wide availability of electric light, attenuated exposure to daylight within buildings, and evening use of light-emitting devices, all of which decrease the strength of natural light–dark signals that entrain circadian systems [3].

    • During the last few decades, the incidence of sleep-onset insomnia, due to delay of circadian phase, has increased substantially among adolescents all over the world. We wanted to investigate whether a small dose of melatonin given daily, administered in the afternoon, could advance the sleep timing in teenagers. Twenty-one students, aged 14-19 yrs, with sleep-onset difficulties during school weeks were recruited. The study was a randomized, double blind, placebo (PL)-controlled crossover trial, lasting 5 wks. During the first 6 d in wks 2 and 4, the students received either PL or melatonin (1 mg) capsules between 16:30 and 18:00 h. During the first 6 d of wk 5, all students received melatonin. Wks 1 and 3 were capsule-free. In the last evening of each week and the following morning, the students produced saliva samples at home for later melatonin analysis. The samples were produced the same time each week, as late as possible in the evening and as early as possible in the morning. Both the student and one parent received automatic mobile text messages 15 min before saliva sampling times and capsule intake at agreed times. Diaries with registration of presumed sleep, subjective sleepiness during the day (Karolinska Sleepiness Scale, KSS) and times for capsule intake and saliva samplings were completed each day. Primary analysis over 5 wks gave significant results for melatonin, sleep and KSS. Post hoc analysis showed that reported sleep-onset times were advanced after melatonin school weeks compared with PL school weeks (p  <  .005) and that sleep length was longer (p  <  .05). After the last melatonin school week, the students fell asleep 68 min earlier and slept 62 min longer each night compared with the baseline week. Morning melatonin values in saliva diminished compared with PL (p  <  .001) and evening values increased (p  <  .001), indicating a possible sleep phase advance. Compared with PL school weeks, the students reported less wake up (p  <  .05), less school daytime sleepiness (p  <  .05) and increased evening sleepiness (p  <  .005) during melatonin weeks. We conclude that a small dose of melatonin given daily, administered in the afternoon, could advance the sleep timing and make the students more alert during school days even if they continued their often irregular sleep habits during weekends.

  • Light and Motion: Effects of Light Conditions and mEDI on Activity and Motion Area under a Sky-Lighting Machine

    • We investigated whether differences in light levels and spectral properties have effects on motion. Twenty-two participants, divided into groups of two, experienced the same room in two diffused light conditions (daylight [DL] or static artificial light [AL]), which were experienced in a repeated measures design layout, controlled for order. Both light conditions offered a stimulus of at least 250 melanopic equivalent daylight illuminance (mEDI) lux, without a view. Participants were observed during an individual reading session and a collaborative construction game session. We measured the connectivity of the built structures, activity by actigraphy, and we automatically extracted motion area and quantity of motion from video analysis. We found a correlation between mEDI values in the two light conditions (DL or AL) and activity and a correlation between light condition and motion area. Diffuse daylight conditions were correlated with lower activity and less extended motion than a diffuse static condition at levels recommended for office lighting and to ensure alerting responses. Indeed, static AL was found to be related to increased spatial exploration, which might indicate restlessness, and high mEDI to a more composed motion. Actigraphy measurements correlate with quantity of motion values; therefore, the two methods provided comparable results. Results also showed a high correlation between all photometric values in the daylight condition. These findings offer arguments for favoring DL conditions in the design of places where it is desirable to avoid fidgetiness, like educational institutions, and to support composed motion, like medical institutions.

  • Light Rhythms : Exploring the Perceptual and Behavioural Effects of Daylight and Artificial Light Conditions in a Scandinavian Context

    • This compilation thesis collects multidisciplinary work on the study of the impact of light rhythms on perception and behaviour. The thesis was structured to answer and discuss the questions: “How does a person feel and behave inan illuminated space?” and “Do variable light conditions influence perception, appraisal and motion?”. In order to answer the questions, I applied methods from design, psychology and behavioural science, conducted literature reviews and performed two experimental studies. In response to the first question, the outcome of the five papers included in the thesis show that light and lighting rhythms elicit specific acute and long-term effects. These effects impact on these categories of aspects: visual and perceptual, appraisal and experience, behavioural and physiological. To structure and visualize these diverse aspects, I introduce the CLAPP framework: Context Light(ing) Action (behaviour), Perception, Person. The framework highlights the complex interplay between light, environment, and human response, by displaying features related to spatial and light rhythms, effects of light on mind and body, and personal features. The framework can provide structure and direction for education and research activities within the scope of Architectural Lighting Design. In response to the second research question, results from the experimental studies reveal that, even after eliminating view and sunlight, variable daylight conditions elicit better mood, higher pleasure, and influence motion, compared to artificial light conditions. The results of this thesis may contribute to achieving the UN sustainability goals, specifically to improve the well-being of the population (SDG3), to design a built-environment that is safe and resilient (SDG 11), and to promote the uses of affordable and clean energy (SDG 7). Building on the experience gained during this thesis work, I am confident that multidisciplinary collaboration will enable to integrate the diverse aspects included in the CLAPP framework, paving the way for the design of spaces that are both resilient and supportive of health.
