  • Validation and normative data on the Revised Child Anxiety and Depression Scale RCADS-25 in a Swedish national probability sample of children and adolescents aged 4–17 years

    • Background Mental health problems among children and adolescents are a growing public health concern. Self-report questionnaires are pivotal for screening and early detection of mental health issues. We evaluate the psychometric properties and provide norm data of the 25-item child and 25-item parent-report versions of the Revised Child Anxiety and Depression Scale (RCADS). Methods A random probability sample of 29,000 children and adolescents (ages 4–17) from the Swedish population registry was invited. Data collection occurred from September 2021 to January 2022, accompanied by one postal and four SMS reminders. Results Participants (N = 7045, 48.98% girls) demonstrate that the RCADS-25 possesses relatively sound psychometric properties, albeit that the child-report version assessed with children and adolescents aged 8 to 17 years rendered relatively better psychometric properties (e.g., ω = 0.87–0.92, CFI = 0.89) compared with the parent-report version and the children 4 to 7 years old (e.g., ω = 0.72–0.82, CFI = 0.84). Conclusion The RCADS-25 instruments may be used as an effective instrument for the initial assessment of anxiety and depression in children and adolescents in Sweden.

  • Preventing E. coli Biofilm Formation with Antimicrobial Peptide-Functionalized Surface Coatings: Recognizing the Dependence on the Bacterial Binding Mode Using Live-Cell Microscopy

    • Antimicrobial peptides (AMPs) can kill bacteria by destabilizing their membranes, yet translating these molecules' properties into a covalently attached antibacterial coating is challenging. Rational design efforts are obstructed by the fact that standard microbiology methods are ill-designed for the evaluation of coatings, disclosing few details about why grafted AMPs function or do not function. It is particularly difficult to distinguish the influence of the AMP's molecular structure from other factors controlling the total exposure, including which type of bonds are formed between bacteria and the coating and how persistent these contacts are. Here, we combine label-free live-cell microscopy, microfluidics, and automated image analysis to study the response of surface-bound Escherichia coli challenged by the same small AMP either in solution or grafted to the surface through click chemistry. Initially after binding, the grafted AMPs inhibited bacterial growth more efficiently than did AMPs in solution. Yet, after 1 h, E. coli on the coated surfaces increased their expression of type-1 fimbriae, leading to a change in their binding mode, which diminished the coating's impact. The wealth of information obtained from continuously monitoring the growth, shape, and movements of single bacterial cells allowed us to elucidate and quantify the different factors determining the antibacterial efficacy of the grafted AMPs. We expect this approach to aid the design of elaborate antibacterial material coatings working by specific and selective actions, not limited to contact-killing. This technology is needed to support health care and food production in the postantibiotic era.

  • Internal Cooling Design Using Multiphysics Topology Optimization

    • This thesis investigates topology optimization (TO) as a tool for designing optimal internal cooling configurations in components subjected to external hot gas streams. The work is motivated by the challenge of simultaneously considering objectives from multiple physics domains, and the rapid development of additive manufacturing (AM) in the industry, which makes it possible to realize highly complex TO designs. Against this background, a multi-physics TO approach is employed, involving the following physics domains: • Fluid mechanics: describes the flow of the coolant • Heat transfer: describes the coolant and component temperatures • Solid mechanics: describes the structural behaviour of the component Density-based TO is used to parametrize the design, such that the interpolation between solid and fluid material properties is governed by a spatially varying design variable. Temperature measures are considered as objectives in the non-convex optimization problems, and coolant mass flow and structural stiffness measures are considered as constraints. The design process is iterative: for a given design, the flow velocities are computed and used to calculate the temperature distribution, which in turn influences the domain’s structural response. These three equilibrium state problems are solved sequentially, followed by solving corresponding adjoint problems in reversed order, to acquire first-order sensitivity information used by the gradient-based optimization solver. Numerical simulations are carried out for geometries of varying degrees of complexity, resembling gas turbine guide vanes. The simulations cover a range of fidelity levels: from simpler 2D setups to more complex 3D setups with fine resolution, intended for execution on high-performance computing (HPC) clusters. Low-fidelity flow models are utilized in the TO process, while more advanced flow models are employed for design comparisons, and flow and heat response comparisons for given TO designs. The thesis consists of two parts: the first provides the theoretical framework, and the second includes appended papers. In Paper I, only the heat problem is included when modelling convection on internal boundaries identified using the design gradient. Fluid and solid mechanics are introduced in Paper II, where a conjugate heat transfer problem is augmented with a structural model coupled through thermal strains, with numerical examples in 3D. In Paper III, the simultaneous consideration of flow and heat objectives is formulated as a mathematical game between two players trying to minimize the average temperature in the domain and the coolant mass flow through the domain, respectively, with examples in 2D and 3D. The flow problems are the computational bottlenecks, and therefore, Paper IV demonstrates a 3D implementation investigating two different numerical techniques for solving the flow problem, with a voxelization approach for efficient meshing of complex geometries. Paper V presents an efficient, massively parallel HPC implementation for three-field flow-heat-structural models, and addresses further implementation details. This thesis highlights challenging aspects of large-scale multiphysics TO considering fluid mechanics, heat transfer, and solid mechanics.

  • Primary retroperitoneal lymph node dissection as treatment for low-volume metastatic seminoma in a population-based cohort : the Swedish Norwegian testicular cancer group experience

    • Background and objective: There is an unmet need to avoid long-term morbidity associated with standard cytotoxic treatment for low-volume metastatic seminoma. Our aim was to assess the oncological efficacy and surgical safety of retroperitoneal lymph node dissection (RPLND) as treatment in a population-based cohort of metastatic seminoma patients with limited retroperitoneal lymphadenopathy.Methods: Sixty-two seminoma patients in Norway and Sweden were included in the cohort from 2019 to 2022. Patients with lymphadenopathy ≤3 cm, having primary clinical stage (CS) IIA/B or CS I with a relapse, were operated with uni- or bilateral template RPLND, open or robot assisted. The outcome measures included surgical complications as per Clavien-Dindo, and Kaplan-Meier survival estimates for 24-mo progression-free survival (PFS) and overall survival (OS).Key findings and limitations: In the cohort, 33 (53%) had CS I with a relapse during surveillance, six (10%) CS I with a relapse following adjuvant chemotherapy, and 23 (37%) initial CS IIA/B. Metastatic seminoma was verified in 58 patients (94%) with a median largest diameter of 18 mm (interquartile range [IQR] 13–24). Robot-assisted RPLND was performed in 40 patients (65%). Clavien-Dindo III complications were observed in three patients (5%); no grade ≥IV complications occurred. Eighteen patients (29%) received adjuvant chemotherapy after surgery. The median follow-up was 23 mo (IQR 16–30), and recurrence occurred in six patients (10%) after a median of 8 mo (IQR 4–14). PFS was 90% (95% confidence interval: 0.86–1) and OS was 100% at 24 mo.Conclusions and clinical implications: RPLND as primary treatment is an option for selected low-stage seminomas with a limited burden of disease, showing low complications and low relapse rates, with the potential to reduce long-term morbidity.Patient summary: In seminoma patients with limited metastatic spread, surgery is a treatment option offering an alternative to chemotherapy or radiation. This paper covers the first 62 patients operated in Norway and Sweden.

  • Trends in kidney transplantation rate across Europe : a study from the ERA Registry

    • Background. The aim of this study was to identify trends in total, deceased donor (DD) and living donor (LD) kidney transplantation (KT) rates in European countries. Methods. The European Renal Association (ERA) Registry and the Global Observatory on Donation and Transplantation (GODT) databases were used to obtain the number of KTs in individual European countries between 2010 and 2018. General population counts were obtained from Eurostat or the national bureaus of statistics. The KT rate per million population (p.m.p.) and the average annual percentage change (APC) were calculated. Results. The total KT rate in the 40 participating countries increased with 1.9% annually [95% confidence interval (CI) 1.5, 2.2] from 29.6 p.m.p. in 2010 to 34.7 p.m.p. in 2018, reflecting an increase of 3.4 p.m.p. in the DD-KT rate (from 21.6 p.m.p. to 25.0 p.m.p.; APC 1.9%; 95% CI 1.3, 2.4) and of 1.5 p.m.p. in the LD-KT rate (from 8.1 p.m.p. to 9.6 p.m.p.; APC 1.6%; 95% CI 1.0, 2.3). The trends in KT rate varied widely across European countries. An East-West gradient was observed for DD-KT rate, with Western European countries performing more KTs. In addition, most countries performed fewer LD-KTs. In 2018, Spain had the highest DD-KT rate (64.6 p.m.p.) and Turkey the highest LD-KT rate (37.0 p.m.p.). Conclusions. The total KT rate increased due to a rise in the KT rate from DDs and to a lesser extent from LDs, with large differences between individual European countries.

  • Low-Grade Syngas Biomethanation in Continuous Reactors with Respect to Gas–Liquid Mass Transfer and Reactor Start-Up Strategy

    • In order to utilize a wider range of low-grade syngas, the syngas biomethanation was studied in this work with respect to the gas–liquid mass transfer and the reactor start-up strategy. Two reactors, a continuous stirred tank (CSTR) and a bubble column with gas recirculation (BCR-C), were used in the experiment by feeding an artificial syngas of 20% H2, 50% CO, and 30% CO2 into the reactors at 55 °C. The results showed that the CH4 productivity was slightly increased by reducing the gas retention time (GRT), but was significantly improved by increasing the stirring speed in the CSTR and the gas circulation rate in the BCR-C. The best syngas biomethanation performance of the CSTR with a CH4 productivity of 22.20 mmol·Lr−1·day−1 and a yield of 49.01% was achieved at a GRT of 0.833 h and a stirring speed of 300 rpm, while for the BCR-C, the best performance with a CH4 productivity of 61.96 mmol·Lr−1·day−1 and a yield of 87.57% was achieved at a GRT of 0.625 h and a gas circulation rate of 40 L·Lr−1·h−1. The gas–liquid mass transfer capability provided by gas circulation is far superior to mechanical stirring, leading to a much better performance of low-grade syngas biomethanation in the BCR-C. Feeding H2/CO2 during the startup stage of the reactor can effectively stimulate the growth and metabolism of microorganisms, and create a better metabolic environment for subsequent low-grade syngas biomethanation. In addition, during the thermophilic biomethanation of syngas, Methanothermobacter is the dominant genus.

    • Innovations and advancements in technology create new opportunities to run and maintain manufacturing plants, which we refer to as digitalised manufacturing. This development is recognised as a socio-technical system (STS) change, where a change in the production system’s goals, technology, processes, people, or environment may lead to ripple effects between those sub-systems. Despite this, technology development and technology use cases account for most of the research within digitalised manufacturing, while little attention has been devoted to leadership practices considering digitalised manufacturing from a socio-technical perspective. This paper focuses on the maintenance organisation, whose mission in a company is to keep production systems functional. We aim to describe leadership in industrial maintenance from an STS perspective. This is a unique interview study where twenty maintenance managers from Swedish manufacturing industry offer their perspective on the changing leadership within maintenance, providing a unique insight into the challenges facing leaders of maintenance in digitalised manufacturing. We frame the empirical findings using an STS framework and propose an overall consideration model for leadership that supports the development of a functional maintenance organisation in the face of pervasive digitalisation.

  • High-order finite element methods for incompressible variable density flow

    • The simulation of fluid flow is a challenging and important problem in science and engineering. This thesis primarily focuses on developing finite element methods for simulating subsonic two-phase flows with varying densities, described by the variable density incompressible Navier-Stokes equations. These equations are commonly used to model a wide range of phenomena, including aerodynamic forces around vehicles, climate and weather prediction, combustion and the spread of pollution.Incompressible flow is characterized by the velocity field satisfying the divergence-free condition. However, numerically satisfying this condition is one of the main challenges in simulating such flows. In practice, this condition is rarely satisfied exactly, which can result in stability and conservation issues in computations. Moreover, enforcing the divergence-free condition is a primary computational bottleneck for incompressible flow solvers. To improve computational efficiency, we explore and develop artificial compressibility techniques, which regularize this constraint. Additionally, we develop a new practical and useful formulation for variable density flow. This formulation allows Galerkin methods to enhance conservation properties when the divergence-free condition is not strongly enforced, leading to significantly improved accuracy and robustness.Another primary difficulty in simulating fluid flows arises from the challenge of accurately representing underresolved flows, where the mesh resolution cannot capture the gradient of the true solution. This leads to stability issues unless appropriate stabilization techniques are used. In this thesis, we develop new high-order accurate artificial viscosity techniques to deal with this issue. Furthermore, we thoroughly investigate the properties of viscous regularizations, ensuring that kinetic energy stability is guaranteed when using artificial viscosity.
