  • Effects of a 16-Week Digital Intervention on Sports Nutrition Knowledge and Behavior in Female Endurance Athletes with Risk of Relative Energy Deficiency in Sport (REDs)

    • Female endurance athletes are considered a high-risk group for developing Relative Energy Deficiency in Sport (REDs). Due to the lack of educational and behavioral intervention studies, targeting and evaluating the effects of the practical daily management of REDs, we developed the Food and nUtrition for Endurance athletes—a Learning (FUEL) program, consisting of 16 weekly online lectures and individual athlete-centered nutrition counseling every other week. We recruited female endurance athletes from Norway (n = 60), Sweden (n = 84), Ireland (n = 17), and Germany (n = 47). Fifty athletes with symptoms of REDs and with low risk of eating disorders, with no use of hormonal contraceptives and no chronic diseases, were allocated to either the FUEL intervention (n = 32) (FUEL) or a 16-week control period (n = 18) (CON). All but one completed FUEL, while 15 completed CON. We found strong evidence for improvements in sports nutrition knowledge, assessed via interviews, and moderate to strong evidence in the ratings concerning self-perceived sports nutrition knowledge in FUEL versus CON. Analyses of the seven-day prospective weighed food record and questions related to sports nutrition habits, suggested weak evidence for improvements in FUEL versus CON. The FUEL intervention improved sports nutrition knowledge and suggested weak evidence for improved sports nutrition behavior in female endurance athletes with symptoms of REDs.

  • A Three Year Case Study with a Multidisciplinary Treatment of Relative Energy Deficiency and Anorexia in a Female Tennis Player

    • Tennis is an all-year racket sport requiring qualities such as speed, agility, flexibility, and endurance, as competitions may last up to five hours.Elite players have an estimated daily need of carbohydrate (CHO) between 6-10 g/kg to sustain high amounts of training and frequent competitions. Low energy availability (LEA), over time, may impair both health and performance. LEA may cause metabolic suppression and altered hormone levels via hypothalamic–pituitary– gonadal axis (HPG). Female athletes with LEA have an increased risk of developing menstrual dysfunction, low bone mineral density (BMD), disordered eating behavior (DE) and an eating disorder (ED). This article provides a case-study of an internationally ranked female tennis player, with a recent performance-driven weight loss of 11 kg. She was diagnosed with functional hypothalamic primary amenorrhea (FHA) and anorexia nervosa (AN). The predetermined goal was to return to play and the treatment was based on the guidelines for relative Energy Deficiency in Sport (REDS-CAT). After three years of multidisciplinary interventions, the athlete safely returned to play. Her weight and body composition improved, her menstrual cycle was normalized and the diagnostically scores of AN indicated non-clinical significance. The multidisciplinary treatment enabled the athlete to return to play despite several years of LEA and AN.

  • Acute Response to One Bout of Dynamic Standing Exercise on Blood Glucose and Blood Lactate Among Children and Adolescents With Cerebral Palsy Who are Nonambulant

    • Purpose: To investigate the acute exercise effects of dynamic standing exercise (DyS) on blood glucose and blood lactate among children and adolescent with cerebral palsy (CP) who are non-ambulant. Methods: Twenty-four participants with CP who are non-ambulant performed 30 minutes of DyS using a motorized device enabling assisted passive movements in an upright weight-bearing position. Capillary blood-samples were taken from the fingertip for measurement of blood glucose and blood lactate at rest and at the end of exercise. Results: At rest, the participants had hyperlactatemia that was unaffected after exercise presented as median and interquartile range at rest 1.8 [1.3:2.7] mmol/L and after exercise 2.0 [1.1:2.5] mmol/L. Children and adolescents with GMFCS-E&R V had higher lactate levels at rest (2.5 [1.8:2.9] vs 1.4 [1.0:2.0]; p=0.030) and after exercise (2.3 [2.0:2.6] vs 1.2 [0.9:2.2]; p=0.032) compared to children and adolescents with GMFCS-E&R IV respectively. A statistically significant larger decrease in blood lactate levels after exercise was observed in children and adolescents with higher resting blood lactate levels (rho=0.56;p=0.004). There were no statistically significant changes in blood glucose. Conclusions: Forty percent of the participants had mild hyperlactatemia at rest and participants with the highest blood lactate levels at rest had the greatest decrease in blood lactate levels after one bout of exercise. Children and adolescents with classified into higher level of GMFCS-E&R had higher blood lactate levels. More studies are needed on how to prevent chronically high resting levels of lactate with exercise in children with CP who are non-ambulant.

  • Physical activity, body composition, inflammation and effects on antioxidative capacity in sedentary adults

    • Background: For the past fifty years part the population has slowly become sedentary to an extent that it is nowadays considered normal behavior. Parallel with sedentary behavior there is an increase in obesity now reaching epidemic level. Metabolic derangements such as diabetes type 2 and cardiovascular diseases has become an economic burden for the modern society. Implementation of training programs for individuals that have been sedentary for years has often been demonstrated inefficient, with few and short-lasting effects. Aim: The primary aim of this thesis was to study effects of increased physical activity at different intensities in distinct sedentary populations. The secondary aim was to investigate if there was a gender difference in adaption to short-term endurance training. Methodology: Study I: Two hundred and twelve (age 45-69 years) sedentary overweight men and women were included. They were classified as normal or impaired glucose tolerance, or diabetes type 2. They were randomized into controls or intervention group. The intervention group was instructed to increase their physical activity by walking with poles (Nordic walking) 5 h per week. Questionnaire, blood samples, anthropometric data and results from an exercise test were collected at baseline and after 4 months. Study II: Fourteen healthy (22-30 year-old) sedentary normal weight women and men participated in a 3 weeks endurance training program including 2 supervised sessions per day. The training period was followed by 4 weeks of detraining. Blood samples including total antioxidative capacity (TAOC) and oxidative stress (OS), body composition (BC), anthropometric data and evaluation of work capacity were obtained at baseline, after training and after detraining. Result: Study I: Nordic walking improved waist circumference, body weight and body mass index in the intervention group with normal glucose tolerance. Participants that reported > 80 % adherence to training improved work capacity in terms of work load, VO2 peak, or both, in all groups. Biochemical markers were unaffected. Study II: TAOC levels decreased significantly in women after training, while no change was noted in men. After training both gender showed increased work load, VO2 peak even adjusted for fat-free mass, and ventilation. After detraining, work load and VO2 peak decreased in women. Adjusted VO2 peak decreased also in men but remained higher compared to baseline. Fat-free mass (kg) was higher after detraining compared to baseline in women. Conclusion: Study I: Nordic walking had positive anthropometric effects among those with normal glucose tolerance. It could be applied in clinical practice, but surveillance of training might be necessary to reach optimal results among sedentary individuals. Study II: Female participants were more sedentary and their exercise capacity improved to a greater degree. Their decrease in TAOC might reflect an increased consumption of antioxidants to prevent negative effects of OS. A higher level of daily regular physical activity in men could explain their maintenance of improved VO2peak after detraining.

At the same time, interventions aimed at reducing DE behaviors in this population are scarce. METHODS: An 8-week exploratory preventive interven-tion for DE behaviors was carried out in a high-level ballet school for 40 teenagers aged 12-15 years (77.5% female). Both risk factors (perfectionism) and potentially protective factors (self-esteem, self-compassion) for the development of DE behaviors were considered. The intervention was created specifically for this study and consisted of five cognitive behavior therapy (CBT) work-shops and four nutrition workshops. Additional compo-nents included newsletters for pupils and educational sessions and social media interactions with staff and parents. The intervention comprised two phases (control and intervention periods), with students acting as their own controls. Standardized questionnaires were com-pleted before and after both phases. RESULTS: Ques-tionnaire results did not indicate any changes in reported perfectionism, self-esteem, or self-compassion, nor were symptoms of DE affected during either the control or intervention periods. CONCLUSIONS: The intervention did not yield any discernible impact. How - ever, it was affected by the onset of the COVID-19 pan-demic, which limits our ability to draw conclusions about intervention effectiveness. Evaluations with pupils offer several considerations for future improvements.

  • Exploring co-occurrence of sensory, motor and neurodevelopmental problems and epilepsy in children with severe-profound intellectual disability

    • Background: Severe to profound intellectual disability (SPID) is associated with multiple neurodevelopmental disorders and problems. In the most severe cases, the term profound intellectual and multiple disabilities (PIMD) is used. This study aimed to explore the co-occurring disorders and neurodevelopmental problems in a sample of twins where the proband had SPID. Method: Within a population-based sample of (30 312) twins, 20 individuals with a national patient register SPID diagnosis were identified. Parent telephone interview data (screening of neurodevelopmental disorders) and register data (APGAR, birth weight, intellectual disabilities, epilepsy, motor and sensory disorders) were gathered for probands and co-twins. Results: The 20 individuals with SPID all had between one and five additional disorders or problems, with autistic traits, motor problems and epilepsy being the most common. Clear discordance was found for ID and all additional disorders and problems between probands with SPID and their non-SPID co-twins. Conclusion: Children with SPID almost never present without neurodevelopmental and/or sensory and/or motor comorbidities. This heterogeneity should be reflected in clinical routine and in research targeting individuals with SPID. The results support a previously suggested conceptualization of a S/PIMD “spectrum”. Autism may be considered for inclusion in future elaborations of such a S/PIMD spectrum.
