  • Low level of education is associated with later stage at diagnosis and reduced survival in cutaneous malignant melanoma : A nationwide population-based study in Sweden

    • BACKGROUND:A worse outcome has been reported for cutaneous malignant melanoma (CMM) patients with low socioeconomic status. We have investigated the association between level of education, clinical stage at diagnosis (stage at diagnosis) and CMM-specific survival in Sweden.METHODS:We identified 27,235 patients from the Swedish Melanoma Register diagnosed with a primary invasive CMM between 1990 and 2007 and linked data to nationwide, population-based, health and census registers with a follow-up to 2010.RESULTS:The odds ratio (OR) of higher disease stage at diagnosis was significantly increased in lower education groups (OR stage II versus I=1.6; 95% confidence interval (CI)=1.5-1.7. OR stage III-IV versus I=2.3; 95% CI=1.8-2.9). The risk of dying of CMM, was significantly increased in patients with low (hazard ratio (HR) low versus high=2.02; 95% CI=1.80-2.26; p<0.0001) and intermediate (HR intermediate versus high=1.35; 95% CI=1.20-1.51; p<0.0001) level of education. After adjustment for age, gender, stage at diagnosis and other known prognostic factors, the HRs remained significant for low versus high (HR=1.13; 95% CI=1.01-1.27; p=0.04) but not for intermediate versus high (HR=1.11; 95% CI=0.99-1.24; p=0.08) education. The HR associated with low level of education was significantly higher among female patients, patients <55years, patients with truncal tumours and during the first 5years after diagnosis.CONCLUSION:Lower level of education is associated with reduced CMM-specific survival, which may at least partially be attributed to a more advanced stage at diagnosis. These results emphasise the need for improved early detection strategies.

  • Combined Gene Expression and Genomic Profiling Define Two Intrinsic Molecular Subtypes of Urothelial Carcinoma and Gene Signatures for Molecular Grading and Outcome

    • In the present investigation, we sought to refine the classification of urothelial carcinoma by combining information on gene expression, genomic, and gene mutation levels. For these purposes, we performed gene expression analysis of 144 carcinomas, and whole genome array-CGH analysis and mutation analyses of FGFR3, PIK3CA, KRAS, HRAS, NRAS, TP53, CDKN2A, and TSC1 in 103 of these cases. Hierarchical cluster analysis identified two intrinsic molecular subtypes, MS1 and MS2, which were validated and defined by the same set of genes in three independent bladder cancer data sets. The two subtypes differed with respect to gene expression and mutation profiles, as well as with the level of genomic instability. The data show that genomic instability was the most distinguishing genomic feature of MS2 tumors, and that this trait was not dependent on TP53/MDM2 alterations. By combining molecular and pathologic data, it was possible to distinguish two molecular subtypes of T(a) and T(1) tumors, respectively. In addition, we define gene signatures validated in two independent data sets that classify urothelial carcinoma into low-grade (G(1)/G(2)) and high-grade (G(3)) tumors as well as non-muscle and muscle-invasive tumors with high precisions and sensitivities, suggesting molecular grading as a relevant complement to standard pathologic grading. We also present a gene expression signature with independent prognostic effect on metastasis and disease-specific survival. We conclude that the combination of molecular and histopathologic classification systems might provide a strong improvement for bladder cancer classification and produce new insights into the development of this tumor type.

    • Det här är en rapport från ett Vinnovafinansierat projekt där syftet har varit att empiriskt visa på de största utmaningarna för ett ökat tillgängliggörande av öppna data och att visa på möjliga vägar fram. Gemensamt för alla öppna data är att det är maskinläsbara datamängder som kan vidareutnyttjas av vem som helst. Det finns framförallt två aspekter man behöver förhålla sig till när man pratar om öppna data: Vilka datamängder det handlar om samt vilka krav som ställs på tillgängliggörandet. • Det finns flera olika drivkrafter för att arbeta med öppna data. Exempelvis är en verklig offentlighetsprincip i en modern förvaltning är knappt möjlig utan öppna data. Rapporten beskriver ett antal grupper av olika men relaterade syften. - Effektivisera den egna verksamheten - Främja innovation och företagsklimat - Främja regional upplevelse- och besöksnäring - Öka insyn och demokratisk delaktighet • Att tillgängliggöra data består av två huvudsakliga processer - det institutionella möjliggörandet och det tekniska tillgängliggörandet. De organisatoriska frågorna underskattas ofta. Medverkande institutioner i framtagandet av rapport: Media Evolution, Sydsvenska Industri- och Handelskammaren, BTH Innovation (wi.se), och Lunds universitets Internetinstitut (LUii).   • Quantitative estimates of tree species selectivity by moose (Alces alces) in a forest landscape

    • An extensive literature is available on browsing preference for certain tree species. However, useful predictive tools for estimating the impact of deer on forests production and biodiversity can still be improved. A step in that direction is not only to rank preference among tree species but also to quantify the relative risk of being browsed. The foraging selectivity of moose was evaluated using three different statistical methods developed to study habitat utilization. The general pattern for the three methods was consistent. From the results, groups of forage species were clustered and a quantitative index of selectivity was calculated for the groups. The selectivity index showed that rowan (Sorbus aucuparia), willow (Salix ssp.) and aspen (Populus tremula) had a 14 times higher probability of being browsed than a group consisting of Scots pine (Pinus sylvestris) and downy birch (Betula pubescens), while juniper (Juniperus communis) and silver birch (Betula pendula) had a 3.5 times higher probability than Scots pine and downy birch. Since the most preferred species were the least abundant, one should be cautious about the generality of the index between areas, as it may indicate that preference depends on plant species composition. The method used can easily be applied in forest management. Information on quantitative selectivity indices may improve the possibility of managing moose in accordance with acceptable browsing damage.

  • Functional results after orthotopic bladder substitution : a prospective multicentre study comparing four types of neobladder

    • Objective: The aim of this study was to evaluate enterocystometry, voiding pattern and urine leakage of four types of orthotopic bladder substitute. Material and methods: At eight urological departments, 78 consecutive men were studied: 66 with an ileal neobladder [30 Studer pouches (S), 24 Hautmann pouches (H) and 12 T-pouches (T)] and 12 with a right colonic [Goldwasser type (G)] neobladder. Enterocystometry, determination of residual urine, micturition protocol and 24 h pad weight test were performed 6 and 12 months postoperatively. Results: Colonic neobladders had higher pouch pressure at first desire, normal desire and strong desire than ileal neobladders (except at first and normal desire at 12 months) (p < 0.02) and contraction was present more often at both 6 and 12 months (p < 0.01 and p < 0.01). Compliance was good in all types of pouch. Intermittent self-catheterization was more common in H patients at 6 months (p = 0.033). All patients with colonic neobladders used pads during the day and night. In patients with ileal pouches 32% used pads during the day and 70% during the night at 12 months. Urine leakage was higher in patients with colonic bladders at 6 and 12 months during the day (mean/median of 98/31 ml and 82/16 ml versus 10/0 ml and 4/0 ml, p < 0.001). T-pouches had excellent day-time continence, but nocturnal leakage was high. Conclusions: The Hautmann pouch and the Studer pouch behaved similarly at enterocystometry and clinically, and continence was good in the majority of patients. The low number of patients with the other two types of pouch precludes definitive statements.

    • Our data demonstrate a significant association between FGFR3 and PIK3CA mutations in UC. Moreover, the identification of mutations in PIK3R1 further emphasizes the importance of the PI3-kinase pathway in UC. The presence of TSC2 mutations, in addition to TSC1 mutations, underlines the involvement of mTOR signaling in UC.
