  • Early complications after laparoscopic gastric bypass surgery : results from the Scandinavian Obesity Surgery Registry

    • OBJECTIVE: To identify risk factors for serious and specific early complications of laparoscopic gastric bypass surgery using a large national cohort of patients.BACKGROUND: Bariatric procedures are among the most common surgical procedures today. There is, however, still a need to identify preoperative and intraoperative risk factors for serious complications.METHODS: From the Scandinavian Obesity Surgery Registry database, we identified 26,173 patients undergoing primary laparoscopic gastric bypass operation for morbid obesity between May 1, 2007, and September 30, 2012. Follow-up on day 30 was 95.7%. Preoperative data and data from the operation were analyzed against serious postoperative complications and specific complications.RESULTS: The overall risk of serious postoperative complications was 3.4%. Age (adjusted P = 0.028), other additional operation [odds ratio (OR) = 1.50; confidence interval (CI): 1.04-2.18], intraoperative adverse event (OR = 2.63; 1.89-3.66), and conversion to open surgery (OR = 4.12; CI: 2.47-6.89) were all risk factors for serious postoperative complications. Annual hospital volume affected the rate of serious postoperative complications. If the hospital was in a learning curve at the time of the operation, the risk for serious postoperative complications was higher (OR = 1.45; CI: 1.22-1.71). The 90-day mortality rate was 0.04%.CONCLUSIONS: Intraoperative adverse events and conversion to open surgery are the strongest risk factors for serious complications after laparoscopic gastric bypass surgery. Annual operative volume and total institutional experience are important for the outcome. Patient related factors, in particular age, also increased the risk but to a lesser extent.

    • A creditor group exists when two or more creditors have claims against the same debtor and the claims of the relevant creditors are in some way interdependent. A creditor group can have come into existence as a result of an execution of a multi-creditor loan agreement of some sort. The group can also have come into existence as a consequence of a creditor transferring the whole or part of e.g. a payment claim vested with the creditor to one or several other persons who thereby also becomes creditors of the relevant debtor. Moreover, the group can have formed because of a heritage whereby two or several beneficiaries inherit rights in one and the same debt obligation. Other common creditor groups are such that are established as a result of some sort of formal, court supervised, insolvency proceeding. The internal dealings of creditors forming part of a creditor group give rise to a great number of legal issues. The answers to these issues are often dependent on the legal paradigm under which the relevant creditor group is to be assigned. The thesis deal with creditor groups that are formed under a syndicated loan agreement and because of a bond issue as well as a consequence of the commencement of a company reorganization and a bankruptcy proceeding. The aim of the study is to examine how the aforesaid creditor groups is to be classified under the law and thereby find a legal foundation for the various group creditors’ internal legal relationship. The thesis is intended to serve as a pre-study, with the expectation that the findings made in the study will constitute a foundation for further research in this area.

    • Chromosomes 2p, 6q and 14q are candidate sites for genes contributing together to high breast cancer risk. A polygenic model is supported, suggesting the joint effect of genes in contributing to breast cancer risk to be rather common in non-BRCA1/2 families. For genetic counselling it would seem important to resolve the mode of genetic interaction.

    • Purpose: Lifestyle-related conditions such as obesity are associated with prostate cancer progression, but the associations with hyperglycemia and dyslipidemia are unclear. This study, therefore, aims to examine the association of glucose, triglycerides, and total cholesterol with prostate cancer death. Methods: From the Swedish AMORIS cohort, we selected 14,150 men diagnosed with prostate cancer between 1996 and 2011 who had prediagnostic measurements of serum glucose, triglycerides, and total cholesterol. Multivariable Cox proportional hazards regressionmodels were used to determine the hazard ratios for death in relation to the aforementioned metabolic markers. Results: Using clinical cut-off points, a non-significant positive association was observed between glucose and prostate cancer death. When compared to those with glucose in the lowest quartile, those in the highest quartile had greater risk of prostate cancer death (HR 1.19; 95% CI 1.02-1.39). However, neither total cholesterol nor triglycerides were associated with prostate cancer death. Glucose and triglycerides were positively associated with overall, cardiovascular, and other deaths. Hypercholesterolemia was only associated with risk of CVD death. Conclusion: Our results suggest that glucose levels may influence prostate cancer survival, but further studies using repeated measurements are needed to further elucidate how glucose levels may influence prostate cancer progression.
